Clinical and laboratory genetic counselor attitudes on the reporting of variants of uncertain significance for multigene cancer panels.

Research suggests variants of uncertain significance (VUSs) present a variety of challenges for genetic counselors (GCs), nongenetics clinicians, and patients. Multigene cancer panels reveal more VUSs than single gene testing as a result of the increase in the number of genes being tested. This study surveyed 87 clinical cancer GCs involved with direct patient care and 19 laboratory GCs who provide guidance to clinicians regarding genetic test results about their attitudes on various options for the reporting of VUSs by laboratories for broad multigene cancer panels. Independent samples t-test

Influenza A viruses circulating in dogs: A review of the scientific literature.

Influenza A viruses (IAV) cause persistent epidemics and occasional human pandemics, leading to considerable economic losses. The ecology and epidemiology of IAV are very complex and the emergence of novel zoonotic pathogens is one of the greatest challenges in the healthcare. IAV are characterized by genetic and antigenic variability resulting from a combination of high mutation rates and a segmented genome that provides the ability to rapidly change and adapt to new hosts. In this context, available scientific evidence is of great importance for understanding the epidemiology and evolution o

Associations Between APOE Variants, Tau and α-Synuclein.

Neurodegenerative diseases are characterized by the aggregation and deposition of misfolded proteins in the brain, most prominently amyloid-β (Aβ), tau and α-synuclein (α-syn), and are thus referred to as proteinopathies. While tau is a hallmark of Alzheimer's disease (AD) and other non-AD tauopathies, and α-synuclein is the pathological feature of the spectrum of synucleinopathies including Parkinson's disease (PD), Parkinson's disease with dementia (PDD) and dementia with Lewy bodies (DLB), the presence of co-pathologies is very frequent in all these diseases. Positive and synergistic associ

The mutational burden in os odontoideum patients.

BACKGROUND: Os odontoideum(OO) is a rare bone malformation at the craniovertebral junction, the presence of which can lead to potential instability of atlantoaxial joints. The cause, prevalence and treatment of OO are still controversial. There are two main theories regarding the pathogenesis of OO: congenital factors and traumatic factors. The congenital hypothesis suggests that the lesion results from segmental defects caused by incomplete fusion of the dens and the axis vertebral body during embryonic development. Our research team previously reported a case series involving three generatio

Association of IL-10 promoter and IL-12 gene polymorphisms with the risk of symptomatic Helicobacter pylori infection.

BACKGROUND: The host's immune response to Helicobacter pylori (H. pylori) infection is largely determined by its cytokine profile. Genetic variations within crucial immunomodulatory genes, including those for interleukin-10 (IL-10) and interleukin-12 (IL-12), are thought to influence an individual's vulnerability to the infection and its clinical consequences by modifying cytokine production. Nonetheless, research data derived from diverse human populations continue to show inconsistent results. AIM: This case-control analysis sought to examine a potential link between symptomatic H. pylori i

Over-expression of XIST, the Master Gene for X Chromosome Inactivation, in Females With Major Affective Disorders.

BACKGROUND: Psychiatric disorders are common mental disorders without a pathological biomarker. Classic genetic studies found that an extra X chromosome frequently causes psychiatric symptoms in patients with either Klinefelter syndrome (XXY) or Triple X syndrome (XXX). Over-dosage of some X-linked escapee genes was suggested to cause psychiatric disorders. However, relevance of these rare genetic diseases to the pathogenesis of psychiatric disorders in the general population of psychiatric patients is unknown. METHODS: XIST and several X-linked genes were studied in 36 lymphoblastoid cell li

Uveal melanoma pathobiology: Metastasis to the liver.

Uveal melanoma (UM) is a type of intraocular tumor with a propensity to disseminate to the liver. Despite the identification of the early driver mutations during the development of the pathology, the process of UM metastasis is still not fully comprehended. A better understanding of the genetic, molecular, and environmental factors participating to its spread and metastatic outgrowth could provide additional approaches for UM treatment. In this review, we will discuss the advances made towards the understanding of the pathogenesis of metastatic UM, summarize the current and prospective treatme

The social transformation of Singapore medicine through 55 years of the SMJ.

The Singapore Medical Journal (SMJ) has in the past 55 years reflected the phenomenal socioeconomic progress of Singapore. Publications in the pre-independent years were mainly on diseases like cholera, diphtheria, leprosy, tetanus and worm infestation. In the new millennium, the research papers included molecular genetics, health economics, obesity, Internet medicine, cancer, cosmetic surgery and palliative medicine. The annual SMA Lecture published in the SMJ provides an ethical compass for doctors to remind them of primum non nocere.

