The effect of single- or multiple-layered garments on interface pressure measured at the backpack-shoulder interface.

Experiments were aimed at determining whether interface pressure measurements are a true reflection of skin contact pressure when made over different layers of clothing. Interface pressures were recorded at the skin interface and above clothing layers, with 11 participants wearing 10 different clothing combinations. The clothing used was standard issue British military clothing, including body armour (without ballistic plate). Participants wore both single and multiple clothing layers whilst walking on a treadmill carrying a loaded backpack (23.5 kg, British military issue). Results showed no

Complex regional pain syndrome type I associated with amyotrophic lateral sclerosis.

BACKGROUND: (CRPS I [formerly called reflex sympathetic dystrophy]) is a syndrome with pain and signs of autonomic dysfunction after trauma or immobilization; the pathophysiologic mechanisms of CRPS I, however, remain unknown. DESIGN: The authors present a case of CRPS I associated with amyotrophic lateral sclerosis. Along with the motor paresis due to amyotrophic lateral sclerosis, pain, swelling, and signs of autonomic disturbance occurred. CONCLUSIONS: This case supports the hypothesis that immobilization is one of the major contributing factors for CRPS I.

Suprascapular nerve entrapment secondary to a lipoma.

Many causes of suprascapular nerve entrapment have been described including a small spinoglenoid notch, a tight ligament, boney spurs, and ganglion cysts. In the current patient, suprascapular nerve entrapment was caused by a lipoma in the suprascapular notch. The patient presented with painful shoulder motion that could have been attributed to rotator cuff and acromioclavicular joint disease. However, magnetic resonance imaging and electromyography were consistent with suprascapular nerve entrapment. Treatment of the rotator cuff disease and excision of the lipoma led to resolution of the pat

Reliability of position sense testing assessed with a fully automated system.

Position sense testing has increased as a tool for augmenting evaluation of joint injury. In the present study, we investigated the inter-day reliability for four different types of position sense tests using a fully automated system. The tests included (1) passive presentation/active replication, (2) passive presentation/passive replication, (3) semi-passive presentation/semi-passive replication (where semi-passive denotes passive movement during antagonist muscle contraction), and (4) active presentation/active replication. The absolute difference between presented target and replicated posi

Comparison of glenohumeral joint laxity in elite and recreational swimmers.

OBJECTIVE: To investigate whether glenohumeral joint (GHJ) hyperlaxity is related to swimming training volume, we evaluated elite and recreational swimmers for differences in GHJ laxity and general joint hypermobility (GJH). PARTICIPANTS: Thirty male and female elite swimmers, aged 15-25 years, were compared with 30 recreational swimmers on five clinical GHJ laxity tests. GJH was assessed based on evaluation of hypermobility in four other joints or joint combinations. OUTCOME MEASURES: Group differences in both GHJ laxity and GJH were analyzed using chi 2 analysis. RESULTS: Compared with th

Late effects of radiotherapy in the treatment of breast cancer.

Late effects after radiotherapy for breast cancer include radiation induced malignancy and changes in irradiated tissues leading to e.g. edema of the arm, decreased mobility of the shoulder joint, brachial plexus neuropathy, pulmonary fibrosis, telangiectasia or atrophic ulceration of the skin. While radiation-induced malignancy depends on the volume of tissue irradiated and the total dose, other late effects are also fractionation dependent. Several reports have shown increased rates of such late effects after changes of the fractionation schedule which should be isoeffective according to the

[Secondary impingement syndrome in athletes].

Dysfunction of the shoulder joint is based not only on anatomic conditions. The consideration of the special kinesiology of the shoulder helps to understand the shoulder pathology. This mainly applies to young "overhead athletes" like swimmers, handball-, basketball-, volleyball-, and racketplayers. These disciplines cause stress on the anterior joint structures (capsule, ligaments, labrum, subscapularis tendon) and lead to anterior instability. This includes anterior subluxation or even dislocation. Finally, an impingement syndrome with the typical symptoms can frequently result from these co

Staple capsulorrhaphy for recurrent posterior shoulder dislocation.

Recurrent posterior shoulder dislocation or subluxation is uncommon but occurs occasionally in athletes. Ten patients were treated with a posterior shoulder staple capsulorrhaphy. A posterior Bankhart-type-lesion was found in all cases. Eight of the 10 patients had pain relief. The range of motion was usually maintained postoperatively, but no patient returned to his former throwing status. Four patients also had anterior instability. Three patients (30%) had postoperative recurrence of their posterior instability. The two "ligamentous lax" conditions in the series both recurred. The procedure

Relations between upper limb soft tissue disorders and repetitive movements at work.

