Moyamoya disease in a Hispanic child: a case report.

BACKGROUND: Moyamoya disease was initially described by Suzuki and Takaku in 1963 as a radiographic phenomenon relating to the tiny collateral vessels characteristic of the disease that resemble a cloud or puff of smoke. The disease is rare and initially it was believed that the disease was confined to the Japanese population. It consists of occlusive vascular disease at the Circle of Willis with a tendency toward multiple ischemic neurological events and small strokes. In older populations it can often be associated with further vascular degeneration and intracerebral hemorrhage. This paper d

[Therapeutic strategy in severe encephalopathies].

INTRODUCTION: The severe epileptic syndromes of infancy are dependent on age, symptomatic or cryptogenic aetiology and are drug-resistant so that overall prognosis control of epilepsy and adequate cognition is poor so that some authors consider it to be catastrophic. All may be identified and differentiated on electroclinical criteria. DEVELOPMENT: We study the neonatal myoclonic encephalopathies in the two clinical forms described by Aicardi and Ohtahara, West s syndrome (infantile spasms), severe myoclonic epilepsy of Dravet-Dalla Bernardina and the Lennox-Gastaut syndrome, briefly describi

Subependymal giant-cell astrocytoma in tuberous sclerosis: endoscopic images and the implications for therapy.

Supratentorial intraventricular tumors are not frequently encountered in childhood. One of the most frequent intraventricular glial tumors is the subependymal giant-cell astrocytoma, mostly associated with tuberous sclerosis. These tumors are diagnosed on computed tomography (CT) or magnetic resonance imaging (MRI) scans. They can occur isolated or multiple and operative resection is advised if these tumors cause symptoms, usually raised intracranial pressure due to obstructive hydrocephalus. However, the number of tumors can be much higher than seen on radiological examination making total re

Our experience with surgical treatment of ischial nerve injuries.

OUTLINE: This report presents the results of 44 surgical interventions performed on 44 patients during a period of 15 years, from 1985 to 1999. The report presents the basic lines of surgical treatment performed on a total number of 50 peripheral nerves of lower extremities--nervus ischiadicus and its rami. PATIENT GROUP AND METHODS: In the whole group of 44 patients, external neurolysis was performed in 23 individuals on 26 nerves. Remaining 21 patients were treated by reconstruction surgery that included 24 injured nerves. In this subgroup, suture of peripheral nerve was performed in 8 trea

[Spontaneous spinal epidural hematoma. Report of two cases].

In the paper 2 cases of non-traumatic spinal epidural haematomas are presented. The possible aetiological factors--vascular malformation, coagulopathy, anticoagulants and neoplasm are discussed. Neurological state, diagnostic procedure and treatment are described. Taking into account our experience we state that only early diagnosis and operation-surgical decompression of spinal cord, could result in complete recovery.

[Surgical treatment of Parkinson disease and essential tremor].

Recognition of the often failing effect of levodopa treatment as Parkinson's disease (PD) progresses has led to the resumption of surgical treatment. In the 1950'ies and 1960'ies pallidotomy and thalamotomy were commonly performed to reduce parkinsonian symptoms, whilst today these permanent lesions are replaced by Deep Brain Stimulation (DBS) of implanted leads. The advantages of DBS in either the subthalamic nuclei (STN) or the internal part of the globus pallidus are that implantation of leads into these nuclei can be performed bilaterally, and that DBS is a reversible treatment. Pallidotom

Versatile intraoperative MRI in neurosurgery and radiology.

BACKGROUND: Several models for the application of intra-operative magnetic resonance imaging (IMRI) have recently been reported, most of them unique. Two fundamental issues need to be addressed: optimal use of the scanner to ensure a wide base for research, development and clinical application, and an organisational model that facilitates such use. METHOD: While in our setting the IMRI project was initiated by the neurosurgeons, the need for wider use of the facilities was recognised since the beginning of the planning phase in 1996. An organisational model was developed that allowed for deve

Endoscopic transsphenoidal surgery for a somatotroph pituitary neuroendocrine tumor associated with an infraoptic course of the anterior cerebral artery: A case report.

BACKGROUND: Identifying cerebrovascular anomalies before neurosurgical procedures is essential to avoid intraoperative complications. An infraoptic course of the anterior cerebral artery (IOA) is a rare variant in which the anterior cerebral artery (ACA) arises near the ophthalmic artery and passes beneath the optic nerve. Despite numerous reports of cerebral aneurysms coexisting with IOA, reports of coexistence with brain tumors are extremely rare, and no studies have reported endoscopic transsphenoidal surgery (eTSS) performed for pituitary tumors. Failure to recognize this IOA anomaly preop…

Effect of tranexamic acid on blood loss in patients undergoing resection of large meningioma: a randomized clinical trial.

