[Asymptomatic course of severe tricuspid valve insufficiency with ateroseptal infarct, coronary fistula and ventricular septum defect as a sequela of blunt thoracic trauma].

Traumatic tricuspid valve insufficiency is a rare in vivo finding, especially in combination with other traumatic cardiac injuries like ventricular septal defect, myocardial infarction, and traumatic coronary fistula. Two-dimensional and, more recently, colorcoded Doppler echocardiography have gained importance for the diagnosis of tricuspid insufficiency. Patients with traumatic tricuspid insufficiency who survive the acute trauma, may remain asymptomatic for many years and even decades. If symptoms develop, tricuspid valve replacement appears feasible even after many years.

Expression of PD-1/PD-L1 pathway molecules in human cardiac allograft according to acute cellular rejection status: insights from a Korean Heart Transplant Cohort.

BACKGROUND: Acute cellular rejection (ACR) following heart transplantation (TPL) compromises graft function and survival. The programmed cell death-1 (PD-1)/PD-1 ligand-1 (PD-L1) pathway represents an immune checkpoint that maintains peripheral immune tolerance, but its expression and significance in human cardiac allografts with ACR remain unclear. Thus, we investigated PD-1/ PD-L1 expression in endomyocardial biopsies from heart TPL recipients to clarify the role of this pathway in the ACR of human cardiac allografts and explore the potential of therapeutic modulation of PD-1/PD-L1 in this s

Device-driven cyclic compression of the superior vena cava as a preload reduction strategy to improve cardiac function in heart failure: a pilot feasibility study.

We evaluated the physiological efficacy and safety of a novel device-based preload reduction strategy that applies external cyclic compression to the superior vena cava (SVC) in a preclinical heart failure model. Heart failure was induced in eleven pigs using ischemia-reperfusion injury, and a 3D-printed SVC compression device was tested under varying compression ratios (70%, 85%, and 100%) and protocols. Hemodynamic responses were monitored using right-heart catheterization and pressure-volume loop analysis. Among the tested conditions, cyclic compression at 85% with 20/5-minute compression-r

Heart-Brain Axis in Viral Myocarditis: Shared Cytokine Pathways, Blood-Brain Barrier Crosstalk, and Neuroinflammatory Consequences.

The heart-brain axis is a bidirectional communication network composed of neural, humoral, and immune pathways that sustain cardiovascular and brain homeostasis. There is mounting evidence that viral myocarditis-a prototype of inflammatory heart disease-acts beyond the myocardium, triggering systemic immune cascades that lead to central nervous system (CNS) involvement. This involvement creates an inflammatory continuum in which cardiac damage and neuroinflammation reinforce each other via common cytokine and molecular mediators. Central mediators in this axis are the proinflammatory cytokines

Compound SJ-12 attenuates streptozocin-induced diabetic cardiomyopathy by stabilizing SERCA2a.

Heart failure (HF) is one of the major causes of death among diabetic patients. Although studies have shown that curcumin analog C66 can remarkably relieve diabetes-associated cardiovascular and kidney complications, the role of SJ-12, SJ-12, a novel curcumin analog, in diabetic cardiomyopathy and its molecular targets are unknown. 7-week-old male C57BL/6 mice were intraperitoneally injected with single streptozotocin (STZ) (160 mg/kg) to develop diabetic cardiomyopathy (DCM). The diabetic mice were then treated with SJ-12 via gavage for two months. Body weight, fast blood glucose, cardiac utr

Adenovirus-IL-10 relieves chronic rejection after mouse heart transplantation by inhibiting miR-155 and activating SOCS5.

Objective: Chronic rejection remains the main factor that influence long-term survival of patients after heart transplantation. Interleukin-10 (IL-10) play critical role in macrophages-mediated transplant immune responses. We investigated the mechanism of IL-10 in macrophage related chronic rejection after mouse heart transplantation. Methods: Mouse heart transplant chronic rejection model was established to evaluate pathological changes in the allograft. Myocardial interstitial fibrosis, apoptosis, and inflammatory factor levels were detected in ad-IL-10-treated mice. The positive iNOS+ and A

Impact of Maturation on Myocardial Response to Ischemia and the Effectiveness of Remote Preconditioning in Male Rats.

Aging attenuates cardiac tolerance to ischemia/reperfusion (I/R) associated with defects in protective cell signaling, however, the onset of this phenotype has not been completely investigated. This study aimed to compare changes in response to I/R and the effects of remote ischemic preconditioning (RIPC) in the hearts of younger adult (3 months) and mature adult (6 months) male Wistar rats, with changes in selected proteins of protective signaling. Langendorff-perfused hearts were exposed to 30 min I/120 min R without or with prior three cycles of RIPC (pressure cuff inflation/deflation on th

Fast in vivo detection of myocardial norepinephrine levels in the beating porcine heart.

