Remaining problems in the staging and treatment of childhood lymphoblastic leukemia.

During the past decade, advances in the treatment of childhood acute lymphoblastic leukemia (ALL) have continued. Progress is largely due to improved disease-free survival of poor-prognosis subgroups, improved sanctuary therapy, shortening of therapy duration, and salvage of relapsed patients by use of better chemotherapy regimens and bone marrow transplantation. Nonetheless, more children continue to die of ALL than of any other childhood cancer. This article reviews the progress and indicates remaining problems in the staging and treatment of ALL. Remaining problems include a lack of standar

Diagnostic Journey and Health Care Burden of Patients With Creutzfeldt-Jakob Disease in the United States: A Real-World Evidence Study.

BACKGROUND AND OBJECTIVES: Evidence on the diagnostic journey and health care burden of patients with Creutzfeldt-Jakob disease (CJD) in the United States is limited. A real-world evidence study using a US health care claims database was undertaken to address this gap. METHODS: A retrospective observational cohort study was conducted using data from the Merative MarketScan Research Databases (01/2012-12/2020). Study population comprised adults aged 18 years or older with evidence of CJD (initial diagnosis = index date), no evidence of selected neurologic conditions after the last CJD diagnosi

A phase 2 trial of chemotherapy plus pembrolizumab in patients with advanced non-small cell lung cancer previously treated with a PD-1 or PD-L1 inhibitor: Big Ten Cancer Research Consortium BTCRC-LUN15-029.

BACKGROUND: Immunotherapy using a checkpoint inhibitor (CPI) alone or in combination with chemotherapy is the standard of care for treatment-naive patients with advanced non-small cell lung cancer (NSCLC) without driver mutations for which targeted therapies have been approved. It is unknown whether continuing CPI treatment beyond disease progression results in improved outcomes. METHODS: Patients who experienced progressive disease (PD) after a clinical benefit from chemotherapy plus a CPI were enrolled. Patients received pembrolizumab (200 mg every 3 weeks) plus next-line chemotherapy. The

Acceptability of Telehealth as the Default Modality for Multiple Sclerosis Care in Switzerland: Cross-Sectional Study.

BACKGROUND: Telehealth can improve access to care for people living with multiple sclerosis (MS), but information on its acceptance is limited in Switzerland. OBJECTIVE: This study aimed to determine the proportion of people living with MS willing to accept telehealth as a new default and the factors associated with their acceptance. METHODS: We conducted a cross-sectional analysis using survey data from the Swiss Multiple Sclerosis Registry. We defined "telehealth as a default" as a health care model where remote consultations (telephone and/or video calls) are the primary mode of interacti

Artificial intelligence and personalized diagnostics in periodontology: A narrative review.

Periodontal diseases pose a significant global health burden, requiring early detection and personalized treatment approaches. Traditional diagnostic approaches in periodontology often rely on a "one size fits all" approach, which may overlook the unique variations in disease progression and response to treatment among individuals. This narrative review explores the role of artificial intelligence (AI) and personalized diagnostics in periodontology, emphasizing the potential for tailored diagnostic strategies to enhance precision medicine in periodontal care. The review begins by elucidating t

The human microbiota: novel targets for hospital-acquired infections and antibiotic resistance.

PURPOSE: Hospital-acquired infections are increasing in frequency due to multidrug resistant organisms (MDROs), and the spread of MDROs has eroded our ability to treat infections. Health care professionals cannot rely solely on traditional infection control measures and antimicrobial stewardship to prevent MDRO transmission. We review research on the microbiota as a target for infection control interventions. METHODS: We performed a literature review of key research findings related to the microbiota as a target for infection control interventions. These data are summarized and used to outlin

Treatment of tobacco dependence in mental health and addictive disorders.

People with mental health and addictive (MHA) disorders smoke at high rates and require tobacco treatment as a part of their comprehensive psychiatric care. Psychiatric care providers often do not address tobacco use among people with mental illness, possibly owing to the belief that their patients will not be able to quit successfully or that even short-term abstinence will adversely influence psychiatric status. Progress in the development of treatments has been slow in part because smokers with current MHA disorders have been excluded from most smoking cessation trials. There are several sm

Progression and regression of cervical lesions. Review of smears from women followed without initial biopsy or treatment.

