Ruptured intracranial dermoid cyst as an incidental finding: Case report.

Intracranial dermoid cysts are rare, benign congenital lesions originating from ectodermal remnants. Though typically asymptomatic, their rupture, although infrequent, can lead to significant neurological complications, such as chemical meningitis, vasospasm, and hydrocephalus. Imaging with computed tomography (CT) and magnetic resonance imaging (MRI) plays a crucial role in diagnosis, providing characteristic features related to the high lipid content of these lesions. An 86-year-old female presented with a 3-day history of headache, nausea, vomiting, and facial cellulitis after an arthropod …

Ruptured Abdominal Aortic Aneurysm as the Initial Manifestation of Undiagnosed Pheochromocytoma.

INTRODUCTION: Pheochromocytoma is a rare catecholamine-secreting tumor that can cause severe cardiovascular complications. Its association with ruptured abdominal aortic aneurysm is extremely rare. CASE PRESENTATION: A 57-year-old man with no significant medical history presented with sudden abdominal pain and hemorrhagic shock. Computed tomography revealed a ruptured infrarenal abdominal aortic aneurysm, and emergency endovascular aneurysm repair was successfully performed. Preoperative imaging incidentally detected a hypervascular left adrenal mass. Although biochemical findings were border…

Ruptured Sinus of Valsalva Aneurysm: An Unexpected Cause of Respiratory Failure.

CASE SUMMARY: We address a case of a young patient with a history of asymptomatic ventricular septal defect, presenting with sudden-onset dyspnea requiring prolonged mechanical ventilation, who developed decompensated right heart failure. A sinus of Valsalva aneurysm rupture, leading to aortopulmonary fistula, was diagnosed. The patient underwent surgical repair with bovine patch and commissural annuloplasty and was discharged after an uneventful postoperative course. TAKE-HOME MESSAGE: This rare case of a ruptured sinus of Valsalva aneurysm in a patient with a known ventricular septal defect…

Spontaneous Closed Extensor Tendon Ruptures in the Nonrheumatoid Hand and Wrist.

Extensor tendon disruptions are frequently seen in emergency rooms and hand surgery offices. Traumatic open injuries are more familiar, but spontaneous closed extensor tendon ruptures represent an important clinical entity. Often occurring without any prodromal warning, they can surprise patients and their providers alike. Spontaneous extensor tendon ruptures often occur in the setting of systemic medical conditions or hand and wrist pathology, with the mechanisms that lead to rupture often dependent upon the underlying comorbidities. Remaining knowledge of the factors that place patients at r…

Annular rupture in TAVI: considerations on the importance of on-site cardiothoracic surgery.

BACKGROUND: Transcatheter aortic valve implantation (TAVI) has proven itself as an effective equivalent to surgical aortic valve replacement for patients with aortic stenosis. However, there is a recognised risk profile for TAVI. Complications such as aortic rupture may require bailout emergency cardiac surgery. Current guidelines dictate the presence of on-site cardiac surgery in centres providing TAVI. However, recent reports suggest that there may be a benefit from increasing access to TAVI to centres without cardiac surgery, the aim of which would be to reduce the morbidity and mortality o…

Spontaneous Rupture of the Urinary Bladder: A Case Report and Review of the Literature.

Rupture of the urinary bladder in the absence of trauma is an uncommon condition with a challenging diagnosis that carries a high mortality rate and usually demands immediate surgical intervention. It is commonly associated with underlying chronic bladder pathologies, such as obstructive uropathy, chronic inflammation, and radiation cystitis; in younger patients, it can occur in the setting of substance abuse. Due to its rarity and nonspecific clinical presentation, which often mimics other more common conditions such as bowel perforation, diagnosis is often delayed and made intraoperatively. …

[Tamponade due to a ruptured mature teratoma : A rare case report].

INTRODUCTION: Mature teratomas, a diverse group of germ cell tumors, rarely present with complications such as rupture. CASE PRESENTATION: We present a case of a 27-year-old male who was admitted to our hospital with acute dyspnea. A tamponade was diagnosed and the patient underwent a pericardial puncture. Further radiological investigations revealed a large mediastinal teratoma with evidence of rupture, leading to pericardial effusion. Surgical intervention was performed, which involved pericardial drainage and complete resection of the teratoma. Histopathological examination confirmed the d…

Boerhaave syndrome complicated by ruptured esophageal varices: a case report.

BACKGROUND: Boerhaave syndrome is a life-threatening condition caused by a spontaneous full-thickness rupture of the esophagus, typically following forceful vomiting. Esophageal variceal rupture is another potentially fatal condition commonly observed in patients with liver cirrhosis. These two conditions are usually reported independently, and their coexistence is extremely rare. Managing this combination is particularly challenging because of the need for both infection control and hemostasis in the presence of portal hypertension. CASE PRESENTATION: We report the case of a 51-year-old man …

Surgical treatment experiences in two cases of spontaneous esophageal ruptures.

Introduction and importance: Spontaneous esophageal ruptures (SREs), also known as Boerhaave syndrome, are rare but potentially fatal medical emergencies characterized by full-layer perforation of the esophagus. Early identification and surgical intervention are critical for improving patient outcomes and reducing mortality rates. Case presentation: This study presents two case reports of patients with SREs who underwent surgical treatment. Both patients experienced lower esophageal segment perforations and were treated with emergency surgery for primary esophageal repair. One patient develope…

Conservative Management of a Ruptured Simple Hepatic Cyst: A Case Report.

Simple hepatic cysts are common benign liver lesions that are typically asymptomatic and discovered incidentally. They are generally managed conservatively unless symptoms or complications develop. Spontaneous rupture of a hepatic cyst is exceptionally rare and often necessitates surgical or interventional management. We report the case of a 79-year-old woman with a large known hepatic cyst who presented with acute right upper quadrant pain, nausea, and vomiting. Imaging revealed rupture of the cyst with associated perihepatic free fluid, but no evidence of haemorrhage or infection. Given her …

Cephalic Pancreaticoduodenectomy With Right Nephrectomy for Traumatic Ruptured Wilms Tumor.

Wilms tumor is the most common pediatric extracranial solid tumor in Peru. Preoperative tumor rupture is an uncommon complication, and its management varies depending on the severity. If the bleeding is self-limited, does not affect hemodynamic stability, or is controlled with medical management, surgery may be delayed until after neoadjuvant chemotherapy. This report describes a three-year-old male patient with a right Wilms tumor extending into the ipsilateral renal vein, accompanied by bulky retroperitoneal lymph nodes and multiple bilateral pulmonary metastases. He presented with severe ab…
