Cholesterol granulomas are benign, foreign body giant cell reactions that occur in response to the presence of cholesterol crystals. Cholesterol granulomas are an exceedingly rare entity in the mediastinum and are often found incidentally without a known cause. In this case report, we describe a cholesterol granuloma developing in the anterior mediastinum secondary to a prolonged episode of pericarditis. In addition to providing unique insights into the etiology of cholesterol granulomas of the anterior mediastinum, this report also demonstrates the feasibility of robotic surgical approaches f

Pigmented villonodular synovitis (PVNS) of the tendon sheath (tenosynovial giant cell tumor) is a rare disorder that is often misdiagnosed because of slow growth and an atypical presentation. Open surgical excision is the treatment of choice, although recurrence is a common complication. In this report, a case of PVNS of the flexor hallucis longus tendon in the ankle and hindfoot is described. The diagnosis was confirmed by endoscopic inspection and biopsy, and complete synovectomy was performed through the endoscope. The patient remained disease free after 24 months of follow up.

Between 1993 and 2003, through experiments involving Drosophila sp., cancer biologists identified the protein kinase known as the mammalian target of rapamycin, its pathway, and its relationship to the genes responsible for tuberous sclerosis. Thereafter, clinical research has resulted in regulatory approval of mTOR inhibitors for four distinct manifestations of the disease: giant cell astrocytoma, angiomyolipoma, lymphangioleiomyomatosis, and epilepsy. These developments are summarized and the practical use of mTOR inhibitors to improve the lives of patients with tuberous sclerosis reviewed.

Denosumab, a human monoclonal antibody that inhibits bone resorption by binding on the receptor activator of the nuclear factor kappa-β ligand, has recently emerged as an additional option in the treatment of musculoskeletal osteolytic tumors. This article focuses on the recent literature regarding the effectiveness of denosumab in the management of giant cell tumor, multiple myeloma, aneurysmal bone cyst, and osteosarcoma. The mechanism of action of denosumab in the management of these tumors and the associated side effects are discussed in detail. [ Orthopedics. 2017; 40(4):204-210.].

Germ cell tumors compromise 15-20% of all anterior mediastinal masses; 50-60% of these are benign mediastinal teratoma. There may be mature, immature, and rarely with malignant component within the tumor mass. There are more chances of malignancy with immature type. We are reporting a case in 20-year young male diagnosed as giant benign cystic teratoma which was adherent to superior vena cava. The patient underwent surgical excision. In follow up of 2 years, the patient is not having any complaints.

An 80 year old man presented subacutely with drowsiness and confusion. Subsequent MRI brain imaging demonstrated multiple posterior circulation infarcts. Extracranial vasculitis was suspected when his ESR was found to be high and vascular imaging showed multifocal irregular narrowing of both vertebral arteries. This was confirmed by targeted temporal artery biopsy, which showed chronic granulomatous inflammation typical of giant cell arteritis (GCA). The patient made a significant recovery following treatment with prednisolone.

Rhabdomyomas are the most common benign cardiac tumours. They are often associated with tuberous sclerosis and can be diagnosed antenatally and postnatally by echocardiography. Rhabdomyomas tend to regress spontaneously and are not usually operated upon, unless they become obstructive or cause severe arrhythmias. We describe the case of a child with tuberous sclerosis who was admitted for the resection of a subependymal giant cell astrocytoma, in whom cardiac rhabdomyomas in the right ventricular outflow tract were diagnosed. These two kinds of tumours are well known in the setting of tuberous

Inflammatory rheumatic diseases with first onset in advanced age have some specific clinical features. Late-onset rheumatoid arthritis and polymyalgia rheumatica/giant cell arteriitis are the most relevant rheumatic diseases among older patients. They are characterized by acute onset, early functional impairment, and difficult differential diagnosis. First-line therapy usually consists of glucocorticoids. During long-term therapy, a spectrum of immunosuppressive agents and biologicals can also be used in elderly patients.

Calcinosis cutis is a condition of accumulation of calcium salts within the dermis. We are presenting four cases of calcinosis cutis, with different clinical presentations, occurring in healthy individuals, with normal serum calcium and phosphorus levels. Histologically, all cases showed similar morphology, the lesions were composed of large and small deposits of calcium. Foreign-body giant cell reaction was seen in one case. Another case had intact and ruptured epidermal cysts and calcification within the cyst.

An 84-year-old woman presented with bilateral visual loss that had appeared 3 days previously. Split lamp examination showed bilateral corneal edema with normal intraocular pressure. The patient complained of headache and vomiting, and finally collapsed. Elevated levels of inflammation markers led to the suspicion of an inflammatory disease. After investigation for internal or neurological diseases, a biopsy of the temporal artery was performed. Giant cell arteritis (Horton's disease) was found, and steroid therapy was begun. The patient's general condition then improved.

