Pyknodysostosis: report of a rare case with review of literature.

Pyknodysostosis is a rare autosomal recessive disorder characterized by the post natal onset of short limbs, short stature, and generalized hyperostosis along with acro-osteolysis with sclerosis of the terminal phalanges, a feature that is considered essentially pathognomonic. Other features include persistence of fontanelles, delayed closure of sutures, wormian bones, absence of frontal sinuses, and obtuse mandibular gonial angle with relative mandibular prognathism. We report a case of 17-year-old girl who presented with a chief complaint of retention of deciduous teeth. General physical exa

Fibrin glue administration to support bronchial stump line.

Bronchopleural fistula is an important cause of mortality and morbidity after pulmonary resection. The use of fibrin glue to reduce the tension and number of sutures in the bronchial stump was assessed in this prospective study of 20 patients between June 2002 and May 2003. They all had a high risk of bronchopleural fistula development because of bronchiectasis, tuberculosis, lung abscess, diabetes mellitus, preoperative neoadjuvant radiotherapy, or residual tumor at the surgical margin. After pulmonary resection, the bronchial stump was closed with separate nonabsorbable sutures supported wit

Management of chordee in children and young adults.

Penile curvature is a spectrum of disease affecting boys with and without hypospadias. The etiology of chordee includes skin tethering, fibrotic bucks or dartos fascia, corporeal body disproportion and rarely a fibrotic urethra. Several surgical techniques (plication, excision, and graft insertion) are currently employed to repair penile curvature. Recent neuroanatomical studies of the developing fetal penis have shown that the dorsal nerve branches from the 11 and 1 o'clock positions to the 5 and 7 o'clock positions, being absent in the midline. Since the neuroanatomy is similar in both the h

Cervical hydatidiform mole causing severe vaginal hemorrhage: a case report.

BACKGROUND: First-trimester vaginal bleeding occurs in approximately 20% of pregnancies and is a frequent cause of emergency department evaluation. Hydatidiform mole, although relatively rare, typically presents as intermittent vaginal bleeding during the first trimester of pregnancy. CASE: A 37-year-old woman had severe vaginal bleeding secondary to cervical implantation of a hydatidiform mole. Treatment included stay sutures, similar to those used in cervical cerclage, to control the hemorrhage and allow stabilization of the patient. Evaluation showed that the hydatidiform mole appeared to

An in utero model of craniosynostosis.

To define better the pathophysiology of craniosynostosis, we developed an in utero model in rabbits. Premature fusion of the sutures was achieved by osteoinduction using demineralized bone matrix (DBM). Thirty-six fetuses from 18 time-dated pregnant rabbits underwent coronal strip craniectomies and implantation of DBM at 25 days of gestation (term = 31 days). Seventeen (47%) survived vaginal delivery and 10 (28%) survived for long-term follow-up. Surviving, operated animals demonstrated bony fusion across the coronal sutures presumably due to osteoinduction by the DBM. Coronal computed tomogra

Morphology of regressive changes in the kidney following experimental ischaemia.

Total or partial renal ischaemia was studied in rats and pigs in a series of experiments, after total devascularization (ligature of renal vessels and ureter); ligature of isolated renal artery; experimental embolization of renal artery by non-resorbable material; devascularization of a specific renal segment by deep, strangulating sutures. Morphological studies showed that the resulting necrotization is represented most frequently by coagulation necrosis, less frequently by colliquating necrosis. Occlusion of the renal artery by embolization or ligature leads only to partial necrotization or

Utility of nonpenetrating titanium clips for dural closure during spinal surgery to prevent postoperative cerebrospinal fluid leakage.

OBJECT: The nonpenetrating titanium clip has been successfully used in peripheral arterial bypass surgery. The purpose of this study was to evaluate the leakage pressures and patterns of nonpenetrating titanium clips using a simple model that mimicked spinal surgery. In addition, the authors describe their surgical experience with these clips and the follow-up results in 31 consecutive patients. METHODS: The authors compared nonpenetrating titanium clips and expanded polytetrafluoroethylene (ePTFE) sutures in relation to the water pressure that could be tolerated by sutured ePTFE sheets, and

Asterion region synostosis.

Posterior plagiocephaly historically has been associated with synostosis of the lambdoid suture. The incidence, diagnosis, and modes of treatment for stenosis of the lambdoid suture remain controversial. Commonly, the lambdoid suture is found to be open both on radiographic examination and at the time of surgery. The study reports on nine patients who presented with unilateral posterior plagiocephaly and who were found to have open lambdoid sutures, but a stenosed region of the asterion. The area of involvement included the distal-most lambdoid suture, the parietomastoid, occipitomastoid, and

Parietal bone mobility in the anesthetized cat.

To quantify parietal bone motion in reference to the medial sagittal suture, a newly developed instrument was attached to the surgically exposed skull of anesthetized adult cats. The instrument differentiated between lateral and rotational parietal bone movements around the fulcrum of the suture. Bone movement was produced by external forces applied to the skull and by changes in intracranial pressure associated with induced hypercapnia, intravenous injections of norepinephrine, and controlled injections of artificial cerebrospinal fluid into the lateral cerebral ventricle. Responses varied co

A New Accessible Six-Strand Flexor Tendon Repair Technique: A Four-Group Comparative Study on a Porcine Model.

