Does bladder capacity assessment by frequency/volume chart correlate well with urodynamic estimation in children with spina bifida?

INTRODUCTION: Reduced capacity is a common feature of the neuropathic bladder seen in children with spina bifida. This impacts on urinary continence and, in severe cases, may require augmentation cystoplasty. The traditional means of assessing bladder size has been urodynamics. A frequency/volume chart may also provide this information, but the technique may not be so scientifically controlled. It is unclear how well the results of these two modalities correlate. MATERIALS AND METHODS: One hundred and twenty-two urodynamic studies and one-week frequency/volume charts were completed by 77 chil

Characterization of global and region-specific transcriptomic alterations in the bladder following acute partial bladder outlet obstruction in female mini-swine.

Partial bladder outlet obstruction (pBOO) is a fibroproliferative disorder that results in poorly compliant, high-pressure bladders due to lower urinary tract occlusion. The mechanisms governing pBOO-induced bladder remodeling remain poorly understood. The goals of the present study were to profile global and region-specific transcriptomes of porcine bladders in response to pBOO using RNA sequencing and to identify genes and pathways differentially affected in the bladder dome and trigone by obstructive injury. Nine adult female Yucatan mini-swine were randomized and divided into either nonsur

Assessing bladder, bowel and sexual symptoms: a neurologist's guide.

Symptoms related to bladder, bowel and sexual dysfunction are common in patients with neurological diseases and are associated with significant morbidity and impacts on quality of life. However, compared with other neurological problems, they are often less visible, under-ascertained and under-treated. This review aims to provide neurologists with an evidence-based, structured approach to evaluating bladder, bowel and sexual dysfunction in patients with neurological disorders. As with any other presenting concern, this requires a structured approach to history, examination and the judicious us

Bladder Cancer and Vesical Imaging-Reporting and Data System: An Updated Review.

Bladder cancer is a prevalent malignancy with a significant societal burden. Treatment strategies and prognosis are largely determined by the degree of local tumor invasion into the bladder wall. The Vesical Imaging-Reporting and Data System (VI-RADS) is a standardized, MR imaging-based scoring system specifically designed to assess muscular invasion in bladder cancer. VI-RADS effectively differentiates between nonmuscle invasive bladder cancer and muscle-invasive bladder cancer. By providing an accurate and non-invasive evaluation of tumor depth, location, and stage, VI-RADS facilitates effec

Explainable and likelihood aware AI framework for MRI-based pixel-level bladder tumour prediction.

Bladder tumours (BTs) pose significant clinical challenges due to their high recurrence rates and risk of progression to invasive malignancies, which emphasises the need for early and accurate detection. Magnetic resonance imaging (MRI), with its superior soft tissue contrast, is a potential modality for BT detection. To analyse the MRI scans, artificial intelligence (AI) models are increasingly being leveraged. However, these models are often limited by a scarcity of annotated datasets, challenges in pixel-level tumour prediction, and insufficient transparency in predictions. This study intro

Bladder Preservation After Neoadjuvant Therapy for Muscle-invasive Bladder Cancer in the Current Era: Myth and Reality.

Bladder preservation after neoadjuvant therapy for muscle-invasive bladder cancer remains investigational. While randomized validation is awaited, growing prospective data, consensus standardization, and biomarker-driven strategies are transforming bladder-sparing strategies from an empirical alternative into a disciplined, evidence-maturing paradigm towards bladder-intact cure.

Effects of different pulse widths on acute tibial nerve stimulation for overactive bladder in cats.

PURPOSE: This study aimed to assess the effectiveness of different pulse widths in suppressing bladder overactivity using tibial nerve stimulation (TNS) in cats. METHODS: Hook electrodes were implanted on the left tibial nerve. Cystometry was conducted by infusing either acetic acid (AA) or normal saline (NS). TNS was applied at intensities of 2-4 times the threshold (T) with pulse widths ranging from 60 to 624 µsec. Cystometrograms were used to evaluate the impact of different pulse widths on the micturition reflex. RESULTS: Bladder capacity (BC) was significantly reduced to 49.71% ± 6.76%

Longer time to radical cystectomy in patients treated with neoadjuvant chemotherapy is associated with worse oncological outcomes.

BACKGROUND: Current muscle-invasive bladder cancer (MIBC) guidelines recommend not delaying radical cystectomy (RC) >3 months after diagnosis as it decreases overall survival (OS). However, literature investigating the impact of delay in RC in patients who receive NAC is limited, especially on a population-based level. OBJECTIVE: To investigate the association between time from diagnosis of MIBC to RC (TTRC) in patients with urothelial bladder cancer (UBC) treated with NAC and RC and 1) 2-year OS and 2) pathological lymph node status (pN+) in a population-based cohort. METHODS: Patients were

MH2AFormer: An Efficient Multiscale Hierarchical Hybrid Attention With a Transformer for Bladder Wall and Tumor Segmentation.

Achieving accurate bladder wall and tumor segmentation from MRI is critical for diagnosing and treating bladder cancer. However, automated segmentation remains challenging due to factors such as comparable density distributions, intricate tumor morphologies, and unclear boundaries. Considering the attributes of bladder MRI images, we propose an efficient multiscale hierarchical hybrid attention with a transformer (MH2AFormer) for bladder cancer and wall segmentation. Specifically, a multiscale hybrid attention and transformer (MHAT) module in the encoder is designed to adaptively extract and a

A Brief Overview of Cholinergic and Phosphodiesterase-5 Inhibitors in Diabetic Bladder Dysfunction.

