[Reconsideration of Secondary Risk Management Strategies in Patients with Ischemic Heart Disease].

The main risk factors in ischemic heart diseases, including myocardial infarction, are hypertension, dyslipidemia, diabetes, obesity and smoking. The incidence of ischemic heart disease in Japan has been lower than that in Western countries because of differences in lifestyle and the anatomy of the coronary arteries, but the situation has been changing recently because of the westernization of lifestyle. Cardiovascular diseases have become the second most common cause of death in Japan, and 40% of those deaths are attributed to ischemic heart disease. Patients with a history of myocardial infa

Acupuncture-Associated Vasovagal Response: Revised Terminology and Hospital Experience.

Background: Vasovagal responses associated with acupuncture therapy are relatively uncommon adverse events, occurring in ∼0.02%-7% of treatments. The complex neurocardiovascular reflexes involved in vasovagal responses can induce a range of symptoms such as dizziness, nausea, sweating, bradycardia, hypotension, and, in some cases, syncope or convulsions. Although patients typically recover quickly with proper support, these events may be of concern and anxiety-producing for both patient and provider. Providers need to be well-versed in methods for prevention and treatment of acupuncture-associ

Developing surveillance for HIV transmission and risk behaviours among high-risk groups in a central London health district.

BACKGROUND: Our aim was to describe surveillance data on HIV transmission and risk behaviours in Camden and Islington, an inner London health district (population 360 000). This information is required to assess the effectiveness of HIV-AIDS prevention. METHODS: We focused on two groups in the local population most severely affected by the HIV epidemic: homosexual and bisexual men, and injecting drug users. Data were drawn from routine and survey data, national and local sources, and a special local survey. RESULTS: There were estimated to be 9250 homosexual men, with an estimated prevalence

Research advances in cuproptosis during cerebral ischemia-reperfusion injury and the therapeutic potential of traditional Chinese medicine.

Cerebral ischemia-reperfusion injury (CIRI) involves oxidative stress, mitochondrial dysfunction, and various cell death mechanisms. Recently, the concept of cuproptosis has revealed that an imbalance in physiological copper (Cu) homeostasis may serve as a contributing factor to neuronal damage in CIRI. This process is characterized by Cu accumulation, proteotoxic stress, oxidative stress, and inflammatory responses. Notably, the observation that Cu chelators can attenuate this damage suggests that cuproptosis is implicated in the pathological progression of CIRI. Traditional Chinese medicine

Intrinsic and niche-dependent metabolic regulation of haematopoietic stem cells and implications for leukaemogenesis.

Haematopoietic stem cells (HSCs) rely on precisely coordinated metabolic programs to preserve their functionality, adapt to environmental cues, and sustain lifelong haematopoiesis. Here we analyse recent advances in understanding the metabolic landscape of HSCs, emphasizing how their intrinsic bioenergetic programs facilitate quiescence, self-renewal and differentiation. We also summarize the dynamic metabolic interactions with the bone marrow microenvironment, including stromal cells, osteoblasts, endosteal cells and adipose tissue, highlighting how they support proper HSC fate. In addition,

Fgk3 interacts with FgMetR to regulate mycelial growth, conidia development, DON production, and pathogenicity in Fusarium graminearum.

Glycogen synthase kinase-3 (Gsk3) is a potential drug target in human medicine, controlling many substrates in cellular processes. In this study, FgMetR was identified as a putative binding partner of Fgk3, the Fusarium graminearum ortholog of Gsk3, although the specific functions of both proteins remain uncharacterized. Notably, the physical interaction between FgMetR and Fgk3 was confirmed via yeast two-hybrid (Y2H) assays, and both proteins were shown to co-localize within the nucleus. FgMetR was vital for mycelial growth, conidia development, deoxynivalenol (DON) production, oxidative stre

Second primary cancers following hematologic malignancies: Epidemiology, pathobiology and clinical management.

Second primary cancers (SPCs) pose an increasingly significant clinical challenge for survivors of hematologic malignancies, attributed to improved therapeutic outcomes and prolonged survival. The development of SPCs is influenced by a complex interplay of treatment-related factors, genetic susceptibility, immune dysregulation, and microenvironmental remodeling. This narrative review summarizes the epidemiological patterns of SPCs in survivors of hematologic malignancies and discusses the underlying biological mechanisms, including therapy-induced genomic instability, clonal hematopoiesis, inf

Bioactive Compounds of Momordica charantia L. Downregulate the Protein Expression of ACE2 and TMPRSS2 In Vivo and In Vitro.

