Staged informed consent for a randomized clinical trial in childhood leukemia: impact on the consent process.

BACKGROUND: Children Cancer Group (CCG) 1991 is the first childhood acute lymphoblastic leukemia trial within CCG that allowed the utilization of a staged approach to the consent process. METHODS: One hundred and forty subjects participated in the Project on Informed Consent which compared the primary outcome measures in the consent process of patients enrolled in CCG-1991 with those enrolled in other CCG leukemia studies. RESULTS: The parents' trust scores were higher for the CCG-1991 compared with other protocols. Eighty percent of parents enrolled in CCG-1991 understood the distinction be

Mothers' experiences of living worried when parenting children with spina bifida.

This study describes the lived experience of mothers of children with spina bifida. Thirteen mothers of children between the ages of 12 and 18 years participated in at-home, audiotaped interviews. Each participant was asked to describe, through narrative, what it is like to be the mother of a child with spina bifida. The researcher used Heideggerian hermeneutical methodology to transcribe and analyze interview texts. Examination of the data revealed the constitutive pattern living worried and its two relational themes: treating them like other children and staying in the struggle. Nurses can u

Familial environment associated with type A behaviors and psychophysiological responses to stress in children.

This study tested two major hypotheses regarding the characteristics of family environments associated with children's Type A behaviors, anger frequency and expression, hostile outlook, hostility displayed during an interview, and cardiovascular responses to laboratory stressors. Two measures of family environment, Positive Affiliation and Authoritarianism, were derived by a factor analysis of the Family Environment Scale completed by parents. The sample consisted of 66 girls and 48 boys enrolled in Grades 2 through 12 from 114 families residing in a predominantly White, upper-middle-class sub

Parental attitudes toward adolescent sexuality: transcultural perspectives.

The problem of teenage pregnancy continues to impact private and public resources, affecting all socioeconomic and cultural groups. A key factor for nurse practitioners to consider when planning sex education programs is the differing parental attitudes toward teenage sexuality. These attitudes are especially important to keep in mind when dealing with parents from minority cultural groups, as these groups are often highly influential in determining the nature of adolescent sexual behavior and attitudes toward reproduction. A study of Cuban and Haitian child-rearing practices clearly demonstra

An ethological view of human adolescence.

In modern times the essential "normality" of adolescence has been acknowledged. With its pan-cultural and functional perspective, ethology is in a position to elaborate a description of normal human adolescence. Ethologists attempt to elucidate four interrelated aspects of a given behavior: its survival function, evolution, development, and elicitation by internal and external factors. The basic research strategy is two-fold: to identify biologically based, or evolved, behaviors (chiefly by controlling for the effects of experience and seeing if a core of evolved behavior remains); and to disc

Anxiety sensitivity and panic attack symptomatology among low-income African-American adolescents.

This study examined the concurrent and prospective relation between anxiety sensitivity (AS) and panic attack symptomatology among a community sample of African-American adolescents (N = 107; mean age 15.6 years) from predominantly low-income, single-parent households. On two occasions, 6 months apart, youth completed self-report measures of AS, measured by the Childhood Anxiety Sensitivity Index (CAS I), and panic symptomatology, measured by the Panic Attack Questionnaire (PAQ) and/or the Panic subscale of the Screen for Child Anxiety-Related Emotional Disorders (SCARED-P). Results indicated

Efficacy of cognitive-behavioral treatment and fire safety education for children who set fires: initial and follow-up outcomes.

The efficacy of cognitive-behavioral treatment (CBT) and fire safety education (FSE) for children who had set a recent fire was evaluated. Assessments were conducted with 38 children who were randomly assigned to CBT or FSE and with another 16 children who received a brief intervention (home visit from a firefighter or HVF) that paralleled routine services. Measures in four domains related to the child's fire history were obtained from children and their parents at pre-treatment, post-assessment, and 1-year follow-up. There were several improvements at post-treatment for all conditions on meas

Compound Heterozygous Mutations in FGA Cause Congenital Afibrinogenemia: A Case Report and Literature Review.

