OBJECTIVES: Deep brain stimulation (DBS) is a well-established therapy for the management of patients with advanced Parkinson's disease and other movement disorders. Patients implanted with DBS require life-long management of the medical device as well as medications. Patients are often challenged to frequently visit the specialized DBS centers and such challenges are aggravated depending on geography, socioeconomic factors, and support systems. We discuss the need for digital health solutions to overcome these barriers to better and safely take care of patients, especially in the current COVI

PURPOSE OF REVIEW: Vascular Ehlers-Danlos syndrome (vEDS) is the most severe form of EDS, affecting the synthesis of type III collagen. It is notable for decreased life expectancy and morbidity, including spontaneous vessel rupture. The present review summarizes recent findings that have improved the ability to manage and delineate the natural history of vEDS.
RECENT FINDINGS: Recent EDS consortium guidelines for the diagnosis of vEDS have emerged and outlined clinical features and molecular diagnostic tools to help facilitate rapid diagnosis. Although medical interventions to help halt the d

Pain and addiction are complex disorders with many commonalities. Beneficial outcomes for both disorders can be achieved through similar principles such as individualized medication selection and dosing, comprehensive multi-modal therapies, and judicious modification of treatment as indicated by the patient's status. This is implicit in the term "medication assisted treatment" (MAT) for opioid use disorders (OUD), and is equally important in pain management; however, for many OUD and pain patients, medication is central to the treatment plan and should neither be denied nor withdrawn if critic

Urinary incontinence is a common problem among women worldwide, resulting in a substantial economic burden and decreased quality of life. The Women's Preventive Services Initiative is the only major organization that recommends annual screening for urinary incontinence in all women despite low to insufficient evidence regarding effectiveness and accuracy of methods. No other major organization endorses screening. Initial evaluation should include determining whether incontinence is transient or chronic; the subtype of incontinence; and identifying any red flag findings that warrant subspeciali

BACKGROUND AND IMPORTANCE: Central retinal artery occlusion (CRAO) is an ophthalmologic emergency due to the sudden cessation of circulation to the inner retinal layer. Without immediate treatment, permanent blindness may ensue. Several treatment options exist, ranging from noninvasive medical management to thrombolysis. Nonetheless, ongoing debate exists regarding the best therapeutic strategy.
CASE PRESENTATION: The authors present the case of a 78-year-old woman with a medical history of hypercholesterolemia and rheumatoid arthritis who experienced complete loss of vision in her left eye.

OBJECTIVES: To evaluate FRAX 10-year fracture probabilities depending on the recommended management strategy in early postmenopausal women who were untreated at baseline.
METHODS: We conducted a descriptive study in 494 untreated women aged 45-60 years seen for the first time at a menopause clinic. Risk factors, physical findings, and bone mineral density (BMD) values determined by dual-energy X-ray absorptiometry were collected. At the end of the clinic visit, 128 (26%) women were prescribed medications. Then, the 10-year fracture probability was estimated using the FRAX tool.
RESULTS: The

Cancer education merits a coordinated, vertical curriculum and an integrated planning strategy. It has become clear that it is as important to teach the techniques of supportive care in oncology as it is to teach the concepts of cancer biology, pathology, epidemiology, prevention, detection and aggressive treatments. Our aim is to determine whether the medical school and nursing school curricula give the students an introduction to the concepts of supportive care of the cancer patient. The spectrum of such supportive care encompasses a wide range of issues working towards a common goal of prov

BACKGROUND: Cold urticaria is a rare subtype of chronic inducible urticaria that is associated with the development of localized or systemic symptoms following exposure to cold stimuli. Although considered benign, it may be associated with severe reactions, including anaphylactoid reactions. This can occur upon direct or indirect exposure to cold temperatures, liquids, and objects. The perioperative environment contains multiple potential cold triggers which can cause generalized hypersensitivity reactions in these patients. Little is known of the perioperative course and management of these p

BACKGROUND: Anti-amyloid therapies (AATs) for Alzheimer's disease complicate emergency stroke care due to the risk of amyloid-related imaging abnormalities (ARIA), which can mimic acute ischemic stroke (AIS) and increase hemorrhagic risk. We evaluated emergency medicine (EM) providers' (physicians and advanced practice providers) and pharmacists' knowledge, confidence, and clinical decision-making related to AAT-associated ARIA.
METHODS: A cross-sectional, web-based survey of EM providers and pharmacists practicing in United States hospital-based emergency care settings between February and A

