The results of utilizing proximal tibial bone graft in reconstructive procedures of the foot and ankle.

BACKGROUND: The proximal tibia is becoming a popular site for harvesting autogenous bone graft. Reports on the use of the proximal tibia for bone graft have focused on donor-site morbidity. The purpose of this study is to determine the clinical rate of bony healing following the use of proximal tibial bone graft in foot and ankle procedures. METHODS: This study is a retrospective review of consecutive patients who underwent proximal tibial bone grafting as an adjunct to a foot and ankle reconstructive procedure. Patients undergoing this combination of procedures at a single institution over a…

Long-term outcomes of patients undergoing war-related amputations of the foot and ankle.

Long-term clinical and functional outcomes for patients undergoing foot and ankle amputations are not well documented. We attempted to document long-term outcomes for patients who required lower extremity amputations as a result of wounds suffered during wartime. For this study, 27 Iranian soldiers who had wounds requiring amputation of the foot and ankle were selected for follow-up. The participants' wartime medical records were reviewed, a clinical examination was performed, and each participant completed a questionnaire. Postamputation follow-up averaged 17.5 years. The most prevalent (66.6…

Empirical therapy for diabetic foot infections: are there clinical clues to guide antibiotic selection?

Initial antibiotic therapy for diabetic foot infections is usually empirical. Several principles may help to avoid selecting either an unnecessarily broad or inappropriately narrow regimen. First, clinically severe infections require broad-spectrum therapy, while less severe infections may not. Second, aerobic Gram-positive cocci, particularly Staphylococcus aureus (including methicillin-resistant S. aureus (MRSA) for patients at high-risk) should always be covered. Third, therapy should also be targeted at aerobic Gram-negative pathogens if the infection is chronic or has failed to respond to…

Foot-and-mouth disease virus crosses the placenta and causes death in fetal lambs.

Eighteen pregnant sheep, six at 45 days gestation and twelve at 75 days gestation, were infected with foot-and-mouth disease virus (FMDV) type O UKG 34/2001. Two sheep from each gestational group were killed at 2, 4, and 7 days post-inoculation (dpi). Three sheep, pregnant for 75 days at infection, were killed at 17 and 18 dpi. Real-time reverse-transcriptase polymerase chain reaction (RT-PCR) and virus isolation (VI) were used to detect viral RNA and infectious virus, respectively, in fetal tissues taken post mortem. Eleven fetuses were obtained from the six sheep inoculated at day 45 of gest…

Severe post-burn hyperextension of metatarsophalangeal joints in a child with bilateral foot contracture.

Achieving a stable plantigrade foot after repair of complex post-burn deformities poses a particularly difficult challenge for the foot and ankle surgeon. We report an unusual case of a child with severe bilateral forefoot contracture deformities treated by soft tissue release and lengthening, and conventional wound coverage with split-thickness skin grafting.

A biomechanical analysis of the effect of lateral column lengthening calcaneal osteotomy on the flat foot.

BACKGROUND: Biomechanical models have been used to study the plantar aponeurosis, medial arch height, subtalar motion, medial displacement calcaneal osteotomy, subtalar arthroereisis and the distribution of forces in the normal and flat foot. The objective was to examine the hypothesis that increased load on the medial arch in the adult flat foot can be reduced through a 10mm lateral column lengthening calcaneal osteotomy 10 mm proximal from the calcaneal cuboid joint. METHODS: A three dimensional multisegment biomechanical model was used with anatomical data from a normal foot, a flat foot a…

Cold-induced vasodilatation in the foot is not homogenous or trainable over repeated cold exposure.

Cold-induced vasodilatation (CIVD) is proposed to be a protective response to preserve tissue integrity in the extremities during cold exposure, but little research exists on either the trainability or the spatial pattern of CIVD response in the foot. We investigated the thermal response across the foot with repeated cold exposure. Ten healthy subjects immersed their left foot to the ankle in 8 degrees C water for 30 min 5 days/week for 3 weeks. Skin temperature was recorded on the medial side of the nail bed of the 5 toes and the dorsum of the foot. The presence of CIVD, defined as an increas…

The importance of proper shoe gear and safety stirrups in the prevention of equestrian foot injuries.

The aim of this study was to compile specific foot injuries occurring in pediatric patients that result from equestrian sports and to highlight the importance of wearing adequate riding boots to protect the feet. During a 12-year period, 258 children were admitted to Children's Hospital of Geneva for injuries resulting from horseback riding. Amongst these children, 8 sustained foot lesions that required hospital admission. Four children had compression-type fractures of the cuboid (nutcracker fracture of the cuboid) associated with other complex midfoot fractures, 2 had Lisfranc fracture dislo…

Magnetic resonance imaging in the evaluation of sports injuries of the foot and ankle: a pictorial essay.

