Fighting a Plague: Doctors' Stories of Challenge and Innovation Combatting the AIDS Epidemic in 1980s New York City.

The human immunodeficiency virus (HIV) has devastated the United States for forty years. Though there are highly effective treatments for HIV and acquired immune deficiency syndrome (AIDS) today, the early years of the HIV/AIDS epidemic in New York City (NYC) were filled with uncertainty, fear, and death-not unlike the period we are now experiencing in the COVID-19 era. Existing scholarship captures the political discourse of the HIV/AIDS era and the narratives of physicians who specialized in HIV medicine. This essay uses eight in-depth interviews of physicians of various specialties who work

Personality of hosts and their brood parasites.

Brood parasites such as the common cuckoo Cuculus canorus exploit the parental abilities of their hosts, hosts avoid brood parasitism and predation by showing specific behavior such as loss of feathers, emission of fear screams and contact calls, displaying wriggle behavior to avoid hosts or potential prey, pecking at hosts and prey, and expressing tonic immobility (showing behavior like feigning death or rapid escape from predators and brood parasites). These aspects of escape behavior are consistent for individuals but also among sites, seasons, and years. Escape behavior expressed in respon

High-definition transcranial direct current stimulation facilitates emotional face processing in individuals with high autistic traits: A sham-controlled study.

The deficit in emotional face processing is a critical impairment for individuals with high autistic traits. The temporalparietal junction(TPJ) is considered to be closely related to emotional face processing. The aim of this study was to examine the effect of highdefinition transcranial direct current stimulation (HD-tDCS) over the right temporal-parietal junction (rTPJ) on facial emotion processing of individuals with high autistic traits using eye-tracking technology. Twenty-nine participants with high autistic traits completed an eyetracking task (including happy, fearful and neutral faces

Fact vs Fallacy: The Anti-Vaccine Discussion Reloaded.

In the light of the COVID-19 pandemic, anti-vaccine sentiments have been on the rise, with a recent seminal study on the development of anti-vaccine views in social media even making its way into Nature Communications. Yet, with the current scientific consensus being in overwhelming agreement over the safety and efficacy of vaccines, many scientists lose their grasp on the fears, concerns, and arguments that the opposition may hold. This paper discusses and evaluates vaccine-hesitant individuals on a socioeconomic, historical, and philosophical landscape. It also provides an analysis of common

Biased Attention to Facial Expressions of Ambiguous Emotions in Borderline Personality Disorder: An Eye-Tracking Study.

Preliminary evidence suggests that biased attention could be crucial in fostering the emotion recognition abnormalities in borderline personality disorder (BPD). We compared BPD patients to Cluster-C personality disorder (CC) patients and non-patients (NP) regarding emotion recognition in ambiguous faces and their visual attention allocation to the eyes. The role of comorbid posttraumatic stress disorder (PTSD) in BPD regarding emotion recognition and visual attention was explored. BPD patients fixated the eyes of angry/happy, sad/happy, and fearful/sad blends longer than non-patients. This vi

The enduring legacy of black lung: environmental health and contested illness in Appalachia.

Over the past twenty years there has been a deadly resurgence of coal workers' pneumoconiosis (CWP), commonly known as black lung disease. While increased prevalence of the disease is alarming, these data only capture cases where CWP has been officially recognised. We argue that many more cases of the disease are going unreported. Drawing from contested environmental illness literature, we examine issues surrounding diagnostic uncertainty and medical surveillance. We draw from qualitative data on black lung that includes in-depth interviews, observation and document analysis. Findings indicate

Sleep enhances exposure therapy.

BACKGROUND: Sleep benefits memory consolidation. Here, we tested the beneficial effect of sleep on memory consolidation following exposure psychotherapy of phobic anxiety. METHOD: A total of 40 individuals afflicted with spider phobia according to DSM-IV underwent a one-session virtual reality exposure treatment and either slept for 90 min or stayed awake afterwards. RESULTS: Sleep following exposure therapy compared with wakefulness led to better reductions in self-reported fear (p = 0.045, d = 0.47) and catastrophic spider-related cognitions (p = 0.026, d = 0.53) during approaching a live

Thromboprophylaxis in the oldest old with atrial fibrillation: between Scylla and Charybdis.

Within a few decades half of the patients with non-valvular atrial fibrillation (AF) will be older than 80 years. These patients are at particularly high risk of thromboembolism (Scylla) but also of the hemorrhagic complications of thromboprophylaxis (Charybdis). A frequent compromise is to use as antithrombotic agents instead of vitamin-K antagonists aspirin or other antiplatelet drugs, which are ineffective for thromboprophylaxis in the oldest old and definitely not free from a high risk of severe bleeding. All the new direct anticoagulants currently licensed (dabigatran, rivaroxaban, apixab

Impact of unfounded vaccine safety concerns on the nationwide measles-rubella immunization campaign, Georgia, 2008.

Vaccine safety fears following media reports of adverse events led to low (50.3%) coverage in a supplementary measles-rubella immunization campaign in Georgia in 2008. Review of adverse events associated with the campaign identified 432 reports (<0.1% of ∼ 493,000 vaccinees) including 338 (78.2%) cases of syncope. There were no deaths. Causality assessment was performed for 79 cases perceived by providers as severe and with clinical details available. Conditions likely caused by the vaccine were identified in 13 (16.5%) cases (allergic and local reactions, thrombocytopenia). Thirty-seven (46.8

[The silent itinerary of people with HIV told through oral history].

