Scimitar syndrome is a rare congenital cardiopulmonary anomaly characterized by partial anomalous pulmonary venous return (PAPVR) from the right lung to the inferior vena cava, often associated with right-lung hypoplasia and dextroposition of the heart. The infantile form usually presents early with respiratory distress and pulmonary hypertension, posing diagnostic and management challenges. We report a term male neonate born at 38 weeks of gestation, who developed severe respiratory distress soon after birth and was initially managed for meconium aspiration syndrome. Echocardiography revealed

Blunt traumatic pericardial rupture is an exceptionally rare condition, often associated with high morbidity and mortality. Isolated pericardial rupture without vascular or cardiac involvement accounts for only 17% of reported cases. We describe a rare case of right-sided pericardial rupture following blunt chest trauma, an unusual presentation since most pericardial tears occur on the left side. A 23-year-old woman involved in a motor vehicle accident sustained multiple injuries, including right hemothorax, bilateral pulmonary contusions, left-sided rib fractures, and a humerus fracture. INTRODUCTION: Injuries of the inferior vena cava (IVC) secondary to blunt trauma is a rare life-threatening condition that has a mortality rate up to 70%. Prompt diagnosis and treatment is mandatory. Endografts are commonly used in arterial trauma, but their use in veins injury is rarely reported in literature.
CASE REPORT: We report a case of a 72-year-old foreign woman with IVC injuries at confluence treated in an emergency setting. She arrived at the emergency room after a car crash. Computed tomography (CT) angiography showed rupture of iliocaval confluence with massive active bleeding. E

Systemic congestion is increasingly recognized as a major contributor to organ dysfunction and adverse outcomes in patients with heart failure and in those at risk of fluid overload. The venous excess ultrasound (VExUS) grading system, introduced in 2020, uses Doppler evaluation of the hepatic, portal, and intrarenal veins to quantify venous congestion at the bedside. While abnormal venous waveforms have been recognized for years, the formalization of VExUS has provided a structured and clinically meaningful framework, generating widespread academic interest with over 500 citations to date. BACKGROUND AND OBJECTIVE: The mediastinum is a complex anatomical region that includes the heart, aorta, superior vena cava, trachea, and mainstem bronchi. Mediastinal masses, while relatively uncommon, can cause life-threatening cardiovascular symptoms from mass effect, including cardiac tamponade, superior vena cava syndrome, mediastinal mass syndrome, and right ventricular outflow tract obstruction, requiring prompt identification and management. Currently, there are no guidelines that discuss the optimal management of these cardiovascular complications, and our review aims to address this.

BACKGROUND Eisenmenger syndrome presents a unique challenge for the acute care surgeon. Even routine operations such as laparoscopic appendectomy or cholecystectomy become challenging due to the cardiopulmonary physiologic changes and anatomic anomalies associated with Eisenmenger syndrome. The care of these patients can be further complicated by the severity of disease, surgical complexity, and the abnormal anatomy associated with the syndrome. CASE REPORT A 35-year-old patient with Eisenmenger syndrome and abdominal heterotaxy presented with acute cholecystitis. She underwent percutaneous ch

OBJECTIVES: To report presumed hypervirulent Klebsiella pneumoniae pyelonephritis causing renal vein-inferior vena cava septic thrombophlebitis, cavitary septic pulmonary emboli, and lung abscesses.
CASE PRESENTATION: A 47-year-old woman with untreated diabetes presented with urinary symptoms, prolonged fever, abdominal pain, and hypoxemia. Blood and urine cultures yielded K. pneumoniae, with bacteremia persisting despite meropenem. An 11-mm positive string test supported a hypermucoviscous phenotype, although hypervirulence was not genetically confirmed. Computed tomography showed left pyelo

AIM: The aim of this study was to highlight the hypothesis that embryological development and morphological aspects necessitate a comprehensive analysis of all situses and a detailed description of each anomaly, rather than attempting to force the malformation into the rigid categories of heterotaxy syndrome (HS) - left or right isomerism. This hypothesis is supported by describing the anatomy of a complex patient with multiple laterality defects resulting in multiple cardiac and organ malformations.
CASE PRESENTATION: The unique aspect of the presented patient's morphological diagnosis lies

BACKGROUND: Giant abdominal tumors exceeding 10 cm in diameter are rare and often involve several critical organs and vascular structures. The primary treatment is surgical excision, which presents substantial challenges, including organ preservation, hemorrhage control, and intact tumor removal.
CASE SUMMARY: We present a 46-year-old male with a massive abdominal mass invading the right kidney and inferior vena cava (IVC). To address this, we developed a novel technique using continuous renal replacement therapy (CRRT) renal vein (RV) shunt procedure. A temporary dialysis catheter was placed

