Are changes in cardiovascular disease risk factors in midlife women due to chronological aging or to the menopausal transition?

OBJECTIVES: This prospective study examined whether changes in traditional and novel coronary heart disease (CHD) risk factors are greater within a year of the final menstrual period (FMP), relative to changes that occur before or after that interval, in a multiethnic cohort. BACKGROUND: Understanding the influence of menopause on CHD risk remains elusive and has been evaluated primarily in Caucasian samples. METHODS: SWAN (Study of Women's Health Across the Nation) is a prospective study of the menopausal transition in 3,302 minority (African American, Hispanic, Japanese, or Chinese) and Ca

Comparative effects of a contraceptive vaginal ring delivering a nonandrogenic progestin and continuous ethinyl estradiol and a combined oral contraceptive containing levonorgestrel on hemostasis variables.

OBJECTIVE: This study aimed to compare the effects on hemostasis variables of a contraceptive vaginal ring with those of an oral contraceptive. STUDY DESIGN: Twenty-three and 22 healthy premenopausal women were randomized to the contraceptive vaginal ring (150 microg Nestorone and 15 microg ethinyl estradiol) or Stediril 30 during 3 cycles. Analysis of covariance was performed with baseline values as covariate. RESULTS: The contraceptive vaginal ring changed most hemostasis variables similarly but raised (95% confidence intervals of percent treatment differences) Factor VIIt (28% to 49%), ex

PAI-1 gene 4G/5G genotype: A risk factor for thrombosis in vessels of internal organs.

Although the common 4G/5G polymorphism in the promoter of the PAI-1 gene was suggested to be a risk factor for some of the thrombotic disorders, its significance in the development of thrombosis is still controversial. This study presents the data on a total of 357 patients with different types of thrombosis and 281 unrelated healthy controls. It was found that the 4G/4G genotype is associated with a higher risk of thrombosis (OR, 1.7; 95% CI, 1.1-2.5). Patients were divided into five distinct groups according to the site of thrombosis. Both 4G/4G and 4G/5G genotypes were associated with a hig

How long is too long? Association of time delay to successful reperfusion and ventricular function outcome in acute myocardial infarction: the case for thrombolytic therapy before planned angioplasty for acute myocardial infarction.

OBJECTIVES: The purpose of this study was to quantify the effect of time delays to reperfusion on ventricular function after myocardial infarction. This allows one to identify a group of patients in whom a strategy using antecedent pharmacologic reperfusion therapy before planned direct angioplasty may offer significant benefit. BACKGROUND: Direct angioplasty for myocardial infarction is associated with a high rate of successful reperfusion compared with pharmacologic reperfusion. However, there is an inherent time delay to treatment with angioplasty compared with pharmacologic therapy. There

Endovascular treatment of dural sinus thrombosis with rheolytic thrombectomy and intra-arterial thrombolysis.

BACKGROUND AND PURPOSE: Cerebral venous thrombosis is a rare entity that can be difficult to manage. Intrasinus thrombolysis is an increasingly applied intervention, but this modality carries an increased risk of hemorrhage. We describe for the first time an option with a potentially lower incidence of intracranial bleeding, the combination of the AngioJet rheolytic thrombectomy catheter with intra-arterial thrombolysis, in 2 patients with extensive dural sinus thromboses, preexisting intracranial hemorrhage, and severe progressive neurological deficits despite heparin therapy. METHODS: Four

Mechanism by which quinapril improves vascular function in coronary artery disease.

Angiotensin-converting enzyme (ACE) inhibition has been shown to improve endothelium-dependent vasodilator responsiveness, but the contribution and mechanism of enhanced nitric oxide (NO) bioactivity to this effect in patients with coronary artery disease are unknown. We investigated the effect of ACE inhibition on brachial artery dilator responsiveness to increased shear stress after forearm ischemia by ultrasonography as a bioassay for endothelial NO available to vascular smooth muscle in 9 men with coronary artery disease. Serum nitrogen oxides were measured after 3 days of nitrate-restrict

Improved cerebral perfusion after stenting of a petrous carotid stenosis: technical case report.

OBJECTIVE AND IMPORTANCE: Atherosclerotic occlusive disease of the intracranial vasculature is associated with increased risk of systemic vascular occlusive disease and stroke. Therapeutic options have included anticoagulation therapy, antiplatelet therapy, or, in a limited number of patients, extracranial-intracranial vascular bypass procedures. We report a patient who had improved cerebral perfusion with silent watershed zone infarctions after endovascular stenting of a severe petrous segment carotid stenosis. CLINICAL PRESENTATION: A 73-year-old man with severe coronary artery disease and

A word of caution: reopening chronic coronary occlusions.

Reopening of chronically occluded coronary arteries by angioplasty, initially considered a low risk procedure, may carry similar risks at PTCA of coronary stenoses. We report 3 cases with ventricular fibrillation or sudden death 2-4 days after reopening of 1- to 5-month-old coronary occlusions.

Early Transcriptional Changes in Neutrophil-Mediated Processes Following Recanalization After Ischemic Stroke.

BACKGROUND: Ischemic stroke is a leading cause of death and long-term disability worldwide. Recanalization therapies, including thrombolysis and mechanical thrombectomy, restore blood flow, yet many patients experience poor outcomes, a phenomenon known as futile recanalization. Given the short therapeutic window for ischemic stroke, identifying early biomarkers to guide targeted interventions and improve outcomes is critical. METHODS: Using a murine middle cerebral occlusion model that mimics a large vessel occlusion with recanalization, a comprehensive microarray analysis from blood samples

Biomarkers of endothelial activation and inflammation in dogs with organ dysfunction secondary to sepsis.