To make a preliminary assessment of whether upper limb soft tissue disorders might be associated with activities at work, we have conducted a case-control study of subjects attending orthopedic clinics in three cities. All subjects between the ages of 16 and 65 years, in whom defined soft tissue conditions of the upper limb were diagnosed by the participating orthopedic surgeons, were invited to take part. Controls were subjects attending the same clinics within the same age range whose clinical diagnosis did not include disease of the upper limb, cervical or thoracic spine. Information concer

Whiplash injury is more than neck pain: a population-based study of pain localization after traffic injury.

OBJECTIVES: To describe the distribution of bodily pain and identify common patterns of pain localization after traffic injury. METHODS: Cross-sectional analysis of a population-based cohort of 6481 Saskatchewan residents who were treated or filed an auto insurance claim within 30 days of traffic injury or both. The prevalence of pain in each of 13 body areas was calculated and compared with pain confined exclusively to each of these areas. Principal component analysis was used to identify the main patterns of pain localization after traffic injury. RESULTS: Irrespective of pain in other are

Symptoms of musculoskeletal disorders in stage rally drivers and co-drivers.

BACKGROUND: During stage rallying, musculoskeletal injuries may be provoked by the high magnitude of vibration and shock to which the driver and co-driver are exposed. Drivers and co-drivers experience similar exposure to whole body mechanical shocks and vibration but different exposure to hand/wrist stressors. OBJECTIVES: To investigate by a questionnaire study the prevalence of symptoms of musculoskeletal injuries after rallying in 13 professional and 105 amateur stage rally competitors. METHODS: The self administered questionnaire investigated whole body and hand/wrist symptoms of musculo

The relationship between the presence of scapula fracture and mortality and morbidity in cases with blunt thoracic trauma.

BACKGROUND: Scapula fractures (SFs) occur as a result of high-energy trauma and are significant in terms of life-threatening injuries. There are few studies showing the relationship between SFs and mortality and morbidity in patients with blunt thoracic trauma (BTT). Our study aims to investigate the relationship between SF and mortality and morbidity in BTT. METHODS: Adult patients admitted to the emergency department of Kahramanmaras Sutcu Imam University, School of Medicine with BTT between January 2019 and April 2021 were retrospectively scanned from hospital records. Patients' age, gende

The impact of faulty posture on rotator cuff tears with and without symptoms.

HYPOTHESIS: We hypothesized that the prevalence of rotator cuff tears would be higher among individuals with poor posture, regardless of the presence of symptoms. METHODS: The study initially comprised 525 residents of a mountain village who participated in an annual health check. Participants completed a background questionnaire, and physical examinations were performed to evaluate shoulder function. Ultrasonographic examinations were also performed to identify rotator cuff tears, and participants were grouped according to the presence or absence of tears. Posture was classified by 2 observe

[Prediction of shoulder dystocia--combining foetal weight estimation by ultrasound and maternal risk factors--a solution for the dilemma?].

BACKGROUND: Although foetal macrosomia is a known risk factor for shoulder dystocia, foetal weight estimation by ultrasound is imprecise. METHODS: We analysed in a retrospective, single-centre study whether the combination of foetal sonographic parameters and maternal parameters could improve the prediction of foetal macrosomia and whether on the basis of this findings the prediction of shoulder dystocia in foetal macrosomia could be enhanced. Likelihood ratios for sonographic parameters have been assessed and have been used to calculate post-test-probabilities for fetal macrosomia. All vagin

Correlation of ultrasonographic findings to Tossy's and Rockwood's classification of acromioclavicular joint injuries.

The aim of this study was to examine the value of ultrasonography to assess high-graded acromioclavicular (AC) joint injuries. We propose a new sonographic technique to evaluate the state of the soft tissues, specifically the deltoid and trapezoid muscles and their common fascia. Radiologic findings graded by Tossy's and Rockwood's classification were compared with sonographic findings for 92 patients with high-grade injuries of the AC joint. Of the 92 patients, 39 underwent operative treatment. A total of 36 patients were radiographically classified as type II and 56 with type III injuries, a

[Stress on the rotator cuff sutures in relation to joint position].

UNLABELLED: In a vitro study we evaluated the influence of surgical technique on the postoperative tension of the supraspinatus tendon in rotator cuff ruptures. In ten shoulder specimens Hall-Effect Strain Gauge (HEST) were implanted in the supraspinatus tendon close to the insertion at the humerus. We documented the tension in specimens with intact rotator cuff as well as after reconstruction of small and large tears in different joint positions. In the specimens with an intact rotator cuff there was almost no tension registered. Only in adduction with external rotation and flexion we found s
