New approach for the diagnosis of extractions with neural network machine learning.

INTRODUCTION: The decision to extract teeth for orthodontic treatment is important and difficult because it tends to be based on the practitioner's experiences. The purposes of this study were to construct an artificial intelligence expert system for the diagnosis of extractions using neural network machine learning and to evaluate the performance of this model. METHODS: The subjects included 156 patients. Input data consisted of 12 cephalometric variables and an additional 6 indexes. Output data consisted of 3 bits to divide the extraction patterns. Four neural network machine learning model

Bone Density and Dental External Apical Root Resorption.

When orthodontic patients desire shorter treatment times with aesthetic results and long-term stability, it is important for the orthodontist to understand the potential limitations and problems that may arise during standard and/or technology-assisted accelerated treatment. Bone density plays an important role in facilitating orthodontic tooth movement (OTM), such that reductions in bone density can significantly increase movement velocity. Lifestyle, genetic background, environmental factors, and disease status all can influence a patients' overall health and bone density. In some individual

An interdisciplinary noninvasive all-ceramic treatment concept for nonsyndromic oligodontia in adolescence.

Oligodontia has a substantial oral functional and psychosocial impact on the quality of life of children. The treatment of oligodontia in adolescence is an interdisciplinary approach which can include extraction of the primary teeth with orthodontic space closure, or prosthodontic rehabilitation. This case report describes a conservative approach for the rehabilitation of a 12-year-old patient with 19 ageneses (excluding third molars) of permanent teeth, infraocclusion of the persisting primary teeth, deep overbite, and reduced mesiodistal dimension of the maxillary incisors with a central dia

Multiple Unerupted Permanent Teeth Associated with Noonan Syndrome.

The present report describes a case of Noonan's syndrome from a dental viewpoint. Noonan syndrome is an autosomal dominant multisystem disorder. Congenital heart deformities, short stature, thoracic deformities, short neck with webbing, hypertelorism, and malocclusions are some of the frequently observed clinical features. Atypical dental anomalies such as multiple unerupted permanent teeth, multiple submerged and retained deciduous teeth, and supernumerary teeth were found in the present case. Oral prophylaxis and preventive resin restorations were done following which the supernumerary teeth

[Primary failure of eruption: diagnostics, treatment, casus and review of literature].

Primary failure of eruption is a rare eruption disorder of above all, the permanent second and sometimes the first molars. It is characterized by infra occlusion of the molars resulting in a severe lateral open bite. Primary failure of eruption is a disorders which affects all molars distal to the most mesial involved tooth. Diagnosis is possible both clinically and with radiographs. A panoramic radiograph combined with clinical findings of impaction or infra occlusion can confirm the suspicion of primary failure of eruption. Primary failure of eruption cannot be treated as other eruption dist

Stress distribution pattern in a root of maxillary central incisor having various root morphologies: a finite element study.

OBJECTIVE: To investigate stress distribution in the roots of maxillary central incisors bearing various types of root morphologies with regard to application of different types of orthodontic forces using the finite element model (FEM). MATERIALS AND METHODS: FEMs of maxillary central incisors with different root morphologies (normal, short, blunt, dilacerated, and pipette) were constructed, and orthodontic forces in various directions (intrusion, extrusion, tipping, and rotational) were applied to the tooth axis at the bracket level. RESULT: On application of various forces, significantly

Intraoral vertical ramus osteotomy improved the stomatognathic function in an elderly patient with mandibular protrusion: a case report.

This article reports the successful surgical-orthodontic treatment of an elderly patient with dentofacial deformity and signs and symptoms of temporomandibular disorder (TMD). The patient was a 63-year-old woman with a concave profile due to mandibular protrusion. To correct skeletal deformities, the mandible was posteriorly repositioned by employing intraoral vertical ramus osteotomy (IVRO) following presurgical orthodontic treatment. After active treatment for 31 months, the facial profile was significantly improved and satisfactory occlusion was achieved. In addition, TMD symptoms of clicki

Predictive model of third molar eruption after second molar extraction.

INTRODUCTION: Extraction of second permanent molars is an option for providing space in orthodontic treatment. Although many articles have described its impact on the outcome, there are few data on the prognosis of the eruption of the adjacent third molars. The aims of this investigation were to provide predictive models of eruption of third molars after second permanent molar extraction and to validate them. METHODS: A total of 48 patients (ages, 11-23 years) who had 128 second permanent molars (54 maxillary, 74 mandibular) extracted during orthodontic treatment were followed until eruption

Does rapid maxillary expansion increase nasopharyngeal space and improve nasal airway resistance?

