Secondary lip and palate surgery.

To properly treat a patient with a secondary cleft lip or palate deformity, one must make an accurate diagnosis of the underlying problem. Recognition of specific recurring patterns and common deformities will help facilitate the surgical planning process and assist the surgeon during future procedures. Through a combination of accurate analysis, clinical patience, and technical precision, successful correction of secondary cleft lip and palate deformities can be delivered.

[Flow-through deep inferior epigastric perforator flaps transplantation for reconstruction of large defects at the extremities].

OBJECTIVE: To investigate the clinical application of flow-through deep inferior epigastric perforator flaps for reconstruction of large defects at the extremities. METHODS: The deep inferior and superior epigastric arteries were designed as the axial vessel and the arterial supply to the flap was the paraumbilical perforator artery. Free deep inferior epigastric perforator flaps were harvested in flow- through manners to reconstruct associated arterial defect in the wound. The sensation assessment,Enneking score, and questionnaire of the flap aesthetic were all performed during follow-up peri

Carpenter syndrome: a review for the craniofacial surgeon.

IMPORTANCE: As of now, there is no review of Carpenter syndrome (CS) for the craniofacial surgeon. This article seeks to unify salient recent studies to provide a resource for surgical planning and overview of this challenging syndrome. OBJECTIVES: The phenotypic characteristics of CS are diverse, and the molecular underpinnings are equally complex. To date, the surgical management of this syndrome has not been fully elucidated, with only a number of selected case studies illustrating proper approach to treatment. This article summarizes treatment approaches from selected CS literature, analy

Pedicled fibular flap for reconstruction of composite defects in foot.

INTRODUCTION: Reconstruction of complex injuries involving bone and soft-tissue in foot remains a tough challenge for surgeons. The free fibular flap is a popular flap for treating these composite defects. However, complications caused by microvascular anastomoses are not uncommon. Herein, we designed a pedicled fibular flap elevated in the ipsilateral leg for reconstruction of multiple defects in foot. METHODS: From July 2005 to April 2013, four patients with composite defects in foot were treated by pedicled fibular flaps. The defects were located in the first metatarsal bone and medial cun

Long-term outcomes following radial polydactyly reconstruction.

PURPOSE: To report long-term outcomes (> 10 y) after radial polydactyly reconstruction. METHODS: We evaluated 43 surgically reconstructed thumbs in 41 patients with radial polydactyly whom we had observed for more than 10 years. The study group included 12 Flatt type II, 8 type III, 17 type IV, and 6 type V. The average age of surgery was 1 year and mean follow-up was 17 years. Objective outcome values and validated patient-oriented outcome evaluations were obtained. RESULTS: No early postsurgical complications were encountered. Eight patients had 10 revision procedures at an average of 8 ye

[Progress of temporomandibular joint prosthesis].

The anatomically and functionally complex nature of the temporomandibularjoint (TMJ) makes its reconstruction one of the most challenging tasks faced by surgeons who operate in the head and neck. TMJ prosthesis is one of the important techniques in the reconstruction of TMJ. The main indications for TMJ prosthesis include ankylosis, fractures of condylar that can't be fixed, trauma or tumor, end-stage TMJ disturbance, and TMJ dysplasia caused by Hallermann-Streiff syndrome. TMJ replacement aims to enhance the function of TMJ, alleviate pain, and prevent serious complications. TMJ prosthesis is

Increased intraocular pressure after extensive conjunctival removal: a case report.

A 50-year-old woman, who had undergone extensive removal of conjunctiva on the right eye for cosmetic purposes at a local clinic 8 months prior to presentation, was referred for uncontrolled intraocular pressure (IOP) elevation (up to 38 mmHg) despite maximal medical treatment. The superior and inferior conjunctival and episcleral vessels were severely engorged and the nasal and temporal bulbar conjunctival areas were covered with an avascular epithelium. Gonioscopic examination revealed an open angle with Schlemm's canal filled with blood to 360 degrees in the right eye. Brain and orbital mag

Carpentering in septorhinoplasty: a novel technique to straighten the deviated nasal septum.

Septal deviation presents a considerable challenge in septorhinoplasty because it leads to functional and esthetic problems. This deformity may displace the nasal tip, disturb nasal valve patency, and affect the final outcomes of septorhinoplasty. This report describes an innovative technique that can be used in patients with C-shaped deviation of the septum in a cephalocaudal direction. Because the procedure is similar to carpentering, it was named a "carpenter spreader graft." In this technique, the deviated dorsal cartilage is separated and displaced to a new straightened position.

Gustav Simon's band and the evolution of labial adhesion.

"Simonart's band" is an eponymic misnomer. In the mid-19th century, Pierre-Joseph Simonart described congenital bands in the limb, not in the lip. Shortly thereafter, Gustav Simon reported preliminary closure of bilateral cleft lip and thus deserves credit for labial adhesion (Simon's band). Labial adhesion was rediscovered 100 years later; however, with increasing use of infant-dentofacial orthopedics, its popularity has declined in many centers. The authors continue to use this procedure before formal repair of unilateral complete cleft lip/palate, unilateral complete cleft lip and alveolus,

Simultaneous correction of deviated columella and wide nostril floor using the Y-V advancement in unilateral cleft lip nasal deformities.

Here, we describe an operative method for the simultaneous correction of deviated columella and wide nostril floor in unilateral cleft lip nasal deformities. The V-shaped flap of the affected nostril floor was inserted into the side wall of the columella using the Y-V advancement. By suturing the anterocaudal septum, which is often deviated to the unaffected side, with the subcutaneous tissue of the affected alar base, it was possible to simultaneously reposition the columellar base in the midline as well as narrow the wide nostril floor. This method was technically simple, with low invasivene

The flexor digitorum longus muscle flap for the reconstruction of soft-tissue defects in the distal third of the leg: anatomic considerations and clinical applications.

Soft-tissue defects of the distal third of the leg are common occurrences in trauma centers. The authors present the anatomic basis and their clinical experience using the flexor digitorum longus muscle flap. Ten cadaver specimens were dissected to evaluate the potential use of this flap for soft-tissue coverage in the lower third of the leg. Eleven patients underwent a soft-tissue reconstruction in the distal third of the leg with this flap. The average size of the defect covered by this flap was 11.3 cm. This transfer flap was used in 7 cases exclusively or in combination in 4 cases. No func

Functional reconstruction of bilateral injuries to the dorsum of hand using radial forearm tendocutaneous flaps.

We treated a patient with skin and tendon defects of both hands as a result of injury by a heat press. There have been no reports of bilateral hand injuries being treated using simultaneous bilateral tendocutaneous flaps. In this case, we reconstructed the injured tissue using simultaneous bilateral radial forearm tendocutaneous flaps, with satisfactory results.

[Adults with congenital heart disease: treatment and medical problems].

By improvement of the medical care, children with congenital heart disease survive in much greater numbers: the figure of ACHD-patients in Germany is estimated up to 203,000 to 301,000. The need for a specialized care of these patients is accepted by the scientific societies, who introduced a certification for doctors and centers and created guidelines. The medical problems include imaging, treatment of arrhythmia, congestive heart failure and cyanosis, prophylaxis of infectious endocarditis, exercise and pregnancy. For the most frequent defects, indication for treatment and postoperative care