Basal cell carcinoma (BCC) is the most common malignancy of the body and most frequently occurs in the head and neck. 'Problematic aggressive' BCCs are either frequently recurrent, often after histologically confirmed clearance, or are 'giant/horrifying' and invasive of critical anatomy. Three patients that illustrate different clinical features and outcomes of 'problematic aggressive' craniofacial BCCs are reported that required craniofacial resection and reconstruction with microsurgical free flap cover, and a re-evaluation of our care pathway for these uncommon presentations.

Vasculitis represents an uncommon but important group of disease entities that may affect older patients. The most common vasculitic disease in humans in giant cell arteritis, a disease process seen almost exclusively in patients older than 50 years in age. Vasculitic disease in geriatric patients presents unique challenges with regard to diagnosis and treatment. A thorough understanding of the vasculitic disease entities that may affect older patients as well as their diagnosis and management is essential in minimizing disease and treatment-related morbidity and mortality.

Noninfectious inflammatory diseases (connective tissue diseases, vasculitis syndromes, granulomatous diseases) emerged as the most frequent cause of fever of unknown origin in western countries. Among these diseases, giant cell arteritis and polymyalgia rheumatica are the most frequent specific diagnosis in the elderly and adult-onset Still's disease the most frequent in younger patients. This article focuses on noninfectious inflammatory diseases as a cause of classic fever of unknown origin (mainly rheumatic diseases, such as vasculitis and connective tissue diseases).

A 14-year-old boy was referred because of a slight swelling of the alveolar mucosa of the right central and lateral maxillary incisor. Radiographically, a well-circumscribed radiolucency was seen between the vital, but partially resorbed roots of the two incisors. Treatment consisted of careful enucleation, including removal of the two teeth. Histopathological examination showed a central giant-cell granuloma. Healing was uneventful. Additional examination for the presence of hyperparathyroidism was negative. During one year follow-up, no signs of recurrence have been noticed.

Osteolytic lesions seen on plain radiographs can be caused by various disorders of the bones such as simple bone cyst, aneurysmal bone cyst, plasmacytoma, giant cell tumor, eosinophilic granuloma and tuberculosis. We studied prospectively Tc-99m-methylene diphosphonate bone scan findings in osteolytic lesions seen radiologically and followed them to histopathology. Interestingly, the scans in these patients helped to show if the lesions were monoostotic or polyostotic and, in some cases, ruled out malignant or infective etiology.

A buffalo disease, called "Degnala", causing lameness, edema, gangrenous ulceration of hooves or tail, emaciation, recumbency and eventual death, occurs in Eastern Nepal. Clinical examinations manifested lice eggs on hairs, bradycardia, hypothermia, dehydration, exanthema and icterus. Hematologically, increase of band neutrophil, giant platelet, hypoalbuminemia and hyperglobulinemia were characteristics. Microscopically, dark blue tiny particles were seen on red blood cell (RBC) after Giemsa staining. Administration of tetracycline at an early stage of the disease was effective.

Brain compartments differing by topical chemical location of alcohol dehydrogenases (ADH) were studied on forensic medical material. ADH were found in magnocytes of giant-cell reticular formation, neuronal nuclei in the blue spot, and capillaries, but not in neurons of the ganglionary layer of the cerebral limbic cortex. ADH activity in the studied brain compartments depended on the level of exogenous ethanolemia. Increased ADH activity in the reticular formation magnocytes characterized the individual tolerance in severe ethanol intoxication. Cerebral ADHs are markers of ethanol intoxication.

A case of malignant melanoma in the thymus is reported. Diagnostic imaging demonstrated a left anterior mediastinal mass in a patient with giant pigmented nevus without malignant change. Histologic and cytologic specimens obtained from the tumor revealed that the tumor was malignant melanoma. Surgery revealed malignant melanoma in the left lobe of the thymus. Many cell nests of pigmented nevi were observed throughout the thymus. The malignant melanoma was thought to have originated from the nevocellular nevus in the thymus. This is the first report of malignant melanoma in the thymus.

Several cases of systemic amyloidosis associated with polymyalgia rheumatica (PMR) or giant-cell arteritis (GCA) have been described. Nevertheless, the type of amyloid deposit has not been characterized in most of them. Here we report on two patients with PMR (one with associated GCA) who developed nephrotic syndrome and end-stage renal failure caused by massive amyloid deposition. Immunohistochemical analysis showed that the amyloid deposits were of AA type (secondary amyloidosis) in both cases.
