Confined to Obscurity: Health Challenges of Pregnant Women in Jail.

Research about pregnant women in jail is scant. This exploratory study begins to fill this gap by examining the demographics; background characteristics; and self-reported physical health, mental health, and substance use challenges reported by 27 pregnant women incarcerated in a large midwestern county jail. It further reports on the prenatal care before and during their incarceration, plans of these pregnant women for delivery of their child, caring for their infant after their release from jail, and their expectations of paternal or family support post-release. Among the sample, 66.7 percen

Original Research: Can a Palliative Care Lay Health Advisor-Nurse Partnership Improve Health Equity for Latinos with Cancer?

PURPOSE: A palliative care infrastructure is lacking for Latinos with life-threatening illness, especially in rural regions of the United States. The purpose of this study was to develop and evaluate a community-based palliative care lay health advisor (LHA) intervention for rural-dwelling Latino adults with cancer. METHODS: An exploratory mixed-methods participatory action research design was carried out by an interprofessional research team that included community and academic members. Fifteen Latino community leaders completed a 10-hour palliative care training program and then served as p

Equity and integration; why the oral healthcare community urgently needs to reflect on its approach to caries management.

The evidence of oral disease burden and their impact on lives and livelihoods is overwhelming. The architecture and process to develop a coherent, consistent and mutually reinforcing approach for the management of dental caries exists through the WHO strategy and action plan. To successfully address equity and integration issues, the oral healthcare community must address the disconnect between advocacy, education, practice and caries management remuneration, finding agreement through negotiation and common ground between all stakeholder groups. Framing and aligning oral and dental health prio

[Epidemiological surveillance and mass events: lessons learned ahead of the 2026 FIFA™ World Cup].

The FIFA World Cup™ 2026 in Mexico will be a major public health challenge worldwide. Large gatherings can facilitate the spread of diseases, raise the number of cardiovascular emergencies caused by stress, and require careful monitoring of vaccine-preventable illnesses, particularly given ongoing concerns about measles and pertussis. This editorial article reviews lessons learned from past events and highlights the need to improve food safety, mental health support, and active disease surveillance. The healthcare system's success will depend on how well it can respond and plan to manage the r

[Reflections on the clinical diagnosis and management of Turner syndrome].

There is a delay in the clinical diagnosis of Turner syndrome (TS), particularly for patients with mosaic karyotypes. This diagnostic delay can hinder age-appropriate treatments and lead to adverse health outcomes. Therefore, it is necessary to explore improvement measures for early diagnosis and treatment plans based on the current clinical situation. Furthermore, as research progresses, the focus of clinical diagnosis and treatment of TS is gradually expanding to multiple aspects, including cardiovascular health, fertility, and transitional care, in order to improve the prognosis and quality

Person-Centered, Skilled Services Using a Montessori Approach for Persons with Dementia.

The current mandate for person-centered care throughout the health care system, and especially in the nursing home industry, requires that speech-language pathologists ensure that the services they provide to elders with dementia are skilled, person centered, and relevant to positive overall health outcomes. Guidelines developed by the Association Montessori International Advisory Board for Montessori for Aging and Dementia are one avenue toward such skilled and person-centered services. The purpose of this article is to provide clinicians with practical strategies for guiding their assessment

[Hypoplastic left heart syndrome. Present experiences and future possibilities].

The authors report experiences from Norwood's staged palliation for hypoplastic left heart syndrome performed in Philadelphia on Norwegian children. Although there is no universal agreement that these children should be operated at all, the Norwegian health authorities have decided that all expenses for transport, examination and treatment shall be covered by the National Health Insurance Scheme. Since 1987 a total of 21 children have been transported to USA for operation. Eight of these children have completed all the planned stages of the operative treatment. Two are waiting for stage 3. 11

Building integrated mental health and medical programs for vulnerable populations post-disaster: connecting children and families to a medical home.

INTRODUCTION: Hurricane Katrina, a Category 3 hurricane, made landfall in August 2005. Approximately 1,500 deaths have been directly attributed to the hurricane, primarily in Louisiana and Mississippi. In New Orleans, Louisiana, most of the healthcare infrastructure was destroyed by flooding, and > 200,000 residents became homeless. Many of these internally displaced persons received transitional housing in trailer parks ("villages") under the auspices of the [US] Federal Emergency Management Agency (FEMA). PROBLEM: The FEMA villages are isolated from residential communities, lack access to h

From Lab to Bedside Testing: Uncovering Gateways and Roadblocks to Syndromic Multiplex Point-of-Care Testing for Infectious Diseases in Canadian Hospitals.

Health leaders must consider various solutions to alleviate the healthcare management burden within Canadian hospitals. Increased access and rapidity of Infectious Diseases (IDs) testing via multiplex Point-of-Care Tests (POCTs) is an example. Twenty-four interviews assessed the facilitators and barriers to implementing such tests in the inpatient hospital setting in Quebec, Ontario, and British Columbia. The perspective of health leaders, including provincial laboratory officials, medical and laboratory directors, POCT managers, physicians, and nurses, revealed the following major themes as b

The Intersection of Incarceration and Reproductive Health: Contraceptive Perspectives of Women in Rural Maine Jails.

