Reflections on the domino effect of a patient with multimorbidity: A case report and literature review.

RATIONALE: As China enters an accelerated stage of global population aging, multimorbidity has emerged as a significant public health challenge. Current management strategies often focus primarily on index diseases, neglecting the complex interplay of multiple conditions in elderly patients. PATIENT CONCERNS: An 86-year-old female with multimorbidity, including diabetes, hypertension, and osteoporosis, sustained a femoral fracture. Following "artificial bone replacement" surgery, she experienced significant loss of consciousness with a drastic decline in hemoglobin from 110 to 39 g/L. DIAGNO

Letter to the editor regarding "osteonecrosis of the femoral head in post-COVID-19 patients: a retrospective comparative study".

We commend Seong et al. for exploring associations between COVID-19 and osteonecrosis of the femoral head (ONFH). However, their retrospective single-center study (N = 84) has critical limitations. The exclusion of patients with chronic comorbidities and highly imbalanced group sizes (e.g., DEX + MPS:12 vs. DEX:66) risk bias and reduced generalizability. Steroid dose equivalence (1 mg MPS = 0.1875 mg DEX) lacks authoritative validation, potentially underestimating MPS’s osteotoxicity. Higher cumulative doses in the DEX + MPS group (380 mg vs. 125 mg DEX) were attributed to “pharmacological syn

Multi-Omics Insights into Gingivitis from a Clinical Trial: Understanding the Role of Bacterial and Host Factors.

Poor oral health is a neglected epidemic, potentially contributing to systemic health issues. We employed a multi-omics approach to investigate the biological changes associated with gingivitis and the effects of stannous fluoride (SnF2) dentifrice on microbial composition and salivary proteomics in an eight-week clinical trial involving 39 participants categorized as high (n = 20) and low bleeders (n = 19). Baseline assessments revealed significant microbial dysbiosis in high bleeders, characterized by a higher abundance of Porphyromonas and Fusobacterium, alongside compromised epithelial bar

Impact of nanostructured formulations for schistosomiasis treatment: a systematic review of in vivo preclinical evidence.

BACKGROUND: Schistosomiasis is a neglected tropical disease caused by Schistosoma sp., and praziquantel (PZQ) is the first-line treatment. However, traditional PZQ formulations have low solubility and fast metabolism, limiting its effectiveness. Thus, nanoparticles have been proposed to improve the bioavailability and efficacy of poorly soluble antischistosomal drugs. AIMS: This systematic review used in vivo preclinical studies to map the available evidence and compare the efficacy of free PZQ and PZQ-based nanostructured formulations (N-PZQ) for schistosomiasis treatment. METHODS: PubMed,

Apoptotic proteins in Leishmania donovani: in silico screening, modeling, and validation by knock-out and gene expression analysis.

Visceral leishmaniasis, a life-threatening vector-borne illness that disproportionately affects children and elderly immunocompromised people, is a primary tropical neglected disease. No apoptotic partner proteins have yet been reported in Leishmania donovani, while their identification could contribute to knowledge on parasite cell death and the establishment of alternative therapeutics. We searched for mammalian Bcl-2 family protein orthologs and found one anti-apoptotic and two pro-apoptotic orthologs in L. donovani. A pro-death aquaporin protein, due to its characteristic BH3 domain known

Leishmania donovani homoserine dehydrogenase: Biochemical and structural characterization of a novel parasite specific enzyme of aspartate pathway.

Visceral leishmaniasis is a neglected tropical disease. Drug resistance and toxicity are the critical issues with the currently available antileishmanial drugs. Therefore, research efforts are underway to identify and validate new drug targets specific to Leishmania parasite. The enzyme homoserine dehydrogenase (HSD) functions in the third step of aspartate pathway. The present study focuses on the biophysical and biochemical characterization of HSD enzyme from Leishmania donovani (LdHSD) which is unique to the parasite with no homologous enzyme in the host. LdHSD gene was cloned in pET28c(+)

Development of the antigenic recombinant tropomyosin of Echinococcus granulosus in a bacterial system as a vaccinal candidate against canine echinococcosis.

Motivation for the study. Cystic echinococcosis is a neglected disease associated with contact between dogs, humans and sheep. In countries such as Peru, control programs include vaccination of sheep; however, vaccination in dogs is a late control strategy to eliminate the adult parasite or to avoid infection with eggs in the environment. Main findings. We were able to clone and express a recombinant protein (tropomyosin) of the adult parasite in a bacterial system with immunogenic properties. Implications. Obtaining the tropomyosin recombinant protein from E. granulosus allows the development

Minimally invasive punch cytology with maceration for diagnosing Jorge Lobo disease in the Brazilian Amazon.

Jorge Lobo disease (JLD) is a neglected cutaneous mycosis prevalent in the Brazilian Amazon, where limited access to histopathology delays diagnosis and care. We assessed minimally invasive punch cytology with maceration (MIPC-M), a simple procedure that uses a 1-mm punch followed by potassium hydroxide maceration and light microscopy, as a field-ready alternative. In a prospective, cross-sectional study (2014-2019) across 11 municipalities in the Brazilian Amazon, individuals ≥12 years with clinically suggestive lesions underwent three cytological approaches in parallel-adhesive tape cytology

Synthesis and antimicrobial evaluation of hybrids methoxynaphthalene-N-acylhydrazones/1,3,4-oxadiazoles.

