Abortion in adolescence.

Sexual attitudes and behavior of adolescent females have been the topic of much interest over the past decade. Feelings about contraception, conception, and abortion have been described in relation to the adolescents' beliefs about the possibility of becoming pregnant, who will or will not "protect" them, and the influence of significant others on their decision making. This study explores differences in 35 women who had abortions during their teenage years with 36 women whose abortions occurred after the age of twenty. A demographic questionnaire, the Millon Clinical Multiaxial Inventory, and

Long-term effects of doping with anabolic steroids during adolescence on physical and mental health.

BACKGROUND: Systematic doping programs like in the GDR were applied in adolescent competitive athletes to induce supramaximal athletic performance. The substances had adverse somatic and psychological effects. The psychological development of the young athletes was impaired and they suffered in adulthood from long-term effects and secondary diseases even years after the doping period. METHOD: The study compared three groups: competitive athletes with doping (I), competitive athletes without doping (II) and persons with no sports activities (III). Somatic and psychological diseases were analyz

Synergising latent profile analysis and network analysis to elucidate the relationships among emotion dysregulation, post-traumatic stress disorder, dissociation, psychosis, and affective symptoms in individuals with trauma and severe mental illness.

BACKGROUND: Trauma exposure and emotion dysregulation are transdiagnostic risk factors for post-traumatic stress disorder (PTSD) and severe mental disorders (e.g., depression, and schizophrenia spectrum disorders). The present study elucidated the latent profiles and interrelationships between emotion dysregulation and psychopathology (PTSD, dissociation, psychosis, and affective symptoms) in a trauma-exposed severe mental illness population. METHODS: We recruited a large sample of trauma-exposed outpatients with mood disorders and schizophrenia spectrum disorders in Singapore (N = 461; M age

Diagnostic and Treatment Concordance Among Primary Care Doctors Delivering Mental Health Care: Results from an Effectiveness-implementation Hybrid Cluster Randomized Controlled Trial of an "Add-on Online Mental Health Training".

BACKGROUND: Training primary care doctors (PCDs) to manage common psychiatric conditions is seen as a key strategy to reduce the treatment gap, but its effects on their diagnostic and treatment skills remain largely unstudied. METHODS: A hybrid cluster randomized controlled trial compared two PCD training methods: training as usual (TAU; two days of in-person lectures, control group [CG]) versus TAU plus online mental health training (OMHT; 18 hours of interactive modules, study group [SG]). Primary outcomes (diagnostic, treatment, and combined concordance) between PCDs and psychiatrists were

Clinical, genetic, and sociodemographic predictors of symptom severity after internet-delivered cognitive behavioural therapy for depression and anxiety.

BACKGROUND: Internet-delivered cognitive behavioural therapy (ICBT) is an effective and accessible treatment for mild to moderate depression and anxiety disorders. However, up to 50% of patients do not achieve sufficient symptom relief. Identifying patient characteristics predictive of higher post-treatment symptom severity is crucial for devising personalized interventions to avoid treatment failures and reduce healthcare costs. METHODS: Using the Swedish multimodal database MULTI-PSYCH, we evaluated novel and established predictors associated with treatment outcome and assessed the added be

Clinical impact of obsessive-compulsive disorder comorbidity in bipolar disorder: a systematic review and meta-analysis.

BACKGROUND: Bipolar disorder (BD) is commonly comorbid with other psychiatric conditions, such as obsessive-compulsive disorder (OCD). Despite increasing interest in this comorbidity, quantitative data on its clinical characteristics remain limited. This systematic review and meta-analysis aimed to evaluate the clinical impact of OCD comorbidity in BD by comparing individuals with BD and OCD (BD-OCD) to those with BD without OCD. METHODS: We systematically searched the PubMed/MEDLINE, Scopus, PsycINFO, and Web of Science databases up to April 15, 2024. Meta-analyses were conducted to compare

Demographic and clinical associations of worst event trauma among a nationally representative sample of individuals with PTSD.

