Free vascularized fibular grafts in Type 3 open tibia fractures.

OBJECTIVE: The aim of this study was to evaluate the results and complications of free vascularized fibular grafting (FVFG) for the reconstruction of bone defects in Gustilo Type 3 open tibia fractures. METHODS: Free vascularized fibular grafting was performed on open tibia fractures with a bone defect of an average of 10 (range: 6 to 18) cm in 21 patients (19 males, 2 females; mean age: 32 years; range: 16 to 47 years). Primary FVFG was performed for 15 patients with open fractures and secondary FVFG in 6 patients with osteomyelitis due to open fractures or defective nonunions. Bony unions w

Patient/caregiver perceived benefits and barriers to elective orthopedic surgery (EOS) in patients with congenital hemophilia with inhibitors.

OBJECTIVE: Congenital hemophilia patients with inhibitors are at greater risk for developing arthropathy and orthopedic complications compared to those without inhibitors. Elective orthopedic surgeries (EOS) may be an option for these patients and may provide long-term cost savings due to reduced bleed frequency. However, patient motivations and goals for undergoing or delaying such surgeries are not well understood. A US-based patient/caregiver survey was designed to describe inhibitor patient experiences and outcomes following EOS and to develop a comprehensive understanding of patient prefe

Current applications of 3-d intraoperative navigation in craniomaxillofacial surgery: a retrospective clinical review.

Intraoperative navigation is a tool that provides surgeons with real-time, interactive access to their patient's diagnostic imaging studies while in the operating room. This modality allows for anatomic localization and facilitates intraoperative planning and diagnosis. The application of intraoperative navigation to neurosurgery, otolaryngology, and orthopedic surgery has been well documented; however, only isolated reports have analyzed its potential in the field of craniomaxillofacial surgery. Advancements in 3-dimensional navigational systems have greatly improved the accuracy of the techn

Treatment of PCL, ACL, and lateral-side knee injuries: acute and chronic.

Combined posterior cruciate ligament, anterior cruciate ligament, and lateral-side disruption is one of the more common patterns of multiligament knee injury. This is a devastating injury with significant long-term functional sequelae, making accurate diagnosis and management extremely important. While surgical intervention is necessary to restore function, the specific management strategies remain controversial. This article will review the current literature and the authors' preferred approach including physical examination, imaging, timing of surgery, surgical technique, and postoperative r

Vertebrectomy and expandable cage placement via a one-stage, one-position anterolateral retroperitoneal approach in L5 tumors.

Spinal reconstruction of the L5 vertebrae after tumor resection remains a challenge. Complex resection followed by circumferential fixation in the same setting, or in staged fashion, is often employed. The added operative time associated with this method potentially increases morbidity and mortality in an inherently high-risk procedure and anatomy in the lumbosacral area makes reconstruction more challenging. The authors describe a technique involving L5 vertebrectomy, placement of an expandable cage, and anterolateral L4-S1 screw fixation via a one-stage, one-position, anterolateral retroperi

Elective orthopaedic surgery for haemophilia patients with inhibitors: single centre experience of 40 procedures and review of the literature.

With the introduction of safe and effective factor VIII/IX-bypassing agents--recombinant activated factor VII (rFVIIa) and plasma-derived activated prothrombin complex concentrates (pd-APCC)--elective orthopaedic surgery (EOS) is a viable option for haemophilia patients with inhibitors. We report a series of patients with haemophilia and inhibitors undergoing EOS between 1997 and 2008 using bypassing agents to provide haemostatic cover. All inhibitor patients undergoing EOS and receiving rFVIIa, plasma-derived prothrombin complex concentrates (pd-PCC) or pd-APCC as haemostatic cover were inclu

Accuracy of the anterior apprehension test as a predictor of risk for redislocation after a first traumatic shoulder dislocation.

BACKGROUND: The treatment options for a first traumatic shoulder dislocation in a young patient are either nonoperative care or primary surgery. It would be valuable to find patient-specific assessments that could predict the risk for redislocation in these patients and thereby identify those who would benefit from primary surgery. HYPOTHESIS: The supine apprehension test, performed after completion of physical therapy in first traumatic shoulder dislocators, can predict risk for redislocation. Patients with a positive test would be at very high risk for redislocation and therefore would be c

Resource utilization in clubfoot management.

UNLABELLED: Both private and socialized healthcare systems require treatments to be not only effective, but also cost-efficient. Although the Ponseti method of clubfoot treatment is effective, its cost-effectiveness has not been demonstrated. We compared the difference in resource use between two prospective cohorts treated for clubfoot by either the Ponseti method or below-knee casting followed by primary surgical release in the socialized healthcare system of New Zealand. Using these cohorts and US billing data, costs of treating these cohorts in the US healthcare system were also calculated

Ponseti method for untreated idiopathic clubfeet in Nepalese patients from 1 to 6 years of age.

