A meta-analysis of treatments for panic disorder.

In a meta-analysis, the authors compared the effectiveness of psychological and pharmacological treatments for panic disorder. Percentage of agoraphobic subjects in the sample and duration of the illness were unrelated to effect size (ES). Type of dependent variable was generally unrelated to treatment outcome, although behavioral measures yielded significantly smaller ESs. Dependent measures of general anxiety, avoidance, and panic attacks yielded larger ESs than did depression measures. Choice of control was related to ES, with comparisons with placebo controls greater than comparisons with

Therapeutic imagery enhanced by hypnosis.

This paper has reviewed the history of using imagery as a powerful change and healing agent in humans. It has been a rather underused technique in the practice of Western medicine and psychiatry. I hope that the specific examples and techniques described herein will stimulate and motivate the reader to adopt them in their clinical practice and creatively develop new strategies and techniques applicable in other fields such as sports, the arts, education, and the human life cycle.

[Therapeutic approach to a case of simulated disorder with psychiatric symptoms].

The case of a 38-year-old female inpatient with the diagnosis of factitious disorder with psychological symptoms--a presentation rarely reported in the psychiatric literature--is presented with special respect to her personality traits. On the basis of her biographical history a therapeutic approach was possible which tried to understand the symbolic value of the histrionic patient's symptoms. We regarded the manifestation of psychological symptoms as an attempt to express conflicts verbally and as an offer of a more mature form of communication in comparison to the former, autodestructive phy

Nonpharmacologic treatments of bulimia nervosa.

The principal psychosocial approach to the treatment of bulimia nervosa has been cognitive-behavioral therapy, the effectiveness of which has been extensively documented in controlled trials, with full recovery (cessation of binge eating and purging) in some 50% to 60% of patients. More recently, interpersonal therapy, first introduced for the treatment of depression, has been shown in preliminary studies to be as effective as cognitive-behavioral therapy, particularly at follow-up. The evidence for effectiveness of psychosocial therapies in bulimia nervosa is first reviewed, and consideration

Breaking the cycle of violence: an examination of factors relevant to treatment follow-through.

To examine differences between a group of battered mothers who followed through with treatment recommended for their children and those who did not, 10 women from each of two comparable women's shelters who followed through with recommended therapy for their children were compared with 10 women from each shelter who did not follow through, in regard to age, years of education, job history, economic status, and prior exposure to therapy. Analysis indicated that women who followed through had significantly more years of education than those women who did not follow-through. Furthermore, women wh

Hypnosis for postpolio syndrome & Type-A behavior.

Many of the hundreds of thousands of survivors of polio are now developing postpolio syndrome. Symptoms include progressive muscle weakness, fatigue, decreased endurance, joint and muscle pain, weight gain, respiratory difficulties, and sleep disturbance, often precipitated or exacerbated by a Type-A Personality pattern. A postpolio patient with Type-A Personality was taught self-hypnosis as a vital component of treatment. Pre-post testing included the Profile of Mood States, the State-Trait Anxiety Inventory, the State-Trait Anger Inventory, and the Personal Orientation Inventory; the patient

Bulimia nervosa: group behavior therapy with exposure plus response prevention.

Exposure plus response prevention has been demonstrated to be effective in the treatment of bulimia nervosa. However, when done individually, it is labor intensive and cost-ineffective. In the present study exposure plus response prevention was used in the context of a 6-wk., 12-session behavioral group. In addition to the exposure plus response-prevention component, other techniques included self-monitoring, cognitive restructuring, eating-habit stabilization and problem-solving. Eight bulimic women, vomiting a minimum of five times per week for at least a year, participated in the group. At

Psychiatric aspects of lung transplant. The Toronto Lung Transplant Group.

Psychiatric problems in a consecutive series of lung transplant applicants (n = 116), candidates (n = 57) and recipients (n = 42) are reported. Fifty percent of all the applicants for lung transplantation reported a history of psychiatric disorder including organic brain syndrome (19%), major depressive episode (16%), panic or anxiety disorder not otherwise specified (11%) and alcohol or substance abuse (11%). In most cases, psychiatric problems had occurred in these patients as a complication of endstage lung disease. The most common new diagnoses in candidates awaiting surgery were adjustmen

[Catamnestic results of 64 patients--5 years following inpatient psychosomatic treatment].

The present study is an attempt at evaluating with clinocosomatic, psychodiagnostic, and sociologic instruments the alterations achieved by (psychodynamically oriented) psychosomatic in-patient therapy. Out of a total of 64 patients who in 1975 had undergone treatment for an average period of two months 46 (i. e. 76.7 per cent) were available for re-examination after a five-year interval. In two thirds of the patients the various complaints and the general autonomic instability had receded. The psychical and social situation had improved for between two thirds and three fourths of all re-exami

[Therapeutic and corrective work with stammering preschool children in a daytime semi-hospital].

Therapeutic and corrective work with stammering preschool children conducted at a day-time semi-hospital institution presents a complex of measures aimed at improving the speech and the entire psychic activity of the child. A semi-hospital form of the organization makes it possible, along with therapeutic and corrective measures, to constantly train the speech in the most diverse situations of the microsocial environment outside the hospital and thus contributes to the development of social and adaptive behaviour which the child needs for further studying in the general education school.

The unstable bladder.

The unstable bladder is a common clinical problem of uncertain aetiology. Current in vitro studies of unstable human detrusor samples show differences in behaviour and response to pharmacological agents from normal detrusor. The fundamental urodynamic abnormality is the occurrence of involuntary detrusor contractions which cause the cardinal symptom of urgency. The incidence of other symptoms is determined by the functional bladder capacity and the patient's ability to resist the unstable contractions. Having excluded outflow obstruction as a causative or associated factor, empirical treatment

Overview: the "wife-beater's wife" reconsidered.

Although not a new phenomenon, wife-beating has been largely ignored by mental health professionals. The author describes a context in which the physical abuse of women as wives can be understood and suggests alternative theoretical constructs to traditional theories of masochism as explanations of why women stay in violent marital relationships. She discusses the societal attitudes that normalize the use of family violence as well as attitudes about women, men, and sex roles that leave women vulnerable to assault; summarizes the research on abused wives; identifies a specific stress response

Locus of control and behavior in treatment groups.

Studied the Rotter Locus of Control scale for effective prediction of behavior in small groups. Subjects were 205 male VA Hospital patients participating in self-directed groups who completed the AGCT, Psychological and Somatic Symptom Scales, and Rotter's I-E measure of locus of control. Group behavior was measured through sociometry and a Group Member Evaluation form. Three hypotheses were not supported: (a) Persons scoring high in the direction of internal control (internals) will be more likely to act to influence their social environment; (b) internals will be more active than externals i

Behavioral treatment of night terrors in a child with acute leukemia.

A 3-year-old girl diagnosed with acute lymphocytic leukemia presented with a 1-month history of recurrent nightmares. Symptoms of the disturbance conformed to a clinical picture of slow wave arousal night terrors, or pavor nocturnus. Behavioral treatment aimed at reducing anxiety related to maternal separation and medical procedures, and at reinforcing appropriate sleep patterns was effective in reducing and eventually eliminating the symptoms. Follow-up revealed no return of nightmares or existence of new problems. A brief review of descriptive, etiological and treatment aspects of night terr