Posttraumatic stress disorder (PTSD) is a highly heterogenous disorders with approximately 636,120 different symptom combinations. The goal of the present study was to examine the association of worst-event trauma with demographic characteristics, psychiatric comorbidities, and PTSD symptoms among a large nationally representative sample of US adults with lifetime PTSD (N = 2339). The study used data from the National Epidemiologic Survey on Alcohol and Related Conditions-III to examine correlates of worst-event trauma using survey-weighted logistic and linear regression analyses. Childhood se

Efficacy of life goals collaborative care for bipolar disorder: A systematic review.

BACKGROUND: Bipolar disorder remains a disabling mental health condition despite the availability of effective treatments. Collaborative chronic care models (CCMs) represent an evidence-based way to structure care for conditions like bipolar disorder. Life Goals Collaborative Care (LGCC) was designed specifically for bipolar disorder, featuring psychoeducation alongside collaborative components (e.g. nurse care management or expert psychiatric consultation). Despite the use of Life Goals across health systems, a systematic review summarizing its effectiveness has never been conducted. METHODS

Comparative risk of suicide by specific substance use disorders: A national cohort study.

Substance use disorders (SUDs) are important risk factors for suicide, yet little is known about how suicide risks vary by specific SUDs. We examined these risks for the first time in a large general population to facilitate comparisons across SUDs. A national cohort study was conducted of all 6,947,191 adults in Sweden. SUDs (opioid, sedative/hypnotic, hallucinogen, cannabis, amphetamine, cocaine, and alcohol use disorders) were identified using inpatient, outpatient, and crime data, and suicide deaths using nationwide death data with follow-up during 2003-2016. Cox regression was used to com

The Swiss Primary Hypersomnolence and Narcolepsy Cohort study (SPHYNCS): Study protocol for a prospective, multicentre cohort observational study.

Narcolepsy type 1 (NT1) is a disorder with well-established markers and a suspected autoimmune aetiology. Conversely, the narcoleptic borderland (NBL) disorders, including narcolepsy type 2, idiopathic hypersomnia, insufficient sleep syndrome and hypersomnia associated with a psychiatric disorder, lack well-defined markers and remain controversial in terms of aetiology, diagnosis and management. The Swiss Primary Hypersomnolence and Narcolepsy Cohort Study (SPHYNCS) is a comprehensive multicentre cohort study, which will investigate the clinical picture, pathophysiology and long-term course of

Targeting separate specific learning parameters underlying cognitive behavioral therapy can improve perceptual judgments of anger.

BACKGROUND AND OBJECTIVES: Anxiety disorders are characterized by biased perceptual judgment. An experimental model using simple verbal instruction to target specific decision parameters that influence perceptual judgment was developed to test if it could influence anger perception, and to examine differences between individuals with social anxiety disorder (SAD) relative to generalized anxiety disorder (GAD) or non-psychiatric controls. METHODS: Anger perception was decomposed into three decision parameters (perceptual similarity of angry vs. not-angry facial expressions, base rate of encoun

P2RX7: expression responds to sleep deprivation and associates with rapid cycling in bipolar disorder type 1.

CONTEXT: Rapid cycling is a severe form of bipolar disorder with an increased rate of episodes that is particularly treatment-responsive to chronotherapy and stable sleep-wake cycles. We hypothesized that the P2RX7 gene would be affected by sleep deprivation and be implicated in rapid cycling. OBJECTIVES: To assess whether P2RX7 expression is affected by total sleep deprivation and if variation in P2RX7 is associated with rapid cycling in bipolar patients. DESIGN: Gene expression analysis in peripheral blood mononuclear cells (PBMCs) from healthy volunteers and case-case and case-control SNP

An overview of the genetic susceptibility to alcoholism.

AIMS: Alcoholism is a multifactorial, genetically influenced disorder. It is a major health and social issue, a highly frequent disease and a cause of premature death. It is also the most expensive addictive disorder due to morbidity, mortality, societal and legal problems. Besides their involvement in alcohol-related fatalities, forensic scientists are also required to assess driving and working ability as well as permanent invalidity due to alcohol-related conditions. Greater knowledge of the genetic basis of alcoholism could improve prevention by identifying specific risk factors and mechan

No evidence for physical anhedonia as a candidate symptom or an endophenotype in bipolar affective disorder.

