Semi-complete versus partial venous obstruction, with and without partial lymphatic obstruction, and benzo-pyrone treatment.

The alterations in the fine structure of the venous wall are reviewed, and the changes which occur with injury. These are very similar in both arteries and veins, and were also found by us in experiments in which the common iliac veins of rats were completely occluded, with or without partial lymphatic obstruction. It was found that the protein concentration was considerably increased in the vein wall, in the muscle, and in the skin of the leg--especially when the lymphatics were blocked as well as the veins. The effects of various procedures, including partial and semi-complete venous obstruc

Circadian Rhythms and Pain: A Narrative Review on Clock Genes and Circadian-Based Interventions.

Pain is a common symptom of many diseases and seriously affects the quality of life. Circadian rhythm is the regular, cyclical physiological, biochemical, and behavioral changes that occur within a 24-hour period in biological organisms, primarily regulated by clock genes. Pain sensitivity may have circadian rhythms, with clock genes likely influencing this pain-related rhythmicity. Therefore, restoring normal circadian rhythms and regulating the expression of clock genes are regarded as viable strategies to combat the development of pain. First, this review elucidates the core operational mec

Traumatic arteriovenous fistula mimicking cutaneous leishmaniasis: A case report.

INTRODUCTION: Traumatic arteriovenous fistula (TAVF) may be challenging to diagnose and can be misjudged as skin lesions or ulcers, including cutaneous leishmaniasis. Here, we present a patient with TAVF misdiagnosed and treated as cutaneous leishmaniasis. CASE PRESENTATION: A 36-year-old male presented with a non-healing venous ulcer in his left leg, which was misdiagnosed and treated as cutaneous leishmaniasis. He was referred to our clinic, where color Doppler sonography showed arterial flow in the left great saphenous vein, and Computed tomographic (CT) angiography revealed left superfici

Crouching Zookeeper, Hidden Dragon: A Case of a Komodo Dragon Bite.

BACKGROUND: The Komodo dragon (Varanus komodoensis) is the world's largest living lizard and exists in private captivity worldwide. Bites to humans are rare and have been proposed to be both infectious and venomous. CASE REPORT: A 43-year-old zookeeper was bitten on the leg by a Komodo dragon and suffered local tissue damage with no excessive bleeding or systemic symptoms to suggest envenomation. No specific therapy was administered other than local wound irrigation. The patient was placed on prophylactic antibiotics and on follow-up, which revealed no local or systemic infections, and no oth

A fatal case of traumatic brain injury with severe coagulopathy due to Rhabdophis tigrinus (yamakagashi) bites: a case report.

BACKGROUND: Yamakagashi venom is a prothrombin activator, leading to disseminated intravascular coagulation. We report a fatal case of severe coagulopathy from head trauma assumed to be caused by a yamakagashi bite. CASE PRESENTATION: An 80-year-old man fell and developed systemic tonic-clonic convulsions. Head computed tomography revealed brain contusion and acute subdural hematoma. Physical examination revealed two bite marks with persistent bleeding on the right lower leg. The patient stated that he had been bitten by some creature 3 days prior, but the bite was left untreated. Laboratory

Recruitment of upper-limb motoneurons with epidural electrical stimulation of the cervical spinal cord.

Epidural electrical stimulation (EES) of lumbosacral sensorimotor circuits improves leg motor control in animals and humans with spinal cord injury (SCI). Upper-limb motor control involves similar circuits, located in the cervical spinal cord, suggesting that EES could also improve arm and hand movements after quadriplegia. However, the ability of cervical EES to selectively modulate specific upper-limb motor nuclei remains unclear. Here, we combined a computational model of the cervical spinal cord with experiments in macaque monkeys to explore the mechanisms of upper-limb motoneuron recruitm

Not all that is 'full house' is systemic lupus erythematosus: a case of membranous nephropathy due to syphilis infection.

