DPAC: A tri-dimensional coordinated framework for breaking the efficiency ceiling in parameter-efficient fine-tuning.

While parameter-efficient fine-tuning (PEFT) has become essential for adapting large pre-trained models, it is challenging to achieve optimal trade-offs between parameter budgets and task performance. The core difficulty lies in reducing parameters and maximizing the utilization efficiency of each allocated parameter across heterogeneous model components. Recent advances like DoRA improve upon LoRA through magnitude-direction weight decomposition. Nevertheless, there is an unexplored limitation: existing methods optimize parameterization structures in isolation but neglect the coordinated dyna

Inflammaging and cytokine-driven cardiovascular senescence in age-related myocardial infarction: mechanisms, biomarkers, and precision immunotherapy strategies.

Ageing is an independent, non-modifiable risk factor for myocardial infarction (MI). It is also characterized by chronic, low-grade inflammation (inflammaging) and progressive cardiovascular senescence. These ageing-associated processes alter cytokine networks and innate and adaptive immune-cell function, promote endothelial dysfunction, and compromise post-infarction repair, thereby increasing myocardial susceptibility to ischaemia and adverse cardiac remodelling in the elderly. Lifestyle modifications, including regular physical activity, reduce inflammaging by facilitating the release of an

The gut-liver-kidney-brain axis in Wilson disease: copper speciation-flux and barrier-mediated organ crosstalk.

Wilson disease (WD) has long been framed as a hepatocentric disorder of copper accumulation. That view is now giving way to a broader model centered on the gut-liver-kidney-brain axis. In WD, copper is not simply stored in tissues as an inert burden. It circulates in dynamic, bioactive pools-particularly relative exchangeable copper (REC)-that disrupt barrier structures, including the intestinal epithelium and blood-brain barrier, and spread toxicity through measurable biochemical mediators. Major pathogenic processes include copper-induced suppression of autophagy, disruption of FXR-regulated

Capturing Elusive Volatile Organics in Atmospheric Particles via Dual-Mode Solid-Phase Microextraction Coupled with Gas Chromatography-Orbitrap Mass Spectrometry.

Molecular-level characterization of organic aerosol (OA) is critical for understanding its sources and atmospheric evolution, yet remains analytically challenging due to its broad volatility distribution and complex chemical composition. In this study, we develop a dual-mode solid-phase microextraction (DM-SPME) approach coupled with gas chromatography-orbitrap mass spectrometry (GC-Orbitrap MS) that enables direct, high-sensitivity detection of OA in complex ambient samples. By integrating headspace and direct immersion extraction, DM-SPME improves the detection of higher-volatility compounds

Diagnostic and Management Approach to Reflux-Related Cough.

Chronic cough, defined as cough persisting longer than 8 weeks, affects approximately 10% of the global population and significantly impairs quality of life. Gastroesophageal reflux disease is a recognized cause of chronic cough, yet the relationship remains complex and poorly understood. Up to 75% of patients with reflux-related cough lack classic gastroesophageal symptoms such as heartburn or regurgitation, making diagnosis particularly challenging. Although direct tissue irritation by refluxate has been proposed as a primary mechanism, evidence also supports a vagally mediated esophageal- t

A review of longitudinal radiology report generation: Dataset composition, methods, and performance evaluation.

Chest X-ray imaging is a widely used diagnostic tool in modern medicine, and its high utilization creates substantial workloads for radiologists. To alleviate this burden, vision-language models are increasingly applied to automate Chest X-ray radiology report generation (CXR-RRG), aiming for clinically accurate descriptions while reducing manual effort. Conventional approaches, however, typically rely on single image, failing to capture the longitudinal context necessary for producing clinically faithful comparison statements. Recently, growing attention has been directed toward incorporating

Beyond first responders: neutrophils shape biomaterial-guided regeneration.

Neutrophils have long been regarded as short-lived effector cells responsible for clearing pathogens and inducing acute inflammation. However, emerging evidence has reshaped this paradigm, revealing remarkable neutrophil plasticity and their capacity to actively orchestrate tissue repair and regeneration. Neutrophils modulate angiogenesis, extracellular matrix (ECM) remodeling, macrophage polarization, and resolution of inflammation. Distinct neutrophil subsets-including pro-inflammatory and pro-resolving phenotypes-have been identified, highlighting their dynamic adaptation to microenvironmen

Tumor-associated macrophages and lineage plasticity in prostate cancer: from established myeloid programs to emerging spatial hypotheses.

