Co-assessment of OCT-A and choroidal vascularity index data in multiple sclerosis patients.

OBJECTIVE: To evaluate optical coherence tomography-angiography (OCT-A) findings and choroidal vascular index (CVI) in patients with multiple sclerosis (MS). METHODS: 113 patients, including multiple sclerosis patients with optic neuritis attack (MSON+) and no optic neuritis attack (MSON-) and healthy control group (HCG), participated in this cross-sectional study. OCT-A images of all patients were taken and CVI was calculated. RESULTS: Superior flow (SF), deep flow (DF), foveal, and parafoveal superior vascular density (sVD) were decreased in the MSON+ group compared to HCG (p < 0.05). Opti

Update in pediatric optic neuritis.

PURPOSE OF REVIEW: Pediatric optic neuritis is an uncommon disorder with significant distinctions from its adult counterpart. Recognizing the features of this disorder and the potential association with progressive demyelinating processes is important for patients' evaluation and prognosis. RECENT FINDINGS: In the last few years, studies have expanded our understanding of demographic and presenting features for optic neuritis in the pediatric population. Emerging research on biomarkers and optical coherence tomography utility offers potential prognostic value in patient management. In additio

Reduced retinal nerve fiber layer thickness and macular volume in pediatric multiple sclerosis.

Retinal atrophy is well known in adult-onset multiple sclerosis but remains unexplored in children. We aimed to determine retinal nerve fiber layer thickness and macular volume in pediatric patients, with and without optic neuritis and their relations with visual evoked potentials. We also examined macular volume changes at month 12. Retinal nerve fiber layer thickness of all quadrants and macular volume were measured in 28 relapsing remitting multiple sclerosis eyes and 30 control eyes using optical coherence tomography and were found reduced in patients compared with controls. This reduction

Monocular temporal hemianopia.

Monocular temporal hemianopia was identified in 24 patients. The field of the fellow eye was normal. Computed tomography or magnetic resonance imaging showed juxta-sellar lesions in 19 patients. Fifteen had pituitary adenomas, two had tuberculum sella meningiomas, one a craniopharyngioma, and one an astrocytoma. One patient had optic neuritis. A relative afferent pupillary defect (RAPD) was detected in most patients. Field loss was functional in two. Two had congenital optic disc dysversion with hemianopia which did not respect the vertical meridian. Monocular temporal hemianopia is attributed

Optic pathway gliomas in neurofibromatosis.

Two patients, a 5-year-old girl and a 3-year-old boy, with documented neurofibromatosis (NF-1) and visual pathway gliomas are reported. In the first patient, the tumor was a typical pilocytic astrocytoma confined to the proximal half of the right optic nerve just near the globe. The tumor was excised; after a follow-up of 7 years recurrence has not occurred. The second patient had a glioma of the left optic nerve, with involvement of the chiasm, optic tracts, probably lateral geniculate body, optic radiations and basal ganglia. Biventricular hydrocephalus, possibly due to the occlusion of Monr

[Hemangiomas of the optic nerve head].

The article presents five cases of various forms of hemangiomas of the optic nerve head (ONH) and juxtapapillary retina that were monitored and analyzed using ocular visualization methods. The obtained data allowed a conclusion that capillary hemangioma of the ONH features clearly-seen boundaries of the lesion both on ophthalmoscopy and optical coherence tomography (OCT) images; on the other hand, juxtapapillary granuloma can be characterized by blurred outlines on ophthalmoscopy and a gradual transition of the altered retina on OCT images with secondary changes seen in the macular area. Analysis of choroidal vascularity index in multiple sclerosis patients without optic neuritis attack.

BACKGROUND: To evaluate the choroidal structural changes in multiple sclerosis (MS) patients without previous optic neuritis (ON) attacks. METHODS: Forty eyes of 20 MS patients without a history of ON and 40 eyes of 20 age-matched healthy volunteers were included in this study. The choroidal thickness (CT) was measured at three points; subfoveal, 1500 µm nasal to the fovea, and 1500 µm temporal to the fovea. Choroidal area (CA), luminal area (LA), and choroidal vascularity index (CVI) were calculated using ImageJ. RESULTS: The mean subfoveal, nasal and temporal CT were decreased in MS patien

A novel OCT signature in leukemic papillopathy masquerading as autoimmune or infectious uveitis.

PURPOSE: With therapeutic advances, central nervous system (CNS) involvement in leukemia has become more common. Leukemic optic disc infiltration, often a clinical diagnosis, can present as an isolated finding in primary or relapsed CNS disease and therefore requires early recognition. Not previously well appreciated, we report here signs of intraocular inflammation accompanying leukemic optic disc infiltration, suggesting infectious or non-infectious uveitis as an alternative diagnosis. We describe a novel optical coherence tomography (OCT) sign favoring leukemic infiltration. METHODS: Retro

Energetic Glaucoma Segmentation and Classification Strategies Using Depth Optimized Machine Learning Strategies.