The U.S. carceral health care system presents a unique opportunity to expand reproductive health services. Evidence suggests that women incarcerated in jails want to initiate contraception, but access varies across facilities and is limited by concerns about cost, reproductive coercion, and a lack of consistent policies. To better understand the reproductive health needs of rural incarcerated women, a cross-sectional study of women incarcerated in four rural Maine jails was conducted using a 59-question survey on contraceptive history, needs, and preferences. Seventy-three women with complex h

The Relationship Between Physician Organization and Health Care Use in Traditional Medicare and Medicare Advantage.

Publicly funded insurance programs increasingly rely on private health plans to manage care, and physicians have organized into larger groups, many of which are owned by hospitals or health care systems. We examine the relationship between physician organization and health care resource use and whether this relationship differs between traditional Medicare and Medicare Advantage. We find that physician group practice is associated with less use of hospital inpatient and outpatient services, but that this relationship is attenuated for patients enrolled in Medicare Advantage. We also find that

[Frontiers and advances in dental caries research].

Dental caries is a prevalent infectious disease caused by microbial metabolism, affecting the hard tissues of teeth. According to data from the Fourth National Oral Health Epidemiological Survey, there is a gradual increase in caries prevalence in China, particularly among children and elderly individuals. As an etiological factor for dental lesions, caries not only impact oral health but also exhibit significant associations with various systemic diseases, suggesting that systemic diseases may serve as potential risk factors of caries development. To effectively implement the plan of national

Make good use of big data: A home for everyone.

The ongoing COVID-19 pandemic should teach us some lessons at health, environmental and human levels toward more fairness, human cohesion and environmental sustainability. At a health level, the pandemic raises the importance of housing for everyone particularly vulnerable and homeless people to protect them from the disease and against other similar airborne pandemics. Here, I propose to make good use of big data along with 3D construction printers to construct houses and solve some major and pressing housing needs worldwide. Big data can be used to determine how many people do need accommoda

HIV/AIDS and the African-American Community 2018: a Decade Call to Action.

HIV/AIDS disproportionately affects African-Americans more than any other racial or ethnic group in the USA. Currently representing only 12% of the US population, African-Americans now comprise close to half of the total reported HIV/AIDS cases in the USA according to the Centers for Disease Control and Prevention since the initial reporting of HIV/AIDS. In this paper, we examined the prevalence and current direction of the HIV/AIDS epidemic in the African-American community especially in comparison to our first call to action in 2008. The situation remains dire and broader attention is necess

Health crises due to infectious and communicable diseases : European preparedness and response tools in an international context.

The combination of changes in eating habits, ways of living, globalisation, extensive travelling and the migration of millions of people around the world may be contributing to increased health risks. Certainly, health crises today are proving highly complex. More and more people are travelling and may carry with them unexpected virus vectors such as mosquitoes. Preparedness is challenging and there is a need for action plans to safeguard the growing at-risk population. Health crises can potentially affect a large proportion of the population and may lead to a significant increase in mortality

Ontario's Poverty Reduction Strategy: A Critical Discourse Analysis.

This article overviews the second phase of a two-phase study which examined experiences of health and social exclusion among mothers experiencing homelessness in Ontario, Canada. A critical discourse analysis was employed to analyze the policy document, Realizing Our Potential: Ontario's Poverty Reduction Strategy, 2014-2019. In nursing, analysis of policy is an emerging form of scholarship, one that draws attention to the macro levels influencing health and health promotion, such as the social determinants of health, and the policies that impact them. The clear neo-liberal underpinnings, with

Vaccine preventable disease incidence as a complement to vaccine efficacy for setting vaccine policy.

Traditionally, vaccines have been evaluated in clinical trials that establish vaccine efficacy (VE) against etiology-confirmed disease outcomes, a measure important for licensure. Yet, VE does not reflect a vaccine's public health impact because it does not account for relative disease incidence. An additional measure that more directly establishes a vaccine's public health value is the vaccine preventable disease incidence (VPDI), which is the incidence of disease preventable by vaccine in a given context. We describe how VE and VPDI can vary, sometimes in inverse directions, across disease o

Trachoma in an omani village - a health care study.

This paper reports the findings of a study carried out on 289 families in rural Oman, to assess the knowledge of trachoma in a defined community, identify the determinants of continued transmission and endemicity of the disease, and to assess the effect of health education provided by medical students. The study identified the prevalence of illiteracy, large family size and overcrowding as potential risk factors. Lack of knowledge in a significant number of families about causes, prevention and transmission of trachoma led to practices that promoted the spread of the disease. We found that episo

RESULT-Hip. The impact of restrictive compared with liberal transfusion strategy on cardiac injury and death in patients undergoing surgery for hip fracture : protocol for a multicentre, randomized controlled trial of two blood transfusion strategies.

AIMS: Hip fracture is a global health issue. Complications are common, leading to reduced quality of life or death. These occur more frequently if anaemia is present. We hypothesize that correcting anaemia in the perioperative period may decrease major adverse cardiac events (MACE) and death after hip fracture. METHODS: This multicentre prospective randomized trial compares transfusion strategies after hip fracture in patients aged ≥ 60 years. We initially planned to randomize 1,984 participants with anaemia, defined as haemoglobin ≤ 90 g l-1, to one of two transfusion strategies: liberal tra