Androgen receptor signaling inhibitors (ARSIs) have transformed the treatment of advanced prostate cancer, yet durable responses are limited by the emergence of therapy-resistant disease states, including neuroendocrine prostate cancer (NEPC) and double-negative prostate cancer (DNPC). Lineage plasticity has traditionally been studied from a tumor-cell-intrinsic perspective, but single-cell and spatial studies increasingly indicate that the tumor microenvironment, particularly tumor-associated macrophages (TAMs), may influence tumor-cell state transitions, immune exclusion, and therapeutic res

Circulating tumour DNA in oligometastatic oesophago-gastric cancers: applications, challenges, and future directions.

Oesophago-gastric cancer (OGC) is associated with a poor prognosis, with most patients presenting with or ultimately developing metastatic disease. Oligometastatic disease (OMD) represents an intermediate disease state characterised by a limited metastatic burden and, in selected patients, a more indolent clinical course. Increasing evidence suggests that carefully selected patients with OMD may benefit from local metastasis-directed therapies combined with systemic treatment. The OligoMetastatic Esophagogastric Cancer (OMEC) consortium has recently proposed a standardised consensus definition

Application and Future Perspectives of Extracellular Vesicle-Loaded Scaffold in Spinal Cord Injury.

Following the initial trauma of spinal cord injury (SCI), the secondary injury phase-characterized by inflammation, oxidative stress, neuronal death, and axonal demyelination-establishes an adverse microenvironment that hinders functional recovery. Despite the availability of existing clinical therapies, they often yield suboptimal functional recovery. In this review, the applications and future prospects of extracellular vesicle (EV)-loaded scaffolds for SCI repair are summarized. This review discusses the biological properties and therapeutic action of EVs, as well as limitations of using th

From pressure to prognosis: establishing a common language for portal hypertension in advanced chronic liver disease.

Risk stratification in compensated advanced chronic liver disease has traditionally been based on the identification of clinically significant portal hypertension (CSPH), defined as a hepatic venous pressure gradient (HVPG) of ≥10 mm Hg. CSPH is a well-established marker of poor prognosis, associated with a substantially higher risk of decompensation compared with the absence of CSPH (29% vs 10% at 4 years).Non-invasive tests (NITs) are increasingly used as surrogates for CSPH, yet their potential to directly predict clinical endpoints of decompensation or liver-related death remains underutil

Smart nanocarriers against MDR-tuberculosis: stimuli-responsive strategies for granuloma penetration and next generation therapy.

Multidrug-resistant tuberculosis (MDR-TB) remains a global health concern due to long treatment durations, limited drug penetration into granulomatous lesions, systemic toxicity, and the persistence of dormant Mycobacterium tuberculosis. The granuloma microenvironment, characterized by hypoxia, acidic pH, enzymatic activity, dense extracellular matrix, and redox imbalance, acts as a barrier to the effectiveness of conventional antitubercular therapy. Recent advances in nanotechnology have led to the development of stimuli-responsive nanocarriers that enable targeted, controlled, and on-demand

Research progress on the mechanisms and clinical applications of neuromodulation techniques for intervening in interhemispheric imbalance after stroke.

Post-stroke interhemispheric inhibitory imbalance acts as one of the major contributing pathological factors driving motor impairment, rather than a universal primary pathological mechanism across all stroke subtypes. This narrative review systematically summarizes preclinical and clinical neuromodulation interventions including optogenetic manipulation and transcranial magnetic stimulation, grounded in the interhemispheric inhibition (IHI) theory and bimodal balance-recovery framework. We elaborate how these techniques restore excitatory/inhibitory (E/I) homeostasis via regulating neurotransm

Current Evidence on the Management of Non-occlusive Mesenteric Ischemia: A Comprehensive Narrative Review.

Non-occlusive mesenteric ischemia (NOMI) is a life-threatening form of acute mesenteric ischemia (AMI), caused by severe intestinal hypoperfusion in the absence of an obstructive lesion of the mesenteric arteries. Early diagnosis and management remain challenging because the condition often develops in critically ill patients and frequently presents with non-specific clinical manifestations. This comprehensive narrative review summarizes the current evidence regarding the management of NOMI. A comprehensive literature search was performed. Evidence relating to hemodynamic resuscitation, imagin