Glaucoma is a major threatening cause, in which it affects the optical nerve to lead to a permanent blindness to individuals. The major causes of Glaucoma are high pressure to eyes, family history, irregular sleeping habits, and so on. These kinds of causes lead to Glaucoma easily, and the effect of such disease leads to heavy damage to the internal optic nervous system and the affected person will get permanent blindness within few months. The major problem with this disease is that it is incurable; however, the affection stages can be reduced and the same level of effect as that for the long

[Glaucoma and optic nerve drusen: Limitations of optic nerve head OCT].

INTRODUCTION: Optic nerve head drusen are congenital calcium deposits located in the prelaminar section of the optic nerve head. Their association with visual field defects has been classically described, but the diagnosis of glaucoma is not easy in these cases of altered optic nerve head anatomy. CASE STUDY: We describe the case of a 67-year-old man with optic nerve head drusen complicated by glaucoma, which was confirmed by visual field and OCT examination of the peripapillary retinal nerve fiber layer (RNFL), but the measurement of the minimum distance between the Bruch membrane opening an

[Quo vadis neuroimaging? The eye as window to the brain. Current options and future perspectives].

A possible manifestation site for multiple sclerosis (MS) is the optic nerve which clinically presents as optic neuritis. In recent years studies have shown that MS patients have a reduced retinal nerve fiber layer (RNFL) thickness. The literature and own investigations could show that MS patients have a thinning of the RNFL (especially in temporal quadrants) with reduction of the total macular volume and this alteration is also correlated with the severity of the disease. Neuromyelitis optica (monophasic) shows an extreme thinning of the RNFL with severe reduction in vision. In neurodegenerat

CHOROIDAL MELANOMA ARISING FROM OPTIC DISK MELANOCYTOMA WITH ASSOCIATED PATHOLOGY AND GENETIC FINDINGS CASE REPORT.

PURPOSE: Choroidal melanoma arising from an optic disk melanocytoma is rare but has been previously described at an incidence of 1-2%. Evaluating these cases for pathology and genetics is relevant for determining metastatic risk. METHODS: Case report. RESULTS: A 73-year-old man with a history of optic disk melanocytoma of the left eye presented with blurred vision after being lost to follow-up at an outside clinic for 5 years. The melanocytoma was found to have undergone transformation to a choroidal melanoma suggested by fundus examination, optical coherence tomography, and B scan ultrasono

Autoimmune Optic Neuropathy: Pathogenesis, Diagnosis, and Therapeutic Advances.

Autoimmune optic neuropathy (AON) encompasses a heterogeneous group of immune-mediated optic nerve inflammatory disorders, characterized by progressive or recurrent visual loss with or without optic disc edema, and distinct autoantibody profiles and neuroimaging features. This review synthesizes current knowledge on AON, highlighting key advances in the discovery of disease-specific biomarkers [eg, aquaporin-4 (AQP4)-IgG for NMOSD, myelin oligodendrocyte glycoprotein (MOG)-IgG for MOG-associated disease (MOGAD), and glial fibrillary acidic protein (GFAP)-IgG for astrocytopathy], while neuroima

Optical imaging of the optic nerve: beyond demonstration of retinal nerve fiber layer loss.

Although we are still early in the evolution of optical imaging of the optic nerve, the available techniques already play an important role in clinical decision making. I would summarize our findings to date as follows: For acute ON: Presentation: OCT shows RNFL swelling, normal GCL + IPL by OCT; 1 month: OCT and SLP show RNFL thinning and swelling, GCL + IPL thinning by OCT; 3 months or later: OCT and SLP show RNFL thinning, further mild GCL thinning by OCT; 6 months: RNFL and GCL + IPL thinning finished. For acute NAION: Presentation: OCT shows RNFL swelling and SLP shows loss of birefringen

Diagnostic Role of Multimodal Imaging in Optic Disc Diseases.

The aim of this narrative review was to provide a disease-oriented diagnostic framework for selecting and integrating multimodal imaging techniques according to the suspected optic disc disorder. A non-systematic PubMed search was performed to identify relevant literature on optic disc diseases and related imaging appearances. Searches included a combination of terms related to disease features and imaging modalities. The recent introduction of non-invasive imaging devices, such as optical coherence tomography (OCT) and OCT angiography (OCTA), has allowed a remarkable step forward in the knowl

Evaluation of Retinal Nerve Fiber Layer and Macular Ganglion Cell Layer Thickness in Relation to Optic Disc Size.

To investigate whether optic nerve ganglion cell amount is dependent on optic disc size, this trial analyzes the correlation between Bruch's membrane opening area (BMOA) and retinal nerve fiber layer (RNFL) thickness as well as macular ganglion cell layer thickness (mGCLT). Additionally, differences in RNFL and mGCLT regarding various optic disc cohorts are evaluated. This retrospective, monocentric study included 501 healthy eyes of 287 patients from the University Hospital Münster, Germany, who received macular and optic disc optical coherence tomography (OCT) scans. Rank correlation coeffic