Cervical smears were reviewed from patients in whom a cytological abnormality was followed, after an interval without interference, either by regression to ;negative' or else by progression to invasive carcinoma. Twenty-eight cases were from a previously analysed series with positive smears and an interval of at least two years before investigation, resulting from refusal or failure to trace. Slides were also reviewed from 25 cases in which ;positive' smears had regressed to negative without escaping from surveillance, and from 10 patients subsequently developing invasive carcinoma whose previ

Hospital as a site of death of cancer patients in Italy: A registry-based analysis and time trends.

BACKGROUND: Place of death is relevant both for individuals and for the society. Home is universally considered the optimal place of death while dying in hospital may be a signal of inappropriate end-of-life care. We aimed at studying the place of death of patients with cancer in Italy in a five-year period. PATIENTS AND METHODS: The death-certificate database published by the Italian National Institute of Statistics (ISTAT) was analysed, searching the place of death and the main disease related to death. Data from 2015 to 2019 were used, to exclude the influence of the COVID pandemic. Cancer

Development of a Lung Rescue Team to Improve Care of Subjects With Refractory Acute Respiratory Failure.

BACKGROUND: A lung-protective mechanical ventilation strategy has become the hallmark of ventilation management for patients with acute respiratory failure. However, some patients progress to more severe forms of acute respiratory failure with refractory hypoxemia. In such circumstances, individualized titration of mechanical ventilation according to the patient's specific respiratory and cardiovascular pathophysiology is desirable. A lung rescue team (LRT) was recently established at our institution to improve the medical care of patients with acute respiratory failure when conventional treat

Checkpoint inhibition in combination with an immunoboost of external beam radiotherapy in solid tumors (CHEERS): study protocol for a phase 2, open-label, randomized controlled trial.

BACKGROUND: While the introduction of checkpoint inhibitors (CPIs) as standard of care treatment for various tumor types has led to considerable improvements in clinical outcome, the majority of patients still fail to respond. Preclinical data suggest that stereotactic body radiotherapy (SBRT) could work synergistically with CPIs by acting as an in situ cancer vaccine, thus potentially increasing response rates and prolonging disease control. Though SBRT administered concurrently with CPIs has been shown to be safe, evidence of its efficacy from large randomized trials is still lacking. The ai

Neutrophil-To-Lymphocyte Ratio and Survival in Pembrolizumab-Treated Oropharyngeal Cancer.

BACKGROUND: A minority of patients receiving immunotherapy for oropharyngeal head and neck squamous cell carcinoma benefit. Finding biomarkers that predict response to ICI is important for improving outcomes in these patients. In this study, we determine the relationship between pretreatment neutrophil-to-lymphocyte ratio (NLR) and 6-month progression-free survival (PFS)/2-year overall survival (OS) among patients with recurrent or metastatic (R/M) oropharyngeal cancer on pembrolizumab. METHODS: This study was a retrospective cohort study of patients with recurrent/metastatic OPSCC who were t

Recurrent Pyoderma Gangrenosum with Initial Onset in Early Pregnancy: A PARACELSUS-Supported Clinical Diagnosis in a Resource-Limited Setting in South-Western Uganda.

BACKGROUND: Pyoderma gangrenosum (PG) is a rare neutrophilic dermatosis characterized by rapidly progressive, painful cutaneous ulceration and remains a diagnosis of exclusion because no single laboratory or histopathological test is diagnostic. Establishing a clinically well-supported diagnosis is particularly challenging in resource-limited settings where histopathology and other advanced investigations are often unavailable, necessitating reliance on structured clinical assessment and validated diagnostic frameworks. [Clinical research within the German competence net "acute and chronic leukemias"].

BACKGROUND: Leukemias are a challenge and a cost factor to society because of their frequency in all age groups. They also serve as a model for a variety of diseases and possess exemplary relevance for basic research and patient care. Leukemia research and therapy have achieved high standards and even a leading position in Germany with regard to clinical trials, standardization of diagnostics and molecular studies of prognostic factors, signal transduction and gene expression. Progress is hampered, however, by fragmentation of leukemia trial groups, diagnostic approaches and treatment research
