Paediatric femoral shaft fractures: what are the concomitant injuries?

INTRODUCTION: Trauma remains a substantial cause of paediatric morbidity and mortality. Femur fractures are common in children, and frequently are the result of high-energy mechanisms of injury. A complete description of missed injuries in this population has not previously been described. MATERIALS AND METHODS: A retrospective chart review of patients presenting to the Alberta Children's Hospital with femoral shaft fractures was performed. Associated injuries, and injuries with delayed diagnosis were recorded. RESULTS: Twenty-percent of the patients suffered at least one other injury with t

A haemoperitoneum does not indicate active bleeding in the peritoneum in 50% of hypotensive blunt trauma patients: a study of 110 severe trauma patients.

BACKGROUND: We hypothesised that in blunt trauma patients with haemodynamic instability and haemoperitoneum on hospital admission, the haemorrhagic source may not be confined to the peritoneum. The purpose of this study was to describe the incidence and location of bleeding source in this population. METHODS: The charts of trauma patients admitted consecutively between January 2005 and January 2010 to our level I Regional Trauma Centre were reviewed retrospectively. All hypotensive patients presenting a haemoperitoneum on admission were included. Hypotension was defined by a systolic blood pr

[Acute diseases and trauma of thorax and abdomen in patients with hemocontact viral infections].

Screening of hemocontact viral infections (HVI) (HIV, hepatitis B and C) was conducted among patients of the emergency thoraco-abdominal surgery unit. During the 8 years of the study the HVI detection had increased on 57.4%. Gender analysis showed greater HVI prevalence among men. Medico-social criteria of HVI risk-groups among patients of the emergency thoraco-abdominal surgery unit were stated.

[The value of sonography in traumatology and orthopedics : Part 2: emergency diagnostics in blunt abdominal and thoracic trauma].

Ultrasound examinations in trauma patients should be done in the emergency department using curved-array (3.5-7.5 MHz) probes. Blunt trauma of the abdomen and thorax must be regarded as a single organ injury. Sonography is the imaging technique of first choice and has completely replaced peritoneal lavage. Paramount advantages are its ability to provide rapid information and reproducible results at short intervals and in a noninvasive manner. The sensitivity and specificity of sonography in detecting intraabdominal fluid are 97-100% and 80-90%, respectively. To achieve such good results, thoug

Cephalomedullary interlocked nail for ipsilateral hip and femoral shaft fractures.

A retrospective study of the management of 23 cases of ipsilateral hip and femoral shaft fractures, between January 1998 and December 2001, is presented. All except two cases were managed by a single implant, i.e. reconstruction nail. There was delayed diagnosis of femoral neck fracture in two cases where the "miss a nail" technique was used for fixation of the femoral neck fracture. All patients managed by reconstruction nail were simultaneously operated on for both fractures and operative treatment was executed as early as the general condition of the patient permitted. Delay in treatment wa

[The so-called "battered child syndrome" from the viewpoint of the pediatric radiologist].

The diagnosis of the Battered-Child Syndrome (BCS) is made by the pediatrician and the radiologist. The recognition of this entity by the radiologist is possible because of the high frequency of the typical skeletal lesions. This skeletal changes are illustrated by X-ray pictures and bone scans. Not only skeletal trauma can be discovered but also visceral injuries may be combined and diagnosed in the BCS. For the detection of all changes in the BCS nowadays all possible imaging procedures should be used. Some forensic problems in this field are added.

Transcatheter gelfoam embolization in abdominal, retroperitoneal, and pelvic hemorrhage.

Clinical experience with Gelfoam embolization for arterial hemorrhage in 55 patients is described. This technique controlled hemorrhage in 51 patients (92.7%). Two patients (3.6%) died of complications related to embolization. Gelfoam can be a permanent occluding agent which does not require subselective catheter position for effectiveness and safety. The advantages of Gelfoam are its availability, low cost, the ease and speed with which it can be applied through conventional catheters, and its safety and effectiveness if properly applied. The data presented here, and those in the literature,

Inferior pancreaticoduodenal artery aneurysm presenting as a highly vascularized lesion by the papilla Vateri: presentation of a case and review of the literature.

We report a case of a 60-year-old male who developed a large retroperitoneal haematoma following endoscopic retrograde cholangiopancreatography (ERCP). Initial imaging suggested a distal common bile duct calculus and a small highly vascularized lesion in the periampullary region, but ERCP revealed no stones, and no sphincterotomy or balloon dilation was performed. Post-procedure, the patient developed severe abdominal pain with subsequent haemodynamic instability. Computed tomography angiography demonstrated active bleeding from aneurysm of the inferior pancreaticoduodenal artery, successfully

Uterine and Sigmoid Colon Perforation Following Hysteroscopy-Guided Microwave Endometrial Ablation (MEA) in a Patient With a Bicornuate Uterus and Multiple Myomas: A Case Report.