We describe an unusual case of membranous nephropathy precipitated by syphilis infection in a patient without systemic lupus erythematosus (SLE). A previously healthy 20-year-old man presented with leg and facial swelling. Laboratory investigation revealed nephrotic range proteinuria, acute kidney injury, hypocomplementaemia and a highly positive rapid plasma reagin. Kidney biopsy showed membranous nephropathy with 'full-house' immunofluorescence (IgG, IgA, IgM, C1q and C3), mimicking lupus nephritis class Vb. However, the patient had no features of SLE and had negative antinuclear and anti-do

Complex Regional Pain Syndrome Developing After a Coral Snake Bite: A Case Report.

Complex regional pain syndrome (CRPS) usually occurs after an inciting injury. Poor understanding of pathophysiology, management, and disease awareness has led to misdiagnosis of this condition. We report a rare case of a 69-year-old male who developed CRPS following a Florida Coral snake bite on his right foot. Initially, it was misdiagnosed as recurrent cellulitis; however, he developed chronic right lower extremity (RLE) pain with worsening flares associated with right leg swelling and erythema. Examination was remarkable for nonpitting edema, erythema, and severe tenderness to light touch

Sleep disorders in traumatic brain injury.

The purpose of the review is to collect the most relevant current literature on the mechanisms of normal sleep and sleep disorders associated with traumatic brain injury (TBI), to discuss the most frequent conditions and the evidence on their possible treatments and future research. Sleep disorders are extremely prevalent after TBI (30-84%). Insomnia and circadian rhythm disorders are the most frequent disorders among the population that has suffered mild TBI, while hypersomnolence disorders are more frequent in populations that have suffered moderate and severe TBI. The syndrome of obstructiv

Distal Tibial Intramedullary Nailing Using an Extraarticular, Lateral Parapatellar Approach in the Semiextended Position.

Semiextended positioning can improve the surgeon's ability to obtain, maintain, and fluoroscopically evaluate a fracture reduction when performing fixation with an intramedullary nail, especially in fractures at the proximal and distal ends. Furthermore, this position allows for evaluation of instrument placement, including the start point, without moving the fluoroscopic unit into extremes of angulation or compromising the quality of the beam orientation. The intraarticular suprapatellar approach has been described as a soft tissue approach to maintain the leg in a position that would not com

Tegafur-uracil-induced rapid development of advanced hepatic fibrosis.

Tegafur-uracil has been reported to have only minor adverse effects and is associated with liver injury in 1.79% of Japanese patients. The development of tegafur-uracil-induced hepatic fibrosis with portal hypertension is rare. Here, we report a case of a 74-year-old woman with rapidly developing tegafur-uracil-induced hepatic fibrosis. The patient had no history of liver disease and had been treated with tegafur-uracil for 8 mo after breast cancer surgery. The patient was admitted to our hospital for abdominal distension and leg edema associated with liver dysfunction. Computed tomography ima

Fluoroscopic and Gait Analyses for the Functional Performance of a Custom-Made Total Talonavicular Replacement.

The present study evaluated the restoration of joint function in a special clinical case: a professional rock climber who underwent an original total talonavicular replacement with a custom-made prosthesis after a complex articular fracture. Full body gait analysis and 3-dimensional joint kinematics using single-plane fluoroscopy were performed on the same day at the 30-month follow-up examination. Gait analysis was performed using stereophotogrammetric, dynamometric, electromyographic, and baropodometric systems. Gait analysis showed good restoration of rotation, as well as moment patterns in

Complete Closing Wedge Osteotomy for Correction of Blount Disease (Tibia Vara): A Technique.

Treatment of Blount disease (tibia vara) can be daunting in adolescents because of their obesity. The goals in performing osteotomy for Blount disease are to correct the deformity, restore joint alignment, preserve leg length, and prevent recurrent deformity and other complications, such as neurovascular injury, nonunion, and infection. In this article, we report on our treatment of 9 limbs in 8 patients (age range, 13-17 years) with Blount disease. In each case, we performed an oblique complete closing wedge osteotomy of the tibia fixed with a compression plate with oblique screw and a fibula

[Functional hemodynamic monitoring should be emphasized in intensive care for burn and trauma patients].

