Granulomatosis with polyangiitis (GPA) (formerly Wegener's disease) is a systemic ANCA (anti-neutrophil cytoplasmic antibody) vasculitis that frequently affects the lungs. We report the case of a patient with isolated pulmonary GPA and a pseudotumoral clinical presentation. In the presence of the major histopathological criteria for GPA (vasculitis lesions, parenchymal necrosis, and granulomatous inflammation), a diagnosis of GPA can be proposed after excluding the main differential diagnoses (infection, necrotizing sarcoidosis granulomatosis, eosinophilic granulomatosis with polyangiitis, and…

Purpose: This study is aimed at describing a case of combined CMV retinitis and vitreous hemorrhage in an immunocompromised patient. Observations: A 38-year-old male who is known to have HIV presented to our emergency department complaining of decreasing vision in his left eye. Vitreous hemorrhage and preretinal hemorrhage were observed upon examination and were thought to be caused by CMV retinitis. After initial treatment and worsening condition upon follow-up, a diagnosis of combined CMV retinitis and vasculitis was considered and was treated accordingly. Conclusions: The presence of vitreo…

Tayakasu arteritis (TA) is a chronic large-vessel systemic vasculitis more frequent in women. The pathogenesis of TA is not fully understood. Due to its silent and heterogenous nature, it is a challenging diagnosis, with no specific diagnostic tests. We report the case of a 45-year-old woman presenting with a concomitant ischemic stroke and myocarditis as an atypical presentation of Takayasu arteritis, revealing its heterogeneous nature. The pre-existent myocardial scar and dilated cardiomyopathy are is a reminder of how silent yet aggressive this vasculitis can be and the need of for high lev…

We sincerely thank Dr Andrew Whyte, who keenly reviewed our case report and came up with critical reasoning to justify his thoughts and critique with regard to our published article, "An Unusual Case of Acquired Angioedema and Monoclonal Gammopathy of Renal Significance in a Middle-Aged Caucasian Female." We agree with the author that hypocomplementemic urticarial vasculitis can be a reasonable contender as a diagnosis in this case. There are indeed some features in this case that do not entirely fit either classic presentation of acquired angioedema or hypocomplementemic urticarial vasculitis…

Microscopic polyangiitis (MPA) is a primary systemic vasculitis characterized by inflammation of small-sized vessels associated with the presence of anti-neutrophilic cytoplasmic antibodies. We report a case of a 39-year-old female diagnosed with microscopic polyangiitis as an incidental finding who presented with signs and symptoms of a stroke at a young age. Usually, it presents with fever, malaise, skin rash, weight loss, mononeuritis multiplex, and arthralgia/myalgia. Very rarely, it can involve meninges to cause meningeal vasculitis which can present as a febrile seizure. The most frequen…

Vasculitides are disease in which injury to blood vessels leads to various degrees and types of organ dysfunction. They are subdivided into … m standard therapies, various biologic agents have been evaluated in the treatment of ANCA positive vasculitis (AAV), cryoglobulinemic vasculitis, Behcet's disease and Takayasu's Arteritis (TAK). In this article, we present a review on biologic agents used in various vasculitides.

Rashes are one of the most common presenting symptoms in the emergency department setting; they may be a primary diagnosis or a symptom of systemic disease. Accurate diagnosis of a rash may be challenging and requires clinical acumen, systematic history and physical examination, and judicious use of diagnostic testing. This case study reviews Henoch-Schönlein purpura (HSP) or IgA vasculitis, a common vasculitis of childhood. Although typically a self-limited disease, HSP may cause serious complications in a small subset of patients and requires monitoring. In addition, there are several differ…

Pulmonary renal syndromes are rare but serious complications of systemic vasculitis. The majority of these cases are related to ANCA-associated vasculitis. These syndromes represent a combination of diffuse alveolar hemorrhage and rapidly progressive glomerulonephritis. Here we report a case of 21-year-old female with a clinical picture of community-acquired pneumonia that rapidly evolved to pulmonary hemorrhage and acute renal failure. Combined pulse steroid treatment, immunosuppressive treatment, and plasmapheresis were instituted and resulted in the improvement of the patient. The diagnosis…