INTRODUCTION: Alteration in endothelial function during sepsis is thought to play a key role in the progression of organ failure. We herein compared plasma concentrations of endothelial activation biomarkers vascular endothelial growth factor (VEGF), hyaluronan (HA), plasminogen activator inhibitor-1 (PAI-1) and von Willebrand factor (vWF), as well as inflammatory mediator concentrations (IL-6, IL-8, IL-10, C-reactive protein and monocyte chemoattractant protein-1) in dogs with sepsis to healthy dogs. METHODS: This study was a multicenter observational clinical trial conducted at two universi

Safety profile of the intravitreal streptokinase-plasmin complex as an adjunct to vitrectomy in the rabbit.

BACKGROUND: The generation of an atraumatic posterior vitreous detachment (PVD), a common goal in vitreoretinal surgery, is a challenge, particularly in children and young trauma patients. Plasmin has been proposed as an adjunct to vitrectomy to enzymatically generate a PVD. Low doses of streptokinase-activated plasmin were tested in human pilot studies. This dose-escalation study assesses the safety range of intravitreal human streptokinase-plasmin in rabbits. METHODS: Plasminogen was isolated from human plasma by affinity chromatography, followed by activation with streptokinase (1:1), to g

Monoclonal antibody [7E3 F(ab')2] prevents arterial but not venous rethrombosis.

The role of the platelet glycoprotein IIb/IIIa (GPIIb/IIIa) receptor and the efficacy of GPIIb/IIIa receptor antagonists on reocclusion after arterial versus venous thrombolysis is unknown. We used a canine model of simultaneous carotid artery and jugular vein thrombosis to evaluate the effect of the murine monoclonal antibody 7E3 [7E3 F(ab')2] on intravascular thrombotic reocclusion after thrombolysis in both vessels. The left carotid artery and right jugular vein were instrumented with flow probes, a critical stenosis, and an electrode through which an anodal current was applied to induce fo

Study on the capture and thrombolytic efficacy of magnetic particle clusters under a deeply focused rotating magnetic field.

Magnetic particle-mediated targeted delivery of tissue plasminogen activator (tPA), assisted by a rotating magnetic field (RMF), represents a highly promising approach for the treatment of venous thrombosis. However, existing magnetic strategies face bottlenecks in deep tissues and complex hemodynamics due to poor focusing and weak flow resistance. Consequently, flow washout induces particle escape, while magnetic attenuation severely limits the clusters ' operating range and fluid perturbation. Achieving deep focusing and flow-resistant retention of magnetic particle clusters (MPCs) under dee

Differential Effects of Direct Plasmin Inhibition and Tranexamic Acid on Atherosclerosis Progression and Vascular Remodeling in Apoe/Ldlr Double-Knockout Mice.

AIM: The fibrinolytic system may protect against or promote atherosclerosis; however, the effects of distinct antifibrinolytic interventions on plaque development and extracellular matrix (ECM) remodeling remain unclear. METHODS: We compared YO-2, a direct plasmin inhibitor, and tranexamic acid (TXA), a lysine analog that interferes with plasminogen/plasmin binding to fibrin, in Apoe/Ldlr double-knockout mice fed a Western diet. The atherosclerotic burden, plaque structure, vascular marker expression, leukocyte-endothelial interactions, and nitric oxide (NO)-related indices were assessed. RE

Association of Glucose Response Shape in OGTT With Markers of Pancreatic Health and Inflammation Across Diverse Ages.

CONTEXT: The shape of glucose response in the oral glucose tolerance test (OGTT) is a biomarker for pancreatic β-cell health. A biphasic response is deemed metabolically healthier compared to monophasic, while persistent increase is a harbinger of diabetes. OBJECTIVE: To assess the relationship of inflammatory markers with the shape of glucose OGTT response in a wide age range and populations. METHODS: Reanalysis of data obtained in cross-sectional prospective clinical studies: individuals of self-reported Black ancestry (adolescents, n = 276, age 16.3 ± 1.7 years, 47.5% female; young adults

A murine stable thrombosis model enabling therapeutic evaluation of antithrombotic drugs.

Over recent decades, numerous experimental models have been developed to investigate arterial thrombosis and evaluate the efficacy of antithrombotic therapies. A major limitation of these models lies in the biphasic dynamics of a thrombus, which prevents assessment of its destabilization/disaggregation/thrombolysis after pharmacological treatment. In this study, we developed a stable thrombosis model based on the external application of 2 pieces of 7.5% FeCl3-saturated Whatman papers to the carotid artery. Histological analysis and transmission electron microscopy revealed no exposure of the s

Hypoxia tumor-associated macrophages facilitate hepatocellular carcinoma metastasis via uPA-uPAR pathway.

BACKGROUND: Hepatocellular carcinoma (HCC) arises within a hypoxic and immunosuppressive tumor microenvironment (TME), where tumor-associated macrophages (TAMs) constitute a major immune population. The impact of hypoxia on TAM functional heterogeneity and their contribution to HCC growth and metastasis remain incompletely understood. METHODS: We integrated multiple single-cell RNA sequencing datasets and developed a machine learning framework to map cellular hypoxia at single-cell resolution. Hypoxic TAMs (H-TAM) were characterized using transcriptomic and functional assays, including pseudo
