Hemorrhagic moyamoya disease in children: clinical features and surgical outcome.

OBJECTIVE: The clinical presentation of moyamoya disease (MMD) typically includes cerebral ischemia in children and intracranial hemorrhage in adults. Because of its rarity, the benefit of surgery in the hemorrhagic type of pediatric MMD has not been clearly established. The purpose of this study was to delineate the clinical features and surgical outcome of hemorrhagic MMD in children. METHODS: Thirteen patients (six boys and seven girls with a combined mean age of 11.1 years and a range of 4-18 years) were retrospectively confirmed as having MMD with intracranial hemorrhage between 1988 and

Infarction limited to both middle cerebellar peduncles.

BACKGROUND: The middle cerebellar peduncle (MCP) is supplied mainly by the anterior inferior cerebellar artery and partly by the superior cerebellar artery. The bilateral MCP infarctions in previous patients were attributed to alternations of two vessels, such as the unilateral vertebral artery and basilar artery or both vertebral arteries. METHODS: We describe a case of bilateral acute infarction of the MCP and the stroke was apparently caused by occlusion of only one vertebral artery. RESULTS: A 63-year-old man presented with vertigo and auditory distortion, accompanied by difficulty in sp

Hydrocephalus secondary to hydrodynamic disequilibrium in an adult patient with a choroidal-type arteriovenous malformation.

We describe an adult patient with an unruptured choroidal-type arteriovenous malformation (AVM) associated with progressive hydrocephalus. There was no evidence of mechanical obstruction of the ventricular system by the AVM nidus itself or a draining vein. However significant reflux into periventricular and transmedullary veins was demonstrated. Following partial targeted embolization of the AVM, no further reflux was observed, the patient's clinical deficits resolved, and the hydrocephalus improved. We suggest a hydrodynamic disorder as a potential pathomechanism of hydrocephalus in this adul

A smartphone client-server teleradiology system for primary diagnosis of acute stroke.

BACKGROUND: Recent advances in the treatment of acute ischemic stroke have made rapid acquisition, visualization, and interpretation of images a key factor for positive patient outcomes. We have developed a new teleradiology system based on a client-server architecture that enables rapid access to interactive advanced 2-D and 3-D visualization on a current generation smartphone device (Apple iPhone or iPod Touch, or an Android phone) without requiring patient image data to be stored on the device. Instead, a server loads and renders the patient images, then transmits a rendered frame to the re

Endovascular treatment of A1 segment aneurysms of the anterior cerebral artery.

Aneurysms of the A1 segment of the anterior cerebral artery (ACA) are rare and challenging to treat. We evaluated our experience of endovascular treatment in 11 patients with A1 segment aneurysms of ACA. Seven aneurysms were treated with coiling; three were treated with stent-assisted coiling and one was treated with balloon-assisted coiling. Endovascular treatments were technically successful and without complication. Follow-up examinations showed complete cure in all 11 patients.

Stent reconstruction of wide-necked aneurysms across the circle of Willis.

OBJECTIVE: To describe a novel technique for single stent reconstruction of wide-necked aneurysms arising from the circle of Willis. METHODS: Neuroform stents (Boston Scientific/Target, Fremont, CA) were placed across the basilar apex (right P1 to left P1) via the posterior communicating artery in two patients, across the left carotid terminus (left A1 to left M1) via the anterior communicating artery in two patients, and across the anterior communicating artery (left A1 to right A1) in one patient for stent-supported coil embolization. RESULTS: Five women underwent successful deployment of

Quantitative evaluation of measurement accuracy for three-dimensional angiography system using various phantoms.

PURPOSE: The purpose of this study was to evaluate the spatial resolution and accuracy of three-dimensional (3D) distance measurements performed with 3D angiography using various phantoms. MATERIALS AND METHODS: With a 3D angiography system, digital images with a 512 x 512 matrix were obtained with the C-arm sweep, which rotates at a speed of 30 degrees/second. A 3D comb phantom was designed to assess spatial resolution and artifacts at 3D angiography and consisted of six combs with different pitches: 0.5 mm, 0.6 mm, 0.7 mm, 0.8 mm, 0.9 mm, and 1.0 mm. Frame rate, field of view (FOV) size, re

Computed tomography angiographic correlates of early computed tomography signs in acute ischemic stroke.

BACKGROUND AND PURPOSE: Early computed tomography (CT) signs of stroke signify major arterial occlusion. CT angiogram (CTA) is useful in detecting major arterial occlusion and may help triaging patients for intra-arterial thrombolysis. The correlation between the early signs of stroke and arterial occlusion on CTA was studied. METHODS: Consecutive patients with suspected acute anterior circulation ischemic stroke presenting within 6 hours of symptom onset underwent noncontrast CT of the head followed by CTA. The scans were later reviewed for evidence of early signs of stroke on CT scan and in

Combined endovascular and microsurgical management of giant and complex unruptured aneurysms.

