The large reservoir of animal plague in the American West led to 121 cases of human plague from 1970 to 1980. The majority (55%) of recent cases have occurred in children 16 years of age and younger. In New Mexico 38 pediatric plague cases were reviewed to determine the epidemiologic, clinical, and laboratory feature of this disease. Currently preferred therapy for CNS Nocardia infection is high-dose sulfonamide coupled with surgical drainage. Neither of these could be used in our patient; this led to therapy with minocycline alone. Several months after completion of minocycline therapy, the patient apparently is cured. His favorable outcome probably resulted from a combination of susceptibility of the organism to minocycline coupled with good CNS penetration of the agent resulting in CNS levels of drug 16 to 22 times higher than the Nocardia inhibitory concentration.

One hundred emergency department charts on cases of suspected pelvic inflammatory disease (PID) were reviewed to determine method of diagnosis and treatment. Seventy-three gonococcus smears, 60 gonococcus cultures, and 53 VDRL tests for syphilis were done. Of these, 20 smears, 19 cultures, and 1 VDRL were positive. All 100 patients were treated for gonorrhea with either spectinomycin, tetracycline, ampicillin or penicillin. Two cases of nocardial mycetoma were successfully treated with a mixture of trimethoprim and sulfamethoxazole (Bacterim). The patient in case 1 showed resistance to treatment with antibiotics and sulfamethoxypyridazine, and the causal agent of the disease could not be isolated. In case 2, bone involvement was seen; the causal agent was N asteroides. No adverse effects of the drug occurred in a period of treatment ranging from three to nine months.
