Increased functional activity, bottom-up and intrinsic effective connectivity in autism.

Sensory perceptual alterations such as sensory sensitivities in autism have been proposed to be caused by differences in sensory observation (Likelihood) or in forming models of the environment (Prior), which result in an increase in bottom-up information flow relative to top-down control. To investigate this conjecture, we had autistic individuals (AS) and neurotypicals (NT) perform a decision-under-uncertainty paradigm while undergoing functional magnetic resonance imaging (fMRI). There were no group differences in task performance and in Prior and Likelihood representations in brain activit

Temporal variations in iliac trabecular bone formation in vertebral osteoporosis.

The histologic heterogeneity of osteoporosis relative to normal controls has attracted great interest. There has been controversy as to whether patients with high turnover osteoporosis may convert to a normal or low turnover form, and vice versa. We have studied 44 patients over 12 years by dynamic histomorphometry and 85Sr kinetics+calcium balance performed within 60 days in 20 patients (Group 1) and 75-808 days apart in the remainder (Group 2). In the first group, the histologic tissue level bone formation rate (BFR/BV or BFR/BS) was predictive of the 85Sr measurements of bone formation (r =

Longitudinal changes in amygdala-supplementary motor area connectivity and their association with recurrent self-harm in adolescents with mood disorders.

BACKGROUND: Adolescents hospitalized with mood disorders face a heightened risk of repeated self-harm (SH) after discharge. Neuroimaging phenotype may complement traditional symptom-based approaches by revealing neural mechanisms of SH vulnerability. This study aimed to investigate longitudinal changes in functional connectivity (FC) in adolescents with repeated SH and to examine how these neural dynamics relate to SH-related symptoms. METHODS: We recruited 201 adolescent inpatients with mood disorders and SH behaviors, who were classified into repeated (RESH; n = 63) and non-repeated (NRESH;

Symptom Network Dynamics during Antipsychotic Treatment in First-Episode Psychosis.

BACKGROUND AND HYPOTHESIS: Treatment response in first-episode psychosis varies substantially, yet underlying factors remain poorly understood. Symptom network theory suggests that inter-symptom relationships may influence treatment response. We hypothesized that symptom networks at baseline, as well as dynamic interactions over time, would differ between remitters and non-remitters, and that specific antipsychotics would show differential symptom-targeting effects. STUDY DESIGN: We compared baseline and temporal symptom networks between remitters (n = 250) and non-remitters (n = 196) from th

Health-Seeking Behaviors After Acute Respiratory Infection Among Urban and Rural Residents and Different Age Groups: Cross-Sectional Questionnaire Study.

BACKGROUND: The choice of pathways in health-seeking behavior following acute respiratory infection (ARI) is critical for health care resource allocation, yet traditional logistic regression methods struggle to capture the dynamic evolution of such behaviors. OBJECTIVE: This study aimed to quantify the dynamic process of changes in health-seeking behaviors following the onset of ARI and to identify differences in behavioral pathways associated with key factors such as urban-rural status and age. METHODS: This study used a multistate Markov model to quantify transition probabilities and inten

Association between serum osmolality trajectories and mortality in sepsis patients: a retrospective multi-cohort study.

BACKGROUND: Sepsis commonly leads to fluid and electrolyte imbalances, often reflected in abnormal serum osmolality. Although static measurements of osmolality have been investigated, the prognostic significance of dynamic changes in serum osmolality over time remains poorly understood in patients with sepsis. METHODS: We conducted a comprehensive analysis of 22,737 septic patients from the MIMIC-IV and eICU-CRD databases, with external validation performed using an independent cohort of 303 patients from Zhejiang Provincial People's Hospital. Latent class trajectory modeling was applied to i

Differences of respiratory mechanics in mechanical ventilation of acute respiratory distress syndrome between patients with COVID-19 and Influenza A.

BACKGROUND: Whether COVID-19-induced acute respiratory distress syndrome (ARDS) should be approached differently in terms of mechanical ventilation therapy compared to other virus-induced ARDS is debatable. Therefore, we aimed to ascertain whether the respiratory mechanical characteristics of COVID-19-induced ARDS differ from those of influenza A induced ARDS, in order to establish a rationale for mechanical ventilation therapy in COVID-19-induced ARDS. METHODS: This was a retrospective cohort study comparing patients with COVID-19-induced ARDS and influenza A induced ARDS. We included intens