AIMS: Paracoccidioidomycosis is a neglected tropical disease caused by the fungi Paracoccidoides spp. In cases of clinical manifestations involving the lung, its symptoms can be confused with tuberculosis, in addition to the possibility of patients presenting a co-infection between the two. Considering previous studies by our research group evaluating N-acylhydrazones and 1,3,4-oxadiazoles containing the 4-methoxynaphthalene ring, this work proposed the synthesis of new N-acylhydrazones and new 1,3,4-oxadiazoles containing the 2-methoxynaphthalene ring. MATERIALS & METHODS: Starting from the

Transmission risk of Trypanosoma cruzi through blood donors in a third-level care hospital.

Blood transfusion is a major way of Trypanosoma cruzi transmission, a neglected parasite infection that produces Chagas disease (CD). CD has high morbidity and is potentially fatal in the chronic phase due to the parasite's ability to invade the myocardium, colon, and esophagus. CD prevalence focuses on Latin America; however, its presence in non-endemic areas has been increasing in the last decade, particularly in North America and Europe, due to high migration rates of infected people. This study established the CD seroprevalence in a blood bank located in Mexico City. A total of 851 donor s

AMF-responsive nanobiomagnetite for targeted delivery of amphotericin B against Leishmania amazonensis.

Cutaneous leishmaniasis remains a neglected tropical disease with limited treatment options. Available therapies include costly and toxic drugs for which recurrent cases of resistance are reported. Drug delivery systems based on the association of approved drugs and nanoparticles have improved pharmacological properties of the drug, such as targeted therapy, enhanced drug solubility, reduced side effects, and potentially lower doses required for effective treatment. In this study, we explored the in vitro potential use of the nanobiomagnetite produced by magnetotactic bacteria functionalized w

Cellulose-lignin wound patch on Janus textile support for dual tissue regeneration and antibacterial action in cutaneous leishmaniasis wounds.

Leishmaniasis is a neglected tropical disease (NTD) caused by Leishmania parasites and the most common form is known as cutaneous leishmaniasis (CL) causing non-healing lesions and ulcers and making the already immunocompromised patients more vulnerable to other infections. Secondary bacterial infections and excess inflammation within the wound area are two critical factors that negatively impact CL wound healing. An optimal wound dressing should address these issues and contribute to the acceleration of wound closure even in a limited clinical setting. Sugar-cane bagasse derived cellulose:lig

Scrub Typhus and Influenza A Co-Infection: A Case Report.

Scrub typhus, caused by Orientia tsutsugamushi, is a neglected and reemerging disease that causes considerable morbidity and mortality. It now extends beyond the Tsutsugamushi Triangle, the region wherein it has traditionally been endemic. Influenza has also resurged since the infection control measures against COVID-19 were relaxed. A few cases of scrub typhus and influenza co-infection have been reported. Herein, we report the case of a 74-year-old woman with fever and upper respiratory symptoms diagnosed with influenza A and treated with oseltamivir; however, her fever persisted, and she de

The development of an Actinomadura madurae grain model in Galleria mellonella larvae.

Mycetoma is a neglected tropical disease characterized by large tumorous lesions. It can be caused by fungi (eumycetoma) or bacteria (actinomycetoma). The hallmark of mycetoma is the formation of grains by the causative agent. Grains can only be formed in vivo; therefore, in vivo models are crucial to studying mycetoma. In vivo, grain models have been developed in the invertebrate Galleria mellonella for eumycetoma, but not for actinomycetoma. Here, we aimed to develop the first actinomycetoma grain model in G. mellonella larvae. Actinomadura madurae strains DSM43236 and DSM44005 were used to

In Vitro Inhibition of Cryptosporidium parvum Infection by the Olive Oil Component Oleocanthal.

Human cryptosporidiosis caused by the zoonotic apicomplexan parasite Cryptosporidium parvum represents a neglected and re-emerging poverty-related disease. C. parvum possesses minimalistic metabolic capacities and highly depends on its intestinal epithelial host cell for intracellular replication. Based on previous results showing that glycolysis and glutaminolysis inhibition diminished C. parvum replication in vitro, we here investigated the impact of the olive oil component oleocanthal on C. parvum infection in HCT-8 cells under physioxia (5% O2) and hyperoxia (21% O2). Oleocanthal targets a

Francisella tularensis in Wild Lagomorphs in Southern Spain's Mediterranean Ecosystems.

Francisella tularensis is a vector-borne zoonotic bacterium that causes tularemia, a disease of great importance for animal and public health. Although wild lagomorphs are considered one of the major reservoirs of this bacterium, information about the circulation of F. tularensis in European wild rabbit (Oryctolagus cuniculus) and Iberian hare (Lepus granatensis) populations in Europe is still very limited. In Spain, F. tularensis is present in northern central regions, with recurrent outbreaks occurring annually. However, southern Spain has been neglected in terms of monitoring and research.

Individual Differences in Bodily Self-Consciousness and Its Neural Basis.

Bodily self-consciousness (BSC), a subject of interdisciplinary interest, refers to the awareness of one's bodily states. Previous studies have noted the existence of individual differences in BSC, while neglecting the underlying factors and neural basis of such individual differences. Considering that BSC relied on integration from both internal and external self-relevant information, we here review previous findings on individual differences in BSC through a three-level-self model, which includes interoceptive, exteroceptive, and mental self-processing. The data show that cross-level factors