BACKGROUND: Congenital afibrinogenemia is an extremely rare autosomal recessive bleeding disorder with an estimated incidence of one in one million, caused by mutations in the fibrinogen-encoding genes. METHODS: We report a genetically confirmed case of FGA-related congenital afibrinogenemia in a 9-year-old female child. Comprehensive coagulation analysis revealed undetectable fibrinogen activity (FIB:C, < 0.2 g/L) as measured by the Clauss assay, along with significantly prolonged prothrombin time, activated partial thromboplastin time, and thrombin time. RESULTS: Next-generation sequencing

Humanity's evolved nest and its relation to cardiac vagal regulation in the first years of life.

BACKGROUND: The Evolved Developmental Niche (EDN) is a millions-year-old developmental system that matches the maturational schedule of the offspring, optimizing health. Every animal has a developmental niche. AIMS: Humanity has fallen away from providing its EDN. Does it matter? STUDY DESIGN: Several components of humanity's EDN were reviewed (breastfeeding, positive touch, allomothers, responsive care, free play) in relation to cardiac vagal nerve regulation, a signal of healthy development. Focal subjects were young children. OUTCOME MEASURES: A review of research on the selected EDN com

Characterization of Sleep Disturbances in Children and Adolescents with Down Syndrome and Their Relation with Cognitive and Behavioral Features.

Despite sleep disturbances are common among youths with Down syndrome (DS), the cognitive and behavioral features associated with sleep problems have not yet been studied extensively. The present study investigated the presence of sleep disturbances in a group of children and adolescents with DS and their cognitive and behavioral correlates. Seventy-one children and adolescents with DS underwent a neuropsychological evaluation, whereas parents completed questionnaires for the screening of the child's sleep, emotional and behavioral problems. We found no association between sleep disturbances a

Development and pilot test of the shriners pediatric instrument for neuromuscular scoliosis (SPNS): a quality of life questionnaire for children with spinal cord injuries.

BACKGROUND/OBJECTIVE: Children and adolescents who have sustained a spinal cord injury (SCI) are at risk of developing spine deformities and secondary complications that may affect their quality of life. The Shriners Pediatric Instrument for Neuromuscular Scoliosis (SPINS) is a condition-specific instrument that was developed to measure the health-related quality of life (HRQOL) of this patient population. A pilot study was conducted to revise the SPINS and assess comprehensibility. METHODS: Fourteen children with SCI (ages 6-16 y) from a pediatric hospital were administered either a child ve

Parent-Reported Health-Related Quality of Life at 3 Years of Age in Children Treated for Sagittal or Metopic Synostosis.

Early surgical intervention is recommended for sagittal (SS) and metopic (MS) synostosis to correct head shape and reduce the risk of later social stigmatization. However, the impact of craniosynostosis (CS) and its surgical treatment on health-related quality of life (HRQoL) in early childhood remains insufficiently understood. To evaluate parent-reported HRQoL at 3 years of age in children treated for SS or MS. At the 3-year follow-up, parents of children treated for SS or MS completed the Pediatric Quality of Life Inventory (PedsQL) version 2 to 4 years. Age- and nationality-matched healthy

A Meta Analysis of Randomised Controlled Trials on Psychological Interventions for PTSD and Depression in Adolescent Victims of Sexual Abuse.

Adolescent sexual abuse (ASA) is a complex, multifactorial, less researched, and challenging issue leading to an enormous immediate and long-term adverse multidimensional impacts. And as these impacts could be unique and different than child sexual abuse (CSA), the treatment plans require special attention. While 2/3rd of the global sexual abuse literature is focused on CSA, the remaining studies focus on miscellaneous issues pertaining sexual abuse. Especially, there is a dearth of literature on ASA focused interventions to guide and provide directions to clinicians for treatment plans. This

Characterizing the within-person variability of food insecurity in everyday life.

Food insecurity (FI), the lack of access to adequate food, is linked with negative health and psychological outcomes. FI is typically measured retrospectively over the last year; although this measurement is useful to understand FI prevalence to inform broad policy, it leaves the experience of FI in everyday life poorly understood. Understanding how FI varies across shorter periods of time (days or weeks) can help inform FI prevention and/or intervention. This study characterizes within-person (day-to-day) variance in FI in everyday life. Low-income parents of school-aged children at risk for
