Frequency and patterns of CMF emergency cases during and after COVID-19.

BACKGROUND: Craniomaxillofacial (CMF) trauma constitutes a significant proportion of hospital presentations, often resulting from high-energ … erved for mandibular and midfacial fractures. In contrast, general trauma indicators such as distal radius fractures remained relatively stable, suggesting a trauma-mechanism-specific effect. CONCLUSION: The COVID-19 pandemic significantly reduced the number of CMF emergency cases in Germany, with partial recovery observed in subsequent years. These findings refl …

Volar locking plate versus external fixation for the treatment of unstable distal radial fractures: a meta-analysis of randomized controlled trials.

BACKGROUND: The choice between volar locking plates (VLP) and external fixation (EF) for unstable distal radius fractures have not reached a consensus. The meta-analysis of randomized controlled trials was performed to compare VLP with EF to determine the dominant strategy. MATERIALS AND METHODS: Meta-analysis was performed with a systematic search of studies conducted by using the PubMed, Embase, and Cochrane Central Register of Controlled Trials databases. The randomized controlled trials that compared VLP with EF was identified. Characteristics, functional outcomes, radiological results, a…

In-line skating injuries.

OBJECTIVE: To assess the type and site of injuries associated with in-line skating in patients encountered during an 18-month period. DESIGN: We retrospectively reviewed the medical records of patients in our emergency department and Sports Medicine Center with injuries that had resulted from in-line skating between July 1992 and December 1993. MATERIAL AND METHODS: A computer search was conducted to identify medical records on which in-line skating was denoted as the mechanism of injury. The records were then manually reviewed to confirm that the injury had occurred during in-line skating.

External fixators for wrist fractures: a biomechanical and clinical study.

The rigidity of four external fixators for the wrist was determined by using the Instron universal testing instrument. Using the equivalent stiffness index, the small A.O. was 7.6, the mini Hoffman was 3.7, Roger Anderson was 3.5, and Ace Colles' was 4.3. Thus, the small A.O. was about twice as rigid overall as the other three external fixators. Twenty-two patients with unstable distal radius fractures were treated with the mini Hoffman external fixator over a 3-year period; and follow-up data were obtained. The average final angle of the distal radial articular surface on the anteroposterior …

Outcome following excision of the radial head in children with open physes for impaired elbow motion.

BACKGROUND: Only few reports have described the outcome of pediatric patients following radial head resection. Therefore, the aim of the present study was to assess clinical and radiologic outcome of patients with open physes following resection of the radial head. MATERIALS AND METHODS: Skeletally immature patients treated with resection of the radial head were included. Range of motion (ROM) of the elbow joint was compared with preoperative values. Grip strength, pronation and supination strength, and carrier angle were compared with the unaffected side. Radiographs were assessed for signs

Osseous destruction by neurofibroma diagnosed in infancy as "desmoplastic fibroma".

A 2-month-old male infant presented with a lytic lesion of the distal radius which was diagnosed as desmoplastic fibroma. Tumor recurred following en bloc resection of the distal radius and forearm, necessitating an elbow disarticulation. Subsequently, the child developed multiple café-au-lait spots, and histologic sections have been reinterpreted as neurofibromatosis. This case illustrates the potential difficulty in diagnosing neurofibromatosis when few or no classic stigmata of the disease are present.

Plate Osteosynthesis for Segmental Forearm Fractures: A Comparison of Outcomes in Floating Elbow and Isolated Forearm Fractures.

BACKGROUND: Segmental forearm fractures are challenging to treat due to their instability. A floating elbow, characterized by ipsilateral fractures of both the humerus and forearm, is a rare but severe injury often caused by high-energy trauma and associated with significant soft tissue damage. The objective of this study is to compare the surgical outcomes of plate osteosynthesis in segmental forearm fractures, with and without the concomitant presence of a floating elbow. METHODS: A total of 35 adult patients who underwent open reduction and internal fixation for segmental forearm fractures

Re-displacement of stable distal both-bone forearm fractures in children: a randomised controlled multicentre trial.

INTRODUCTION: Displaced metaphyseal both-bone fractures of the distal forearm are generally reduced and stabilised by an above-elbow cast (AEC) with or without additional pinning. The purpose of this study was to find out if re-displacement of a reduced stable metaphyseal both-bone fracture of the distal forearm in a child could be prevented by stabilisation with Kirschner wires. METHODS: Consecutive children aged <16 years with a displaced metaphyseal both-bone fracture of the distal forearm (n = 128) that was stable after reduction were randomised to AEC with or without percutaneous fixatio

Multiple fractures in the elderly.

