Extended-spectrum β-lactamases (ESBLs), particularly CTX-M enzymes, represent a significant concern among Enterobacterales. This cross-sectional, descriptive study reports the first detection and genomic characterization of CTX-M-15-producing Enterobacter hormaechei and Leclercia adecarboxylata recovered from high-contact surfaces in a Brazilian veterinary teaching hospital, based on analysis of 12 environmental samples. Three multidrug-resistant isolates were identified: E. hormaechei ST78 (UFRGS-TABLE-2023), E. hormaechei ST68 (UFRGS-TAP-2023), and L. adecarboxylata (UFRGS-BED-2023), all car

Carbapenem-resistant Enterobacterales (CRE) are a major global health threat with limited treatment options. Aztreonam/avibactam is a promising therapy against metallo-β-lactamase (MBL)-producing and other CRE, but emerging resistance threatens its effectiveness. Insertions in penicillin-binding protein 3 (PBP3), which are well described in Escherichia coli, are linked to reduced aztreonam/avibactam susceptibility but remain poorly characterized in Klebsiella pneumoniae. We report clinical K. pneumoniae carbapenemase-producing K. pneumoniae sequence type 11 isolates carrying a novel PBP3 YRIT

UNLABELLED: Antimicrobial resistance is a major public health threat, as resistant infections are harder to treat and carry greater risks. Using a One Health framework, this survey of freshwater samples investigated the influence of local land use on the occurrence of waterborne third-generation cephalosporin-resistant Enterobacterales (3GC-E) mediated through extended-spectrum β-lactamases (ESBLs) or plasmid-mediated AmpC (pAmpC) genes. A cross-sectional survey of 339 freshwater samples from 49 New Zealand sites was undertaken, encompassing catchments with urban, agricultural (namely dairy; s

INTRODUCTION: The aim of this study was to analyze the clinical characteristics and risk factors of bloodstream infections (BSI) caused by extended-spectrum β-lactamase (ESBL)-producing Klebsiella pneumoniae (KP).
METHODOLOGY: A retrospective study was conducted by enrolling clinically confirmed KP-BSI cases (2019-2023). Clinical data were collected and analyzed through review of the electronic medical records. Patients with ESBL-positive KP-BSI were compared to those with ESBL-negative KP-BSI. Multivariate logistic regression analysis was used to identify risk factors associated with ESBL-pr

BACKGROUND: The emergence of carbapenem-resistant Klebsiella pneumoniae (CR-Kp) has become a major global public health concern. Infections caused by CR-Kp strains that also possess hypervirulence traits present significant challenges for clinical management and are associated with increased morbidity and mortality. This study aimed to investigate the prevalence of β-lactamase genes and hypervirulence-associated markers among clinical CR-Kp isolates collected in Cyprus.
METHODS: A total of 96 K. pneumoniae isolates were investigated. Species-level identification and antimicrobial susceptibili

To elucidate the zoonotic potential of carbapenemase-producing carbapenem-resistant Enterobacterales (CP-CRE) in US companion animals (i.e., dogs and cats), we queried the National Center for Biotechnology Pathogen Detection database to identify One Health clusters containing CP-CRE isolates from companion animals and humans. The 11 One Health clusters we found included most (69% [169/246]) publicly available CP-CRE sequences from US companion animals and were from 8 internationally disseminated, high-risk sequence types from 3 bacterial species (Escherichia coli, Klebsiella pneumoniae, and En

BACKGROUND: Water is essential for life; however, it serves as the route of transmission for many infectious pathogens. The increasing prevalence of multidrug-resistant (MDR) bacteria in water samples poses a significant public health risk. AIM: This study aims to reveal the transmissible resistance genes linked to antibacterial resistance in bacterial pathogens found in water and sediment samples from the Oko-Agbon stream in Makoko, Lagos. METHOD: The bacteria were identified using conventional methods and subsequently subjected to antimicrobial susceptibility testing using the Kirby-Bauer di

BACKGROUND: Carbapenem-resistant Pseudomonas aeruginosa (CRPA) constitutes a serious source of global healthcare-associated infections, and the exploration of its resistance mechanism represents an important approach to address this issue. Because current research on antibiotic resistance predominantly focuses on multidrug-resistant P. aeruginosa which is widely isolated clinically and the resistance mechanism is complicated. CRPA generally has a higher tolerance to other antibiotics than carbapenem-sensitive P. aeruginosa, yet the specific mechanism of resistance remains poorly understood.