BACKGROUND: Inflammation-induced injury of venous endothelium is the first trigger of deep vein thrombosis (DVT). We previously showed NLRP3 regulates platelet function and arterial thrombosis. However, whether platelet NLRP3 involves in venous thrombosis remains unclear.
OBJECTIVES: In this study, we intended to investigate the role of NLRP3 in venous thrombosis by using NLRP3 knockout mice and platelet-specific NLRP3 knockout mice.
METHODS: DVT model was established through ligation of the inferior vena cava. After 48 hours of ligation, inferior vena cava sample was excised to measure thro

A 19-year-old female presented with a 2-month history of abdominal discomfort and palpable abdominal mass; a computer tomography scan revealed a large retroperitoneal mass as well as high suspicion of thrombus in her inferior vena cava. Right kidney core biopsy showed diagnostic features of synovial sarcoma. While awaiting workup for her mass she was placed on anticoagulants. However, she re-presented to hospital 3 weeks later complaining of 3 days of shortness of breath. A repeat computer tomography scan revealed extensive thrombus burden with tumor thrombus involving the inferior vena cava,

BACKGROUND: Superior sinus venosus defect (SVD) is a complex congenital heart disease (CHD) with a wide spectrum of anatomical variants. The innovative transcatheter correction of SVD (TCSVD) is feasible in selected cases. Patient selection requires a detailed anatomical evaluation. This study aims to provide an anatomical classification of SVD using 3D models.
METHODS: Computed tomography (CT) scans of hearts with superior SVD were 3D-modeled using semi-automatic segmentation. Key parameters such as superior vena cava (SVC) size, SVC overriding, caudal defect extension, and size/orientation

An 18-year-old female presented to the emergency department with progressive bilateral lower extremity pain and swelling. She reported a two-week history of back pain radiating down both legs, initially treated with steroids and muscle relaxants without improvement. Initial imaging, including an MRI and hip X-rays, was unremarkable. She subsequently developed worsening left leg swelling, prompting further evaluation for deep vein thrombosis (DVT). She had no history of oral contraceptive use, recent travel, surgery, or prolonged immobility. A family history of "thick blood" in her mother and a

An aortocaval fistula is a rare vascular connection between the aorta and inferior vena cava, often caused by abdominal aortic aneurysm, trauma, or infection. It can result in high-output heart failure due to increased venous return and reduced systemic vascular resistance, typically presenting with right-sided symptoms. Renal arteriovenous and reno-caval fistulas are even rarer, with no prior reports of spontaneous idiopathic reno-caval fistulas. A 67-year-old man with hypertension and paroxysmal atrial fibrillation presented with 10 months of generalized edema and exertional dyspnea. He had

Adrenocortical carcinoma (ACC) is a malignancy of the adrenal cortex with a high morbidity and mortality. More than half of the cases are functional tumors. As different hormones can be co-secreted above physiologic levels, it causes a very broad variety of symptoms and makes differentiating from more common entities hard. Here we present a case of a patient with a newly diagnosed ACC who initially presented with acute pulmonary embolism and recurrent deep vein thromboses (DVT) in the setting of hypercortisolism. Imaging showed a left adrenal mass invading adjacent structures including a nonoc

Protein C deficiency is a rare genetic disorder caused by mutations in the protein C, inactivator of coagulation factors Va and VIIA gene, affecting approximately 1 in 200-500 individuals. It leads to a hypercoagulable state, increasing the risk of blood clots. Symptoms vary with age, ranging from life-threatening purpura fulminans in neonates to venous thromboembolism, particularly deep vein thrombosis, in adults. A recent case involved a 21-year-old South Asian male presenting with persistent fever, weight loss, epistaxis, abdominal tenderness, and acute pain in the right thigh and leg, rais

Intravenous leiomyomatosis (IVL) is a benign disease that originates in the uterus and is characterized by aggressive intravenous growth that can extend to the inferior vena cava and even involve the intracardiac cavity. This extension of the IVL into the right heart is known as intracardiac leiomyomatosis (ICL). The clinical incidence of this disease is extremely low, and the characteristics of atypical aggressive growth lead to diagnostic dilemmas. Thus, studies on this disease have profound clinical significance. Here, we report a case of intracardiac leiomyomatosis and review the relevant
