Autoantibody pain.

As autoantibodies bind to target tissues, Fc-region dependent inflammation can induce pain via mediators exciting nociceptors. But recently another possibility has emerged, where autoantibody binding to nociceptors can directly cause pain, without inflammation. This is thought to occur as a result of Fab-region mediated modification of nerve transduction, transmission, or neuropeptide release. In three conditions, complex regional pain syndrome, anti-voltage gated potassium channel complex autoimmunity, and chronic fatigue syndrome, all associated with no or only little inflammation, initial l

[Central regulation of pain in patients with joint disease and approaches to therapy].

The paper reviews investigations studies that have demonstrated that chronic pain syndrome is mixed in rheumatic diseases. The nervous system is involved in its pathogenesis with different frequency and different mechanisms. Under the influence of afferent pain impulses from damaged joints, there are changes in the excitability of spinal cord neurons, which is called central sensitization (CS). A number of patients have enhanced CS and clinical manifestations as neuropathic sensitive phenomena. The mixed model of the development of chronic pain in joint diseases and its presence along with noc

Cytokines in Neuropathic Pain and Associated Depression.

Neuropathic pain occurs as a result of lesion or disease affecting the somatosensory nervous system and is present in a diverse set of peripheral and central pathologies such as nerve trauma, diabetic neuropathy, post-herpetic neuralgia, chemotherapy-induced peripheral neuropathy, spinal cord injury and multiple sclerosis. Debilitating symptoms including allodynia, hyperalgesia and spontaneous pain have a substantial negative impact on patients' quality of life. The currently available therapeutic treatments are generally ineffective and characterised by poor response rates. Accumulating evide…

Burning Eye Syndrome: Do Neuropathic Pain Mechanisms Underlie Chronic Dry Eye?

OBJECTIVE: Dry eye is a multi-factorial disorder that manifests with painful ocular symptoms and visual disturbances, which can only be partly attributed to tear dysfunction. This disorder may also involve neuroplasticity in response to neuronal injury. This review will emphasize the key characteristics of dry eye pain and its pathologic mechanisms, making the argument that a subset of dry eye represents a neuropathic pain disorder of the eye, more appropriately called "burning eye syndrome." METHODS: A literature review was conducted using a PubMed search focusing on dry eye, corneal nocicep

Special issue on microglia and chronic pain.

This special issue of Experimental Neurology is devoted to the role of Microglia and Chronic Pain. Chronic pain affects 116 million people p … pain after peripheral and central nervous system injury, as well as in diabetic pain, post-herpetic neuralgia pain and related diseases. Collectively, the body of work indicates the importance of understanding the roles of microglial cells in chronic pain which will lead to targeted treatment to attenuate or alleviate chronic neuropathic pain syndromes.

[Metatarsalgia and neuropathies of the foot. Differential diagnosis].

INTRODUCTION: Metatarsalgia is the main symptom of a group of frequent ailments characterised by pain in the balls of the feet. Clinical knowledge and the availability of efficient complementary examinations are useful tools for the neurologist. AIM: To describe the range of processes related with metatarsalgia and a basic diagnostic schema that allows the aetiology to be differentiated. DEVELOPMENT: We perform an elementary review of the neurological anatomy of the foot and outline the different neuropathies affecting the region, as well as the extra neurological processes that could call f

Pulsed radio frequency: a non-neurodestructive therapy in pain management.

PURPOSE OF REVIEW: To appraise and discuss the current available evidence on pulsed radio frequency (PRF), with an emphasis on published ran … ainful conditions successfully treated with PRF include chronic cervical radicular pain, trigeminal neuralgia, chronic shoulder pain and chronic low back pain. SUMMARY: At present very few well designed randomized controlled trials have been carried out on patients comparing PRF to another technique. Further scientific research and clinical trials are requ …

Complex regional pain syndrome type I: neuropathic or not?

Complex regional pain syndrome (CRPS) is clinically characterized by pain, abnormal regulation of blood flow and sweating, edema of skin and subcutaneous tissues, active and passive movement disorders, and trophic changes. It is classified as type I (reflex sympathetic dystrophy) and type II (causalgia). CRPS cannot be reduced to one system or to one mechanism only. In the past decades, there has been absolutely no doubt that complex regional pain syndromes have to be classified as neuropathic pain disorders. This situation changed when a proposal to redefine neuropathic pain states was recent

Tri-cortical pedicle screw fixation in the most cranial instrumented segment to prevent proximal junctional kyphosis.

BACKGROUND CONTEXT: Proximal junctional kyphosis (PJK) is a common complication of deformity correction for degenerative kyphoscoliosis (DKS … he last follow-up. Complications, including PJK, proximal junctional failure (PJF), and intercostal neuralgia were recorded. PJK was defined as: 10° or higher increase in kyphosis angle which between the inferior endplate of upper instrumented vertebra (UIV) and the superior endplate of the UIV + 2 (PJA). PJF was defined as: fracture of UIV or UIV + 1, need …