Hemodynamic monitoring is a very important measure for critically ill patients with burn and trauma, and it should be carried out throughout the course of treatment. Functional hemodynamic monitoring consists of the assessment of the dynamic interactions of hemodynamic variables in response to a defined volume change. Accordingly, response of fluid volume can be assessed in a quantitative fashion by measuring variation of both arterial pulse pressure and left ventricular stroke volunime during positive pressure breathing, or the change in cardiac output response to passive leg raising maneuver

The anterior aspect is the safest area to split a leg cast in the supine position: a study protocol.

BACKGROUND: Involuntary cast-saw burns are attributable to poorly padded casts, cast material, or improper technique. Another factor potentially associated with the risk of injury is the distance between the inner perimeter of the cast and the patient's skin (safety distance). METHODS: Eighty computed tomographic scans of the lower limb following cast application were analyzed. Safety distance was measured above, below, and at the level of the knee and ankle joints at 4 different aspects (posterior, anterior, medial, and lateral). RESULTS: The highest safety distance measured was at the ante

[Squamous cell carcinoma--case report].

Carcinoma that develops in chronic ulcers, wounds, burn scars and fistulas is known as Marjolin's ulcer. It occurs primarily as a squamous cell carcinoma and rarely as basal cell carcinoma. Marjolin's ulcer represents 2% to 5% of all squamous cell carcinomas of the skin. We present a 60-year-old male patient with chronic ulceration on the right leg. The ulceration appeared 15 years before at the localization of the burn scar following an injury and was treated for years as a chronic venous ulcer. After sudden enlargement of the ulcer, biopsy confirmed it to be invasive squamous cell carcinoma.

Talus fractures: a current concepts review of diagnoses, treatments, and outcomes.

The talus is the key articular segment linking the leg and foot, and as such, is subject to complex loads and may occasionally fracture. Fracture patterns provide clues to the underlying pathomechanics and energy of the injury, both of which can help guide treatment and suggest prognosis. Talus fractures have a wide variety of presentation from low-energy avulsion fractures of the lateral or posterior processes, to high-energy comminuted talar body fractures. Appropriate, expedient treatment provides the patient the best chance of obtaining a good functional outcome. Treatment relies on approp

Walking dreams in congenital and acquired paraplegia.

To test if dreams contain remote or never-experienced motor skills, we collected during 6 weeks dream reports from 15 paraplegics and 15 healthy subjects. In 9/10 subjects with spinal cord injury and in 5/5 with congenital paraplegia, voluntary leg movements were reported during dream, including feelings of walking (46%), running (8.6%), dancing (8%), standing up (6.3%), bicycling (6.3%), and practicing sports (skiing, playing basketball, swimming). Paraplegia patients experienced walking dreams (38.2%) just as often as controls (28.7%). There was no correlation between the frequency of walkin

Central pain in the face and head.

All kinds of lesions in the central nervous system can induce central neuropathic pain (e.g., stroke, multiple sclerosis, spinal cord injury, syringobulbia, syringomyelia). The location is the most important feature of the lesion. In stroke, the onset of pain is often delayed. It is mostly constant, but it may be intermittent or paroxysmal. In stroke patients the pain is frequently a hemipain (75%), whereas in the 28% of all multiple sclerosis patients who develop central pain it dominates in the legs (87%) and 5% have trigeminal neuralgia caused by lesions in the brainstem. There is a large v

Microparticles initiate decompression-induced neutrophil activation and subsequent vascular injuries.

Progressive elevations in circulating annexin V-coated microparticles (MPs) derived from leukocytes, erythrocytes, platelets, and endothelial cells are found in mice subjected to increasing decompression stresses. Individual MPs exhibit surface markers from multiple cells. MPs expressing platelet surface markers, in particular, interact with circulating neutrophils, causing them to degranulate and leading to further MP production. MPs can be lysed by incubation with polyethylene glycol (PEG) telomere B surfactant, and the number of circulating MPs is reduced by infusion of mice with PEG or ant
