BACKGROUND: There are currently no pharmacological therapies to address the intellectual disability associated with Down syndrome. Excitatory/inhibitory imbalance has been hypothesized to contribute to impairments in cognitive functioning in Down syndrome. Negative modulation of the GABAA-α5 receptor is proposed as a mechanism to attenuate GABAergic function and restore the excitatory/inhibitory balance.
METHODS: Basmisanil, a selective GABAA-α5 negative allosteric modulator, was evaluated at 120 mg or 240 mg BID (80 or 160 mg for 12-13 years) in a 6-month, randomized, double-blind, placebo-c

BACKGROUND: Many people with cancer experience moderate to severe pain that requires treatment with strong opioids, such as oxycodone and morphine. Strong opioids are, however, not effective for pain in all people, neither are they well-tolerated by all people. The aim of this review was to assess whether oxycodone is associated with better pain relief and tolerability than other analgesic options for adults with cancer pain. This is an updated version of the original Cochrane review published in 2015, Issue 2 on oxycodone for cancer-related pain.
OBJECTIVES: To assess the effectiveness and t

Oral administration is preferred for cancer pain, and when it fails the switch to a parenteral or transdermal route is usually made once and in one direction. We describe a woman with recurrent cervical cancer managed at home for approximately three years in whom several causes of route failure coexisted-short bowel syndrome with a stoma (impairing enteral absorption of sustained-release opioids), profound cachexia (impairing subcutaneous absorption), and recurrent indwelling-device infections (threatening vascular access)-so that the opioid route had to be changed repeatedly and in both direc

Hutchinson-Gilford Progeria Syndrome (HGPS) is a rare premature aging disorder caused by de novo LMNA mutations. Patients develop severe systemic symptoms limiting life quality and ultimately causing death from cardiovascular events. Despite extensive research, treatment options remain limited. Here, we investigated the role of long non-coding RNAs (lncRNAs) in the development of vascular pathology in HGPS. We analyzed an available single-cell RNA sequencing dataset from aortic arch cells of wild-type and LmnaG609G/G609G mice aged 6, 10 and 12 weeks. Several lncRNAs implicated in cardiovascula

Copper (Cu) is a common heavy metal pollutant in aquatic environment, jeopardizing the life safety of aquatic animals. This study aims to investigate the effects of Cu2+ stress on the serum biochemical indicators and alternative splicing(AS) of the liver in rainbow trout. In this study, rainbow trout were divided into a control group and 3 treatment group (75 µg/L, 150 µg/L, and 300 µg/L of Cu2+), which were stressed for 4 d, and restored for 2 d. We determined the dynamics of serum antioxidant indices, and analyzed the changes in liver AS by RNA-seq. The results showed that T-SOD, CAT and MDA

Dementia is a leading and growing cause of disability worldwide. Insomnia is highly prevalent in mild cognitive impairment (MCI) and dementia and is associated with impaired cognition, functional decline, and reduced quality of life (QoL). Although sleep disturbance represents a potentially modifiable risk factor within the trajectory of cognitive impairment, there have not been many studies conducted to examine the feasibility or preliminary efficacy of digital cognitive behavioural therapy for insomnia (dCBT-I) in this population. The aim of this pilot randomised controlled feasibility study

INTRODUCTION: Oral hygiene is associated with fewer adverse events and improved quality of life in patients with stroke. However, these patients tend to experience poor oral hygiene due to self-care limitations. Despite clinical guidelines recommending structured oral hygiene protocols, compliance remains suboptimal in hospital settings.
OBJECTIVES: This project aimed to improve nurses' compliance with best practice recommendations for oral hygiene in patients with stroke.
METHODS: This project followed the JBI Evidence Implementation Framework. A baseline audit and a follow-up audit were co

BACKGROUND: Psoriasis is a systemic inflammatory disease frequently comorbid with obesity, as both conditions share common pathogenic pathways. Acupoint catgut embedding (ACE), a sustained-release form of acupuncture, has demonstrated potential in modulating both immune and metabolic responses. This trial aims to evaluate the efficacy, safety, and underlying mechanisms of ACE in patients with mild psoriasis and concurrent overweight/obesity.
METHODS: This is a multicenter, randomized, double-blind, sham-controlled trial. Participants aged ≥ 18 years with mild plaque psoriasis (PASI< 3, BSA <

Anti-programmed death-1 (PD-1) and programmed death-ligand 1 (PD-L1) inhibitors have made significant advances in the field of immunotherapy, revolutionizing the treatment of metastatic cancers. These immune checkpoint inhibitors have been approved by the U.S. Food and Drug Administration for a wide range of malignancies. However, as the clinical use of PD-1/PD-L1 inhibitors has expanded, concerns have arisen regarding immune-related adverse events (irAEs). Among these, skin reactions can manifest as atypical, non-specific lesions, including vitiligo, eczema, psoriasis, and other cutaneous cha