To determine whether intraoperative tranexamic acid (TXA) administration reduced intraoperative blood loss in patients undergoing large meningioma resection. Patients undergoing large meningioma resection face a high risk of significant blood loss. While TXA reduces blood loss and transfusion requirements in many non-neurosurgical procedures, its effect on patients undergoing supratentorial large meningioma resection remains unclear. This study was a single-center randomized double-blind trial. 228 patients having elective resections of supratentorial meningiomas with diameters ≥ 5 cm were enr…

Coil-based middle meningeal artery embolization for chronic subdural hematoma: a scoping review.

Chronic subdural hematoma (cSDH) is a common neurosurgical condition with meaningful recurrence after conventional treatment. Middle meningeal artery embolization (MMAE) has emerged as an adjunctive and, in selected cases, a treatment strategy. Although liquid embolic agents and particles have been more extensively studied, coil-only MMAE is used in practice and may be particularly useful in anatomically challenging cases. We aim to synthesize the available evidence on coil-only MMAE for cSDH. This scoping review was conducted in accordance with the Preferred Reporting Items for Systematic rev

Regional cerebral oxygen saturation metrics for predicting postoperative delirium in elderly patients undergoing non-cardiac, non-neurological surgery: a propensity-matched cohort study.

PURPOSE: Regional cerebral oxygen saturation reflects cerebral oxygenation, but it remains unclear which cerebral oxygenation metric is most strongly associated with postoperative delirium during non-cardiac surgery. METHODS: This retrospective observational cohort study analyzed patients aged ≥ 65 years undergoing non-cardiac, non-neurosurgical procedures under general anesthesia between January 2020 and December 2024. Baseline regional cerebral oxygen saturation, the absolute lowest intraoperative value, and the percentage decrease from baseline were evaluated. Postoperative delirium was as…

Postoperative adverse events after spinal surgery: Sex-specific risk profiles and ICU utilization.

BACKGROUND: Sex differences in surgical outcomes are well-documented, yet data specific to spinal surgery remain conflicting. RESEARCH QUESTION: We investigated whether postoperative risk truly differs between sexes or if disparities reflect case mix and ICU triage practices. METHODS: We analyzed prospectively collected data from 1337 spinal procedures (2023-2025) at a tertiary neurosurgical center. Multivariable logistic and Poisson regression models estimated the independent effect of sex on 30-day outcomes (complications, neurological deficits, rebleeding, ICU admission) and length of sta

Dual Impact: Coup-Contrecoup Brain Injury in a Young Cyclist-A Case Report.

INTRODUCTION: Coup-contrecoup brain injury occurs when trauma produces focal damage both at the site of impact (coup) and at the contralateral side (contrecoup) due to intracranial pressure (ICP) dynamics and rotational forces. These injuries are frequently seen in high-velocity accidents and often demand staged neurosurgical intervention. CASE PRESENTATION: We report a case of a 14-year-old boy presenting with altered sensorium following a road traffic accident. Initial CT revealed a large left frontal extradural hematoma (EDH) with mass effect and contralateral right frontotemporoparietal c

Morphological Analysis of the M1 Segment of the Middle Cerebral Artery and Its Implications for Neurosurgical Approaches: A Cadaveric Study.

Anatomical variations of the middle cerebral artery (MCA) and its lenticulostriate perforator branches can confound imaging interpretation and increase ischemic injury risk during various procedures, particularly around the M1 segment, where aneurysms arise. This cadaveric study characterized M1 segment morphology and perforator distribution. Thirty formalin-fixed cerebral hemispheres (27 males and three females) with a mean age of 68 years (ranging from 48 years to 77 years) were dissected under 2.5× magnification with arachnoid removal; M1 origin relative to optic nerve and chiasma were docu

Middle meningeal artery arteriovenous shunting after surgical evacuation of chronic subdural hematoma: a retrospective angiographic cohort study.

Arteriovenous shunting involving the middle meningeal artery (MMA) has been reported after cranial trauma, neurosurgical procedures, and endovascular interventions. However, its angiographic frequency and characteristics after chronic subdural hematoma (cSDH) surgery remain poorly defined. This study aimed to assess the frequency and angiographic features of MMA arteriovenous shunting in patients undergoing MMA embolization after surgical evacuation of cSDH. We retrospectively analyzed consecutive patients who underwent selective and superselective MMA angiography during MMA embolization after…
