Programmable atrial inhibited pacemakers were implanted in two patients with orthostatic hypotension due to autonomic failure. They were paced at 95 beats/min during the day and programmed themselves to 55 beats at night. This treatment resulted in virtual disappearance of orthostatic symptoms during a two-year follow-up. Haemodynamic studies showed a mean increase in erect systolic blood pressure from 47 mmHg pre-implantation to 85 mmHg at nine months post-implant during pacing. A 58-year-old man with the Shy-Dräger syndrome underwent laryngo- and tracheoscopies in general anaesthesia for evaluation of the larynx after tracheostomy following bilateral vocal cord paralysis. A profound fall in arterial pressure during anaesthesia induced with thiopentone has been demonstrated in previously reported patients with the Shy-Dräger syndrome. In the present patient, anaesthesia induced with ketamine, maintained with nitrous oxide in oxygen and supplemented with fentanyl, diazepam and suxamethonium did not cause a fall in arterial pressure.

A young, diabetic woman suffering from fainting spells, vomiting, and diarrhea is described. Extensive investigations showed total cardiac denervation, orthostatic hypotension, and disturbances in the the pupillary and sudomotor functions, as well as impairment of glucagon secretion during hypoglycemia. These disturbances were found to be caused by autonomic neuropathy. No signs of peripheral neuropathy could be detected. To the best of our knowledge this is the second case of total cardiac denervation due to diabetic neuropathy described in the literature.

The response of the urethra in patients with chronic neurogenic bladders to autonomic stimulation was investigated by urethral pressure profile. A consistent supersensitivity to alpha-adrenergic stimulation was documented. Experiments with various autonomic blocking drugs confirmed that this response is alpha-adrenergically mediated via the short neuron system. The results appear to offer pharmacological evidence for the recently evolving intricate innervation pattern of the urethra including its distal portion, where the alpha-adrenergic system is believed to be important.

Uterine response to electroacupuncture stimulation of specific sites of the extremities is demonstrated in 60 pregnant women--48 cases for labor induction and 12 cases for inhibition of premature labor. In the 34 term, post-term, and 7 intrauterine fetal death cases, induction of labor was attempted; in 32 cases delivery was achieved, resulting in a success rate of 78%. Rectal biopsy in a case of lumbosacral agenesis showed the presence of autonomic nerve elements normally supplied by spinal levels distal to the agenesis.