We present the prevalence of multiple fractures in the elderly in a single catchment population of 780,000 treated over a 12-month period and describe the mechanisms of injury, common patterns of occurrence, management, and the associated mortality rate. A total of 2335 patients, aged ≥ 65 years of age, were prospectively assessed and of these 119 patients (5.1%) presented with multiple fractures. Distal radial (odds ratio (OR) 5.1, p < 0.0001), proximal humeral (OR 2.2, p < 0.0001) and pelvic (OR 4.9, p < 0.0001) fractures were associated with an increased risk of sustaining associated fractu

Nonunion of distal radial fractures associated with distal ulnar shaft fractures: a report of four cases.

Although complications following distal radial fractures are common, nonunion is rare. We report four cases of radial nonunion following distal radial fractures that occurred in elderly women after low-energy falls. The distinctive feature of these injuries was the presence of an associated distal ulnar shaft fracture. All patients had initial closed reduction and cast immobilization, which was unsuccessful. Reconstruction was complicated by deformity, disuse osteoporosis, radiocarpal stiffness, and distal radioulnar joint disruption. Wrist arthrodesis improved function and decreased pain and

Outcomes after arthroscopic treatment for scaphoid nonunion using ipsilateral radius bone graft and a headless compression screw: a comparison between the patients with and without avascular necrosis.

PURPOSE: To determine whether avascular necrosis can affect clinical outcomes or the union incidence after arthroscopic bone grafting for the treatment of scaphoid nonunion. METHODS: This retrospective comparative study included thirty-four patients with scaphoid nonunion that underwent arthroscopic bone graft from the ipsilateral radius and internal fixation. The patients were divided into two cohorts (group A, with avascular necrosis, n = 15; group B, without avascular necrosis, n = 19) based on pre-operative magnetic resonance imaging findings. Additionally, the patients were grouped in ac

Epidemiology, Treatment, and Treatment Quality of Overriding Distal Metaphyseal Radial Fractures in Children and Adolescents.

BACKGROUND: The incidence of completely displaced overriding distal metaphyseal radial fractures in children is unknown, and the optimal treatment is controversial. METHODS: All completely displaced distal metaphyseal radial fractures in patients <16 years old treated in our tertiary-level institution from 2014 to 2019 were identified with use of the Kids' Fracture Tool Helsinki. Etiology, fracture characteristics, management, and quality of treatment were assessed. A telephone interview with the guardian of the patient was performed for 100 (89%) of 112 patients at 1.5 to 7.2 years (median,

A research protocol on leap motion tracking device: A novel intervention method in distal radial fracture rehabilitation.

INTRODUCTION: Physiotherapeutic rehabilitation are used to optimize functional recovery following a distal radial fracture (DRF). Being most common upper limb fracture in all age groups, the DRF peaks in young men and in post-menopausal women with incidence ratio of 1:4. Leap motion control based rehabilitation of patients with DRF is limited. This research aims to assess the efficacy of leap motion control based rehabilitation in patients with DRF. METHODS: In an randomized parallel group trial, subjects (n = 40) with DRF will be recruited. The participants will be enrolled into either exper

Intermediate Cuneiform Resurfacing Arthroplasty for Combined Defects of the Sigmoid Notch and the Lunate Fossa of the Distal Radius.

Posttraumatic comminution and bone loss at the sigmoid notch and the lunate fossa of the distal radius result in substantial impairment. Established salvage methods do not meet the physical demands of young patients. Replacement of the articular cartilage is challenging owing to the loss of 2 articular surfaces in different planes and the lack of a defined autogenous source to replace them simultaneously. The use of an intermediate cuneiform osteoarticular autograft to reconstruct the sigmoid notch and the lunate fossa is a new surgical technique that restores joint congruity and stability whi

Volar locking plates not touching the flexor pollicis longus tendon appear as prominences on radiographs: a cadaver study.

BACKGROUND: Plate protrusion is a risk factor for flexor pollicis longus (FPL) rupture following volar locking plate (VLP) surgery. However, plate prominence on follow-up radiographs is common. We hypothesised that a VLP that does not touch the FPL tendon can appear as a plate prominence projected over the volar ridge on lateral radiographs. MATERIALS AND METHODS: We studied six current designs of widely used plates in formalin-fixed cadavers. Each plate was placed in six cadavers. We analysed 36 different plate-cadaver combinations. The main aim of plate fixation was to position the plate in

Point-of-care ultrasound compared with conventional radiographic evaluation in children with suspected distal forearm fractures in the Netherlands: a diagnostic accuracy study.

BACKGROUND: Distal forearm fractures are common in children. The reference standard to diagnose these fractures is by conventional radiography, which exposes these patients to harmful radiation. Ultrasound (US) seems to be a good alternative. However, emergency physicians (EPs) in the Netherlands have limited experience in using US for diagnosing fractures in children. OBJECTIVE: The primary objective was to determine the accuracy of US, performed by a Dutch EP, compared with conventional radiography, in diagnosing distal forearm fractures in children. As a secondary objective, differences in