OBJECT: The purpose of this study was to assess the efficacy and describe the technical features of combined endovascular and microsurgical treatments for complex and giant unruptured intracranial aneurysms. METHODS: A prospectively maintained database was reviewed to identify all patients with unruptured intracranial aneurysms who were treated with combined techniques. Twenty-one lesions were treated in as many patients: six lesions involved the posterior cerebral artery (PCA); seven the cavernous portion of the internal carotid artery (ICA); two the basilar apex; two the basilar trunk; and

Microsurgical anatomy of the early branches of the middle cerebral artery: morphometric analysis and classification with angiographic correlation.

OBJECT: The cortical arteries arising from the main trunk of the middle cerebral artery, proximal to its bifurcation or trifurcation, are called "early branches." The purpose of this study was to characterize these early branches. METHODS: The early branches were characterized according to their sites and patterns of origin, diameters, and relative proximity to the internal carotid artery bifurcation, as well as the course and area of supply of their cortical branches based on an examination of 50 hemispheres. Special attention was directed to the perforating arteries that arose from the earl

Saccular aneurysm associated with posterior cerebral artery fenestration manifesting as a subarachnoid hemorrhage in a child.

Fenestrations of the cerebral arteries are uncommon, and aneurysms in association with fenestrations are even more uncommon. We report the case of a saccular aneurysm associated with a posterior cerebral artery fenestration in a child with subarachnoid hemorrhage. The patient was successfully treated with Guglielmi detachable coils. This case is unusual because of the patient's age and the location of the fenestration.

Blood supply of the posterior cerebral artery by the carotid system on angiograms.

BACKGROUND AND PURPOSE: Occipital lobe infarcts are traditionally attributed to vertebrobasilar disease. However, anatomical studies indicate that in some people the supply of the posterior cerebral artery is via the carotid system. We investigated how often such a developmental variant in the cerebral blood supply was present during life. METHODS: We retrospectively studied 212 conventional four-vessel cerebral angiograms. Eighteen subjects were excluded beforehand, because of vascular abnormalities causing important hemodynamic changes. We determined whether a fetal variant was present, and

Enigma of raised intracranial pressure in patients with complex craniosynostosis: the role of abnormal intracranial venous drainage.

OBJECT: In this study the authors investigated whether patterns of intracranial venous drainage in children with complex craniosynostosis associated with raised intracranial pressure (ICP) were abnormal and, thus, could support the theory that venous hypertension is a major contributor to raised ICP that can lead to impaired visual function or even blindness in these patients. METHODS: The authors analyzed the anatomy of intracranial venous drainage as demonstrated in the results of 24 angiography studies obtained in 23 patients, all of whom had either a craniosynostosis-related syndrome (18

Meningiomas of the lateral ventricles of the brain.

Meningiomas of the lateral ventricles of the brain are rare tumours with an approximate incidence of 0.5-4.5% among all intracranial meningiomas. Four cases of intraventricular meningioma arising from the trigone are presented. In 1-st case the calcified mass was visible on straight radiographs. In 2-nd and 3-rd cases CT and angiography was performed. In 4-th case MRI and MRA proved to be very useful. The tumours were removed by transcortical approach in the posterior parietal area. The available literature on intraventricular meningiomas has been discussed.

Treatment of aneurysms by excision or trapping with arterial reimplantation or interpositional grafting. Report of three cases.

The authors report three cases of ruptured, large or giant aneurysms that were treated by excision or trapping, followed by revascularization of distal vessels by means of arterial reimplantation or superficial temporal artery interpositional grafting. In the first case, a large serpentine aneurysm arising from the anterior temporal branch of the right middle cerebral artery (MCA) was excised and the distal segment of the anterior temporal artery was reimplanted into one of the branches of the MCA. In the second case, a giant aneurysm, fusiform in shape, arose from the rolandic branch of the M

[Internal carotid artery agenesis and multiple aneurysms in the posterior communicating artery].

Internal carotid artery agenesia is a rare anomaly of the embryonic development. We present a case with this anomaly and a vascular feeding of this territory out of the contralateral carotid system and the posterior vascular system. There were multiple aneurysms in the right posterior communicating artery, never described before in association with carotid agenesia, that are similar to the angiographic image of the fibromuscular dysplasia ("atypical" pattern). We review and discuss the embryonic development of this anomaly and its relationship with the hemodynamic stress and aneurysm formation

Vertebral-artery dissection following a judo session: a case report.

A few days after a judo session, an 11-year-old boy presented with an ischemic stroke with dizziness, aphasia and ataxia. CCT scan revealed a left thalamic infarct. Angiography showed a fibromuscular dysplasia (FMD) of the left vertebral artery probably complicated by dissection. Subsequent evolution was favorable. This observation points out the fact that the association of a cervical pain with neurological signs of vertebrobasilar stroke, especially occurring after a cervical trauma or rotatory motion, should alert to the possibility of vertebral-artery dissection. The diagnosis is mainly ba