Periprosthetic joint infection (PJI) is a devastating complication of total joint arthroplasty. Patients with autoimmune and inflammatory diseases, psoriasis, rheumatoid arthritis (RA), and systemic lupus erythematosus (SLE) face substantially elevated infection risk through three convergent mechanisms: skin barrier dysfunction, systemic immune dysregulation, and the immunosuppressive therapies required to control their disease. This narrative review synthesizes evidence from cohort studies, registry analyses, mechanistic investigations, and clinical practice guidelines to examine PJI pathophy

BACKGROUND: Despite the increasing prevalence of pediatric irritable bowel syndrome (IBS), multifaceted treatment, and stomach pain being one of the most common reasons youth see the doctor, there is a dearth of literature concerning self-management in this population.
OBJECTIVES: This pilot study addresses this by providing a characterization of these variables and associated factors via a microlongitudinal and longitudinal design.
METHODS: Daily diary data from 50 youth (10-18 years old) diagnosed with IBS across three 7-day time points were analyzed via multi-level modeling. This study to

BACKGROUND: Reducing lipid levels is essential for preventing major adverse cardiovascular and cerebrovascular events (MACCE) in management of coronary artery disease (CAD) patients with obstructive sleep apnea (OSA). The first line treatment of OSA is continuous positive airway pressure (CPAP), however, its impact on the association between lipid profile and MACCEs is uncertain.
METHODS: This was a secondary analysis of the RICCADSA cohort. In all, 224 revascularized CAD patients with OSA (apnea-hypopnea index [AHI] ≥15 events) were allocated to CPAP, 103 to no-CPAP, and 86 patients had no O

BACKGROUND: Malaria remains the leading parasitic disease worldwide with a significant global morbidity and mortality burden. Plasmodium malariae, the least prevalent of the five Plasmodium species that cause human malaria, has unique characteristics including prolonged prepatent periods and life-long persistance. In non-endemic countries and particular in neonates with coexisting diseases diagnosis and therapy pose challenges.
CASE PRESENTATION: We report a rare case of severe congenital P. malariae malaria in a 2-month-old female infant born in Germany to a Nigerian mother. The infant prese

INTRODUCTION: Patients with life-limiting illnesses are at increased risk of mucosal bleeding. Usual management includes anticipatory planning and sedation, alongside anti-fibrinolytics, despite a lack of evidence for their use. Anti-fibrinolytic agents (tranexamic acid and aminocaproic acid) produce effective haemostasis in different clinical settings. Our aim was to synthesise the evidence for anti-fibrinolytic medication use in adult patients with life-limiting illnesses at risk of, or experiencing, mucosal bleeding.
METHODS: We searched MEDLINE, Embase, CINAHL, Web of Science Conference P

INTRODUCTION: Optimal management of patients with chronic hepatitis B (CHB) requires surveillance for hepatocellular carcinoma (HCC) and identification of antiviral therapy candidates, but few dedicated CHB surveillance models have been described. Kaiser Permanente Northern California developed a systematic CHB surveillance and management program in 2012. We report the results of the program's performance over the initial 8-year period.
METHODS: We conducted a retrospective cohort study of all patients with CHB meeting guideline criteria for HCC surveillance. Eligible patients were invited in

Whether clinical features that establish if GN etiology is medication or TB related, and possible benefits of immunosuppression remain uncertain.
METHODS: A scoping review was completed, searching MEDLINE, EMBASE, Cochrane Central Register of Controlled Trials, Web of Science and Conference Abstracts from Inception to December, 2023. The study population included patients with TB infection who developed GN and under

Inhaled corticosteroid (ICS)-containing therapies are the mainstay of pharmacological management of asthma. They can be administered alone or in combination with a long-acting bronchodilator, depending on asthma severity, and may also be supplemented with short-acting bronchodilators for as-needed rescue medication. Adherence to asthma therapies is generally poor and characterized by underuse of ICS therapies and over-reliance on short-acting bronchodilators, which leads to poor clinical outcomes. This article reviews efficacy versus systemic activity profiles for various dosing regimens of bu