OBJECTIVE: To evaluate the effect of rapid maxillary expansion (RME) on the dimension of the nasopharyngeal space and its relation to nasal airway resistance. METHODS: Twenty-five school-age children (from 7 to 10 year-old) with mouth and/or mixed breathing, with mixed dentition and uni- or bilateral posterior crossbite involving the deciduous canines and the first permanent molars, were evaluated. RME was placed and remained during 90 days. Rhinomanometry and orthodontic documentation were performed at four different times, i.e., before (T(1)), immediately after (T(2)), 90 days (T(3)) and 30

The behaviour of the periodontal ligament is influencing the use of new treatment tools.

Orthodontic treatment can cause resorptions on teeth. Factors causing resorptions are forces and moments. Other factors that cause resorptions are less influenced by an orthodontic treatment. By carrying out experiments and finite element calculations, it was possible to demonstrate that forces and moments will produce stress in certain parts of the periodontal ligament and the root. This stress reaches or exceeds the blood pressure in some areas. To avoid high pressure in those areas, new treatment devices were constructed consisting of NiTi wires or a combination of NiTi and stainless steel.

Four impacted permanent canines: an unusual case.

Impacted permanent canines in a child, particularly in the maxilla, pose a functional as well as an esthetic concern for the patient and parents alike. Informing the parties involved that all the permanent canines are impacted is, needless to say, a distressful dilemma for the parents and a unique challenge for the orthodontist. The response to the treatment in this case was quite acceptable. All 4 canines were properly aligned with the aid of orthodontic traction and surgical assistance, and now the patient possesses confidence to smile and has enhanced self-esteem. There is no additional cos

Ehlers-Danlos type VIII. Review of the literature.

Ehlers-Danlos type VIII is a rare disorder characterized by soft, hyperextensible skin, abnormal scarring, easy bruising, and generalized periodontitis with early loss of teeth. To illustrate the clinical dermatological and dental features, we present the case history of a 20-year-old patient who has suffered from poor healing of wounds at the shins and knees since childhood, which have developed into hyperpigmented atrophic scars. In the course of orthodontic treatment during the last 3 years, severe apical root resorption, gingival recession, and loss of alveolar bone were observed. Family h

A longitudinal study on the effect of unilateral extraction of primary molars.

The effect of unilateral extraction of the first or second primary molar on space conditions, need for orthodontic treatment, and eruption stage of permanent canines and second molars has been studied longitudinally in two groups. In Group 1 the first primary molar had been extracted early in 13 cases in the maxilla and in 14 cases in the mandible. In Group 2 the corresponding figures for the second primary molar were 14 and 24 for the maxilla and mandible, respectively. The space on the extraction side in Group 1 was significantly less than on the control side at the age of 9 but not at the a

Correction of the long face syndrome by Le Fort I osteotomy. A report on some new technical modifications and treatment results.

The combined efforts of multiple specialists are needed for the successful treatment of patients with the long face syndrome. The plan of therapy is designed to correct the individual's dentofacial deformity and achieve the best possible occlusion and facial esthetics. Reduction of facial height by surgical means and proper alignment of the teeth by orthodontic and restorative procedures are the common denominators of successful treatment. By properly planned and executed Le Fort I maxillary osteotomies, the vertical dimension of the face can be shortened to improve the esthetic balance betwee

Diagnosis, treatment, and management recommendations for cleidocranial dysplasia: A modified Delphi panel.

Cleidocranial dysplasia is a rare genetic condition negatively impacting skeletal development. Clinical guidelines for patients, their family members, and clinicians on the diagnosis, treatment, and management of this disease are lacking. The aim of this study was to align expert opinion on standard of care (SoC) medical recommendations for patients with cleidocranial dysplasia, primarily for adults and children (≥2 yrs of age). A modified Delphi panel comprised of a 3-round survey was used to determine consensus among a multidisciplinary team of 13 experts with experience treating patients wi

Clinical Soft Tissue Adaptation to Biomechanical Modulation with the Bone Protection System (BPS): A Two-Case Report in Thin-Biotype Patients.

Background: Patients with a thin gingival phenotype and a narrow buccal alveolar plate are highly susceptible to periodontal complications during orthodontic expansion. Traditional biomechanics often fail to maintain root control in thin alveolar housing. This report presents two clinical cases illustrating soft- and hard-tissue responses to a novel biomechanical approach, the Bone Protection System (BPS), designed to reduce buccal cortical overload during expansion. Case Presentation: Two adult patients with a thin gingival phenotype assessed by a standardized periodontal probe transparency t
