Patients' experiences of everyday life after lung transplantation.

AIM: To investigate the experiences of everyday life after lung transplantation of patients with previous chronic obstructive pulmonary disease (COPD). BACKGROUND: Compared with patients being transplanted due to other indications, those with COPD prior to lung transplantation report more problems in the form of shortness of breath, fatigue, sexual problems, insomnia and increased appetite. In addition, they are often faced with problems returning to normal working life. How these problems influence the patient's everyday life is unknown. DESIGN: An exploratory qualitative study. METHODS: T

Holding Complex Spaces Through Relational Work: A Qualitative Descriptive Study of Co-Design Facilitation.

INTRODUCTION: Involving patients and the public in healthcare research, development and innovation is becoming mandatory. Co-design is a structured approach to collaboratively investigating and finding solutions to complex problems that brings together relevant actors and interest groups. Including experiences and insights from patients and the public in multi-actor co-design processes can help to generate and evaluate solutions to complex problems. While toolkits support co-design project organisation, little is known about the relational work required when facilitating co-design workshops, e

Participation in eHealth Communication Interventions Among Patients Undergoing Hemodialysis: Scoping Review.

BACKGROUND: eHealth communication interventions have been shown to offer individuals with chronic kidney disease the opportunity to embrace dialysis therapies with greater confidence, the potential to obtain better clinical outcomes, and an increased quality of life. eHealth is an emerging field that offers diverse, flexible designs and delivery options. However, existing evidence on eHealth communication among patients undergoing hemodialysis is sparse and scattered and lacks systematization. OBJECTIVE: This scoping review aims to identify and map the current evidence on patient participatio

"More work means less pay": a qualitative understanding of physicians' patient selection decision-making under DRG reform in Nanjing, China.

BACKGROUND: Patient selection, also known as 'cream-skimming', can lead to the systematic exclusion of high-cost, clinically complex patients from necessary care. It has been identified as an opportunistic response under Diagnosis-Related Groups (DRGs) payment systems globally. As China advances the rollout of DRG reform alongside commitments to promoting healthcare equity, understanding the ecology of physicians' cream-skimming behaviour and how to mitigate it is imperative. METHODS: We purposively sampled 21 physicians from two hospitals with different incentive structures in 2024 and condu

Ligneous periodontitis exacerbated by Behçet's disease in a patient with plasminogen deficiency and a stop-gained variant PLG c.1468C > T: a case report.

BACKGROUND: Plasminogen serves as the precursor to plasmin, an essential element in the fibrinolytic process, and is synthesized primarily in the liver. Plasminogen activation occurs through the action of plasminogen activator, converting it into plasmin. This conversion greatly enhances the fibrinolytic system within tissues and blood vessels, facilitating the dissolution of fibrin clots. Consequently, congenital deficiency of plasminogen results in impaired fibrin degradation. Patients with plasminogen deficiency typically exhibit fibrin deposits in various mucosal sites throughout the body,

Influenza vaccination coverage and factors associated with severe laboratory-confirmed influenza-related illness in patients receiving care at a tertiary hospital in Catalonia (Spain) during the 2018-2019 epidemic season.

INTRODUCTION: Influenza vaccination rates in risk groups remain suboptimal. Evidence supporting a significant association between influenza vaccination and severe illness is limited. METHODS: We retrospectively analyzed the epidemiological characteristics of out- and inpatients with laboratory-confirmed influenza infection attended during the 2018-19 epidemic season. Influenza vaccination coverage by indication was analyzed. Logistic regression was used to compare the odds of vaccination between severe and non-severe influenza-positive patients. Severe cases were defined as presenting pneumon

Integrating Neurology, Palliative Care and Emergency Services in ALS: A Community-Integrated Neuropalliative Pathway in Modena, Italy.

BACKGROUND: Amyotrophic lateral sclerosis (ALS) is a progressive neurodegenerative disease that causes severe motor, respiratory and communication impairment and imposes a high psychosocial burden on patients and families. Recent evidence shows that integrated neuropalliative care-early collaboration between neurology and palliative services with community support-improves quality of life and reduces avoidable hospitalisations. Yet there are few descriptions of how such integration is operationalised. OBJECTIVE: This study examines a Community-Integrated Neuropalliative Pathway (CINP) impleme

Direct and mediated effects of treatment context on low back pain outcome: a prospective cohort study.

OBJECTIVES: Contextual components of treatment previously associated with patient outcomes include the environment, therapeutic relationship and expectancies. Questions remain about which components are most important, how they influence outcomes and comparative effects across treatment approaches. We aimed to identify significant and strong contextual predictors of patient outcomes, test for psychological mediators and compare effects across three treatment approaches. DESIGN: Prospective cohort study with patient-reported and practitioner-reported questionnaire data (online or paper) collec

"Life still has meaning for me!" Exploring sources of meaningful life among palliative care patients: a qualitative study.

PURPOSE: Palliative care has increasingly shifted from a biomedical model toward holistic approaches that address physical, psychological, social, and spiritual needs. Meaning in life is a key determinant of quality of life in advanced illness, yet patient perspectives on how meaning is constructed within inpatient settings remain underexplored. This study aimed to explore how individuals receiving inpatient palliative care at a tertiary hospital in Turkey construct and sustain meaning in their daily lives. METHODS: This qualitative study used in-depth, semi-structured interviews. Nine patien

Western Dietary Pattern, Prudent Dietary Pattern, and Cancer-Specific Quality of Life in Prostate Cancer Survivors in the Health Professionals Follow-Up Study.

BACKGROUND: Prostate cancer survivors often experience reduced health-related quality of life (QOL). Diet is related to QOL in the general population and prostate cancer survivors, with benefits observed from greater consumption of a plant-based diet after treatment. We examined whether postdiagnostic Western and prudent dietary patterns were associated with cancer-specific QOL. METHODS: We studied 1,032 participants in the Health Professionals Follow-Up Study diagnosed with nonmetastatic prostate cancer (2005-2014). Diet scores were cumulatively averaged from validated food frequency questio

Patient experiences of a 10-weeks weight-neutral treatment program for clinical binge eating disorder in a higher weight population.

BACKGROUND: Weight based stigma might drive the development of both higher weight and binge eating disorder (BED). To improve treatment and outcomes, a deeper understanding of how stigma and shame are correlated in clinical encounters is needed. The current study was designed to gain insight into how participating in a 10-weeks weight-neutral treatment program for patients with binge eating disorder and higher weight was experienced. METHODS: Semi-structured interviews were conducted with 10 patients who had completed the BED treatment. The intervention was group based, addressing stigma and

Which clinical conditions are responsible for impaired alertness?

The complaint of excessive daytime sleepiness, commonly encountered in clinical practice, may arise from many medical and psychiatric disorders and behavioural lifestyle changes. The list of possibilities spans virtually every major area of medicine, neurology and psychiatry. Assessment of impaired alertness is largely based on clinical interview and direct observation, but it may also include subjective self-report, objective physiological and behavioural measures, and performance tests. In the primary care setting, a clear and detailed history is invaluable in negotiating these numerous diag
