Nanoindentation was used to characterize the intrinsic mechanical properties of bone tissue from eight (8) children with type III Osteogenesis Imperfecta (OI). The bone samples were harvested from the cortex portion at the site of bowing (the mid 2/3 of the shaft of the tibia/femur). Unlike normal bone tissue, OI type III cortical bone exhibited more isotropic material properties. Young's modulus and hardness values measured in the longitudinal direction did not show significant differences from the transverse measurements. No differences were observed in modulus or hardness in an analysis of …

Upper airway stenosis in patients with faciocraniosynostosis is very common and often severe. Midface advancement, either with a Le Fort III or concomitantly to a monobloc frontofacial advancement, may prevent a tracheotomy or result in its ablation. The amelioration of respiratory function appears to be much better if the midface advancement is combined with distraction osteogenesis, although large studies with long-term follow-up are rare. In this study, we reviewed the respiratory outcome between Le Fort III with distraction and monobloc advancement with distraction in 54 faciocraniosynosto…

Minimal trauma fractures in bone diseases are the result of bone fragility. Rather than considering bone fragility as being the result of a reduced amount of bone, we recognize that bone fragility is the result of changes in the material and structural properties of bone. A better understanding of the contribution of each component of the material composition and structure and how these interact to maintain whole bone strength is obtained by the study of metabolic bone diseases. Disorders of collagen (osteogenesis imperfecta and Paget's disease of bone), mineral content, composition and distri…

The mechanisms involved might include improved osteogenesis and angiogenesis. This new therapeutic approach should modify the treatment of early-stage osteonecrosis of the femoral head.

The role of bisphosphonates in osteogenesis imperfecta was further confirmed, and novel roles in ankylosing spondylitis, myelofibrosis, and hypertrophic pulmonary osteoarthropathy were suggested.

Genu varum is a relatively common finding in children. Physiologic bowing, which is seen most often, has a well-documented favorable natural history. Idiopathic tibia vara is the most common of the pathologic conditions that are associated with bowed legs; treatment strategies vary with the patient's age and the stage of disease and deformity. Genu varum may also accompany systemic conditions, such as achondroplasia, vitamin D-resistant rickets, renal osteodystrophy, and osteogenesis imperfecta-all of which can result in short stature. Indications for intervention are not always well defined. …

OBJECTIVE: We examined the accuracy of prenatal diagnosis of skeletal dysplasias and ways to refine this ability.
STUDY DESIGN: A total of 226 fetuses and stillbirths referred for suspected skeletal dysplasia were evaluated. The fetal age, mode of ascertainment, and referring diagnosis were analyzed with regard to the final diagnosis.
RESULTS: The leading mode of diagnosis was routine ultrasonography performed between 16 and 24 weeks' gestation. Twenty-two cases (9.7%) had previous sibs, parents, or cousins affected. The most common final diagnosis was osteogenesis imperfecta. In 15 cases (7…

With a background of experimental studies on macroporous biphasic calcium phosphate (MBCP) in canine spine fusion, MBCP was investigated in 11- to 18-year-old patients with scoliosis treated by spinal fusion. Twelve cases were chosen for whom enough bone graft was difficult to obtain (severe neurologic scoliosis and osteogenesis imperfecta). MBCP blocks were used in combination with a specific strong fixation (Cotrel-Dubousset instrumentation). Clinical and roentgenogram assessments were performed up to 24 months. In two cases, biopsies were obtained. Histologic, ultrastructural, and microanal…

Between 1970 and 1981, 12% of patients undergoing mitral valve surgery were found to have chordal rupture. Spontaneous or primary rupture accounted for 74.6% of patients (primary group); in the remainder (secondary group) chordal rupture complicated chronic rheumatic valvular disease (8.9%), bacterial endocarditis on both normal (8.5%) and rheumatic valves (4.7%), ischaemic heart disease (2.3%), acute rheumatic fever (0.5%), and osteogenesis imperfecta (0.5%). Isolated posterior rupture was seen most frequently (54%), with anterior rupture in 36% and rupture of both mitral cusps in 10% of pati…

This paper provides an overview of one aspect of the differentiation of cartilage and bone, namely, the degree of control provided by the extracellular matrix and microenvironment. A brief review of the diagnostic features of cartilage and bone is followed by a discussion of stem cells, emphasizing how to identify them using cytochemical, ultrastructural or experimental procedures. The role of extracellular matrices in the initiation of differentiation is discussed with reference to the initiation of chondrogenesis in the vertebral skeleton of the embryonic chick and of osteogenesis in the man…

BACKGROUND: Opioid abuse is among the most ubiquitous issues world-wide, and when it happens in mothers, it puts them at risk of diseases that can be transferred to the next generation. Previous studies have indicated that morphine addiction during pregnancy could inhibit development in rat embryos and infants. The present study focused on the effects of maternal consumption of morphine on rat skeletal system development and also investigate the molecular pathway of chondrogenesis and osteogenesis of infants from control and addicted rat groups.
METHODS: Thirty-two female rats were randomly a…