Neutrophilic dermatosis of the dorsal hands (NDDH) is a rare and recently described disorder regarded as a subset of neutrophilic dermatosis, similar to superficial pyoderma gangrenosum and pustular vasculitis. Many currently consider it to be a localized variant of Sweet Syndrome. We describe the case of a 63-year-old male patient with NDDH associated with hypopharyngeal carcinoma. A biopsy demonstrated changes consistent with those seen in Sweet Syndrome, with leukocytoclasia but no signs of true vasculitis. Treatment with oral prednisolone resulted in complete resolution of the lesions in 1…

OBJECTIVE: To report the clinical, pathological, and mutational features of hereditary C1 inhibitor (C1INH) deficiency as a cause of isolated vasculitic neuropathy.
PATIENT: A 35-year-old woman with sensorimotor mononeuritis multiplex and facial palsy.
RESULTS: The sural nerve biopsy results showed a decrease of myelinated fibers with axonal degeneration and severe hypersensitivity vasculitis, with deposition of C1q on vessel walls. Mutational analysis of the C1INH gene found a new mutation, a heterozygous 2-base pair deletion in exon 8. The patient was treated with plasmapheresis and intrav…

Behçet's syndrome (BS; Adamantiades syndrome) is the association of the triple symptom complex of recurrent aphthous stomatitis (RAS) with genital ulceration, and eye disease (especially iridocyclitis) though a number of other systemic manifestations may also be seen. BS mainly affects young adult males, and there is an association with HLA-B5 and HLA-B51 (B5101). Features such as arthralgia and leucocytoclastic vasculitis suggest an immune-complex mediated basis, which is supported by finding circulating immune complexes and, although the antigen responsible is unidentified, heat shock protei…

PURPOSE: To describe a case of peripheral keratitis associated with erythema elevatum diutinum (EED), an unusual immune-complex-mediated cutaneous vasculitis.
DESIGN: Observational case report.
METHODS: A 25-year-old man who was diagnosed 15 months previously with erythema elevatum diutinum presented with an inflammatory peripheral keratitis of the left eye.
RESULTS: Serologic investigations for systemic disorders associated with vasculitic peripheral ulcerative keratitis were unremarkable. As the sclerokeratitis was thought to represent an ocular extension of the patient's cutaneous vascul…

A 28-year-old male patient suffering from Wegener's granulomatosis died suddenly with signs of cardiac failure after clinical symptoms had basically subsided under chemotherapy. Autopsy revealed pulmonary granulomata, necrotizing vasculitis of the lungs and kidneys, focal and segmental necrotizing glomerulonephritis, and diffuse granulomatous and necrotizing giant cell myocarditis. Histological confirmation of inflammation of the heart in Wegener's disease has rarely been reported. Although cardiac involvement in Wegener's granulomatosis sometimes is suspected, it is usually thought to have no…

Chronic meningococcal septicaemia is an unusual and infrequently recognised presentation of disease caused by Neisseria meningitidis. Clinical features are immunologically mediated and include fever, rash and arthritis, which may mimic cutaneous vasculitis or reactive gonococcal arthritis. Diagnosis is difficult to confirm as blood cultures commonly do not grow the organism despite weeks of symptoms. Culture of the organism from the nasopharynx may provide supportive evidence for diagnosis. Chronic meningococcaemia should be considered in the differential diagnosis of a cutaneous vasculitis. I…

The study of ten cases of cutaneous periarteritis nodosa by direct immunofluorescence microscopy of excision biopsy specimens revealed positive findings in nine. Deposition of IgM was found in the vessel walls in six cases, and C3 was observed in four; however, only in two cases was C3 found with IgM in the vessel walls. Deposits of IgM in the superficial vessels were found in five cases, although only the deep muscular vessels showed evidence of the vasculitis in cutaneous periarteritis nodosa. Serum hepatitis B antigen was absent in the nine cases that were tested. Positive immunofluorescenc…