Splenic artery aneurysm (SAA) is an uncommon and life-threatening vascular disease. This pathology is frequently diagnosed during pregnancy. The major complication of this aneurysm is rupture, which happen frequently in pregnant women due to physiological changes. We report, two rare cases of sudden death due to SAA rupture occurring during the first two trimesters of pregnancy and discovered during autopsy. Case 1: A 26-weeks pregnant woman consulted the Emergency for a sudden onset of abdominal pain and dizziness. An ultrasound showed fetal death. She underwent two surgeries, and the origin

Alzheimer's disease (AD) is not a single entity but a biologically heterogeneous disorder, with substantial inter-individual variability in clinical presentation, molecular drivers, and disease trajectories. This heterogeneity limits current diagnostic and therapeutic strategies, which largely rely on late-stage protein biomarkers that, while sensitive to prodromal impairment, remain insufficient for primary prevention and patient stratification. Lipids are fundamental to brain structure and function, yet their role in the earliest stages of AD remains poorly defined. Here, we identify a robus

Cancer has become a major disease that seriously threatens human health and life worldwide. Cytological diagnosis serves as a cornerstone for clinical oncology. However, conventional cytomorphological evaluation and immunohistochemistry (IHC) remain time-consuming and inadequate for large-scale screening and rapid on-site precision diagnosis. Consequently, novel cytological technologies integrating morphological and molecular information are urgently needed to enable rapid, accurate, and scalable detection. To address this need, we report here a tumor detection strategy based on the MNAzyme-ap

PURPOSE OF REVIEW: Multiple cystic fibrosis transmembrane conductance regulator (CFTR) modulators are approved for the treatment of cystic fibrosis (CF) and show significant improvement in lung function, BMI, quality of life, and sweat chloride. However, their ability to impact liver disease is unclear. This review highlights the current published literature on CFTR modulators and liver health and briefly reviews considerations for clinical management of hepatobiliary disease in the CFTR modulator era.
RECENT FINDINGS: Currently, the primary data available on the clinical efficacy of CFTR mod

TBI-223 is an oxazolidinone antibiotic under clinical development for the treatment of tuberculosis. Preclinical data indicate potent antituberculosis activity and a potentially improved safety profile over linezolid. In a single-ascending dose study and a multiple-ascending dose study in 114 healthy adults, TBI-223 was generally safe and well tolerated at single doses up to 2,600 mg and multiple doses up to 2,400 mg daily over 14 days. No deaths, serious or severe adverse events, or discontinuations resulting from adverse effects (other than COVID-19) occurred. Except for two instances of ort

Functional hypothalamic amenorrhea (FHA) is a form of hypogonadotropic hypogonadism that accounts for approximately 30-35% of secondary amenorrhea in women of reproductive age. This condition is characterized by anovulation due to a reduced gonadotropin-releasing hormone (GnRH) drive, activation of the hypothalamic‒pituitary‒adrenal (HPA) axis and sick euthyroid syndrome, resulting in nonorganic hypoestrogenemia, hypercortisolemia, and hypothyroidism. The stressors contributing to FHA include psychological factors such as perfectionism and mood disorders; psychosocial factors such as adverse l

Resilience is a dynamic adaptation process defined as an individual's ability to overcome and recover from stress or unhappiness. A person's resilience is determined by the balance between risk factors and protective factors. Risk factors increase the likelihood of negative outcomes, whereas protective factors modify responses to the negative event, thereby avoiding potential negative outcomes. Studies on the neurobiology of resilience are heterogeneous and have associations in the structure, activity, and connectivity of prefrontal and subcortical areas. Chronic diseases, which have increased

Podocytopathies are glomerular diseases caused by initial podocyte injury or dysfunction that lead to proteinuria and often nephrotic syndrome. The term encompasses characteristic histological patterns, most commonly focal segmental glomerulosclerosis, minimal changes, membranous nephropathy, diffuse mesangial sclerosis and collapsing glomerulopathy. However, proteinuria of glomerular origin is frequently managed without biopsy; importantly, when the protein loss is mostly albumin, it is a direct readout of podocyte injury and a strong predictor of cardiovascular events, kidney failure and red

Neurological complications during thoracic and thoracoabdominal aortic surgery remain significant issues affecting both postoperative quality of life and long-term survival. Inadequate cerebral and spinal cord perfusion, as well as embolic events, are major contributors to such outcomes. Near-infrared spectroscopy (NIRS)-based regional cerebral oxygen saturation (rSO2) monitoring allows continuous, non-invasive assessment of cerebral perfusion and has become a routine adjunct in high-risk procedures. Factors such as mean arterial pressure, arterial carbon dioxide tension (PaCO2), hemoglobin le