The emergence of AIDS as a social and historical phenomenon has brought along with it specters held in social imagination, aggravating the concept of plague, generating strong feelings and prejudice, becoming a stigma. Such factors ultimately affect HIV bearers' well being. Thus, this study aims to understand the dimension of the problem of HIV bearers who have not seeked treatment in the National Health System and the reasons which make this search so difficult. The adopted methodological approach was thematic oral accounts. Five HIV bearers collaborated in this study. By using content analys

An experimental analysis of hypochondriasis.

Hypochondriasis (HC) involves preoccupation with fears of having a serious medical illness based on the misinterpretation of benign bodily perturbations. Individuals with HC also perform behaviors such as checking and reassurance-seeking presumably to reduce health-related fears. Experimental behavioral analyses of HC symptoms, however, are lacking. In the present study, 27 patients with HC were exposed to personally relevant health-related stimuli under one of two conditions: (a) subsequently performing safety-seeking behaviors (e.g., checking) (n=14) or (b) subsequently being instructed not

Emotional and psychiatric issues in hypertrophic cardiomyopathy and other cardiac patients.

Hypertrophic cardiomyopathy (HCM) exposes young patients to the risk of sudden death. The risk of sudden death separates the young patient from his peers, but connects him more closely to his physician. The physician may have a powerful effect replacing uninformed fears with accurate knowledge, restoring hope, and helping the patient shift focus from the fear of dying to engaging in life and living with a medical illness. Depression and panic disorder are two psychiatric illnesses that are common in cardiac practices, cause significant morbidity and mortality, and may be overlooked and undertr

Misrepresentations about palliative options and prognosis in motor neurone disease: some legal considerations.

If euthanasia were legalized, clinicians would be under a duty to explain to patients requesting euthanasia what the prognosis and palliative options were. Anecdotal evidence suggests that some patients suffering from motor neurone disease may request euthanasia because of a fear of choking to death. The literature indicates that with competent palliative care this is unlikely to occur. It is assumed, for the purposes of argument, that responsible clinicians would accordingly reassure patients that such a fear was unwarranted, and that such a reassurance would cause patients for whom choking w

A trial of a relapse prevention strategy in women with bulimia nervosa who respond to cognitive-behavior therapy.

OBJECTIVE: This study examines a relapse prevention strategy for bulimia nervosa (BN). Subjects in a multicenter BN treatment trial who initially achieved abstinence after a course of cognitive-behavioral therapy (CBT) were told to recontact the clinic if they had a recurrence of symptoms or feared such a reoccurrence so that they could receive additional therapy visits. METHOD: At the end of CBT, subjects whose scores on the Eating Disorders Examination indicated that they were abstinent from binge eating and purging, and therefore considered to be treated successfully, were assigned randoml

Coping strategies of panic and control subjects undergoing lactate infusion during magnetic resonance imaging confinement.

The psychological reactions and coping strategies used in response to a behavioral contingency (confinement in a magnetic resonance imaging [MRI] scanner) during a biological challenge (sodium lactate infusion) were systematically studied in 13 subjects with panic disorder (PD) and 11 control subjects using the Claustrophobia Questionnaire (CQ) and the Revised Ways of Coping Checklist (RWCCL). All participants were able to successfully complete the experimental procedure. Findings suggest between-group coping strategy differences in response to general stressors, but relative convergence of co

Ten-year stability and predictive validity of five bulimia-related indicators.

OBJECTIVE: The authors examined the temporal stability and predictive utility of bulimic symptoms and related variables over the course of 10 years, from 1982 to 1992. METHOD: The subjects were 459 women who were aged 18-22 years in 1982 and were surveyed in both 1982 and 1992. Each respondent completed five subscales of the Eating Disorders Inventory (bulimia, drive for thinness, maturity fears, perfectionism, and interpersonal distrust) and answered questions based on the DSM-III criteria for bulimia nervosa. RESULTS: The temporal stability of bulimic symptoms and related variables was rel

Occupational and personal risk estimates for HIV contagion among incoming graduate nursing students.

In an effort to measure both fear of HIV/AIDS contagion and possible co-factors, two cohorts (n = 331) of incoming graduate level nursing students were surveyed during two consecutive academic years. All completed a Risk Perception Scale and an AIDS Attitude Scale; additionally, 118 completed an AIDS Knowledge Scale. Findings indicate this population had tremendous fears of the possibility of HIV infection. Estimates of potential contagion far outweighed scientific estimates of risk, and in some cases corresponded to behaviors that are scientifically purported to have no risk. Of interest in t

Characteristics of Venezuelan school refusers. Toward the development of a high-risk profile.

Parent, teacher, and child reports were used to identify situational and personal factors associated with school refusal in 114 3- to 13-year-old Venezuelan children. The sample consisted of 57 school refusers and 57 nonrefusers matched on age, school, and sex. As compared with nonrefusers, the refusers had changed schools more often, were rated as more dependent, had more school-related fears, and were perceived by their parents as more difficult to manage. Stepwise multiple regression analyses revealed that school refusal status could be predicted by both situational and personality variable

Examining Suicidal Ideation and Capability Across the Menstrual Cycle.

INTRODUCTION: Suicidal thoughts and behaviors have been linked to menarche and the perimenstrual phase of the menstrual cycle. This study examines whether acute changes in fearlessness about death (FAD) occur across the menstrual cycle. It also explores relationships between FAD and other known correlates of suicidal behaviors across the cycle. METHOD: The sample included 46 participants in good health, with natural menstrual cycles, past-month suicidal ideation, and ongoing outpatient mental health. Participants were excluded for exogenous hormone or cycle-altering medication use, nicotine u
