Reducing cardiac injury during minimally invasive repair of pectus excavatum.

Minimally invasive repair of pectus excavatum (MIRPE) provides a minimal access approach to correct pectus excavatum deformities. Cardiovascular complications represent a rare but catastrophic complication of this cosmetic operation. We describe a modification to the technique following a case of cardiac puncture.

Different characteristics of nonthymomatous generalized myasthenia gravis with and without oropharyngeal involvement.

BACKGROUND: Thymectomy represents a safe and valid approach for patients with myasthenia gravis. However, some factors may influence the efficacy of thymectomy. The objective of this study was to evaluate the clinical characteristics of generalized myasthenia gravis with oropharyngeal involvement and compare the postoperative outcome of generalized myasthenia gravis without and with oropharyngeal involvement. METHODS: From 1991 to 2002, there were 202 patients with nonthymomatous generalized myasthenia gravis (GMG), who underwent thymectomy by the transsternal approach or thoracoscopy. Accord

[Case of multiple intrapulmonary lymph nodes].

A 45-year-old man who had hypertension, hyperthyroidism, and bronchial asthma was admitted to our hospital because of a low-grade fever and chest pain. The physical findings and laboratory data were almost all within normal limits except for evidence of mild inflammation and liver dysfunction. The chest X-ray findings seemed normal, but a computed tomography (CT) scan showed multiple nodules in both lower lung fields. We suspected the cryptococcosis or lung cancer. Biopsy by video-assisted thoracoscopic surgery (VATS) yielded a diagnosis of multiple intrapulmonary lymph nodes. In cases with th

A case of dermoid cyst ruptured into the lung.

Ruptured cystic teratomas (dermoid cysts) are rare and always symptomatic, presenting as haemothorax, pleural effusion or pericardial effusion. We present an extremely rare case of a 45-year-old woman who, during a routine health assessment was noted to have a well-defined anterior mediastinal tumour with peripheral ground glass opacity on chest CT. The patient was asymptomatic. She underwent video-assisted thoracoscopic surgery and a ruptured dermoid cyst was observed and the contents had infiltrated into the right pulmonary parenchyma. There were no complications and no evidence of recurrenc

[A comparison of T3 sympathectomy versus T4 sympathectomy in the treatment of primary palmer hyperhidrosis].

OBJECTIVE: To evaluate the effect and side effect of T(3) versus T(4) sympathectomy in the surgical management of primary palmar hyperhidrosis through prospective multi-center randomized controlled trial. METHODS: From Sep 2004 to Feb 2006, 106 consecutive patients (49 males and 54 females) with primary palmar hyperhidrosis were enrolled in the study and randomly assigned into T(3) or T(4) level sympathectomy groups. 66 patients received T(3) sympathetic chain transection and 37 patients received T(4) sympathetic chain transection. The two groups were comparable in all other parameters. RESU

Videothoracoscopic talc poudrage in primary spontaneous pneumothorax: a single-institution experience in 861 cases.

OBJECTIVE: We sought to evaluate the outcome of 861 patients treated with videothoracoscopic talc poudrage for primary spontaneous pneumothorax. METHODS: From September 1995 through January 2004, a total of 861 patients (578 male, 283 female, mean age 28.6 years) underwent videothoracoscopy for recurrent and complicated primary spontaneous pneumothorax. Patients were treated with videothoracoscopic talc poudrage only (group A: Vanderschueren's stage I, 196 patients; stage II, 112 patients) or videothoracoscopic talc poudrage plus stapling of the blebs/bullae (group B: stage III, 391 patients;

[Laparoscopic surgery: from clinic to legal medicine].

The laparoscopic technique introduced a new way of operating but inevitably causing new problems for the surgeon. After a comprehensive review of the history and the evolution of laparoscopic surgery from its beginning, the technical aspects of minimally invasive surgery and its fields of application are described. The close dependence on instruments and technology is emphasized. A detailed analysis of the advantages and limitations of laparoscopy is made with emphasis on the importance of a risk-benefit evaluation by the health care provider. Of key importance is to obtain a detailed and clea

[Esophageal gastrointestinal stromal tumor surrounding the middle esophagus with dysphagia for 8 years; report of a case].

