Differences in the Health Status of Older Adults in Community and Hospital Cohorts.

BACKGROUND: Older adults frequently utilize healthcare services across diverse medical settings to maintain continuity of care. However, data from the hospital records of older adults is often not linked to their health status in community settings, preventing a full understanding of their healthcare needs. METHODS: This study aimed to compare the multidimensional health status (i.e., self-rated health, depression, physical function/activity, mobility, nutrition, lifestyle factors, blood laboratory, and diseases) of older adults between community and hospital cohorts. The goal was to identify

Sarcopenia and liver transplant: The relevance of too little muscle mass.

Loss of muscle mass and function is a common occurrence in both patients with decompensated cirrhosis and those undergoing liver transplantation. Sarcopenia is associated with morbidity and mortality before and after liver transplantation. The ability of skeletal muscle mass to recover after transplant is questionable, and long term adverse events associated with persistent sarcopenia have not been well studied. Limited data is available examining mechanisms by which decreased muscle mass might develop. It is not clear which interventions might reduce the prevalence of sarcopenia and associate

Phenotypical characterization of changes in thymus and spleen associated with lymphoid depletion in free-ranging harbor porpoises (Phocoena phocoena).

Harbor porpoises from the North and Baltic Seas exhibit a higher incidence of bacterial infections compared to whales from less polluted arctic waters. Furthermore, thymic atrophy and splenic depletion, associated with elevated concentrations of environmental contaminants, such as polychlorinated biphenyls (PCB) and polybrominated diphenyl ether (PBDE) body burdens, have been found in wildlife harbor porpoises. Thus, there is currently a debate about the potential adverse effect of xenobiotics on the immune system and therefore on the health status of this and other marine mammal species. The

Malnutrition and the role of nutritional support for radiation therapy patients.

The nutritional status of a tumor patient can be negatively influenced by the local and systemic effects of the malignant tumor (tumor cachexia, anorexia, difficult oral food intake), by the effects of the various antitumoral therapy modalities (surgery, radiotherapy, chemotherapy), and by the complications associated with such modalities (anorexia, nausea, vomiting, mucositis, xerostomia, alterations of the smell and taste sensations, odynophagia, dysphagia, maldigestion, malabsorption, diarrhea, steatorrhea, conditioned aversions, radiogenic late effects), as well as by the psychological rea…

The GLIM criteria for defining malnutrition can predict physical function and prognosis in patients with cardiovascular disease.

BACKGROUND & AIMS: The Global Leadership Initiative on Malnutrition (GLIM) published a new international standard for defining malnutrition in 2018. The GLIM criteria were compared with the European Society for Clinical Nutrition and Metabolism (ESPEN) criteria in relation to physical function and mortality risk in patients with cardiovascular disease (CVD). METHODS: A total of 921 CVD patients ≥ 20 years old (67.8 ± 13.4 years, 631 men) hospitalised for heart failure, acute coronary syndrome and other conditions were stratified according to the presence or absence of malnutrition according t

Impact of Maillard reaction products on nutrition and health: Current knowledge and need to understand their fate in the human digestive system.

The Maillard Reaction (MR) is a non-enzymatic chemical reaction which results in the linkage between the amino group of amino acids and the carbonyl group of reduced sugars. MR products (MRPs) are common components of processed foods, mainly as a result of heating, especially in the Western diet. MRPs are classified as into three stages: initial, intermediate, and final stages, indicative of increased complexity and size, incurring different flavor, aroma, and texture. MRPs presence is known to reduce the nutritional quality of foods, particularly by reducing protein digestibility. Early repor

Levodopa and nutrition support: A case report of Parkinsonism-hyperpyrexia syndrome.

Levodopa is competitively inhibited by amino acids for absorption across the intestinal wall and blood-brain barrier. An acute withdrawal of levodopa increases the risk of Parkinsonism-hyperpyrexia syndrome, a life-threatening condition characterized by muscular rigidity, mental status changes, hyperthermia, and autonomic instability. This report discusses the case of a patient with Parkinson's disease who was critically ill who developed Parkinsonism-hyperpyrexia syndrome while receiving enteral and parenteral nutrition support. Clinicians should be aware of this potential drug-nutrient inter

Sinus bradycardia associated with peripheral lipids and total parenteral nutrition.

This case report describes a 23-yr-old woman with Crohn's disease who developed sinus bradycardia 4 hr after initiation of total parenteral nutrition with lipids through a peripheral vein. The bradycardia persisted for 1 wk, with only slight improvement when the lipids were held for two 4-hr periods. The heart rate began to improve when lipids were changed from 10 to 20%, and continued to rise after Synthroid was started. The possible relationships between nutritional and thyroid status, as well as lipids and heart rate are discussed.

Total abdominal wall reconstruction.

A patient had full-thickness loss of abdominal wall from clostridial myonecrosis. Initial care consisted of resuscitation, debridement, and transfer to a hyperbaric chamber facility. After control of sepsis, multiple enteric fistulas were managed by enterotomies, gastric and duodenal defunctionalization with closed-loop gastrojejunostomy, gastrostomy, and end jejunostomy. Good nutritional status was maintained with total parenteral nutrition over a three-month period. Total abdominal wall reconstruction was accomplished by rotation of bilateral tensor fascia lata musculocutaneous flaps. Recons…

The clinical utility of nutritional and immunological indices as prognostic factors in pancreatic cancer after pancreatectomy.

BACKGROUND: Pancreatic cancer (PC) exhibits aggressive malignant behavior and is the most lethal cancer type worldwide. It is often accompanied by malnutrition, which exacerbates as the disease progresses. We investigated the clinical utility of nutritional and immunological indices as prognostic factors in patients with PC using five major nutritional indices: the Controlling Nutritional Status (CONUT) score, C-reactive protein/albumin ratio, neutrophil/lymphocyte ratio, platelet-to-lymphocyte ratio, and Prognostic Nutritional Index. METHODS: We retrospectively analyzed 328 patients with PC …

The novel nutritional metabolic risk index and established triglyceride-glucose indices in type 2 diabetes: modifying role of biological aging assessed by telomere length.

BACKGROUND: Triglyceride-glucose (TyG) derivatives are recognized markers of type 2 diabetes (T2DM) risk. However, the association between composite indices integrating nutritional and metabolic status and biological aging in T2DM patients remains unclear. METHODS: Data from 4,572 adults were analyzed to evaluate the association between four indices, including the geriatric nutritional risk index (GNRI), TyG, triglyceride glucose-waist-to-height ratio (TyG-WHtR), and the composite nutritional metabolic risk index (NMRI), and T2DM prevalence using logistic regression and restricted cubic splin

Machine learning prediction of complications after surgery in cirrhotic patients: Integrating nutritional risk with liver function.

Liver disease is among the most common medical conditions in the world, leading to high rates of morbidity and death. Early identification of liver diseases enables prompt care, which may prevent many conditions from progressing to more serious stages, like cirrhosis or liver cancer. Traditionally, models often overlook the predictive importance of nutritional status and focus primarily on liver-specific measures. This research aims to propose a machine learning (ML) approach to predict surgical complications in patients with liver cirrhosis by integrating existing liver function metrics with …
