Are gastric and duodenal ulcers separate diseases or do they form a continuum?

It is argued that separation of chronic peptic ulcers into gastric and duodenal may be artificial and confuse the search for common factors in their etiology. Differences are often exaggerated by contrasting the extremes of a continuous distribution of ulcers from high gastric lesser curve to duodenal bulb and beyond. Genetic and epidemiological factors appearing to favor one or the other type of ulcer may act by determining the background cellular changes on which ulcers develop. The extent of the mucosal changes may influence the site of ulceration and incidentally the level of acid secretio

Handling of multicohort data in pooled analysis of environmental exposures: confounding by cohort of the relationship between perfluoroalkyl substances and birthweight in the ECHO Program.

The relationship between exposure to perfluoroalkyl substances (PFAS) and birthweight remains unclear. Pooling data across multiple cohorts can increase power, leading to more representative populations and exposure distributions, but confounding by cohort can be a major source of bias. To understand this potential bias, we assessed the relationship between exposure to 5 PFAS and birthweight utilizing data from 5480 mother-infant dyads across 17 Environmental influences on Child Health Outcomes (ECHO) Cohort sites. The relationship was assessed in several ways: covariate-adjusted models withou

A time-delayed epidemic model for control of highly pathogenic avian influenza (HPAI H5Nx) with vaccination, compliance, and non-pharmaceutical interventions.

INTRODUCTION: Highly pathogenic avian influenza poses an ongoing zoonotic risk with the potential for severe human outbreaks. Existing epidemic models often assume immediate vaccine effectiveness and homogeneous populations, which may overestimate the impact of vaccination during the early phase of an outbreak. This study develops a time-delayed epidemic model to examine how vaccination timing, delayed immune protection, behavioral heterogeneity, and non-pharmaceutical interventions jointly shape the transmission dynamics of a hypothetical human outbreak of highly pathogenic avian influenza H5

Evaluating empirical calibration of P values under unmeasured confounding bias: a simulation study and real-world application.

OBJECTIVES: To evaluate whether empirical calibration of P values using negative controls can effectively control type I and type II errors under unmeasured confounding bias in both simulated and real-world observational settings. STUDY DESIGN AND SETTING: A simulation study was conducted under five settings reflecting different degrees of adherence to the U-comparability assumption-that is, the extent to which negative controls share the same unmeasured confounding structure as the exposure of interest. These included three primary scenarios (ideal, realistic, and violation of U-comparabilit

Transmission matrix parameter estimation of COVID-19 evolution with age compartments using ensemble-based data assimilation.

The COVID-19 pandemic, with its multiple outbreaks, has posed significant challenges for governments worldwide. Much of the epidemiological modeling relied on pre-pandemic contact information of the population to model the virus transmission between population age groups. However, said interactions underwent drastic changes due to governmental health measures, referred to as non-pharmaceutical interventions. These interventions, from social distancing to complete lockdowns, aimed to reduce transmission of the virus. This work proposes taking into account the impact of non-pharmaceutical measur

Mortality and Ventricular Arrhythmia in Patients with Early Ventricular Repolarization.

BACKGROUND: Recent research has linked early repolarization (ER) with increased ventricular fibrillation risk, especially in leads V1-V3 and in inferior and lateral regions. However, data on the Brazilian population are limited. OBJECTIVE: To estimate the impact of ER on survival and the occurrence of ventricular arrhythmias (VA) in patients over a 10-year period at a university hospital. METHODS: This retrospective cohort study included patients with ER on electrocardiogram from the hospital database. Descriptive analysis was conducted to describe patients' profile and characteristics. Kapl

Implementing Negative Control Outcomes to Assess Comparability of Treatments for Psoriasis and Psoriatic Arthritis.

PURPOSE: Treatment selection is typically associated with prognosis, leading to potential confounding in comparative studies. We used negative control outcomes (NCOs) to identify potential residual confounding when comparing apremilast initiators to other psoriasis (PsO) or psoriatic arthritis (PsA) treatment initiators. METHODS: Adults with PsO/PsA who initiated treatment from September 23/March 21, 2016, respectively, with apremilast, topicals, methotrexate, interleukin (IL)-17 inhibitor (i), IL-12/23i, or tumor necrosis factor inhibitor (TNFi) were identified in the OPTUM Clinformatics Dat

Epidemiological surveillance of Crimean-Congo haemorrhagic fever virus in the Iberian lynx (Lynx pardinus), 2010-2022.

Crimean-Congo haemorrhagic fever virus (CCHFV) is a zoonotic pathogen whose main vector and reservoir are Hyalomma ticks. The recent emergence of the severe disease in humans caused by this virus in southwestern Europe has alerted sanitary authorities about its public health significance. This study was aimed to assess the role of the Iberian lynx (Lynx pardinus) in the epidemiology of CCHFV in the Iberian Peninsula. Of 554 lynxes tested, anti-CCHFV antibodies were detected in 66 animals (11.9 %; 95 % confidence intervals (CI): 9.2-14.6). Age was a risk factor associated with exposure to CCHFV

Twenty-first century epidemiology of dyslipidemia in Greece: EMENO national epidemiological study.

BACKGROUND: Greece was recently reclassified from low- to medium-risk country in terms of cardiovascular disease, with 27% of cardiovascular deaths attributed to hypercholesterolemia. EMENO nationwide survey (2013-2016) assessed the epidemiology of dyslipidemia in the general population in Greece. METHODS: A random sample of adults was drawn by multistage stratified random sampling based on 2011 census. Standardized questionnaires and blood tests for total cholesterol (TC), low-density (LDL-C), and high-density lipoprotein cholesterol (HDL-C), and triglycerides were used. Hypercholesterolemia

National Epidemiological Study about Hepatitis C Virus Infection among Dialysis Patients.

