Tuberculosis (TB), caused by Mycobacterium tuberculosis (Mtb), is one of the most threatening infectious disease worldwide. Selenium, an essential micronutrient, is incorporated into selenoproteins such as glutathione peroxidase, which modulate lipid peroxidation and immune responses, thus playing pivotal roles in host defense against infection and control of Mtb dissemination. As an emerging class of selenium-based medicines, selenium nanoparticles (SeNPs) have demonstrated attractive anti-tuberculosis potential based on their favorable bioactivity, lower toxicity compared, high drug-loading …

BACKGROUND: Tuberculosis (TB) remains a leading infectious cause of death globally. While control strategies have traditionally prioritized symptomatic cases, growing evidence highlights the substantial burden of asymptomatic or subclinical TB, microbiologically confirmed disease without classical symptoms, posing challenges for detection, treatment, and elimination efforts.
METHODS: We conducted a scoping review in accordance with the Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR) guidelines. We selected epidemiological studies,

Tuberculosis (TB) is the leading cause of infection-related mortality in the world, and cases in the United States are increasing. Prompt diagnosis, isolation, and treatment improve patient outcomes and reduce onward transmission. Screening of contacts and high-risk persons for asymptomatic TB infection and treatment of those testing positive reduces progression to disease. TB disease should be suspected in persons with compatible symptoms, risk factors, and consistent imaging. Microbiological confirmation requires an acid-fast smear, nucleic acid amplification test, and culture. Treatment, gu

INTRODUCTION: The WHO endorsed the Xpert MTB/RIF (Xpert) technique since 2011 as initial test to diagnose rifampicin-resistant tuberculosis (RR-TB). No systematic review has quantified the proportion of pretreatment attrition in RR-TB patients diagnosed with Xpert in high TB burden countries.Pretreatment attrition for RR-TB represents the gap between patients diagnosed and those who effectively started anti-TB treatment regardless of the reasons (which include pretreatment mortality (death of a diagnosed RR-TB patient before starting adequate treatment) and/or pretreatment loss to follow-up (P

Tuberculosis (TB), caused by Mycobacterium tuberculosis (Mtb), remains a significant global health challenge, affecting millions annually and leading to substantial mortality, particularly in developing countries. The pathogen's ability to persist latently and evade host immunity, combined with the emergence of drug-resistant strains, underscores the need for innovative therapeutic strategies. This review highlights the crucial role of interleukin-12 (IL-12) in coordinating immune responses against TB, focusing on its potential as an immunotherapy target. IL-12, a key Th1 cytokine, enhances ce…

Pulmonary tuberculosis (PTB), caused by Mycobacterium tuberculosis (MTB), is a serious chronic infectious disease. Although significant progress has been made in the prevention and treatment of MTB, current anti-tuberculosis therapies still face numerous challenges. The human gut microbiota, a complex ecosystem, plays a role in host metabolism, immune regulation, and health maintenance. Recent studies have increasingly highlighted a close relationship between gut microbiota and PTB. The gut microbiota, through the gut-lung axis, mediates the immune processes of PTB, while MTB infection can dis…

Pulmonary tuberculosis (TB), caused by the Mycobacterium tuberculosis (MTB) bacterium, remains a significant health problem worldwide, intensified by the emergence of multidrug-resistant (MDR) and highly resistant (XDR) strains. The current treatment protocols, related side effects, and the increasing incidence of drug resistance limit the efficacy of conventional therapeutic strategies. Traditional medicinal constituents rich in diverse phytoconstituents offer multi-target action with reduced toxicity, minimal risk of resistance, and immunomodulatory properties. Network pharmacology (NP), an …

Tuberculosis (TB) remains one of the top infectious disease killers worldwide, with an estimated 10.6 million new cases and 1.3 million deaths reported in 2022 alone (WHO, 2023). The COVID-19 pandemic has further disrupted TB control efforts by limiting access to healthcare services, interrupting treatment regimens, and delaying diagnoses and leading to a resurgence in TB transmission. Tuberculosis is caused by Mycobacterium tuberculosis and spread through the air, TB posing a serious threat to vulnerable populations, especially those with weakened immune systems such as individuals living wit…