The rise of multidrug-resistant (MDR) extraintestinal pathogenic Escherichia coli (ExPEC) poses a severe healthcare threat, necessitating alternative treatment strategies such as bacteriophage therapy. In this study, four lytic phages (EC.W2-9, EC.W5-4, EC.W8-1, and EC.W14-2) were isolated from hospital wastewater and characterized for their efficacy against MDR ExPEC isolates. A total of 44 ExPEC isolates were analyzed, with 41% belonging to sequence type (ST) 131 and 59% to ST410, all exhibiting MDR phenotypes. Among these, 49% produced extended-spectrum β-lactamases (ESBLs), and 86% were id

This systematic review assessed the global epidemiology of metallo-β-lactamase (MBL)-producing Acinetobacter clinical isolates and the associated antimicrobial resistance. A total of 475 relevant articles from the Cochrane Library, Google Scholar, PubMed, Scopus, and Web of Science were identified and screened as potentially eligible articles. Data from 85 articles were extracted for the analysis. Most reports on MBL-producing Acinetobacter clinical isolates originated from Asia [68/85 (80%) studies] and Africa [14/85 (16.5%) studies]. There were also scarce reports from Europe and America. Th

Carbapenemase-producing Klebsiella pneumoniae is among the mainly reasons for death from bacterial infection associated with antibiotic resistance. Its widespread dissemination, especially due to KPC enzyme, is one of the main challenges in One Health perspective. Here, we studied 42 KPC-producing K. pneumoniae isolates from hospital wastewater, municipal wastewater from wastewater treatment plant (WWTP), and urban stream which receives treated municipal effluent. The isolates presented broad resistance to β-lactams antibiotics, as well as to fluoroquinolones, and show antibiotic resistance pr

No clinically available antibiotic adjuvants can overcome metallo-β-lactamase (MBL) resistance in carbapenem-resistant Gram-negative ESKAPE pathogens. Divalent cations in MBL and quasicrystalline lipopolysaccharides (LPS) are key determinants of acquired and intrinsic resistance in these bacteria. Chemically depriving these cations would block multiple resistance pathways simultaneously and restore carbapenem susceptibility. Here, we chemically engineer a class of anionic adjuvants to tailor their interactions with MBL, enhancing the enzymatic inhibition by up to ~5885-fold compared to parent

Extended-spectrum β-lactamases (ESBLs) and plasmid-mediated AmpC (pAmpC) β-lactamases represent a threat for public health. Their dissemination is often mediated by mobile genetic elements (MGEs), but plasmid identification and characterization could be hindered by sequencing limitations. Hybrid assembly may overcome these barriers. Eight ESBL/pAmpC-producing E. coli isolates from broilers were sequenced using Illumina (short-read) and Oxford Nanopore MinION (long-read). Assemblies were generated individually and using a hybrid approach. Plasmids were typed, annotated, and screened for antimic

Comamonas species have been isolated from different sources, with Comamonas testosteroni and Comamonas resistens commonly related to human diseases and multidrug resistance, respectively. During a surveillance study to monitor carbapenem resistance in bacteria from wastewater samples in Brazil, a carbapenem-resistant strain, named M13, was obtained and identified as C. resistens (ANI 98.90%, dDDH 94.60%) by genomic analysis, being a species distinct from C. testosteroni. It exhibited multidrug resistance and presented small inhibition zones around disks containing novel β-lactams and β-lactam-

Bacteria showing tolerance to antibiotic killing are characterized by a slower kill rate compared with other isolates from the same species. Antibiotic persistence is defined by a subpopulation of bacteria showing a reduced kill rate when an isolate is exposed to a bactericidal antibiotic. Both are best studied using time-kill experiments and have been associated with treatment failure and the development of antibiotic resistance. Despite their potential clinical importance, quantitative data for the variation in antibiotic tolerance and persistence are lacking. Here, we performed time-kill ex

Febrile neutropenia can lead to life-threatening infections in patients with haematological malignancies or after hematopoietic cell transplantation. Infection management is challenged by rising antibiotic resistance and regional differences in bacterial epidemiology. The 10th European Conference on Infections in Leukaemia panel recommends a personalised approach guided by local resistance patterns and individual risk factors. For patients who are haemodynamically stable without colonisation or infection by resistant Gram-negative bacteria in low-resistance prevalence settings, empirical monot

Carbapenem-resistant Enterobacterales (CRE) are important pathogens that can develop resistance via multiple molecular mechanisms, including hydrolysis or reduced antibiotic influx. Identifying these mechanisms can improve pathogen surveillance, infection control, and patient care. We investigated, both phenomenologically and mechanistically, how resistance mechanisms influence the carbapenem inoculum effect (IE), a phenomenon where inoculum size affects antimicrobial susceptibility testing (AST). We demonstrated that any of seven different carbapenemases were sufficient to impart a meropenem