The sympathetic nervous system modulates cardiac function by controlling key parameters such as chronotropy and inotropy. Sympathetic control of ventricular function occurs through extrinsic innervation arising from the stellate ganglia and thoracic sympathetic chain. In the healthy heart, sympathetic release of norepinephrine (NE) results in positive modulation of chronotropy, inotropy, and dromotropy, significantly increasing cardiac output. However, in the setting of myocardial infarction or injury, sympathetic activation persists, contributing to heart failure and increasing the risk of ar

Effects of Anchomanes difformis on Inflammation, Apoptosis, and Organ Toxicity in STZ-Induced Diabetic Cardiomyopathy.

Persistent hyperglycemia is known to cause enhanced generation of reactive oxygen species in diabetes. Several inflammatory cytokines are induced by oxidative stress, and their release also leads to increased oxidative stress; this makes oxidative stress one of the important factors in the development of chronic inflammation and other immune responses. These have been implicated in the development of diabetic complications such as nephropathy and cardiomyopathy. Anchomanes difformis has been shown to possess antioxidant and anti-inflammatory potentials. The present study investigated the immun

Hypoxic hepatitis: clinical and hemodynamic study in 142 consecutive cases.

The centrilobular liver cell necrosis observed in hypoxic hepatitis is generally attributed to failure of hepatic blood perfusion. Accordingly, this injury of the liver is commonly recognized under the terms "shock liver" or "ischemic hepatitis." During a 10-year period, 142 episodes of hypoxic hepatitis were consecutively identified in the intensive care unit of a general hospital, and the clinical, biological, and hemodynamic parameters were prospectively collected on individual files. We conducted the current study to assess retrospectively the role of the hemodynamic mechanisms of tissue h

Ischemia-reperfusion injury: beneficial effects of mesenchymal stromal cells.

PURPOSE OF REVIEW: Organ transplantation and other major surgeries are impacted by ischemia-reperfusion injury (IRI). Mesenchymal stromal cells (MSCs) recently became an attractive alternative therapeutic tool to combat IRI. The present review highlights the effects of MSCs in the preclinical animal models of IRI and clinical trials, and explains their potential modes of action based on the pathophysiological IRI cascade. RECENT FINDINGS: The application of MSCs in animal models of IRI show anti-inflammatory and anti-apoptotic effects, particularly for damage to the kidneys, heart and lungs.

[Role of chest physician in the management of patient with thoracic trauma].

Thoracic traumas are frequent and potentially fatal, because of the associated neurological and abdominal lesions. They are observed in car crashes, combat environments and urban terrorist bombings. The mechanisms of the traumatic injury are complex and account for the diversity of the lesions. The management of a chest trauma patient is a model of multidisciplinary collaboration where the chest physician can make a significant contribution.

Thoracic trauma.

The physiologic equilibrium of chest injury patients is frequently precarious, and mild stress during examination and treatment may precipitate acute decompensation and death. This is particularly true with the respiratory system, where the normally large respiratory reserve capacity may be rapidly lost. Accurate assessment of the nature of the thoracic injury and the severity of that injury must be determined in order to formulate a therapeutic plan. Many thoracic injuries, such as pneumothorax, pulmonary contusions, or rib fractures, will be self-limiting. Other conditions must be recognized

Tissue perfusion pressure as a physiologic indicator of renal failure risk after heart transplantation.

BACKGROUND: Mean arterial pressure (MAP) guides hemodynamic management following heart transplantation, yet renal failure remains common. Tissue perfusion pressure (TPP), defined as MAP minus critical closing pressure, predicts acute kidney injury in non-transplant cardiac surgery. We hypothesized that postoperative TPP predicts continuous renal replacement (CRRT) requirement after heart transplantation. METHODS: We analyzed all adult heart transplants at a single institution (01/2020-06/2025) and excluded multiorgan transplants, congenital heart disease, and patients requiring post-transplan

The anaesthesiologist's role in heart transplant: An Armed Forces Medical Services perspective.

BACKGROUND: Heart transplantation remains the definitive therapy for refractory endstage heart failure. While surgical and immunosuppressive advances have been pivotal, the anaesthesiologist's role is underrepresented-particularly within Armed Forces Medical Services (AFMS), with logistical and geographic constraints. This study examines perioperative anaesthetic strategies in AFMS heart transplants and their impact on patient and graft outcomes. METHODS: This retrospective case series included 33 consecutive heart transplants performed across AFMS centres between 2018 and 2024. Data were ana

BRD4 inhibition rewires cardiac macrophages toward a protective phenotype marked by low MHC class II expression.

Bromodomain and extraterminal domain (BET) proteins, including BRD4, bind acetylated chromatin and coactivate gene transcription. A BET inhibitor, JQ1, prevents and reverses pathological cardiac remodeling in preclinical models of heart failure. However, the underlying cellular mechanisms by which JQ1 improves cardiac structure and function remain poorly defined. Here, we demonstrate that BRD4 knockdown reduced expression of genes encoding CC chemokines in cardiac fibroblasts, suggesting a role for this epigenetic reader in controlling fibroblast-immune cell cross talk. Consistent with this, J