BACKGROUND: Whether bone markers have prognostic value in patients with bone metastases is unknown. We investigated this question in patients with bone metastases secondary to prostate cancer and to non-small-cell lung cancer (NSCLC) and other solid tumors assigned to the placebo arms of two phase III trials of zoledronic acid. METHODS: Levels of the urinary bone resorption marker N-telopeptide and the serum bone formation marker bone-specific alkaline phosphatase were assessed every 3 months for patients with prostate cancer (n = 203) or NSCLC or other solid tumors (n = 238) and were categor

[Outcome of minor amputations at the diabetic foot in relation to bone histopathology: a clinical audit].

AIM OF THE STUDY: to assess the clinical course of minor amputations at the diabetic foot in relation to bone histopathology at the affected site, and at the osteotomy site. PATIENTS AND METHODS: A total of 54 toes or metatarsal bones were amputated by general surgeons. Amputation level and mode of wound closure (primary wound closure) was left at the surgeons' discretion. The subjects were 45 diabetic patients, suffering from critical foot ischaemia (n = 27), polyneuropathy (n = 41) and chronic hemodialysis because of endstage renal failure (n = 4). RESULTS: Histopathology revealed osteomye…

Epithelioid sarcoma of the foot with subsequent lesion in hand: metastatic lesion or second primary?

Epithelioid sarcomas (ES) are rare tumors of soft tissue that have a propensity to occur in the extremities. Epithelioid sarcomas are known to metastatise to draining lymph nodes and commonly to the lungs. Herein, a case of epithelioid sarcoma which recurred in an unusual site namely the distal phalanx of left middle finger, six months post amputation of the primary lesion in the left foot is being reported. The ipsilateral inguinal lymph node showed metastatic deposits. The tumor at both these sites had similar histology and an identical immunohistochemical (IHC) pattern showing reactivity to…

A clinico-microbiological study of diabetic foot ulcers in an Indian tertiary care hospital.

OBJECTIVE: To determine the microbiological profile and antibiotic susceptibility patterns of organisms isolated from diabetic foot ulcers. Also, to assess potential risk factors for infection of ulcers with multidrug-resistant organisms (MDROs) and the outcome of these infections. RESEARCH DESIGN AND METHODS: Pus samples for bacterial culture were collected from 80 patients admitted with diabetic foot infections. All patients had ulcers with Wagner's grade 3-5. Fifty patients (62.5%) had coexisting osteomyelitis. Gram-negative bacilli were tested for extended spectrum beta-lactamase (ESBL) p…

Molecular epidemiology of the foot-and-mouth disease virus outbreak in the United Kingdom in 2001.

The objective of this study was to quantify the extent to which the genetic diversity of foot-and-mouth disease virus (FMDV) arising over the course of infection of an individual animal becomes fixed, is transmitted to other animals, and thereby accumulates over the course of an outbreak. Complete consensus sequences of 23 genomes (each of 8,200 nucleotides) of FMDV were recovered directly from epithelium tissue acquired from 21 farms infected over a nearly 7-month period during the 2001 FMDV outbreak in the United Kingdom. An analysis of these consensus sequences revealed very few apparently …

Sports injuries in the pediatric and adolescent foot and ankle: common overuse and acute presentations.

Care of the youth athlete requires knowledge of developmental anatomy and specific injury patterns, which are acute or chronic in nature. We may expect that the incidence of overuse and acute foot and ankle injuries in this population is likely to increase in proportion to the number and intensity of competitive youth teams with demanding training schedules. We, as physicians, must exercise our best judgment in regard to recognizing these patterns early and instituting appropriate treatments. Return to play decisions should be based on objective criteria when available and always keeping the b…

[Preservation of the foot in a five-year-old child with bilateral congenital deficiency of the tibia].

Congenital tibial deficiency is a rare anomaly causing shortening and varus deformity of the lower extremity. Recent limb lengthening and foot repositioning techniques enable functional results with preservation of the foot. We present a five-year-old boy with bilateral congenital tibial deficiency of type 2 according to the Jones classification, who was treated with tibia-fibular fusion without Syme amputation. His ambulation depended on crawling. Radiographic examination showed a normally developed fibula with thickening in the distal portion, and only proximal tibia with absence of the dist…

[Tuberculous osteoarthritis of the foot in the diabetic: not unrecognize this diagnostic trap. About one case].

Osteoarticular tuberculosis is the fourth leading extrapulmonary localization of tuberculosis. The disease has a progressive course and is often diagnosed in the stage of bone destruction, causing an important diagnostic problem in diabetics with nervous osteoarthropathy. We report the case of a 23-year-old patient with multicomplicated diabetes type 1, treated for pulmonary tuberculosis who consulted after a trauma distended the left ankle. Bone biopsy was performed because of the diagnostic doubt between diabetes and infectious osteoarthropathy. Pathology reported active tuberculosis. Osteoa
