[Methicillin-cephem-resistant Staphylococcus aureus (MRSA) mediastinitis following open heart surgery].

Mediastinitis following cardiac surgery occurs frequently in association with high mortality and morbidity. Patient was a 6-year-old boy suffering from Sotos syndrome with secundum type of ASD. He was operated upon to repair ASD. Following the surgery, he suffered from MRSA mediastinitis and osteomyelitis of the sternum. Routine method of closed irrigation after reopening of the sternal wound was ineffective. Patient recovered following aggressive debridement repeatedly, open drainage and topical irrigation with vancomycin. This appears to be a useful method to treat MRSA mediastinitis of the

Survival versus nonsurvival determinants for neonatal harbor seals.

Of 29 neonatal harbor seals that had been abandoned, separated from their mothers, or stranded in the wild, 22 succumbed during hospitalization at the California Marine Mammal Center in the spring of 1982. Compared with standard reported harbor seal pup measurements, 8 of the nonsurviving pups were premature (ie, creamy-white lanugo coat, early pupping season birthdate, and decreased body size [6 to 10 cm shorter and 2.3 to 4.5 kg less than normal]). The premature pups and the young pups (less than or equal to 10 days old) had at least two of the following findings: jaundice, emaciation, labor

Chromosome 7 short arm deletion, 7p21----pter.

A case report of a de novo deletion in the short arm of chromosome 7 is presented (46,XX,del(7)(p21----pter)). The five-month-old girl's major symptoms are: trigonocephalus with craniosynostosis, median bony forehead bulge, high palate, atrial septal defect, anal atresia and perineal fistula, thumb insertion far to ulnar-proximal, and a slightly retarded psychomotor development. The other cases with monosomy 7p described in the literature are reviewed.

Pulmonary vein blood flow velocity waveform--with special reference to pulmonary "systolic runoff" in patients with atrial septal defect.

In order to evaluate the magnitude of pulmonary "systolic runoff", we studied the pulmonary vein blood flow velocity waveform by positioning a catheter-tip velocity-pressure transducer into the extraparenchymal pulmonary vein just distal to the left atrium. We recorded blood flow velocity and pressure simultaneously, and subsequently identified the zero blood flow velocity with blood flow velocity level of the pulmonary artery in diastole. Patients with atrial septal defect were used as subjects because of the technical ease although the altered hemodynamics were present. Two kinds of flow vel

Superior and inferior vena cava flow velocity in patients with anomalous pulmonary vein connection.

The superior and inferior vena cava flow velocities (SVC and IVC flow velocities) of 3 patients with anomalous pulmonary vein connection were recorded with the Doppler flowmeter catheter. The cases consisted of a patient with partial anomalous pulmonary vein connection to the SVC (Case 1), one with total anomalous pulmonary venous connection to the SVC (Case 2), and one with partial anomalous pulmonary vein connection to the IVC (Case 3). The SVC and IVC recordings of these patients were compared with those of 10 normal subjects, 20 patients with atrial septal defect and 150 patients with othe

Left Ventricular Diastolic Function in Children with Atrial Septal Defects Improves After Closure by Means of Increased Hydraulic Force.

Hydraulic force aids diastolic filling of the left ventricle (LV) by facilitating basal movement of the atrioventricular plane. The short-axis atrioventricular area difference (AVAD) determines direction and magnitude of this force. Patients with atrial septal defect (ASD) have reduced LV filling due to the left-to-right shunt across the atrial septum and thus potentially altered hydraulic force. The aims were therefore to use cardiac magnetic resonance images to assess whether AVAD and thus the hydraulic force differ in children with ASD compared to healthy children, and if it improves after

Multiperforated atrial septal aneurysm and a single coronary artery.

Multiperforated atrial septal aneurysm (ASA) with left-to-right shunt and elevated pulmonary arterial pressure is rare. We present an adult patient with this unusual anomaly combined with a single coronary ostium emerging from the right sinus of Valsalva. The ASA was excised and the defect repaired successfully.

