Survivors of sudden death without structural heart disease and a normal coronary angiogram are often diagnosed with idiopathic ventricular fibrillation (VF). The criteria for idiopathic VF have been reviewed elsewhere. However, a careful analysis of the electrocardiogram (ECG) is essential in the differential diagnosis of unexplained sudden cardiac death. We report two patients with unexplained VF, intermittent right bundle-branch block (RBBB) and ST-segment elevation in the precordial leads V1-V3. This syndrome is named after Brugada who first described a clinical series. This paper stresses …

In 1994 Hans Asperger (1906-80), an Austrian physician, described a group of children with impaired social interaction and communication abilities. The name of this disorder today is Asperger's syndrome, and it is currently defined under the category of pervasive developmental disorder in DSM-IV and ICD-10. In this article the following aspects of Asperger's syndrome are focused on: personality, epidemiology, etiology, examination, differential diagnosis, management and prognosis. The article is based on a literature study. Asperger's syndrome seems to be considerably more common than "classic…

In 1937, Wiskott described three brothers with congenital thrombocytopenia, bloody diarrhea, eczema, and recurrent ear infections. Seventeen years later, Aldrich showed X-linked (a gene carried on the X chromosome) inheritance. Subsequently, the characteristic immune defects of Wiskott-Aldrich syndrome (WAS) were reported, including lymphopenia, lymphocyte depletion in the thymus, T-dependent pericortical areas of lymph nodes, defective delayed type hypersensitivity, and failure to produce antibodies to polysaccharides and to a variety of bacterial, protein, and viral antigens. The consistent …

Fragile X syndrome (FXS) is the most common cause known of inherited developmental disability. New and widely available techniques in DNA analysis allow both affected individuals and carriers to be tested from a simple blood sample. It is now recognised that a wide range of clinical severity occurs in both sexes. Specific treatment and interventional strategies are now available and of great benefit to individuals, families and carers. Genetic counselling allows families to make informed decisions. General practitioners can play a central role in detection and management of this syndrome.

Two cases of Marfan's syndrome with acute dissection in relation to labour are presented. One patient died of aortic rupture while the other was electively operated. The haemodynamic changes during pregnancy and labour can have catastrophic consequences. The Marfan patient must be well informed before pregnancy and carefully monitored during pregnancy and the peripartum period. Serial echocardiographic studies evaluating aortic root size and aortic valve function are essential. In case of severe cardiovascular manifestation the pregnancy should be interrupted.

A 32-year-old short statured woman with alopecia, typical facies, shortened angulated fingers and toes with Trichorhinophalangeal syndrome type I (TRPS I) is reported. The absence of exostosis and mental retardation rule out TRPS II. The absence of generalized shortness of all phalanges, metacarpals and metatarsals distinguish it from TRPS III. Possibly the various types of Trichorhinophalangeal syndrome are genetically identical but have a varied clinical spectrum.

This development of pathology in conserved ovaries is defined as the residual ovary syndrome (ROS). It consists of pelvic pain, pelvic mass, dyspareunia as a single or a cluster of symptoms. It is estimated that at least 5% of patients will develop ROS and require surgery for it following hysterectomy. We describe a case of ROS in a 41-year-old woman who developed an abdominopelvic mass the size of a pregnancy of 24 weeks' gestation within 11 weeks of hysterectomy and right salpingo-oophorectomy. At laparotomy, the mass was found to be a huge benign ovarian cyst measuring 11.5 x 11.0 x 14.1 cm…

We report an 18-month old male presenting with a right-sided inguinal hernia and undescended testes. At herniotomy, a uterus and two fallopian tubes were found in the pelvic peritoneum adjacent to the two gonads which received their blood supply partly along the müllerian duct remnants. The gonads were testes by histological examination. Bilateral orchiopexy was performed without removal of the müllerian structures for fear of jeopardizing the testicular blood supply. The diagnosis of persistent müllerian duct syndrome was confirmed postoperatively by genetic and hormonal investigations.

BACKGROUND: Pituitary stalk transection is a non-negligible cause of growth hormone (GH) deficiency.
POPULATION AND METHODS: We studied 22 … stimulating hormone (TSH) deficiency. Diabetes insipidus was present in six cases and revealed the syndrome in two. All children older than normal age of puberty (n = 10) had gonadotropin deficiency. In our study, these hormonal anomalies progressed from isolated GH deficiency to multiple hormonal deficiencies.
CONCLUSION: The recently described stalk tr …

Two patients, men aged 47 and 64 years, were found in a comatose condition in their homes, after a period of fatigue, polyuria and polydipsia. They had not been known to suffer from diabetes mellitus, but now displayed a hyperglycaemic hyperosmolar non-ketoacidotic disorder as the first manifestation of diabetes mellitus type 2. In that condition, just sufficient insulin is present to counteract ketone production, but not enough to prevent hyperglycaemia. Neurological and thromboembolic manifestations, possibly fatal, may result from severe dehydration brought about by a vicious circle in whic

PURPOSE: To describe the anaesthetic management of a child with Poland syndrome for CT scan.
CLINICAL FEATURES: An eight-month-old child presented with left upper limb hypoplasia and chest wall deformity with absence of ribs on the left side for CT scan of thorax. Pulsations of the heart could be seen along with paradoxical respiration over the defect. The trachea was intubated and the lungs ventilated manually to avoid inadequate ventilation and hypoxia.
CONCLUSIONS: The unilateral absence of ribs leads to poor development of subatmospheric pressure in the thorax and paradoxical respiration…

The antiepileptic drug hypersensitivity syndrome (AHS) is an adverse drug reaction associated with the aromatic antiepileptic drugs (AEDs) phenytoin (PHT), carbamazepine (CBZ), phenobarbital (PB), and primidone. The syndrome is defined by the triad of fever, skin rash, and internal organ involvement. It can also be caused by other drugs, such as sulfonamides, dapsone, minocycline, terbinafine, azathioprine, and allopurinol. Diagnosis of AHS may be difficult because of the variety of clinical and laboratory abnormalities and manifestations and because the syndrome may mimic infectious, neoplast…

Cubital tunnel syndrome is the second most common peripheral compression neuropathy. The unique anatomic relationships of the ulnar nerve at the elbow place it at risk for injury. Normally with elbow range of motion, the ulnar nerve is subjected to compression, traction, and frictional forces. As the elbow is flexed the arcuate ligament elongates producing a decrease in canal volume of 55%. Intraneural and extraneural pressures increase and have been shown to exceed 200 mm Hg with elbow flexion and flexor carpi ulnaris contraction. Because the ulnar nerve courses behind the elbow axis of rotat…

Fixed head malleus syndrome is a rare anatomoclinical entity first described by Goodhill in 1966. We present a series of 9 patients who underwent surgery between 1991 and 1997 and discuss the technical procedures used and functional outcome. Ossicular mobility can be re-established with two surgical methods. The more simple method consists in a classical incus transposition with malleus neck section. The more physiological method consists in drilling the synostosis fixing the malleus without disrupting the ossicular chain; stapedotomy is associated in certain cases (Type III).

Although the carpal tunnel is open at both ends, it has the physiologic properties of a closed compartment bounded by synovium proximally and distally. When the intracarpal canal interstitial pressure rises above a critical threshold pressure, capillary blood flow is reduced below the level required for median nerve viability. Acute carpal tunnel syndrome is recognized frequently as occurring secondary to wrist trauma and infrequently due to a variety of infectious, rheumatologic, and hematologic disorders. This condition warrants prompt recognition and the treatment is early carpal tunnel rel…
