Biparietal osteodystrophy in an 86-year-old woman: An incidental finding following minor head trauma.

Biparietal osteodystrophy (bilateral parietal bone thinning) is a rare but well-documented phenomenon characterized by symmetric thinning of the parietal bones of the skull. It predominantly affects older adults, particularly women. We report the case of an 86-year-old woman who was incidentally diagnosed with this condition after a minor head injury. She presented with a low-impact fall and underwent neuroimaging to exclude intracranial hemorrhage. Computed tomography (CT) of the head revealed bilateral concave thinning of the parietal bones with an intact inner table, and no acute fracture o…

Jael's syndrome: Removal of a retained intracranial kitchen knife blade - A case report.

BACKGROUND: Jael's syndrome is defined as an intentional injury caused by a knife in the skull or the face. It is a rare yet challenging situation in clinical practice. Initial triage is the key to optimal management. CASE DESCRIPTION: We describe the case of a right-handed 30-year-old man presenting to the emergency for headaches 15 days after a stabbing attack into the skull using a kitchen knife. He was conscious with no neurological deficit or history of seizures. The wound had already healed. A cerebral computed tomography (CT) scan showed a retained kitchen knife blade. The stab wound e…

Two case reports of fatal injuries sustained from tear gas guns loaded with pebbles as missiles.

These two case reports are of fatal injuries from less-lethal weapons (tear gas guns) using pebbles as an unconventional ammunition. Reported here are the fatal outcomes of two 19-year-old males, one with head trauma and the other with abdominal trauma. They were admitted to the ED and subjected to immediate exploratory operations, but they passed away. On autopsy, the first case demonstrated a right frontal bone fracture with subdural hemorrhage and a 3 × 2.5 cm pebble retained in the skull. The second case showed severe liver laceration in close vicinity to a cubical pebble (3.2 × 3 cm). The…

Craniofacial dysmorphology in Down syndrome is caused by increased dosage of Dyrk1a and at least three other genes.

Down syndrome (DS), trisomy of human chromosome 21 (Hsa21), occurs in 1 in 800 live births and is the most common human aneuploidy. DS resul … one of these causative genes as Dyrk1a. We show that the earliest and most severe defects in Dp1Tyb skulls are in bones of neural crest (NC) origin, and that mineralisation of the Dp1Tyb skull base synchondroses is aberrant. Furthermore, we show that increased dosage of Dyrk1a results in decreased NC cell proliferation and a decrease in size and cellula …

Ontogenetic endocranial shape change in alligators and ostriches and implications for the development of the non-avian dinosaur endocranium.

Birds and crocodiles show radically different patterns of brain development, and it is of interest to compare these to determine the pattern of brain growth expected in dinosaurs. Here we provide atlases of 3D brain (endocast) reconstructions for Alligator mississippiensis (alligator) and Struthio camelus (ostrich) through ontogeny, prepared as digital restorations from CT scans of stained head and dry skull specimens. Our morphometric analysis confirms that ostrich brains do not change significantly in shape during postnatal growth, whereas alligator brains unfold from a cramped bird-like sha…

Extended middle fossa approach with anterior petrosectomy for resection of upper petroclival meningioma involving Meckel's cave: operative video and technical nuances.

The surgical management of petroclival meningiomas remains a formidable challenge. These tumors are deep in the base of the skull and arise medial to the fifth cranial nerve. In this operative video, the author demonstrates the extended middle fossa approach with anterior petrosectomy to resect an upper petroclival meningioma extending into Meckel's cave with brainstem compression. This approach is very useful for accessing deep tumors located above and below the tentorium, and between the fifth and seventh cranial nerves. Access to Meckel's cave is readily achieved by opening the fibrous ring…

Imaging of the craniovertebral junction anomalies in children.

The craniovertebral junction (CVJ) is interposed between the unsegmented skull and the segmented spine; it is functionally unique as it allows the complex motion of the head. Because of its unique anatomy, numerous craniometric indices have been devised. Because of its complex embryology, different from that of the adjacent skull and spine, it is commonly the seat of malformations. Because of the mobility of the head, and its relative weight, the craniovertebral junction is vulnerable to trauma. Like the rest of the axial skeleton, it may be affected by many varieties of dysplasia. In addition…

Anatomical and clinical considerations regarding the greater palatine foramen.

OBJECTIVE: In this study, we mapped the topography of the greater palatine foramen (GPF), within the South-Eastern European population, according to clinically identifiable anatomical landmarks. MATERIALS AND METHODS: We used 100 dry adult human skulls, of which 75 were bilaterally dentate, whereas 25 were partially edentulous. RESULTS: The shape of the GPF was oval in 84% of the cases. The foramen was located internally from the third molar in 73% of the skulls. The greater palatine canal showed an antero-infero-medial direction in 82% of the cases. The average values of the distances betwe…

The association between dental status and temporomandibular osseous changes: a morphological study on Roman-Byzantine skeletons.