The World Health Organization estimates that 3% of the general population is infected with this virus. Hepatitis C remains the main viral infection in dialysis patients, and the severity of this infection is the risk of developing cirrhosis or hepatocarcinoma. We aim to determine the prevalence of hepatitis C in dialysis patients, to calculate the rate of hepatitis C virus (HCV) seroconversion, and to identify the risk factors for seroconversion. This is a nationwide multicenter observational study including all dialysis patients regardless of age and gender. Those with acute renal failure and

Association Between Fibrosis Stage and Outcomes of Patients With Nonalcoholic Fatty Liver Disease: A Systematic Review and Meta-Analysis.

BACKGROUND & AIMS: Biopsy-confirmed liver fibrosis is a prognostic factor for patients with nonalcoholic fatty liver disease (NAFLD). We performed a systematic review to quantify the prognostic value of fibrosis stage in patients with NAFLD and the subgroup of patients with nonalcoholic steatohepatitis (NASH) and to assess the evidence that change in fibrosis stage is a surrogate endpoint. METHODS: We searched the MEDLINE, Embase, Cochrane Library, and trial registry databases through August 2018 for prospective or retrospective cohort studies of liver-related clinical events and outcomes in

Invasive Pneumococcal Disease in UK Children <1 Year of Age in the Post-13-Valent Pneumococcal Conjugate Vaccine Era: What Are the Risks Now?

BACKGROUND: Invasive pneumococcal disease (IPD) has declined significantly since the introduction of pneumococcal conjugate vaccines (PCVs). It is not known whether certain infant populations remain at higher risk of IPD in countries with established 13-valent PCV (PCV13) programs. We aimed to describe the epidemiology, clinical characteristics, serotype distribution, and outcomes of IPD in infants, and to estimate the relative risk of PCV13-type, non-PCV13-type, and overall IPD in premature infants compared to term infants during a 4-year period after the PCV13 program was established. Prevalence of back pain in a group of elite athletes exposed to repetitive overhead activity.

BACKGROUND: The prevalence of back pain in athletes has been investigated in several studies, but there are still under- or uninvestigated sports discipline like sports exposed to repetitive overhead activity. Elite athletes spend much time in training and competition and, because of the nature of their disciplines, subject their bodies to a great deal of mechanical strain, which puts a high level of stress on their musculoskeletal systems. From this it is hypothesized that elite athletes who engage in repetitive overhead motions experience a higher strain on their spine and thus possibly a hi

Timing of influenza epidemics and vaccines in the American tropics, 2002-2008, 2011-2014.

BACKGROUND: Influenza-associated illness results in increased morbidity and mortality in the Americas. These effects can be mitigated with an appropriately chosen and timed influenza vaccination campaign. To provide guidance in choosing the most suitable vaccine formulation and timing of administration, it is necessary to understand the timing of influenza seasonal epidemics. OBJECTIVES: Our main objective was to determine whether influenza occurs in seasonal patterns in the American tropics and when these patterns occurred. METHODS: Publicly available, monthly seasonal influenza data from t

Propensity Score-Based Approaches to Confounding by Indication in Individual Patient Data Meta-Analysis: Non-Standardized Treatment for Multidrug Resistant Tuberculosis.

BACKGROUND: In the absence of randomized clinical trials, meta-analysis of individual patient data (IPD) from observational studies may provide the most accurate effect estimates for an intervention. However, confounding by indication remains an important concern that can be addressed by incorporating individual patient covariates in different ways. We compared different analytic approaches to account for confounding in IPD from patients treated for multi-drug resistant tuberculosis (MDR-TB). METHODS: Two antibiotic classes were evaluated, fluoroquinolones--considered the cornerstone of effec

Real-time, portable genome sequencing for Ebola surveillance.

The Ebola virus disease epidemic in West Africa is the largest on record, responsible for over 28,599 cases and more than 11,299 deaths. Genome sequencing in viral outbreaks is desirable to characterize the infectious agent and determine its evolutionary rate. Genome sequencing also allows the identification of signatures of host adaptation, identification and monitoring of diagnostic targets, and characterization of responses to vaccines and treatments. The Ebola virus (EBOV) genome substitution rate in the Makona strain has been estimated at between 0.87 × 10(-3) and 1.42 × 10(-3) mutations

Potential risks associated with increased plasma plant-sterol levels.

The consumption of plant sterols is associated with a decrease in LDL cholesterol. However, it is also associated with an increase in plasma plant-sterol (sitosterol, campesterol) levels that may be detrimental. Indeed, the genetic disease sitosterolaemia, which is characterized by elevated plasma levels of plant sterol, is associated with premature atherosclerosis. Yet, although plasma plant-sterol levels are recognized markers of cholesterol absorption, the relationship between such levels and atherosclerosis is not clear. Several studies have analysed the association between plasma plant-st

The emergence and spread of finch trichomonosis in the British Isles.

Finch trichomonosis, caused by the protozoal parasite Trichomonas gallinae, was first recognized as an emerging infectious disease of British passerines in 2005. The first year of seasonal epidemic mortality occurred in 2006 with significant declines of greenfinch Carduelis chloris and chaffinch Fringilla coelebs populations. Here, we demonstrate that large-scale mortality, principally of greenfinch, continued in subsequent years, 2007-2009, with a shifting geographical distribution across the British Isles over time. Consequent to the emergence of finch trichomonosis, the breeding greenfinch
