The ileal ureter neobladder is associated with a high success and a low complication rate.

PURPOSE: Several different methods to construct a bladder substitute after cystectomy have been described. We evaluated our experience with the Studer ileal ureter neobladder during the last 5 years. MATERIALS AND METHODS: We reviewed retrospectively the results in 32 patients who underwent construction of a slightly modified ileal neobladder from that originally described. Mean followup was 25 months (range 6 to 68). RESULTS: Patients experienced few complications and only 1 required reoperation. Daytime and nighttime continence rates were 94 and 74%, respectively. One patient sustained a u

[Usefulness++ of total parenteral nutrition in radical surgery for bladder cancer].

Thirty patients diagnosed with non-metastatic infiltrating vesical cancer (pT2-3, NoMo) due to receive radical cystectomy and transintestinal urinary by-pass, underwent Pre-operative Nutritional Assessment (PRNA), Cystectomy, post-operative Total Parenteral Nutrition (TPN), and Post-operative Nutritional Assessment (PONA) in day 7. The purposes of this prospective, uncontrolled clinical trial were: to identify, prior to surgery, undernourished patients, to assess the effectiveness of post-operative TPN with regard to a decrease in morbidity and mortality, and to evaluate the influence of a def

Vesicoureteral reflux in complete ureteral duplication: surgical options.

Vesicoureteral reflux is the most common abnormality seen with complete ureteral duplication. The majority of cases require surgical correction. The techniques used in 62 patients with 71 duplicated systems with reflux are reviewed. Reflux into the lower pole was encountered in 51 duplicated systems, both poles in 19 systems and the upper pole in 1. A total of 42 conjoint ureteral reimplantations was performed but in 8 cases only the lower pole ureter with reflux was reimplanted after it was separated from the nonrefluxing ureter. Ipsilateral ureteroureterostomy was performed in 19 cases of lo

Percutaneous ureteral fulguration: a nonsurgical technique for ureteral occlusion.

Three patients with lower urinary tract fistulas after multiple operations and radiation therapy for pelvic cancer were treated with percutaneous ureteral fulguration and nephrostomy tube drainage. This technique occluded the ureter and allowed for maintenance of a dry fistula site in all 3 patients. There were no complications. The longest followup in these patients was 21 months. The procedure is simple technically and it can be performed with the patient under local anesthesia. The technique of percutaneous ureteral fulguration is described and other techniques for nonoperative occlusion of

Urine cytology. Part I: urinary tract cytology.

Urinary tract cytology is an excellent means of detecting urinary tract neoplasia. It is not a good localizing technique but is an exceptionally useful diagnostic tool for monitoring high-risk patients. The closer the lesion is to the collecting point, the higher the degree of diagnostic accuracy. Thus, cells from a bladder cancer are more frequently seen than malignant cells from the ureter or renal pelvis. Properly collected urine specimens examined by qualified cytologists can also aid in the diagnosis of infections and inflammatory diseases.

The Sex Bias of Cancer.

In hormone-dependent organs, sex hormones and dysregulated hormone signaling have well-documented roles in cancers of the breast and female reproductive organs including endometrium and ovary, as well as in prostate and testicular cancers in males. Strikingly, epidemiological data highlight significant differences between the sexes in the incidence of various cancers in nonreproductive organs, where the role of sex hormones has been less well studied. In an era when personalized medicine is gaining recognition, understanding the molecular, cellular, and biological differences between men and w

Nuclear matrix proteins as cancer markers.

The transformation of normal cells to a malignant state has long been detected by light microscopy as visible changes in nuclear morphology. These changes include abnormal nuclear shape, increased nuclear to cytoplasmic ratio, and the presence of additional and abnormal nucleoli. Metaplasia, dysplasia, carcinoma-in-situ, and gross malignant tumors are diagnosed and graded pathologically by this traditional method. The resulting relative increase in DNA concentration within these cells produces a greater affinity for Hematoxilyn and Eosin staining, and thus, the characteristic blue color of canc

Integrating urinary dual-gene methylation and VI-RADS score to predict residual tumors after TURBT in NMIBC: a nomogram-based model.

BACKGROUND: Bladder cancer (BC) is the ninth most prevalent cancer globally, with approximately 75% of cases classified as non-muscle-invasive bladder cancer (NMIBC). Although transurethral resection of bladder tumor (TURBT) is the standard treatment for NMIBC, there remains a considerable risk of postoperative residual tumors. Accurately identifying patients who require repeat transurethral resection of bladder tumor (Re-TURBT) remains a significant clinical challenge. Magnetic resonance imaging (MRI) exhibits high accuracy in detecting residual tumors after TURBT, aiding clinical decision-ma

Predictive Value of Prior Non-muscle-invasive Bladder Cancer for the Effectiveness of Neoadjuvant Chemotherapy in Localized Muscle-invasive Bladder Cancer: A Real-world Analysis of the BLADRAC Cohort.

BACKGROUND AND OBJECTIVE: It remains currently unknown whether the effectiveness of neoadjuvant chemotherapy (NAC) before radical cystectomy (RC) for muscle-invasive bladder cancer (MIBC) is the same in patients with or without prior non-muscle invasive bladder cancer (NMIBC). This study aims to perform a real-world analysis of the predictive value of prior NMIBC for the effectiveness of NAC in patients undergoing RC for MIBC. METHODS: We included 2378 patients from the BLADRAC cohort who received RC with (n = 870; 37%) or without (n = 1508; 63%) NAC for primary (n = 1825; 77%) or secondary (

Adjuvant intravesical therapy in intermediate-risk non-muscle-invasive bladder cancer.

