Mucinous nodules on the skin overlying a rheumatoid arthritic joint.

A rare case of mucinous nodules on the skin overlying rheumatoid synovitis in a 79-year-old man is described. The patient had a five year history of RA. The patient had complained of arthralgia in the left elbow joint, and X-ray demonstrated narrowing of thejoint space along with bone destruction. He underwent an intraarticular injection of dexamethazone and lidocaine. Three weeks later, he noticed two dome-shaped nodules about 5 mm in size developing on the elbow. Histopathological examination demonstrated poorly defined mucinous nodules in the upper dermis. The mucinous material positively s…

Fracture of the radial head and neck of Mason types II and III during growth: a 14-25 year follow-up.

Twenty-four individuals, who were 16 years of age or younger when they sustained a fracture of the radial head or neck, were examined at a m … 14%) had occasional pain. Flexion was decreased in the formerly injured compared with the uninjured elbow (139 +/- 8 degrees versus 142+/-5 degrees; P<0.05). None had developed elbow osteoarthritis. Isolated, closed fracture of the radial head and neck during growth has a favourable, long-term outcome.

Employer, physical therapist, and employee outcomes in the management of work-related upper extremity disorders.

The purpose of this study was to investigate the relationship among employer, provider, and patient outcome measures and how they are related to clinical factors and other occupation-related factors. Data were obtained from a New England workplace physical therapy (PT) clinic. Successful outcomes were defined as remaining-on/returning-to preinjury job (employer); achieving PT goals (provider); and increases in the SF-36 scale scores (patient). Variation in outcomes was explored across injury regions (shoulder, elbow, wrist/hand), work categories, number of visits, and length of PT care. Employ…

Prevalence of orthopaedic maladies in people who flyfish: an Internet-based survey.

In this study, we define the prevalence of back and joint pains in people who flyfish. We use the Internet as a source of data. Requests for participation were posted on flyfishing Web sites. Eighty-nine people were surveyed (E-mail group). A control group of 42 flyfishing club members participated (Club group). Epidemiologic information, flyfishing data, and location of pain were collected. Low back pain in the 131 participants was 59%. No differences between the two groups in location or prevalence were noted. Saltwater fishermen had the highest rates of shoulder and elbow pain (31%). Trout …

[Mononeuritis multiplex associated with measles virus infection].

We describe a 19-year-old patient who developed mononeuritis multiplex associated with measles virus infection. On April 30th, 1995, he suffered from fever and skin eruption, which disappeared ten days later. Thirteen days later, he noticed weakness of the fingers of both hands and forearms, and pain on both elbow joints. He was admitted to our hospital on June 12th, 1995. Neurologic examination revealed weakness and sensory disturbance, confined to the distal aspects of the right radial nerve and left median nerve. The electrodiagnostic findings confirmed those of axonal neuropathy in the rig…

Somatotopic mapping of the human primary motor cortex with functional magnetic resonance imaging.

We applied functional magnetic resonance imaging (FMRI) to map the somatotopic organization of the primary motor cortex using voluntary movements of the hand, arm, and foot. Eight right-handed healthy subjects performed self-paced, repetitive, flexion/extension movements of the limbs while undergoing echo-planar imaging. Four subjects performed movements of the right fingers and toes, while the remaining subjects performed movements of the right fingers and elbow joint. There was statistically significant functional activity in the left primary motor cortex in all subjects. The pattern of func…

Major limb replantation: a Thai experience.

From May 1986 to December 1992, we performed 36 major limb replantations in 35 patients; 30 were complete and 6 were incomplete amputations. Twenty-eight of these patients were male. Their ages ranged from 5 to 81 years, the average being 22 years. The levels of injury were 13 hemihand, 8 wrist, 5 lower forearm, 5 upper forearm, and 5 above the elbow amputation. The degrees of injury were 8 guillotine, 14 local crush, 9 diffuse crush, and 5 avulsion amputation. There were a partial loss of digits in 4 hemihand amputations and one failure in an above-elbow amputation--a limb survival rate of 95…

Intraneural ulnar nerve pressure changes related to operative techniques for cubital tunnel decompression.

To evaluate the effect of critical anatomic structures on the ulnar nerve after cubital tunnel decompression, we determined the intraneural ulnar nerve pressure in 50 fresh cadavers after the following surgical procedures: simple decompression, medial epicondylectomy, subcutaneous transposition, and submuscular transposition by the Learmonth and by the musculofascial lengthening technique. Intraneural pressure was measured in 0 degrees, 30 degrees, 60 degrees, and 90 degrees elbow flexion at locations that were proximal, within, and distal to the cubital tunnel. Statistical analysis compared t…

Humeral mobility after treatment with hanging cast.

A retrospective review of 23 patients after treatment of humeral fractures with hanging cast was performed. The average followup time was 8.59 years (range, 1.0 to 15.5 years). Functional results were good in eight (34.8%) and excellent in 15 patients (65.2%). To assess the flexibility of shoulder and elbow joints 24 parameters were measured on each body side using the neutral-zero method of Debrunner. A cumulative comparison of each parameter revealed decreased flexibilities of the upper limb after treatment with hanging cast: reduced abduction without moving shoulder blade (84.08 +/- 7.53 vs…

Functional status in ulnar deficiency.

Function is the most important consideration in the evaluation of patients with congenital ulnar deficiency. The upper extremity function of 8 patients with ulnar deficiency was evaluated. None of these patients had been treated with surgical procedures directed to the elbow, forearm, or wrist. The functional criteria included: (a) active ranges of elbow, forearm, and wrist motion, (b) power grip, (c) prehension, (d) dexterity, and (e) a patient's activities questionnaire. The averaged total active range of joint motion was 229 degrees or 46% of predicted normal active motion. Power grip avera…

Splinting tips.

Both plaster of Paris and fiberglass can be used for temporary or definitive treatment of acute extremity fractures, sprains, and strains. Longitudinal slab splints are appropriate for immobilizing inflamed tendons or joints of the wrist, elbow, and knee. "Sugar-tong" splints work well for humerus, forearm, and ankle fractures or sprains.

The Pulseless Supracondylar Humerus Fracture: To Explore or Not.

Supracondylar humerus fractures are the most common elbow fracture requiring surgical intervention in the pediatric population; therefore, any orthopaedic surgeon who treats pediatric orthopaedic trauma patients must be well versed in the appropriate evaluation and treatment of supracondylar fractures. It is important to focus on a subset of pediatric supracondylar humerus fractures in which the patient has an associated loss of a palpable radial pulse, termed the pulseless supracondylar. Essential considerations for these injuries include the initial evaluation and care in the emergency depar…