Diabetic bladder dysfunction (DBD) comprises a wide spectrum of lower urinary tract symptoms that impact diabetic patients' lives, including urinary frequency, urgency, incontinence, and incomplete bladder emptying. To relieve symptoms, anticholinergics have been widely prescribed and are considered an effective treatment. There is increasing evidence that diabetic patients may benefit from the use of phosphodiesterase 5 (PDE5) inhibitors. This narrative review aims to provide a brief overview of the pathophysiology of DBD along with a focus on cholinergic and phosphodiesterase inhibitors as t

Integrating Patient Preferences with Guideline-Based Care in Neurogenic Lower Urinary Tract Dysfunction After Spinal Cord Injury.

Individual and social factors are important for clinical decision-making in patients with neurogenic bladder secondary to spinal cord injury (SCI). These factors include the availability of caregivers, social infrastructure, and personal preferences, which all can drive bladder management decisions. These elements can be overlooked in clinical decision-making; therefore, there is a need to elicit and prioritize patient preferences and values into neurogenic bladder care to facilitate personalized bladder management choices. For the purposes of this article, we review the role of guideline-base

Vaginal pessary: a culprit for intraperitoneal bladder perforation.

Vaginal pessaries are widely considered to be a safe and effective non-surgical management option for women with pelvic organ prolapse. Complications may occur, and are more frequent with improper care and certain device designs and materials. It is imperative to provide information to patients about potential complications. We present the case of a woman in her 70s who presented to the Emergency Department with increasing groin and abdominal pain following a vaginal pessary insertion 2 days prior for grade 3 vaginal vault prolapse. On presentation, her abdomen was markedly distended with guar

Cytokeratin 20 expression is linked to stage progression and to poor prognosis in advanced (pT4) urothelial carcinoma of the bladder.

Cytokeratin 20 (CK20) expression is limited to umbrella cells in the normal urothelium. Since CK20 is often upregulated in neoplastic urothelial cells including dysplasia and carcinoma in situ, immunohistochemical CK20 analysis is often used for the assessment of bladder biopsies. CK20 expression is a feature of luminal bladder cancer subtype, but its prognostic relevance is disputed. In this study, we investigated CK20 on >2700 urothelial bladder carcinomas in a tissue microarray format by immunohistochemistry. Cytoplasmic and membranous CK20 staining was seen in 1319 (51.8%) cancers. The fra

Multiparameter interferometric polarization-enhanced imaging differentiates carcinoma in situ from inflammation of the bladder: an ex vivo study.

SIGNIFICANCE: Successful differentiation of carcinoma in situ (CIS) from inflammation in the bladder is key to preventing unnecessary biopsies and enabling accurate therapeutic decisions. Current standard-of-care diagnostic imaging techniques lack the specificity needed to differentiate these states, leading to false positives. AIM: We introduce multiparameter interferometric polarization-enhanced (MultiPIPE) imaging as a promising technology to improve the specificity of detection for better biopsy guidance and clinical outcomes. APPROACH: In this ex vivo study, we extract tissue attenuatio

Clinical application of bladder MRI and the Vesical Imaging-Reporting and Data System.

Diagnostic work-up and risk stratification in patients with bladder cancer before and after treatment must be refined to optimize management and improve outcomes. MRI has been suggested as a non-invasive technique for bladder cancer staging and assessment of response to systemic therapy. The Vesical Imaging-Reporting And Data System (VI-RADS) was developed to standardize bladder MRI image acquisition, interpretation and reporting and enables accurate prediction of muscle-wall invasion of bladder cancer. MRI is available in many centres but is not yet recommended as a first-line test for bladde

[NONRISING BLADDER PARAGANGLIOMA SUCCESSFULLY TREATED BY THE OPEN BLADDER SURGERY: A CASE REPORT].

The patient is a 47-year-old female. MRI revealed a well-defined submuscular mass in the bladder muscle layer. Bladder paraganglioma was suspected based on MRI findings. Endocrinologic Testing showed no significant elevation. 123I-MIBG scintigraphy of the mass showed a significant uptake, and we made diagnosis of bladder paraganglioma. The mass was nonrising and showed no color differentiation making its location undetectable. Using MRI with a ureteral stent and urethral catheter in place, we were able to determine its location. The possibility of damage to the ureteral or internal urethral op

Single-cell RNA sequencing reveals the immune microenvironment and signaling networks in cystitis glandularis.

INTRODUCTION: Cystitis glandularis (CG) is a rare chronic bladder hyperplastic disease that mainly manifests by recurrent frequent urination, dysuria and gross hematuria. The current lack of unified diagnosis and treatment criteria makes it essential to comprehensively describe the inflammatory immune environment in CG research. METHODS: Here, we performed scRNA-sequencing in CG patients for the first time, in which four inflamed tissues as well as three surrounding normal bladder mucosa tissues were included. Specifically, we isolated 18,869 cells to conduct bioinformatic analysis and perfor