The aim of this study was to evaluate the association between dental status and the prevalence and severity of osseous changes in the temporomandibular joints of human skulls from the Roman-Byzantine period. Fifty-eight skulls from 36 men and 22 women between the ages of 19 and 63 years were studied, and the following parameters were evaluated: morphological osseous changes in the articular surface of the condyles, tooth wear and molar support. A significant correlation between age and dental wear or loss of molar support was observed, although no correlation was noted between age and morpholo…

Weapon identification using antemortem computed tomography with virtual 3D and rapid prototype modeling--a report in a case of blunt force head injury.

A frequent request of a prosecutor referring to forensic autopsy is to determine the mechanism of an injury and to identify the weapons used … lyze antemortem clinical data, especially CT scans, with virtual 3D reconstruction of the fractured skull. A physical model of a part of the broken skull was created with the use of 3D printing. [Pierre Marie-Sainton cleidocranial dysplasia].

The paper presents a family with hereditary transmitted Marie-Sainton dysplasia affecting the father and his both children. This is a rare syndrome presenting an autosomal pattern of inheritance, characterized by a generalized defect in both membranous and endochondral bone formation resulting in clavicular aplasia, delayed ossification of the fontanelles and the sutures of the skull and prolonged retention of deciduous dentition with delayed eruption of the permanent teeth. The diagnosis is suggested by more or less complete clinical picture and confirmed by multiple radiological explorations…

Middle fossa approach to a temporosphenoidal encephalocele -technical note-.

Temporosphenoidal encephalocele (TSE) is a rare entity caused by herniation of the anteromedial temporal lobe into the sphenoid sinus (SS) through a middle fossa (MF) defect. A 45-year-old woman presented with a spontaneous TSE manifesting as a 4-year history of recurrent cerebrospinal fluid rhinorrhea and meningitis. Coronal computed tomography showed a skull defect in the superior wall of the right lateral recess of the SS. This homogeneous intrasphenoidal lesion appeared hypointense on T(1)-weighted magnetic resonance (MR) imaging and hyperintense on T(2)-weighted MR imaging. The patient un…

Nasopharyngeal carcinoma presenting with rapidly progressive severe binocular optic neuropathy and periocular pain in a young man.

A 27-year-old man presented with rapid and severe visual loss in both eyes, together with pain behind the eyes. Visual acuities were light perception in both eyes. Pupillary constriction to light was minimal, and ophthalmoscopy results were normal. For a presumptive diagnosis of retrobulbar optic neuritis, he was treated with intravenous corticosteroids, and vision improved transiently. But vision later worsened to no light perception, and MRI revealed bilateral optic nerve enhancement with dural enhancement and thickening in the anterior skull base, sella, and retroclival areas, findings init…

Accidental traumatic head injury in infants and young children.

This article will discuss accidental head injuries in infants and young children. The first category of injury is the crushing head injury. Static forces applied slowly to the head result in multiple fractures of the skull and contusions and lacerations of the brain resulting from the bone fragments striking the brain. This article will discuss the subject of short falls in young children and the resulting head injuries. Because falls are frequent events in early life, many cases have been collected and many papers written on the subject. Study of these cases is informative about the injuries …

Attenuation of signaling pathways stimulated by pathologically activated FGF-receptor 2 mutants prevents craniosynostosis.

Craniosynostosis, the fusion of one or more of the sutures of the skull vault before the brain completes its growth, is a common (1 in 2,500 births) craniofacial abnormality, approximately 20% of which occurrences are caused by gain-of-function mutations in FGF receptors (FGFRs). We describe a genetic and pharmacological approach for the treatment of a murine model system of Crouzon-like craniosynostosis induced by a dominant mutation in Fgfr2c. Using genetically modified mice, we demonstrate that premature fusion of sutures mediated by Crouzon-like activated Fgfr2c mutant is prevented by atte…

[Traumatic atlanto-axial rotatory fixation with lateral mass fracture of the axis].

Traumatic atlanto-axial rotatory fixation (AARF) is a relatively uncommon injury, especially in association with fractures of the axis. The rotatory dislocation and fixation is normally caused by intercalated facet joints of axis and atlas. A traumatic AARF in a 21 year old female is presented with special emphasis on the diagnostic and therapeutic approaches. This high velocity injury was caused by a traffic accident. The trauma service which was initially involved made the correct diagnosis and tried to reduce the dislocation by skull traction during analgesia and sedation, but without succe…
