From 1979 to 1983, Lyme disease was studied longitudinally in the 162 long-term residents of Great Island, Massachusetts. In retrospect, the index case occurred in 1962, and the peak years of disease transmission (about three new cases per 100 residents per year) were the late 1970s. Thereafter, during the period of active surveillance, attack rates declined by half. Altogether, 26 (16%) of the 162 residents developed symptoms of the disease. Most of those affected had erythema chronicum migrans, and when untreated, they subsequently developed arthritis or, in one instance, myocarditis. A mino

Lyme disease, caused by a tick-transmitted spirochete, typically begins with a unique skin lesion, erythema chronicum migrans. Of 314 patients with this skin lesion, almost half developed multiple annular secondary lesions; some patients had evanescent red blotches or circles, malar or urticarial rash, conjunctivitis, periorbital edema, or diffuse erythema. Skin manifestations were often accompanied by malaise and fatigue, headache, fever and chills, generalized achiness, and regional lymphadenopathy. In addition, patients sometimes had evidence of meningeal irritation, mild encephalopathy, mi

Blastomycosis is the infection caused by the dimorphic fungus Blastomyces dermatitidis. The fungus was believed to be limited in distribution to North America but is found in Africa and northern South America, too. The exact natural habitat of B. dermatitidis is still uncertain with only rare reported isolation of the fungus from the environment. The inability to recover the organism from nature along with the absence of both a reliable skin test antigen and a sensitive serological test have significantly restricted our understanding of the epidemiology and the full clinical spectrum of blasto

Twenty-four hips in 21 patients suffering septic arthritis at infancy were followed 11-30 years (mean 17 years). Fourteen hips with none to moderate femoral head destruction proved to have a fairly good chance of becoming located following closed treatment. If it failed, open reduction was successful, provided the femoral head-neck was big enough to achieve a stable reduction. Out of the 10 hips with absent femoral head-neck and an iliac dislocation, 6 underwent trochanteric arthroplasty: 3 hips remained located though with limited motion, and one of the 3 became painful requiring arthrodesis.

The aetiological role of immunodeficiency in acute septic arthritis of the hip in infancy, the management of the condition after the acute infection has subsided, and the special hazards of infection in the region of the hip joint in the older child are discussed. The principles of treatment of congenital dislocation of the hip are examined in relation to the maintenance of acetabular growth potential. The factors determining the outcome of treatment in Perthes' disease are discussed and a comparison of the result in a series of cases treated by femoral osteotomy with those in untreated contro

Coccidioidomycosis, a disease induced by the dimorphic fungi Coccidioides immitis and Coccidioides posadasii, is endemic to the southwestern United States and occurs with a myriad of clinical and imaging manifestations, which range from limited pulmonary disease to disseminated multiorgan infections. Humans are infected when arthroconidia in the soil are disturbed and dispersed into the air. Once inhaled, arthroconidia form spherules, which trigger a host cell-mediated immune response. If this response is adequate, it results in granuloma formation, halting the infection. However, an insuffici

The pathogenesis and pathophysiology of rheumatic diseases is complex and relies on the interaction of different factors. The common view is that the pathological autoimmunity develops in genetically predisposed individuals upon exposure to an environmental trigger. This highlights the importance of recognizing and deconstructing the effects of environmental agents in rheumatic diseases. Several factors have been identified in the last decades, with detrimental or protective effects, impacting not only on disease onset, but also on its natural history. Cigarette smoking has been identified as

BACKGROUND: In clinical practice, some cases gently challenge our assumptions, reminding us that not everything is what it seems. This is the story of a young woman receiving treatment for tuberculosis who began showing signs that pointed beyond infection alone. It underlines how autoimmunity can surface in the shadows of another illness, demanding careful attention and an open mind.
CASE PRESENTATION: A 27-year-old woman receiving treatment for pulmonary tuberculosis presented to us with worsening joint pain, swelling, facial rash, and muscle weakness. She had trouble with basic movements li

OBJECTIVE: Infectious conditions are a significant cause of mortality in autoimmune rheumatic diseases (ARD). Among patients hospitalized with an infection, we compared in-hospital and long-term (3-year) mortality between those with and without ARD.
METHODS: This retrospective analysis included members of the largest health maintenance organization in Israel, aged > 18 years at the first episode of infection, who required hospitalization during 2003-2019. We compared in-hospital mortality and the results of a 3-year landmark analysis of those who survived the index hospitalization between pat

BACKGROUND: Gender- and sex-specific differences in medicine were long-time disregarded. Despite numerous indications of gender- and sex-specific influences on the treatment of dermatological conditions, these have not yet been systematically investigated.
OBJECTIVES: To meet this unmet need, we conducted the present systematic review on the topic of gender and sex differences in the treatement outcome of skin diseases.
METHODS: Embase (via Ovid), PubMed Medline and Web of Science were searched, in between January 2001 and December 2022. English and german randomized controlled trials, prosp

OBJECTIVE: Uveitis may occur in the course of systemic inflammatory rheumatic diseases (IRD), or it may be the first clinical manifestation of these diseases. The aim of this study was to determine the incidence of IRD in patients whose initial clinical manifestation was non-infectious uveitis.
METHODS: The study included adult patients diagnosed as having noninfectious uveitis in the department of ophthalmology and referred to rheumatology for further investigation of potential rheumatic diseases as underlying etiology of uveitis. The patients' demographic and clinical features, laboratory,

INTRODUCTION: The advent of biological and targeted synthetic therapies has revolutionized rheumatoid arthritis (RA) treatment. However, this has come at the price of an increased risk of infections. The aim of this study was to present an integrated overview of both serious and non-serious infections, and to identify potential predictors of infection risk in RA patients using biological or targeted synthetic drugs.
METHODS: We systematically reviewed available literature from PubMed and Cochrane and performed multivariate meta-analysis with meta-regression on the reported infections. Randomi

INTRODUCTION: Systemic sclerosis (SSc) is characterized by a complex etiopathogenesis encompassing both host genetic and environmental -infectious/toxic- factors responsible for altered fibrogenesis and diffuse microangiopathy. A wide spectrum of clinical phenotypes may be observed in patients' populations from different geographical areas. We investigated the prevalence of specific clinical and serological phenotypes in patients with definite SSc enrolled at tertiary referral centres in different Italian geographical macro-areas. The observed findings were compared with those reported in the

This paper reports the presentation and management of an older female patient who was diagnosed with Coronavirus disease (COVID-19) and discharged from an intensive care unit (ICU) after prolonged hospitalization. The patient's COVID-19 test was negative; therefore, she was monitored in the COVID-19 general clinic with normal levels of oxygen saturation (SpO2 ). The patient had been taking Plaquenil for rheumatoid arthritis for a long time. Azithromycin was administered first, and then, the treatment continued with favipiravir according to the national treatment protocol in Turkey. On the thir