Early cardiac perforation after atrial septal defect closure with the Amplatzer septal occluder.

We report a case of aortic perforation after transcatheter closure of an atrial septal defect by an Amplatzer septal occluder. During emergency surgery, perforations of the dome of the right atrium and the noncoronary sinus of Valsalva of the aorta were repaired. Atrial septal defect was primarily closed. A short anterosuperior rim should be considered a risk factor for aortic perforation in transcatheter atrial septal defect closure.

Myocardial contractility in premature neonates with and without patent ductus arteriosus.

Controversy exists as to whether a hemodynamically significant left-to-right shunt due to a patent ductus arteriosus (PDA) affects ventricular contractility. Load-dependent indices such as ejection fraction and shortening fraction have traditionally been used to assess contractility, but the relationship between the rate-corrected velocity of fiber shortening (MVCFc) and wall stress may be more suitable, as it is a preload-independent, afterload-adjusted method of assessing ventricular contractility. Age-related differences have been established for these variables in normal adults and childre

The floppy mitral valve syndrome--a review of fourteen patients requiring valve surgery.

The correlation of clinical features with the operative findings in 14 patients with a floppy mitral valve is discussed. The clinical course is typified by rapidly progressive disability. An abrupt deterioration was present in five of our subjects and this may be due to rupture of chordae tendinea or stretching of the valve apparatus. The redundancy of the anterior cusp in 13 of our series may explain the absence of mid systolic clicks in all except one patient. The diagnosis of prolapse can be difficult to make prior to surgery because the prolapsed cusp may be masked by the regurgitant contr

Exome analysis of a family with pleiotropic congenital heart disease.

BACKGROUND: A number of single gene defects have been identified in patients with isolated or nonsyndromic congenital heart defects (CHDs). However, due to significant genetic heterogeneity, candidate gene approaches have had limited success in finding high-risk alleles in most cases. The purpose of this study was to use exome sequencing to identify high-risk gene variants in a family with highly penetrant pleiotropic CHD. METHODS AND RESULTS: DNA samples from 2 members of a family with diverse CHD were analyzed by exome sequencing. Variants were filtered to eliminate common variants and sequ

Clinical Importance of Concomitant Cleft Lip/Palate in the Surgical Management of Patients With Congenital Heart Disease.

BACKGROUND: Congenital heart disease (CHD) frequently occurs in conjunction with extracardiac developmental anomalies, including cleft malformations. The clinical impact of concomitant cleft disease on the surgical management of CHD has not been studied. We evaluated cardiac surgical outcomes in patients with concomitant CHD and cleft lip and/or palate (CL/P). METHODS: Patients with CHD + CL/P managed at our institution between January 2004 and December 2018 were included. Demographic, operative, and follow-up data were retrospectively collected and analyzed using SAS 9.4. Chi-square tests we

Usefulness and limitations of precordial phonocardiography and external pulse recordings.

Phonocardiography provides one of a growing list of valuable intermediary studies between physical examination and cardiac catheterization. It not only provides a medium for instruction in physical examination, but also allows one to make certain diagnoses and assess severity of numerous cardiovascular lesions. It is especially valuable in the evaluation of all valvular stenoses, especially aortic stenosis. One can also use phonocardiography for screening and diagnosis of idiopathic hypertrophic subaortic stenosis. It is a helpful adjunct in the study of several congenital abnormalities as wel

Prenatal detection of major congenital heart disease - optimising resources to improve outcomes.

INTRODUCTION: Congenital heart disease (CHD) is the most common major structural fetal abnormality and the benefits of prenatal detection are well described. The objective of this study was to evaluate the precision of prenatal diagnosis at a single tertiary referral unit over two three year periods (2006, 2007, 2008 and 2010, 2011, 2012), before and after a prenatal screening protocol for CHD was developed to include extended cardiac views, mandatory recall for suboptimal views, and a multidisciplinary Fetal Cardiac clinic was established. There exists a single national centre for paediatric