Marfan syndrome.

Marfan syndrome is a heritable disorder of the connective tissue that affects the cardiovascular, skeletal and ocular systems, and often involves the skin, nervous system and lungs. Historically, a person with Marfan syndrome had a poor prognosis due to the cardiovascular effects of this disorder. The life expectancy has improved dramatically over the past 30 years for individuals with Marfan syndrome because of aggressive medical and molecular research and advances in surgical technology. This article discusses the history, genetics, manifestations, diagnosis and treatment of Marfan syndrome.

Pathogenesis of Tourette's syndrome.

This review presents a models of disease pathogenesis in the context of CNS development. It begins with an exploration of the clinical features and natural history of Tourette's syndrome. This is followed by a consideration of the role of genetic and nongenetic factors. An effort is then made to review the anatomical organization of the basal ganglia and related cortical sites. These circuits are intimately involved in the normal processing of sensorimotor, cognitive, and emotionally laden information. Evidence implicating these circuits in the pathobiology of Tourette's syndrome is then consi

Animal models of blastomycosis.

Mice offer many advantages as a model system for the study of experimental blastomycosis. The murine model has allowed quantitative assessments of disease initiation and progression. These methods of assessment have enabled studies of histopathology, and of the effect of age, host genetic background, fungal isolate virulence, and different routes of challenge on the outcome of infection. The model has been a particularly fertile soil for researching dissection of the immune response to fungal infections and the study of new agents (particularly polyenes and azoles) for the treatment of the myc

Heterogeneity and natural history of hereditary breast cancer. Surgical implications.

The properly compiled cancer family history can prove to be one of the most cost-effective and powerful instruments for cancer control in breast cancer. Needed, however, is an understanding of the heterogeneity of hereditary breast cancer and knowledge of those facets of its natural history that can expedite a syndrome diagnosis. Priority concerns for cancer control in hereditary breast cancer are the development of registries of cancer-prone families, willingness by third-party carriers to help defray the costs of surveillance, and more research in molecular genetics.

Heterotopic Ossification: Molecular Drivers, Subtype-Specific Mechanisms, and Translational Therapeutic Advances.

Heterotopic ossification (HO), the pathological formation of mature bone in non-skeletal soft tissues (e.g., muscles, tendons), severely impairs patient mobility and quality of life. Despite decades of research, systematic analysis of signaling networks across HO subtypes (acquired traumatic HO, hereditary Fibrodysplasia Ossificans Progressiva (FOP), Progressive Osseous Heteroplasia (POH)) remains insufficient, and clinical therapies suffer from high recurrence and severe side effects. This review synthesizes recent advances in HO pathogenesis: FOP involves gain-of-function activin A receptor

Organoids as Novel Models for Embryo Implantation Study.

In the last decade, organoids have become emerging novel models for biomedical research. Organoids are small, self-organized three-dimensional (3D) tissue cultures derived from stem cells that mimic certain tissues or organs. In reproductive medicine, researchers have generated numerous organoids including blastoid (blastocyst organoid), endometrial organoid, and trophoblast organoid. These organdies provide useful models for studying the embryo implantation mechanism through observation of cell differentiation, gene expression, and epigenetic profiles at the implantation stage. As in vitro ti

Women's perspectives on the ethical implications of non-invasive prenatal testing: a qualitative analysis to inform health policy decisions.

BACKGROUND: Non-Invasive Prenatal Testing (NIPT) is a technology which provides information about fetal genetic characteristics (including sex) very early in pregnancy by examining fetal DNA obtained from a sample of maternal blood. NIPT is a morally complex technology that has advanced quickly to market with a strong push from industry developers, leaving many areas of uncertainty still to be resolved, and creating a strong need for health policy that reflects women's social and ethical values. We approach the need for ethical policy-making by studying the use of NIPT and emerging policy in t

Intracoronary Cytoprotective Gene Therapy: A Study of VEGF-B167 in a Pre-Clinical Animal Model of Dilated Cardiomyopathy.

BACKGROUND: Vascular endothelial growth factor (VEGF)-B activates cytoprotective/antiapoptotic and minimally angiogenic mechanisms via VEGF receptors. Therefore, VEGF-B might be an ideal candidate for the treatment of dilated cardiomyopathy, which displays modest microvascular rarefaction and increased rate of apoptosis. OBJECTIVES: This study evaluated VEGF-B gene therapy in a canine model of tachypacing-induced dilated cardiomyopathy. METHODS: Chronically instrumented dogs underwent cardiac tachypacing for 28 days. Adeno-associated virus serotype 9 viral vectors carrying VEGF-B167 genes we