Bromoepiandrosterone (BEA), a synthetic analog of the adrenal steroid DHEA, holds promise as a host-directed therapy for both active and latent tuberculosis (TB). Unlike DHEA, BEA lacks hormonal side effects yet retains potent immunomodulatory activity. It promotes a Th1-skewed immune response by enhancing interferon-γ (IFN-γ) and tumor necrosis factor-α (TNF-α), critical cytokines for macrophage activation and intracellular control of Mycobacterium tuberculosis (Mtb), while suppressing Th2 cytokines such as IL-4. BEA also inhibits 11β-hydroxysteroid dehydrogenase-1, lowering intracellular cor…

Pulmonary Tuberculosis (PTB) is an infectious disease caused by a bacterium called Mycobacterium tuberculosis. This paper aims to create Symbolic Artificial Intelligence (SAI) system to diagnose PTB using clinical and paraclinical data. Usually, the automatic PTB diagnosis is based on either microbiological tests or lung X-rays. It is challenging to identify PTB accurately due to similarities with other diseases in the lungs. X-ray alone is not sufficient to diagnose PTB. Therefore, it is crucial to implement a system that can diagnose based on all paraclinical data. Thus, we propose in this p…

Tuberculosis (TB) is one of the major killer diseases affecting lung parenchymal tissues. Mycobacterium tuberculosis (Mtb) is the bacterium that causes it. It most commonly affects the lungs, although it can affect any part of the body, including the stomach, glands, bones, and nervous system. Although anti-mycobacterial drugs are available, it remains a major threat to public health due to the rise of drug-resistant strains, and early and accurate diagnosis is very important. Currently, research science and medical communities are focusing on the use of cost-effective biosensors to manage hum…

Blood-based correlates of vaccine-induced protection against tuberculosis (TB) are urgently needed. Here, we analyze the blood transcriptome of rhesus macaques immunized with varying doses of intravenous (i.v.) BCG followed by Mycobacterium tuberculosis (Mtb) challenge. We use high-dose i.v. BCG recipients for "discovery" and validate our findings in low-dose recipients and in an independent cohort of macaques receiving BCG via different routes. We identify seven vaccine-induced gene modules, including an innate module (module 1) enriched for type 1 interferon and RIG-I-like receptor signaling…

Tubercular uveitis (TB-uveitis) remains a conundrum in the uveitis field, which is mainly related to the diverse clinical phenotypes of TB-uveitis. Moreover, it remains difficult to differentiate whether Mycobacterium tuberculosis (Mtb) is present in the ocular tissues, elicits a heightened immune response without Mtb invasion in ocular tissues, or even induces an anti-retinal autoimmune response. Gaps in the immuno-pathological knowledge of TB-uveitis likely delay timely diagnosis and appropriate management. In the last decade, the immunopathophysiology of TB-uveitis and its clinical manageme…

The causative agent of tuberculosis is still a widespread pathogen, which caused the death of ca. 1.6 million people globally in 2021. The paleopathological study of human remains revealed the antiquity of the disease and its continuous presence throughout the history of humankind. The Carpathian Basin has always been a biocultural melting pot, since it has seen several migrations over the centuries, and served as a location of admixture and interaction for numerous populations of different cultures. Thus, this geographical territory is ideal for the examination of the coevolutionary processes

Two in every five patients with active tuberculosis (TB) remain undiagnosed or unreported. Therefore community-based, active case-finding strategies require urgent implementation. However, whether point-of-care (POC), portable battery-operated, molecular diagnostic tools deployed at a community level, compared with conventionally used POC smear microscopy, can shorten time-to-treatment initiation, thus potentially curtailing transmission, remains unclear. To clarify this issue, we performed an open-label, randomized controlled trial in periurban informal settlements of Cape Town, South Africa,

Our study was done to identify patient or health system related delay in diagnosis of treatment naïve sputum positive PTB patients and its impact on sputum conversion at 2 months. A total of 280 patients were enrolled and followed up for 2 months following which repeat sputum examination was done. Detailed clinical history was taken with emphasis on duration of symptoms, day of presentation, diagnosis and starting of therapy. The median(range) of delay in treatment initiation was 15 (0-82) days, most commonly because of delay due to the health system. Patients with positive sputum at the end o

Mycobacterium tuberculosis (Mtb) is the causative agent of human tuberculosis (TB) which primarily infects the macrophages. Nearly a quarter of the world's population is infected latently by Mtb. Only around 5%-10% of those infected develop active TB disease, particularly during suppressed host immune conditions or comorbidity such as HIV, hinting toward the heterogeneity of Mtb infection. The aerosolized Mtb first reaches the lungs, and the resident alveolar macrophages (AMs) are among the first cells to encounter the Mtb infection. Evidence suggests that early clearance of Mtb infection is a…