OBJECTIVE: To evaluate the impact of adjuvant therapy on oncological outcomes in patients with intermediate-risk non-muscle-invasive bladder cancer (NMIBC), as due to the poorly-defined and overlapping diagnostic criteria optimal decision-making remains challenging in these patients. PATIENTS AND METHODS: In this multicentre study, patients treated with transurethral resection of bladder tumour for Ta disease were retrospectively analysed. All patients with low- or high-risk NMIBC were excluded from the analysis. Associations between adjuvant therapy administration with recurrence-free surviv

HIVEC as an alternative option in non-muscle-invasive bladder cancer: Experiences from a high-volume center.

BACKGROUND: High risk non-muscle invasive bladder cancer (NMIBC) is usually treated with intravesical BCG-therapy. In case of BCG failure radical cystectomy (RC) is the treatment of choice. Nevertheless, many patients are unfit for or unwilling to undergo RC. Hyperthermic intravesical chemotherapy (HIVEC) is a promising bladder sparing therapy in such cases. It was the purpose of the study to evaluate the efficacy of HIVEC in patients with BCG failure as well as in BCG naïve patients in case of BCG shortage or given contra-indications for BCG. METHODS: We analyzed the first 60 patients who re

Real-world Patterns and Trends of Intravesical Chemotherapy for Non-muscle Invasive Bladder Cancer: Insights from a Large Prospective German Cohort.

BACKGROUND/AIM: Intravesical instillation therapy is the standard of care for patients with non-muscle-invasive bladder cancer (NMIBC) following transurethral resection (TURBT). Adherence to early and adjuvant instillation guidelines in real-world settings remain largely unknown. This was a large, prospective, non-interventional study aimed to evaluate practice patterns, temporal trends, adverse events, and guideline adherence in Germany. PATIENTS AND METHODS: Between 2011 and 2019, 960 patients with NMIBC were enrolled across 102 urology practices. Data were collected using standardised case

Differential response of low- and high-grade intermediate-risk non-muscle-invasive bladder cancer to bacillus Calmette-Guérin and gemcitabine-docetaxel therapy.

BACKGROUND AND OBJECTIVE: Sequential intravesical gemcitabine/docetaxel (Gem/Doce) is a potential alternative to bacillus Calmette-Guérin (BCG) for treating non-muscle-invasive bladder cancer (NMIBC). Intermediate-risk NMIBC group (IR-NMIBC) includes both low-grade (LG) and high-grade tumors (HG). This study investigates the response of HG and LG IR-NMIBC to Gem/Doce compared to BCG therapy. METHODS: We included patients with IR-NMIBC who received either BCG or Gem/Doce between 2013 and 2023. Maintenance regimens were administered to patients without recurrence after induction for 1 year. Fol

Small cell size circulating tumor cells predict the prognosis of high-risk non-muscle invasive bladder cancer patients.

The study aimed to explore the distribution of circulating tumor cells (CTCs), circulating tumor endothelial cells (CTECs), and their subtypes in non-cancer and bladder cancer individuals, focusing on their prognostic value in high-risk non-muscle invasive bladder cancer (NMIBC). Researchers analyzed 59 fresh peripheral blood samples using subtraction enrichment and immunostaining fluorescence in situ hybridization (SE-iFISH). Samples were collected from healthy individuals (n = 18), patients with benign urinary conditions (n = 2), newly diagnosed bladder cancer patients (n = 20), and NMIBC pa

A Novel Gene Expression Scoring System Predicts Recurrence in Non-Muscle-Invasive Bladder Cancer Patients.

BACKGROUND: Despite the high recurrence rate of non-muscle-invasive bladder cancer (NMIBC), there are limitations in accurately predicting recurrence after transurethral resection of bladder tumor (TURBT) based on clinicopathological factors alone. However, prediction of recurrence using biomolecular characteristics of bladder tumors has not been applied to clinical practice. The objective of this study was to establish a new gene expression scoring system for identifying patients at high risk of recurrence. METHODS: NMIBC and normal bladder samples were subjected to microarray analysis to ob

Radiotherapy for female non-gynecologic pelvic and adjacent cancers and secondary uterine Cancers: a SEER analysis.

BACKGROUND AND PURPOSE: Population-based evidence on the incidence and long-term risk of secondary uterine cancer after radiotherapy (RT) for non-gynecologic pelvic cancers remains limited. This study evaluated the incidence, temporal risk patterns, and survival outcomes of secondary uterine cancers after RT. MATERIALS AND METHODS: Female patients with rectal cancer (RC), bladder cancer (BC), or lower pelvic cancers (LPC) were identified from the Surveillance, Epidemiology, and End Results database. Competing-risk regression models were used to estimate hazard ratios for secondary uterine can

99mTc-DTPA and 99mTc-DMSA renal gamma imaging in the surveillance of patients with conduit urinary diversion.

We studied renal anatomy and function using 99mTc-2-3 dimercaptosuccinic acid (DMSA) and 99mTc-diethylenetriaminepentaacetic acid (DTPA) in 27 patients with conduit urinary diversion. In this condition, free ureteral reflux is often associated with bacteriuria, and these factors are thought to precipitate progressive renal deterioration. Gamma-camera images provided valuable information concerning the structure of the renal parenchyma, the function of individual kidneys and possible ureteral obstruction, thus helping us to decide whether or not to instigate further treatment. The information g

Neuromuscular dysfunction of the lower urinary tract in patients with lesions of the cauda equina and conus medullaris.

Bladder and urethral function was studied in 21 patients with lesions of the cauda equina or conus medullaris using gas cystometry, integrated sphincter electromyography, uroflowmetry and computer assisted measurement of detrusor and urethral innervation. An areflexic cystometrogram, impaired electromyographic sphincter activity and delayed reflex-evoked potentials from stimulation of the detrusor muscle and urethra were the most consistent indicators of lesions of the conus medullaris and/oather than the ice water test is suggested.
