This report deals with two brothers suffering from chondrodystrophic myotomy (Schwartz-Jampel syndrome), and presents, through the study of these cases, the orthopedic and neurological problems presented by this condition. In the case of our patients there is no confirmation of the stunted growth mentioned in other reports, nor are there any changes in the vertebral column. In contrast to cases reported to date, pathologically high sero-enzyme amounts were found in both children. The congenital changes in the hip joints worsen during the pre-puberty phase, so that in one case bilateral adducti

A case of extremely severe birth asphyxia is reported. In spite of successful resuscitation the arterial pH was still only 6.6 at the age of 1 h. Assessment at 3 years 10 months revealed that the child was not only completely normal, but was of superior intelligence. This case demonstrates the advisability of maintaining an optimistic approach even when very severe fetal acidosis is detected around the time of birth.

From 1969 through 1973, 335 consecutive patients (mean age, 60 years) underwent 390 carotid endarterectomies using hypercarbic general anesthesia and no carotid shunting. Early neurologic complications were most common among patients with previous neurologic symptoms and among those with subtotal stenosis or occlusion of the contralateral internal carotid artery. The introduction of routine carotid shunting without hypercarbia during a subsequent series of 626 procedures from 1974 through 1978 has been associated with significantly fewer operative strokes in comparable groups of patients. The opiate antagonist naloxone has been used to treat cats subjected to cervical spinal trauma. In contrast to saline-treated controls, naloxone treatment significantly improved the hypotension observed after cervical spinal injury. More critically, naloxone therapy significantly improved neurologic recovery. These findings implicate endorphins in the pathophysiology of spinal cord injury and indicate that narcotic antagonists may have a therapeutic role in this condition.

It has been shown experimentally that irreversible brain injury and death may follow after 5-7 min of cerebral ischemia and that even brief periods, 2 min or less, can produce focal damage to the nervous system. All published studies demonstrating a protective effect of barbiturates have been performed in animals. A patient is presented who recovered full neurological function after 13 min of hypovolemic hypotension. This remarkable outcome may have been due to the rapid institution of high-dose barbiturate therapy. The extent to which such therapy affected his outcome is unclear, but does add

A variety of neurological complications may occur in the various connective tissue and "collagen-vascular" diseases. Most of these complications are due to vasculitis affecting various sites in the central or peripheral nervous system. While the evidence for definitive vasculitis in SLE is not strong, small vessel damage usually is present in anatomic sites which correlate well with clinical features. Although patients with rheumatoid arthritis also may have vasculitis, neurological complications are usually related to nerve compression by rheumatoid nodules or the arthritic process itself. Co

Cerebral damage remains a major hazard of open-heart surgery. A one-year follow-up investigation of 100 consecutive patients who underwent open-heart operation for valve replacement revealed an incidence of postoperative cerebral disorders of 37%. The occurrence of brain damage was clearly related to the presence of a history of previous neurological diseases, to operative hypoxia, and to unexpected events during operation, but long perfusion time proved to be the most significant risk factor. In contrast to previous findings, age and moderate operative hypotension proved unimportant. The abno

Nervous-system specific aldolase C has been detected in human cerebrospinal fluid (CSF) by radioimmunoassay. Measurement of 138 samples of CSF showed a mean level of 92 +/- 28 ng/ml. There was no correlation between the level of CSF aldolase C and the CSF total protein, albumin, IgG, or IgA levels. Aldolase C immunoreactivity present in concentrated CSF diluted out in parallel with the standard curve in the assay and showed an elution profile on ion-exchange and gel filtration chromatography similar to that of aldolase present in whole human brain extracts. Addition of known quantities of puri