The emergence of SARS-CoV-2, the etiological agent of COVID-19, has resulted in widespread global infection and millions of deaths. Viral entry is initiated by the interaction between the viral spike (S) protein and the host cell receptor ACE2, followed by TMPRSS2-mediated proteolytic activation that facilitates membrane fusion. Bitter melon (Momordica charantia L., MC), a traditional medicinal and edible plant widely used in tropical Asia, possesses notable anti-inflammatory, antioxidant, antitumor, and hypoglycemic properties. In this study, the ethanol extract of bitter melon (EMC) markedly

Bioisostere-Driven Discovery of SePP: A Selenium-Containing Polypharmacological Agent Relevant to Fragile X Syndrome.

Diphenidine is a prototypical 1,2-diarylethylamine that functions as an uncompetitive N-methyl-d-aspartate receptor (NMDAR) antagonist and monoamine reuptake inhibitor. To examine the effects of phenyl-ring bioisosteric replacement within this scaffold, a series of diphenidine analogs incorporating chalcogen heterocycles (2-furan, 2-thiophene, 3-thiophene, and 2-selenophene) was synthesized. Compounds were evaluated for in vitro binding to rat forebrain NMDARs and inhibition of human DAT, NET, and SERT in cell-based assays, enabling assessment of polypharmacology. Rational use and regulatory stewardship for paediatric cough syrups in India: A public health imperative.

Recurrent paediatric deaths from diethylene glycol (DEG)-contaminated cough syrups in India highlight critical failures in regulatory stewardship and rational medicine use. Despite global alerts and domestic advisories, children continue to be exposed to hazardous, non-essential multiple-drug combinations. This commentary analyses the intersection of clinical irrationality and regulatory gaps, arguing that persistent reliance on over-the-counter (OTC) cough syrups for self-limiting viral illnesses in children under five is ethically indefensible. We review recent incidents (2022-2025) and poli

Immunology of pregnancy and sepsis: shared and specific pathways guiding future precision care.

Maternal sepsis remains a leading cause of preventable death globally, yet its underlying biology within the context of pregnancy is poorly understood. This review examines the extent to which pregnancy and sepsis share physiological, immunological, and regulatory pathways. We propose a unifying model for maternal sepsis that integrates immune, transcriptomic, epigenetic, and metabolic frameworks to advance a precision medicine approach, restoring immune equilibrium, controlling pathological inflammation while preserving pregnancy-specific adaptations. We address key knowledge gaps in current

Molecular mechanisms and translational prospects in osteoporosis.

Osteoporosis is the most prevalent chronic metabolic bone disease, affecting approximately 200 million individuals worldwide. With the rapid acceleration of global aging, osteoporosis has emerged as a major global health challenge, significantly increasing the occurrence rate of fractures and the associated medical burden. In recent years, considerable progress has been made in understanding the molecular mechanisms underlying osteoporosis, including bone remodeling, osteoblast-osteoclast communication, and signaling pathways such as RANKL/RANK/OPG and Wnt/β-catenin. These mechanistic studies

[Expert consensus on joint diagnosis and treatment of multimorbidity of upper airway allergic diseases in children].

Objective:Upper airway allergic diseases in children refer to chronic non-infectious inflammatory diseases of the upper airway caused by allergic inflammation. These diseases have high prevalences and great harm, furthermore often co-occur. Focusing only on the treatment of individual diseases may lead to ineffective control. To more comprehensively and effectively relieve symptoms and signs, and prevent recurrence or prolongation, pediatric upper airway allergic diseases must be diagnosed and treated jointly as an entity. Although there are currently several guidelines and consensuses on diag

Tetracyclines, a timely resurgence or doomed to fail again?

Tetracyclines are bacteriostatic antimicrobials used in the treatment of both bacterial and protozoan pathogens. They abolish protein synthesis by binding to the 16S rRNA, blocking aminoacyl-tRNA access and preventing polypeptide elongation. Tetracyclines are polyketide antimicrobials, with members of this drug family (doxycycline, minocycline and tigecycline) on the World Health Organization (WHO) list of essential medicines. Bacterial resistance to this antimicrobial family is widespread and arises via multiple mechanisms. The most common of these resistance mechanisms are tetracycline efflu