UNLABELLED: Although the Ponseti method has been effective in patients up to 2 years old, limited information is available on the use of this method in older patients. We retrospectively reviewed the records of 171 patients (260 feet) to determine whether initial correction of the deformity (a plantigrade foot) could be achieved using the Ponseti method in untreated idiopathic clubfeet in patients presenting between the ages of 1 and 6 years. A mean of seven casts was required, and there were no differences in the number of casts between the different age groups. Two hundred fifty (95%) of the

The influence of femoral technique for graft placement on anterior cruciate ligament reconstruction using a skeletally immature canine model with a rapidly growing physis.

PURPOSE: The purpose of this study was to evaluate 3 different femoral techniques of anterior cruciate ligament (ACL) reconstruction using a skeletally immature canine model. METHODS: A soft-tissue autograft ACL reconstruction was performed in 25 ten-week-old canines via a central transphyseal tibial tunnel and 1 of 3 femoral techniques: epiphyseal, over the top, or transphyseal. The contralateral hind limbs served as controls. The canines were killed at 16 weeks postoperatively and evaluated by gross inspection, plain radiographs, photography, magnetic resonance imaging, and histomorphometry

How to Improve Outcomes of Spine Surgery in Geriatric Patients.

BACKGROUND: The incidence of spine surgery in elderly patients is increasing. Geriatric spine surgery has 3 main concerns: osteoporosis, continuing degeneration and more deformity, and comorbidities. Measures taken regarding these concerns would improve results, and elderly patients will experience much more benefit from surgery. This study reviewed the most recent literature to improve outcomes of geriatric spine surgery. METHODS: A literature search of the last 10 years was done. RESULTS: Outcomes of spine surgery using decompressive techniques only are similar to outcomes in younger patie

Construction and evaluation of a PONV prediction model in patients undergoing orthopedic surgery under spinal anesthesia: a prospective cohort study.

BACKGROUND: Postoperative nausea and vomiting (PONV) is a common complication following anesthesia. Most existing studies have focused on patients undergoing non-orthopedic surgeries with general anesthesia, neglecting the occurrence of PONV in patients receiving spinal anesthesia. This study aims to investigate the potential risk factors for PONV in patients undergoing orthopedic surgery with spinal anesthesia through a prospective cohort study and to construct and evaluate a clinical prediction model. METHODS: Relevant factors associated with PONV in patients undergoing elective orthopedic s

Surgical outcomes after superior peroneal retinaculum repair without bony procedures for chronic peroneal tendon instability: A consecutive case series.

BACKGROUND: Although bony procedures tailored to retromalleolar groove morphology have been advocated to reduce recurrence in chronic peroneal tendon instability, concerns remain regarding tendon adhesion and procedure-related morbidity. Consequently, it remains unclear whether isolated superior peroneal retinaculum repair alone is sufficient across different fibular morphologies, providing the rationale for the present study. PURPOSE: This study evaluated whether isolated superior peroneal retinaculum repair, without adjunctive bony procedures, can restore stability and function in chronic p

The effect of preoperative oral carbohydrate intake on perioperative well-being in patients undergoing hip and lower extremity orthopedic surgery under spinal anesthesia: a randomized controlled trial.

BACKGROUND: The administration of preoperative oral carbohydrate (POC) has been shown to enhance patient well-being and expedite postoperative recovery. Nevertheless, evidence regarding its efficacy in orthopedic patients remains insufficient and warrants further investigation. This study aimed to address gaps by evaluating the effect of preoperative oral dextrose (POD) intake on perioperative well-being parameters (including thirst, hunger, anxiety, pain, nausea, and vomiting) in patients undergoing hip and lower extremity surgeries with spinal anesthesia. METHODS: In this randomized control

Evaluation of the functional outcome measures of recurrent clubfeet after posteromedial or posterior release treated using the Ponseti method: a preliminary study.

BACKGROUND AND AIM: The Ponsetimethod has been popularized for treating recurrent idiopathic clubfoot and, more recently, recurrent clubfoot following posteromedial (PMR) or posterior (PR) release. A retrospective study was performed to determine the functional outcome in Ponseti treatment of recurrent idiopathic clubfoot after PMR or PR. The study also aimed to determine whether clinical scoring systems (Pirani and Dimeglio scores) could be predictive for the initial numbers of casts, Achilles tenotomy, ankle dorsiflexion after initial treatment, relapse, and functional outcomes following the