Background: Six-strand flexor tendon repairs offer superior biomechanical strength, yet commonly used techniques often require looped sutures or proprietary materials that limit global accessibility. The Elsaftawy repair technique was developed as a six-strand construct using universally available braided polyester, aiming to provide high strength without reliance on specialty sutures. Methods: Sixty porcine digital flexor tendons were randomized into four groups: Elsaftawy 6-strand, Jin Bo Tang 6-strand, FiberLoop®, and modified Kessler 4-strand. Core repairs only were performed by a single e

Endoscopic Gastric Mega-plication (MEGA) for the Primary Treatment of Obesity: "A European Multicenter Retrospective Cohort Study, 6-month Outcomes".

BACKGROUND: Obesity is a chronic and highly prevalent disease. Although bariatric surgery is the most effective treatment, access remains limited. Endoscopic bariatric therapies have emerged as minimally invasive alternatives. MEGA (Endoscopic Gastric Megaplication) is a modified version of POSE 2.0 that uses two G-LIX devices, a 33-mm G-PROX, and fewer sutures (mean: six) to create double gastric plications, with the goal of streamlining the procedure. OBJECTIVE: To evaluate the technical reproducibility, clinical efficacy, and safety of the MEGA technique at 6 months in a multicenter cohort

Syndromic craniosynostosis: current concepts in intracranial hypertension, cranial base pathology, and surgical management.

Syndromic craniosynostosis is characterized by premature fusion of multiple cranial sutures accompanied by progressive craniofacial dysostosis and neurological complications. Although reduction of intracranial volume (ICV) was historically regarded as the principal mechanism underlying elevated intracranial pressure (ICP), contemporary evidence suggests that intracranial hypertension in syndromic craniosynostosis is multifactorial. Abnormal cerebrospinal fluid circulation, venous hypertension, upper airway obstruction, and premature cranial base fusion appear to contribute substantially to dis

Innovations in Rhytidoplasty: 480 Cases Validating the Efficacy of the Inner Hemostatic Net.

BACKGROUND: Cervicofacial rejuvenation procedures often face challenges related to soft tissue repositioning, dead space management, and long-term contour stability. The internal hemostatic net (IHN) technique offers a systematic approach to deep fixation that improves surgical outcomes and reduces complications such as hematomas or tissue displacement. OBJECTIVES: The authors of this study aim to describe a reproducible, layered suture technique for cervicofacial lifting that addresses deep neck contouring and flap stabilization through a combined submental and lateral approach, while minimi

Comprehensive Management of Obstetric Rectal Buttonhole Tears: Insights From a Case Report and Literature Analysis.

BACKGROUND Obstetric rectal buttonhole tears (ORBT) are rare and underreported complications during vaginal births. They offen go undiagnosed, as they appear with intact perineum or with a low-degree perineal laceration. If missed, they can progress to a rectovaginal fistula. They are not a part of the Sultan classification and there is no consensus on their management. Various repair techniques have been proposed, but the limited evidence and experience make it challenging to standardize an approach. CASE REPORT We present a case of a 2-cm ORBT with intact anal sphincters revealed after a dig

An unusual case of small-bowel obstruction: Broad ligament hernia.

Broad ligament hernias (BLHs) are rare internal hernias that can lead to serious complications if left untreated. In this case report, we present the case of a BLH in a female patient and discuss its clinical presentation, diagnosis and management. A 40-year-old woman presented with subacute intestinal obstruction symptoms, including the inability to pass flatus and faeces and recurrent bilious vomiting. A computed tomography (CT) scan confirmed small-bowel obstruction adjacent to the uterus within the left broad ligament. Diagnostic laparoscopy revealed a loop of obstructed small intestine wi

A randomised controlled trial of intrapleural balloon intercostal chest drains to prevent drain displacement.

BACKGROUND: Chest drain displacement is a common clinical problem that occurs in 9-42% of cases and results in treatment failure or additional pleural procedures conferring unnecessary risk. A novel chest drain with an integrated intrapleural balloon may reduce the risk of displacement. METHODS: A prospective randomised controlled trial comparing the balloon drain to standard care (12 F chest drain with no balloon) with the primary outcome of objectively defined unintentional or accidental chest drain displacement. RESULTS: 267 patients were randomised (primary outcome data available in 257,

A fully stapled technique for gastrojejunal anastomosis creation in robotic Roux-en-Y gastric bypass.

PURPOSE: The purpose of this paper is to showcase a fully stapled approach to creating a gastrojejunostomy during a robot-assisted Roux-en-Y gastric bypass. METHODS: We utilize two robotic 12-mm ports, two robotic 8-mm ports, and one 8-mm assistant port. The tools used are a fenestrated bipolar forceps, vessel sealer, cadiere grasper, needle driver, and a robotic stapler. After the partial gastrectomy, the roux limb is brought up to the gastric pouch where monopolar scissors are used to create a gastrotomy and enterotomy. The gastrotomy is made just above the staple line of the gastric pouch.