Enteroviruses--4 polio, 30 Coxsackie and 20 echovirus strains--were isolated from 54 patients with neurologic disease. In 29 cases (53.7%) viruses were isolated from the CSF, blood clot and nerve tissue homogenates. The correlation of the complex clinical data and the localisation of the lesions in the CNS demonstrate the involvement of cranial nerves, of the long bulbar motor pyramidal and extra-pyramidal tracts, of the reticular formation, a.o. These aspects are in accordance with the localisation of the lesions especially in the brain stem and in the hypothalamus, in most of our patients. T

A good tumoricidal activity of vindesine (VDS) has been reported in a variety of animal tumors and in human leukemias and lymphomas. We treated 22 patients who had received no prior chemotherapy and were suffering from a variety of malignant neoplasms with 0.5 mg/m2 to 3.0 mg/m2 VDS i. v. once or three times at weekly intervals and recorded the clinical, hematologic, and especially, neurological side effects. Clinically we observed fatigue in nine patients, paresthesias in seven, myalgias in three, vertigo and diarrhea in two, and skin pains, tinnitus, gastric pains, alopecia, and tremor in on

28 patients with spondylolisthesis who underwent anterior interbody fusion at the Orthopaedic Department of the University of Basle between 1968 and 1976, have been follow-up. In 90% satisfactory or good clinical results were achieved. In 82% the neurological symptoms - without laminectomy - were markedly improved or eliminated. According to the authors anterior interbody fusion (with reduction and enlargement of the intervetebral space) is indicated even when neurological symptoms are present.

Relapsing polychondritis is a rare disease of undetermined cause. The most frequently seen symptom is redness and swelling of the cartilaginous ear, followed by cartilage inflammation elsewhere in the body. Our five cases had neurologic symptoms that posed diagnostic difficulties. Because vasculitis is not uncommon in relapsing polychondritis, it was difficult to determine whether these neurologic symptoms were attributable to relapsing polychondritis or to other unrelated causes. In our cases, specific neurologic complaints were caused by a concomitant but unrelated disease, rather than by re

T lymphocytes were determined according to their ability to form rosettes with sheep erythrocytes, in cerebrospinal fluid (CSF) from 120 patients, and in peripheral blood from 59 patients. Normal CSF contained 74.9 +/- 9.6% T lymphocytes. Increased T lymphocyte percentage was found in CSF from patients with active multiple sclerosis (MS), and Guillain-Barré syndrome, as well as the 2 patients with retrobulbar neuritis and the 1 with subacute sclerosing panencephalitis. Patients with stable MS showed no significant change in CSF T lymphocyte percentage. CSF from patients with viral meningo-ence

This review evaluates the theoretical background and experimental data behind a new development: the replacement therapy of deficient central cholinergic systems with the dietary precursors choline or lecithin. Cholinergic deficiency states are possibly present in five neurological entities: Huntington's chorea, Tardive Dyskinesia, Gilles de la Tourette's disease, Friedreich's ataxia and pre-senile dementia. Preliminary data from various laboratories, including our own, in each of these disorders indicate that some clinical improvement can occasionally be seen, and that this approach deserves

An adult with acute nonlymphoblastic leukemia involving the central nervous system is presented. Unusual features included: (1) Focal signs and radiographic evidence of sagittal sinus occlusion early in the course of disease; (2) progressive meningeal, cranial nerve, and spinal nerve involvement despite a 4-year bone marrow remission; (3) intracerebral tumor formation, and (4) retrobulbar optic neuritis associated with microscopic findings of herpeslike viral particles. The incidence of clinically overt neurologic disease in adults with acute nonlymphoblastic leukemia seems to have increased i

Ultrasonography is the most innocuous and noninvasive procedure, ideally suited for screening patients suspected of having cerebrovascular insufficiency. The Doppler ultrasound and real-time techniques complement each other. The latter can actually display a two-dimensional dynamic image of the carotid artery. B-scan can also be used in fetuses, infants, and young children for precise assessment of the size of the ventricular system. It can be used for serial follow-up evaluation of hydrocephalus. Ultrasound can be utilized in determination of the depth and width of the lumbar spinal canal. Di