In September 2002, a 24-year-old woman complaining dysphagia with an abnormal shadow in a chest X-ray was admitted to our hospital. Endoscopic ultrasonography, computed tomography (CT), and magnetic resonance imaging (MRI) showed a hypo-echoic, low-density mass surrounding the middle esophagus. Bronchofiberscopy and gastrofiberscopy showed compression from the outside of bronchus and esophagus. No ulcer formation was found. Transbronchial aspiration biopsy and esophageal biopsy showed no malignancy. After 14 cm thoractomy, extirpation of the tumor was performed. The solid tumor was 10.5 x 3.0

Lung wedge resection improves outcome in stage I primary spontaneous pneumothorax.

BACKGROUND: To evaluate the role of apical lung wedge resection in patients with recurrent primary spontaneous pneumothorax with no endoscopic abnormalities at surgery as compared with simple apical pleurectomy. METHODS: We performed a retrospective analysis on 126 consecutive video-assisted thoracoscopic surgery (VATS) procedures in 113 patients treated for stage I recurrent PSP between January 1994 and December 2001. Two surgical strategies were applied: simple apical pleurectomy (57 procedures, 45.2%: group A) and apical pleurectomy together with an apical lung wedge resection (69 procedur

Endoscopic aortic valve replacement.

During the last 8 years, many different approaches for minimally invasive aortic valve surgery have emerged. We have developed a technique that enables total endoscopic aortic valve replacement with port access, via a small right lateral thoracotomy with only soft tissue retraction and minimally invasive aortic crossclamping. The operation is performed under video guidance, since no direct eye vision is possible. We believe this is the first such operation performed in cardiac surgery and that it mak es possible broadening of indications for nonsternotomy-video-directed surgery in the future.

Initial surgical staging of lung cancer.

Many patients with early stage lung cancer (stage I and II) are curable by surgical resection. In patients with locally advanced disease surgery plays an important role in order to provide local tumor control. Therefore, the aim of all staging efforts in NSCLC must be to identify all patients, who might be potential candidates for a surgical approach. Current staging tools include imaging techniques like CT- and PET-scan, transthoracic, transbronchial or transeosophageal needle biopsies and finally surgical staging methods including mediastinoscopy and video-assisted thoracoscopic surgery (VAT

Congenital Heart Surgery Nomenclature and Database Project: pulmonary venous anomalies.

The extant nomenclature for pulmonary venous anomalies is reviewed for the purpose of establishing a unified reporting system. The subject was debated and reviewed by members of the STS-Congenital Heart Surgery Database Committee and representatives from the European Association for Cardiothoracic Surgery. All efforts were made to include all relevant nomenclature categories using synonyms where appropriate. The basis for classification are the prenatal errors of embryologic development. The major categories include: partially anomalous pulmonary venous connection, totally anomalous pulmonary …

Reconstruction of congenital chest wall deformities using solid silicone onlay prostheses.

Congenital chest wall deformities include five types: pectus excavatum (funnel chest), pectus carinatum (pigeon breast), Poland's syndrome, defects of sternal fusion, and miscellaneous dysplasias and skeletal disorders. Of these five types, two, pectus excavatum and Poland's syndrome, are defects of the skeletal chest wall. These two specific anomalies comprise the vast majority of congenital defects of the chest wall and, as depression deformities, are readily amenable to surgical correction.

Thoracoscopic truncal vagotomy.

BACKGROUND: Nowadays the only indications to truncal vagotomy is recurrent ulceration after previous gastric surgery. Truncal vagotomy allows us to obtain a reduction in acid production and to promote ulcer healing, but this technique causes pylorospasm in about 20% of cases and this requires further synchronous or metachronous pyloric drainage procedure. For this reason, videothoracoscopic truncal vagotomy is reserved to patients with gastroresection. METHODS: The authors describe 15 patients treated with videothoracoscopic truncal vagotomy. In 12 patients, a gastrojejunostomy was done accor

[The video-assisted thoracoscopic treatment of spontaneous pneumothorax--our experience with 42 patients].

We present our experience with video-assisted thoracoscopic surgical (VATS) treatment of spontaneous pneumothorax (SP) in 42 consecutive patients. We performed 45 video-thoracoscopic procedures during 44 hospitalisations. The most common procedures were: Endo-stapling of bullae in 12 pts, 4 ligations of bullae, 8 suturing of bullae, 27 bleb ablations and catheterizations. In all patients a parietal pleural abrasion was performed. We describe a new variation of mechanical pleurodesis for prevention of recurrences--"linear pleurotomy"--performed in 15 patients. No major complication occurred dur

Pediatric tracheal problems.

Awareness of the special problems of airway repair in children and precise application of principles of tracheal and laryngotracheal surgery are necessary for success in treating acquired pediatric airway lesions. Long segment congenital lesions require special techniques, which are still evolving.
