Sexual abuse of children: intersection with the HIV epidemic.

OBJECTIVE: Sexual transmission of human immunodeficiency virus (HIV) is the predominant risk exposure among adolescents and adults reported with HIV infection and acquired immunodeficiency syndrome (AIDS). Although perinatal transmission accounts for the majority of HIV infection in children, there have been reports of HIV transmission through sexual abuse of children. We characterized children <13 years of age who may have acquired HIV infection through sexual abuse. METHODS: All reports by state and local health departments to the national HIV/AIDS surveillance system of children with HIV i

Psychological and psychosocial implications of differences of sex development and differentiation in families of children with this condition, with relevance to surgical care: A cross-sectional study.

INTRODUCTION: Disorders of sex development (DSD) include congenital disorders with incongruence among phenotype, genotype, and gonadal sex. The psychological aspects and response of the parents of a child with DSD towards this issue are crucial, as they are considered the fundamental care providers for their child. Hence, their perceptions affect the overall clinical and social care. We hypothesise that parents having children with DSD face marked challenges, not only social, but also psychological, such as anxiety and depression. Also, their quality of life may be negatively affected. This st

Effects of addictive substances during pregnancy and infancy and their analysis in biological materials.

The use of addictive substances during pregnancy is a serious social problem, not only because of effects on the health of the woman and child, but also because drug or alcohol dependency detracts from child care and enhances the prospect of child neglect and family breakdown. Developing additive substance abuse treatment programs for pregnant women is socially important and can help ensure the health of babies, prevent subsequent developmental and behavioral problems (i.e., from intake of alcohol or other additive substances such as methamphetamine, cocaine,or heroine) and can reduce addictio

Climate change and child wellbeing: a systematic evidence and gap map on impacts, mitigation, and adaptation.

We developed a systematic evidence and gap map (2014-24) to assess how climate change impacts, mitigation, and adaptation affect the wellbeing of children aged 0-18 years globally, and discussed findings with the Children in All Policies 2030 Youth Advisory Board. Health was the most researched child wellbeing domain (84%; 948 of 1127 studies), followed by education (15%; n=171), and food security and nutrition (14%; n=160). Research on children's agency and resilience, displacement, socioeconomic distress, and safety received less attention. Health research gaps included limited studies on ve

Human milk oligosaccharides are associated with maternal genetics and respiratory health of human milk-fed children.

Breastfeeding provides many health benefits, but its impact on respiratory health remains unclear. This study addresses the complex and dynamic nature of the mother-milk-infant triad by investigating maternal genomic factors regulating human milk oligosaccharides (HMOs), and their associations with respiratory health among human milk-fed infants. Nineteen HMOs are quantified from 980 mothers of the CHILD Cohort Study. Genome-wide association studies identify HMO-associated loci on chromosome 19p13.3 and 19q13.33 (lowest P = 2.4e-118), spanning several fucosyltransferase (FUT) genes. We identif

The Full Picture: Incorporating Ecological Factors When Conducting Developmental Screening with Young Children Experiencing Homelessness.

Conducting a neurodevelopmental screening without considering ecological factors is insufficient and may underestimate the actual risk to development for children living in poverty. This article describes ecological risk factors among a nationwide sample (n = 231) of young children experiencing homelessness. Researchers examined rates of health care access and continuity, food insecurity, lead exposure, ACEs, and access to safe places to play using a new ecological screening tool developed for the population. Children in the sample experienced high rates of food insecurity, faced significant c

Clinical Competence for Youth Suicide: Use of Simulation in Pediatric and Psychiatric-Mental Health Nurse Practitioner Programs.

Approximately one in five adolescents in the United States has a mental health concern and suicide is the second leading cause of death in this population. It is vital to prepare nurse practitioner (NP) students with the required skills to assess and manage youth with suicidal ideation. Unfortunately, it is challenging for NP students to accumulate these critical competencies due to limited preceptors and the infrequent presentation of suicidality in certain clinical sites. Furthermore, the serious nature of a child in mental health crisis often requires students to take a passive role while d

Factors Related to Immigrant/Nonimmigrant Children's Experience of Being Bullied: An Analysis Using the Multiple Disadvantage Model.

Applying the multiple disadvantage model, a study of children in the United States examined experiences of being bullied in terms of 5 factors: social disorganization, social structural factors, social relationships, mental health and access to care, and acculturation. The study was a secondary data analysis of 19,882 immigrant and non-immigrant children, using data from the 2018 National Survey of Children's Health. Logistic regression results show children's likelihood of being bullied to be associated positively with racial discrimination; child mental health problem (either attention defic

Valuing the adult health effects of air pollution in Chinese cities.

China's ongoing economic growth is accompanied by a large amount of air pollution that exacts significant health and economic costs on its people. Following up on some earlier work focusing upon general mortality and child-specific health effects, this article uses a larger data set, covering more than 90 Chinese cities, along with a set of China-based epidemiological functions, to estimate some of the adult health benefits of reducing urban air pollution. Projecting future air pollution based upon current conditions, it calculates the averted mortality and morbidity effects that would result

Uniformed families, unequal outcomes?: A multicenter retrospective study of non-accidental trauma in military and civilian children.

BACKGROUND: Non-accidental trauma (NAT) or child physical abuse (CPA) is associated with many social factors, including low socioeconomic status, mental health difficulties, and substance use. Military families are uniquely vulnerable to abuse, particularly during periods of increased stress. NAT outcome data in military-dependent children are limited to a single-center study showing longer length of stay (LOS), higher mortality, and more complications. METHODS: A retrospective review was conducted of patients (≤18 years) with NAT at eight US children's hospitals from 2018 to 2023, diagnosed

Prenatal exposure to nonpersistent endocrine disruptors and early childhood BMI: single pollutant and mixture analyses from the ECHO Program.

BACKGROUND: Exposure to non-persistent endocrine-disrupting chemicals (EDCs), including phenols and phthalates, has been linked to obesity risk. While many studies focus on single-chemical exposures, mixture effects on child adiposity remain unclear. OBJECTIVES: To assess associations of prenatal phenol and phthalate exposures, singly and as mixtures, with early childhood BMI in the U.S. Environmental influences on Child Health Outcomes (ECHO) Program. METHODS: We included 586 mother-child pairs from three ECHO sites with 23 phenols and phthalate metabolites measured in maternal urine during…

Parental Low Education, Low Income, Deprivation and School Refusal Among Japanese Elementary School Students in First Grade: Results From A-CHILD Study.

BACKGROUND: School refusal has negative consequences throughout life. While parental education and child poverty are closely linked to school refusal, the mechanism by which parental low education influences school refusal in offspring remains unclear. This cross-sectional study examined the association between parental low education and several dimensions of child poverty (low income and deprivation) with school refusal among first grade (6-7 years old) elementary school students in Japan. METHODS: Data were obtained from the Adachi Child Health Impact of Living Difficulty (A-CHILD) study co…

Global Pathways to Thriving Children: Implementing Large-Scale Multicomponent Nutrition and Child Development Programs.

Thriving children are likely to develop the human capital necessary to become healthy, competent, and economically stable adults, beginning with achieving expected rates of growth and development as infants. However, millions of young children in low-and middle-income countries are not thriving, largely due to nutritional deficiencies and lack of learning opportunities, often caused by poverty. Although nutrition interventions can support children's growth and nutrition and child development and parenting interventions can support children's learning and responsive caregiving, there are few la