OBJECTIVES: Bipolar affective disorder (BPAD) is clinically and genetically heterogeneous and the affected phenotype is poorly defined, hampering studies of its genetic basis. Studies of specific, familial, clinical indicators of BPAD may be useful for identifying heritable forms. Homogeneous forms of the disease may be identified in patients (candidate symptom approach) and some vulnerability markers may be sought in unaffected relatives of patients (intermediate traits or endophenotypes). Physical anhedonia (PA) is considered a possible candidate symptom and endophenotype in schizophrenia, b

Association of Lithium Treatment With the Risk of Osteoporosis in Patients With Bipolar Disorder.

IMPORTANCE: Osteoporosis, a systemic skeletal disorder associated with substantial morbidity and mortality, may be particularly common among individuals with bipolar disorder. Lithium, a first-line mood-stabilizing treatment for bipolar disorder, may have bone-protecting properties. OBJECTIVE: To evaluate if treatment with lithium is associated with a decrease in risk of osteoporosis among patients with bipolar disorder. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study included 22 912 adults from the Danish Psychiatric Central Research Register who received an initial diagn

The availability of dialectical behavior therapy in partial hospitalization and residential services for borderline personality disorder: An exploratory longitudinal study of the National Mental Health Services Survey from 2014 to 2021.

OBJECTIVE: Treatment guidelines on borderline personality disorder (BPD) recommend day-hospital or residential treatments for patients with BPD who cannot tolerate outpatient treatment (American Psychiatric Association, 2010; National Health & Medical Research Council, 2013). However, the current literature suggests that evidence-based treatment for BPD may be difficult to access (Lohman et al., 2017). The present study aims to characterize the accessibility of dialectical behavioral therapy (DBT) in day-treatment and residential programs in the United States in the last 7 years and examines w

Identifying Women with Post-Delivery Posttraumatic Stress Disorder using Natural Language Processing of Personal Childbirth Narratives.

BACKGROUND: Maternal mental disorders are considered a leading complication of childbirth and a common contributor to maternal death. In addition to undermining maternal welfare, untreated postpartum psychopathology can result in child emotional and physical neglect, and associated significant pediatric health costs. Some women may experience a traumatic childbirth and develop posttraumatic stress disorder (PTSD) symptoms following delivery (CB-PTSD). Although women are routinely screened for postpartum depression in the U.S., there is no recommended protocol to inform the identification of wo

Criminal multilation of the human body in Sweden--a thirty-year medico-legal and forensic psychiatric study.

During the 30-year period 1961-1990, a total of 22 deaths with criminal multilation/dismemberment of the human body were registered in Sweden. The multilations occurred in time clusters, mostly during the summer and winter periods, and increased during the three decades, with incidence rates of 0.05, 0.1, and 0.125 per million inhabitants and year, respectively. Multilation was noted 6.6 times more often in large urban areas than in the rest of Sweden. Defensive mutilation, in order to get rid of the corpse or make its identity more difficult, was noted in ten instances, aggressive mutilation

Prediction of risk of prolonged post-concussion symptoms: Derivation and validation of the TRICORDRR (Toronto Rehabilitation Institute Concussion Outcome Determination and Rehab Recommendations) score.

BACKGROUND: Approximately 10% to 20% of people with concussion experience prolonged post-concussion symptoms (PPCS). There is limited information identifying risk factors for PPCS in adult populations. This study aimed to derive a risk score for PPCS by determining which demographic factors, premorbid health conditions, and healthcare utilization patterns are associated with need for prolonged concussion care among a large cohort of adults with concussion. METHODS AND FINDINGS: Data from a cohort study (Ontario Concussion Cohort study, 2008 to 2016; n = 1,330,336) including all adults with a