In Japan, the microwave endometrial ablation system (MEA, Microsulis PLC, Waterlooville, UK) is the most widely used device for endometrial ablation and is considered a minimally invasive, uterus-preserving treatment for menorrhagia. Although MEA is generally safe, uterine and/or bowel perforation can occur as a rare but serious complication. We report a case of uterine and sigmoid colon perforations following hysteroscopy-guided MEA in a patient with a bicornuate uterus, endometriosis, and multiple uterine myomas. To our knowledge, this is the first reported case of simultaneous uterine and b

Child with Closed Head Injury and Persistent Vomiting.

CASE PRESENTATION: We present the case of a six-year-old child with autism who presented with persistent vomiting in the setting of a closed head injury (CHI). Computed tomography of the head was normal, but due to persistent vomiting a radiograph of the abdomen was done, which showed multiple, rare-earth magnets in the abdomen. There was no history of witnessed ingestion. These magnets had caused enteroenteric fistula formation leading to persistent vomiting. DISCUSSION: In the setting of CHI, vomiting can be a sign of concussion or intracranial hemorrhage. In cases of CHI where intracranial

Empyema Presentation Secondary to Streptococcus constellatus.

A 60-year-old male presented to our institution for abdominal pain and was later admitted to the intensive care unit for shock, acute hypoxemic respiratory failure, and acute kidney injury. He was subsequently found to have a large left-sided pleural effusion with empyema secondary to Streptococcus constellatus. With the emerging threat and growing prevalence of Streptococcus anginosus group pathogens, there is now greater clinical importance in identifying viridans streptococci at the species level. While immunosuppressed individuals are at greater risk of opportunistic pathogens, this case p

Complications associated with incorrect use of nasal CPAP.

Nasal continuous positive airway pressure (nCPAP) is a safe, effective, non-invasive respiratory modality to deliver positive end expiratory pressure in neonates. Many studies have established its associated improved respiratory outcomes without increase in major morbidities associated with preterm neonates. In contrast, there is paucity in literature addressing complications such as nasal injury, abdominal distention, air leak syndromes (especially pneumothorax), hearing loss, heat and chemical burns, swallowing and aspiration of small components of the nasal interface and delay in escalation

Renal transplant injury at caesarean delivery: A cautionary tale and a plan for the future.

Pregnancy following renal transplantation is increasingly common. Overall pregnancy outcomes are favourable; however, specific transplant-related risks do exist. In particular, the risk of caesarean delivery is much higher in renal transplant recipients when compared to the general obstetric population. This is owing to the necessity for preterm delivery in cases of severe and early-onset pre-eclampsia and/or fetal growth restriction. We describe two recent cases of renal transplant injury at caesarean delivery at our institution, a tertiary/quaternary obstetric service, which highlight the po

EVALI versus MIS-C, one more overlapping diagnosis to consider.

We describe six teenagers presenting with fever and severe abdominal symptoms admitted with concerns for multisystem inflammatory syndrome in children (MIS-C). Laboratory evaluation revealed elevated markers of inflammation, lymphopenia, and increased D-dimers. Imaging studies revealed multifocal airspace disease and ground-glass opacities. SARS-CoV-2 polymerase chain reaction and serologies were negative. All patients reported a history of vaping, prompting E-cigarette, or vaping, product use-associated lung injury (EVALI) diagnosis. MIS-C has overlapping clinical and laboratory features high

LAPRA-TY for laparoscopic repair of traumatic diaphragmatic hernia without intracorporeal knot tying.

CASE PRESENTATION: A 38-year-old man was brought in by ambulance as a trauma activation after sustaining a self-inflicted stab wound in the left upper quadrant with a kitchen knife. His primary survey was unremarkable and his vital signs were normal. Secondary survey revealed a 2 cm transverse stab wound inferior and medial to the left nipple. Extended focused assessment with sonography for trauma (FAST) did not show intra-abdominal or pericardial fluid and chest X-ray did not show a definite pneumothorax or hemothorax. Transfusion-Associated Circulatory Overload as a Result of Plasma Transfusion to Correct International Normalized Ratio Before an Invasive Procedure: A Case Report.

Plasma transfusion is commonly used to correct elevated international normalized ratio (INR) before invasive procedures. A 54-year-old woman presented to the emergency department with abdominal pain. Workup revealed Streptococcus pneumoniae peritonitis. Her hospitalization was complicated by respiratory failure, fluid overload, atrial fibrillation, and acute kidney injury. Patient underwent 2 paracentesis (9 L removed). Four units of plasma were transfused to correct an INR of 3.0 (goal 1.5) for a transjugular intrahepatic portosystemic shunt procedure. INR remained at 1.9, and she developed a

[How to do - the chest tube drainage].

A chest tube is used to drain the contents of the pleural space to reconstitute the physiologic pressures within the pleural space and to allow the lungs to fully expand. Indications for chest tube placement include pneumothorax, hemothorax, pleural effusion, pleural empyema, and major thoracic surgery. The most appropriate site for chest tube placement is the 4th or 5th intercostal space in the mid- or anterior- axillary line. Attention to technique in placing the chest tube is vital to avoid complications from the procedure. Applying the step-by-step technique presented, placement of a chest
